Typically you see agents who are working for the health insurance company and not really working for the employer companies they are selling to. Health insurance is primarily run by the Big 4 – BUCA: Blue Cross United Healthcare, CIGNA, Aetna. Health insurance employer group plans can be broken down into 4 Parts: 1. TPA or third-party administrator 2. Network PPO or HMO 3. Pharmacy benefit RX 4. Insurance that covers the caps the limits on the stop loss PPO is your primary network and open on using referrals HMO is a non-preferred network typically has less offerings and tight on referrals Ethos Benefits helps employers to break up the four parts of a group plan and customize Network PPO is what people typically think of when they think of health insurance. Network and the Big 4 health companies have a tighter deal with doctors and contract prices. A lot is pre-negotiated with a set of rates, which is the point of a PPO. This is where you get larger claims and they run in the system of healthcare. Seg 2 Pay more for healthcare in the U.S. than the rest of the world The biggest pharmaceutical companies are in the US Pharmaceutical companies in flight prices, and also set the prices They make money through spread pricing Employers can actually pick up their own Pharma benefit and get the rebates that the big health insurance companies are not giving them Healthcare system is a rigged game The fraud waste and abuse extremely high in health 401(k) and retirement benefit industry is actually tighter with more disclosure than the healthcare industry Regulated better since the creation of the Securities Act in the 1930s and updates that ran through the 1970s and beyond with things like ERISA There have been revisions to the healthcare act passed in 2021 - started in Jan.2022 Actions had 3 disclosures: · Brokers comp and bonus · Data with gag, clauses, and full access to data upon request · Benchmarking for drug cost Further Notes via Google: The revisions you are likely referring to are part of the Consolidated Appropriations Act of 2021 (CAA). Whistleblower program. In addition to the methods of proof the IRS has developed, the Tax Relief and Health Care Act of 2006 created the IRS Whistleblower Office, which allows anonymous whistle blowers to receive 15 to 30 percent of any recovery by the IRS which comes to at least $2 million including all penalties, interests and any other monies collected from the government. The whistleblower program seeks information based on evidence and analysis which can provide a solid basis for further investigation rather than speculation and hearsay. The program is designed to provide incentive to ordinary citizens to inform on tax cheats. The program provides far greater incentives for whistleblowers than previous programs because under prior programs the government was not required to compensate whistleblowers. Under this program, a taxpayer may file a lawsuit in court if he or she does not receive a deserved award. Whistleblower Office. Established by the Tax relief and Health Care Act of 2006, the IRS Whistleblower Offices processes tips received from eligible individuals who spot tax problems in their workplace, in their day-to-day personal business, or anywhere else. After determining the degree of credibility, an appropriate IRS office is assigned the case for further investigation. The IRS office assigned varies by the type of issue that the whistleblower alerted it to. Individuals must meet qualifications to be eligible to receive the reward and must submit Form 211 with supporting documentation to the Internal Revenue Service Office in Ogden, Utah. To claim eligibility the individual must not be an employee of the Department of Treasury or have been an employee there when they obtained the information, must not have obtained the information through the individual's official duties as an employee of the federal government, or who obtained the information based on a contract with the federal government. Whistleblower incidents happen in greater frequency in the private sector opposed to the government. The reward can be worth between 15 and 30 percent of the total proceeds that the IRS collects. To claim the reward, the IRS must move ahead based on the information provided and the amount identified, including taxes, penalties, and interests, must be worth more than $2 million. A gross income of $200,000 or more is required if the taxpayer in question is an individual. The Bipartisan Budget Act of 2018 added subsection 7623(c) which expanded the definition of proceeds for whistleblower awards and was applied to open whistleblower claims. The Taxpayer First Act was signed by President Trump on July 1, 2019. This law made changes surrounding the notification process to whistleblowers and increased protection against retaliation available to whistleblowers. During his conscripted service with the German Navy, Christian Dierks learned that if you're the person in charge of rules and administration, you can bring about a lot of change. He took this lesson with him to law school and in the creation of his own law and healthcare consultancy firm, Dierks+Company. Christian was instrumental in the creation of Germany's 2019 Digital Health Care Act (known as DVG in Germany), which mandates full reimbursement of any prescribed digital health application. In fact, Christian was involved in setting up the legal framework for digital health applications (DiGA) in Germany—and importantly, he convinced health insurers in Germany not to nickname them DiVA, an acronym which would have carried distinctly less gravitas. Starting with