Willkommen zu Folge #129 vom MS-Perspektive-Podcast. Heute begrüße ich Prof. Dr. Dr. Henri-Jacques Delecluse vom Deutschen Krebsforschungszentrum (DKFZ) in Heidelberg als Gast im Interview. Wir sprechen über die Bedeutung des Epstein-Barr-Virus (EBV) für die Entwicklung verschiedener Krebsarten, aber auch der Multiplen Sklerose. Von einer erfolgreichen Impfung könnten vermutlich viele Menschen profitieren, aber das Virus macht es den Forschern nicht einfach. Seit 40 Jahren wird bereits nach einem Impfstoff gesucht, doch bisher gibt es noch keine zugelassene Impfung gegen EBV.  Aktuell arbeiten sieben Firmen an einer Lösung. Wir sprechen über die Herausforderungen denen die Forscher begegnen, wie das Epstein-Barr-Virus das Immunsystem angreift und warum es für unser Immunsystem so schwierig ist das Virus zu eliminieren. Er ist Abteilungsleiter am Deutschen Krebsforschungszentrum (DKFZ) in Heidelberg und erforscht die Pathogenese (Entstehung und Entwicklung) infektionsbedingter Tumore. Mit seiner Frau, die ebenfalls Ärztin ist, hat er drei Kinder. Zu seinen Hobbys gehören Gesang und Mathematik. Bedeutung des EBV-Virus Wie verbreitet ist das Epstein-Barr-Virus (EBV) in Deutschland und weltweit? Weltweit, also auch in Deutschland sehr verbreitet, über 95% der Bevölkerung über 50 sind infiziert. In welchem Alter / welcher Altersspanne infiziert sich der Großteil der Bevölkerung mit EBV? Es kommt darauf an wo auf der Welt, in Afrika sind die meisten Kinder vor dem ersten Geburtstag infiziert, in industrialisierten Ländern zwischen 15 und 25 Jahren. Welche Krankheiten stehen in Zusammenhang mit dem Epstein-Barr-Virus? Es stehen sehr viele Krankheiten in Zusammenhang, allen voran das Pfeiffersche Drüsenfieber, eine gutartige meist sich selbst zurückbildende Entzündung der Lymphknoten und des Rachenraums. Allerdings wird das Virus mitverantwortlich gemacht für die Entstehung von Tumoren des Nasopharynx, des Magens sowie des Lymphdrüsensystems wie etwa Morbus Hodgkin. Letztlich spielt das Virus eine entscheidende Rolle bei der Entwicklung von Multipler Sklerose und womöglich von anderen autoimmunen Krankheiten. Wieso forschen Sie am Deutschen Krebsforschungszentrum daran? Weil Krankheiten, die von Viren verursacht werden, leichter vorgebeugt oder therapiert werden können. Auch weil Herpesviren sehr komplex und interessant sind. Welche Bedeutung hat EBV für die Multiple Sklerose und warum ist der Zusammenhang so eindeutig? Man weiß, dass die relativ wenigen Menschen, die EBV nicht tragen, fast nie MS entwickeln, d.h. die Infektion ist Voraussetzung für ihre Entwicklung. Allerdings haben die meisten Menschen, die das Virus tragen keine MS. Also braucht man mehr als EBV, um MS zu entwickeln. Der Zusammenhang zwischen EBV und MS basiert auf epidemiologischen Studien. Letztere können eindeutige Zusammenhänge erkennen, wenn sie gut gemacht sind. Die Studien, die den Zusammenhang zwischen EBV und MS untersucht haben, sind mittlerweile sehr gut und liefern eindeutige Ergebnisse. Komplexität des Virus und seiner Wirkungsweise auf das Immunsystem Was ist das Besondere am Epstein-Barr-Virus? Es ist sehr großes und daher komplexes Virus, das sehr erfolgreich Menschen lebenslang infiziert, meist, aber eben nicht immer, ohne Konsequenzen für seinen Wirt. Welche Phasen gehören zu einer EBV-Infektion und wie sind sie zeitlich einzuordnen? Zunächst erfolgt die primäre Infektion, also der erste Kontakt mit dem Virus. Diese verläuft symptomlos oder höchstens mit einem milden grippalen Infekt in der Kindheit. Wenn man später infiziert wird, kann sich ein Pfeiffersches Drüsenfieber entwickeln, mit geschwollenen Lymphdrüsen und Mandeln, Fieber, Veränderung des Blutbildes und manchmal größeren Problemen wie Milzruptur oder Leberentzündung. Nach diesem ersten Kontakt bleibt das Virus im Körper, in den meisten Menschen ohne Konsequenzen. Bei manchen Menschen kann sich jedoch eine Krankheit entwickeln, jeder Zeit nach der ersten Infektion. Welche Zellen des Immunsystems greift das EBV-Virus an und wie schädigt es die Abwehrzellen? EBV infiziert sehr erfolgreich B-Lymphozyten und zwingt sie, sich zu vermehren. Wenn diese Vermehrung unbegrenzt bleibt, kann es zur Tumorbildung führen. In manchen Zellen werden jedoch neue Viren produziert, dabei sterben die Zellen ab. Ist die Wirkung der mehr als 30 Proteine, die jeder einzelne EBV-Virus beinhaltet, auf den menschlichen Körper bereits klar oder wird daran noch geforscht? Bei manchen Proteinen ist die Wirkung relativ klar, bei anderen weiß man sehr wenig bis gar nichts. Allgemein kann man sagen, dass die Rolle des Virus in der Krebsentstehung wenig bekannt bleibt, mit der Ausnahme von bestimmten Tumoren. Gibt es ein oder mehrere Proteine aus dieser großen Menge, die ein geeignetes Ziel für eine Impfung sind? Im Prinzip stellen alle Proteine ein mögliches Ziel dar, es sei denn sie haben schädliche Eigenschaften, wie die Fähigkeit Krebs auszulösen. Aktueller Stand der Forschung für mögliche EBV-Impfungen Seit wann wird an einem EBV-Impfstoff geforscht, und warum ist es so schwer, eine funktionierende Impfung zu entwickeln? Seit den 1980ern wird daran geforscht. Generell ist es schwer, gegen Herpesviren zu impfen, weil sie dem Immunsystem so gut entgehen können und sich in Zellen verstecken. In ihrem Versteck produzieren sie keine Proteine mehr und werden deshalb auch nicht mehr vom Immunsystem erkannt. Außerdem werden Menschen im Laufe der Zeit immer wieder vom Epstein-Barr-Virus infiziert. Deshalb wird es vermutlich nie einen absoluten Schutz gegen das Virus geben. Allerdings könnte eine Impfung die akuten Symptome der Infektion wie das Pfeiffersche Drüsenfieber möglicherweise mildern. Das würde wahrscheinlich wiederum die Frequenz mancher Tumore wie Morbus Hodgkin oder Krankheiten wie MS verringern. Sie haben sieben Ansätze für Impfungen gegen EBV untersucht. Welche davon sind besonders vielversprechend? Alle haben das Potential, wirksam zu werden, oder auch nicht. Manche werden T-Zellen besser stimulieren, andere die Antikörperproduktion. Da bis jetzt nur mit lebendigen abgeschwächten Viren eine erfolgreiche Impfung gegen Herpesviren erreicht wurde, wissen wir nicht, ob die anderen Technologien überhaupt funktionieren werden. Natürlich kann man aus ethischen Gründen eine Lebendvakzine beim Epstein-Barr-Virus nicht anbieten. Wie sieht es mit der mRNA-Technologie aus? Könnte diese Technik als Impfung gegen den EBV-Virus erfolgreich zum Einsatz kommen oder eher spezifisch für einzelne Erkrankungen, wie die MS? Die mRNA Technologie ermöglicht die schnelle Entwicklung von Impfstoffen. Allerdings ist die mRNA Technologie, wie wir es beim Coronavirus gesehen haben nicht effizienter als andere Strategien z.B. mit Vektoren oder mit rekombinanten Proteinen. Da es meines Wissens derzeit unmöglich ist, herauszufinden welche Menschen zukünftig MS entwickeln werden, sollte die gesamte Bevölkerung im Kindesalter geimpft werden. Es ist auch denkbar, dass die Impfung MS-Patienten nutzen würde, auch wenn sie bereits das Virus tragen. Allerdings ist das nur eine weit hergeholte Hypothese, die keineswegs bewiesen ist. Wer würde von einer erfolgreichen Impfung profitieren? Potentiell alle Menschen, da das Pfeiffersches Drüsenfieber relativ häufig vorkommt. Weiterhin würde die Impfung den 200.000 Krebsfällen, die jährlich von EBV verursacht oder mitverursacht werden, zu einem gewissen Teil vorbeugen. Und natürlich besteht die Hoffnung, dass die Impfung die Frequenz von Multipler Sklerose auch reduziert. Allerdings bleibt unklar, ob eine effiziente Impfung möglich ist. Welchen Durchbruch in der Forschung zu EBV-Impfstoffen wünschen Sie sich in den kommenden 5 Jahren? Eine erfolgreiche klinische Studie, die eben 5 Jahre Beobachtungszeit hat und beweist, dass Vakzinierung die Frequenz von EBV-assoziierten Krankheiten senken lässt. Wo können sich Patienten über den aktuellen Forschungsstand bezüglich EBV-Impfungen informieren? Ich selbst habe keine direkten Informationen. Die Firmen, die Impfungen entwickeln, werden bestimmt Werbung machen. Verabschiedung Möchten Sie den Hörerinnen und Hörern noch etwas mit auf dem Weg geben? Ich denke, dass die EBV-Forschung große Fortschritte macht. Seriously, a document has apparently been unearthed by Foreign Policy magazine that appears to show either the incredible level of preparedness of the U.S. national security community, or it demonstrates how the people tasked with keeping the nation safe are insane. The zombie attack training manual, “CONOP 8888,” or “Counter Zombie Dominance,” is dated April 2011, and was supposedly found buried on a secret military computer. It tells military planners how to defeat an invasion of flesh eaters, (Editor's note: Remember he outbreak of zombies that were devouring peoples faces.) and how to preserve the sanctity of “non-zombie life.” From magic zombies to chicken zombies, even vegetarian zombies, they're all in the file, with a plan for how to take them out.  “Zombies pose a threat to all non-zombie human life,” says CONOP8888. “[Strategic Command] will be prepared to preserve the sanctity of human life and conduct operations in support of any human population—including traditional adversaries.” While the manual says in its disclaimer that “this plan was not acutally designed as a joke,” it still kind of is. What the intelligence community has done here is use “zombie” as a kind of code word for “enemy,” so as not to look like the Pentagon is planning for an invasion of a particular country, religion, or plague. To prepare for all possible situations, it says, “We elected to use a completely-impossible scenario that could never be mistaken for a real plan.” All of which begs the question, of course, if zombies are being used as metaphors for real threats, what... are “Vegetarian zombies” doing in there? Read the full zombie apocalypse training manual below: CONPLAN 8888 Now if that was all bad enough… the airlines are panicking over 5G (The opening scene of the movie “Cell” takes place in airport!!)  