this episode, Eugene says farewell (for now) to DTx trailblazers in the United States, and shifts the focus to leaders in Europe, where Christian and other thought leaders are working to broaden access to digital health solutions. Eugene and Christian recorded this episode in January, and since then, there have been some exciting updates to the status of DVG. January Laws HR 26: Terrorism Risk Insurance Program ReAuthorization Act of 2015 The bill reauthorizes and changes the terms of the program that provides Federal insurance to businesses damaged in a terrorist attack. The program was dead for 12 days after expiring on December 31, 2014. Extends the program until December 31, 2020 Decreases the Federal share of compensation from 85% to 80% over the course of the next five years. There's a $100 billion cap on Federal losses. The program trigger, which is the point at which insurance companies get Federal money, gradually increases from $100 million now to $200 million. The Secretary of Treasury alone will certify the act of terrorism; the Secretary of State will no longer be involved. A rollback of the Dodd Frank financial reform bill was attached. The attachment prevents the SEC from telling swaps traders how much cash they need to put up front to make a swaps trade. The attachment is the text of the Business Risk Mitigation and Price Stabilization Act, which was written by Rep. Michael Grimm. He tried to get it passed in the 112th and 113th Congresses, before he resigned on the first day of the 114th Congress after pleading guilty to tax fraud. The Securities and Investment industry was his #2 contributor, giving him over $400,000. Rep. Jeb Hensarling, who fought to keep this provision in the bill, has taken at least $2.8 million from the financial industry. January Bills H.R. 22 and S. 12: Hire More Heroes Act Veterans with government health care will not count towards the 50 employee Affordable Care Act threshold which triggers a company's obligation to give employees health insurance. The bill is sponsored by Rep. Rodney Davis in the House, who has taken over $180,000 from health professionals and over $160,000 from the insurance industry. The bill is sponsored by Senator Roy Blunt of Missouri, who has taken almost $1.3 million from health professionals and almost a million from the insurance industry. H.R. 23: National Windstorm Impact Reduction Act Reauthorization of 2015 Reauthorizes and updates the National Windstorm Impact Reduction Program The program designed to improve weather modeling, coordinate post-storm investigations, improve understanding of wind's impact on buildings and vital infrastructure, and promote adoption of storm preparation measures. Appropriates about $21 million per year for the next three years Written by Rep. Randy Neugebauer of Texas H.R. 34: Tsunami Warning, Education, and Research Act of 2015 Consolidates tsunami warning systems for the Pacific and Arctic Oceans and for the Atlantic Ocean into a single warning system, which will cooperate with other countries' warning systems. Appropriates $27 million per year through 2017 to get this done. Passed unanimously. Written by Rep. Suzanne Bonamici of Oregon H.R. 203: Clay Hunt Suicide Prevention for American Veterans Act Requires annual independent evaluations of Veteran's Administration mental health programs Requires the Veteran's Administration to create a website for information about their mental health services that needs to be updated at least every 90 days. Creates a three year pilot program to repay psychiatrists' loans ($30,000 a year) if they work for at least two years at the Veteran's Health Administration. Prohibits any additional money to get this done. Written by Democrat Rep. Timothy Walz of Minnesota H.R. 351: LNG Permitting Certainty and Transparency Act Forces the Department of Energy to decide on applications to construct, expand, or operate liquified natural gas export facilities within 30 days of the completed NEPA review. Requires the applicant to publicly disclose the specific destination of the liquified natural gas exports. Written by Rep. Bill Johnson of Ohio, whose top three contributing industries are #1 Leadership PACs, who have given him over $387,000, #2 Mining, who has given him over $250,000, and #3 Oil and Gas, who has given him over $244,000. H.R. 3: and S. 1 Keystone XL Pipeline Act Explicitly approves the Keystone XL pipeline. Forces any lawsuits against the pipeline to be filed in Washington D.C. or in the Supreme Court Written by Rep. Kevin Cramer of North Dakota, whose #1 contributing industry is Oil & Gas, who have given him over $322,000. The House version would be vetoed by the President H.R. 30: Save American Workers Act of 2015 Makes people work for 40+ hours to be eligible for employer provided health insurance. Includes the Business Risk Mitigation and Price Stabilization Act, which is the bill that has already been signed into law as an attachment to the Terrorism Risk Insurance Program reauthorization. Would delay the Volcker rule until 2019 (the Federal Reserve already delayed it until 2017), which prohibits commercial banks from trading collateralized loan obligations. Would allow companies to exclude historical data from their financial reports at their discretion. Companies with under $250 million in revenue wouldn't have to submit their financial statements in computer readable form (this would include roughly 60% of publicly traded stocks). Exempts some private equity firms from having to register as brokers with the SEC, which will exempt them from more frequent examinations. The bill was sponsored by Rep.