What do they know????    Planned 5G Wireless Deployment Could Compromise Airline Safety, Aviation Executives Warn  Newsweek - Justin Klawans - Dec 21, 2021    Aviation executives expressed their fears Monday that the potential coming deployment of 5G wireless technology could harm the airline industry. In a joint letter acquired by Reuters addressed to the U.S. Department of Transportation (DOT), Boeing CEO Dave Calhoun and Airbus Americas CEO Jeffrey Knittel urged DOT Secretary Pete Buttigieg to postpone the upcoming rollout of the technology, citing multiple concerns. "5G interference could adversely affect the ability of aircraft to safely operate," the letter stated, adding that the rollout of the technology could have "an enormous negative impact on the aviation industry.” Both AT&T and Verizon are planning to deploy C-band spectrum 5G wireless networks across the United States on Jan. 5. The technology was originally slated to go online in November, but the companies agreed to push the date into 2022 after concerns were cited, and now the airline executives are hoping to slide that date even further down the calendar. The airplane manufacturers, who are typically considered direct competitors, came together in a rare show of unity to voice concerns that the 5G initiative could lead to numerous delays at airports throughout the country. The pair of companies represent the vast majority of the airplanes built for commercial aviation use, and they also have a large market share of military aircraft. The letter from Boeing and Airbus cited a statistic from trade group Airlines for America, which stated that if 5G had been in effect in 2019, it would've resulted in the delay or cancellation of 345,000 passenger flights and 5,400 cargo flights. The sentiment from the airline executives echoed a report released Dec. 7 from the U.S. Federal Aviation Administration (FAA) that said 5G technology could lead to scheduling problems and safety issues in the skies. After issuing its report, the FAA went on to present a pair of airworthiness directives that would prohibit aircraft from operating certain radio functions while in the area of 5G signals. "Because radio altimeter anomalies that are undetected by the aircraft automation or pilot, particularly close to the ground ... could lead to loss of continued safe flight and landing," one of the directives stated. However, the FAA later issued a statement in which it said that the "expansion of 5G and aviation will safely co-exist." Verizon also released a statement pushing back against the FAA guidelines, stating that "there is no evidence that 5G operations using C-band spectrum pose any risk to aviation safety, as the real-world experience in dozens of countries already using this spectrum for 5G confirms." The communications giant added that it was taking "additional steps to minimize energy coming from 5G base stations." And then there's a unit within the DNA strand called a sulfuric bridge.  This sulfuric bridge holds our entire DNA strand together. It only appears after every ten nucleic acids have been laid down, and then after every five nucleic acids, then after every six, and then again, after every five nucleic acids. So, we get this repeating numerical value of 10, 5, 6, 5 within our DNA strand. Well, it turns out the Biblical name for God, YHWH, contains the same numerical value as our DNA strand. Yod He Vav He equals ten, five, six, five. What's even more interesting is that if you take the Hebrew name of YHWH and place it on its side, you get the image of a human being. Part three of this video series suggests that DNA is a language. Many people were wondering what god made the language of DNA. Researchers at Harvard University announced today that they have found what appears to be a message from God written inside the human genome. The chip is said to be able to deploy an experimental new vaccine, developed jointly by Moderna and U.S government, that could change human DNA. The chip and the vaccine built on a breakthrough made by then-Harvard University professor and eventual Moderna co-founder Derek Rossi in 2010. In his discovery, which the now-retired scientist says came about purely by accident, Rossi claimed that he found a way to “reprogram” messenger ribonucleic acid (mRNA) – the molecules that carry genetic instructions for cell development in the human body. The promise provided by Rossi's breakthrough was such that it allowed him to co-found Moderna with private equity firm Flagship Pioneering. The company then attracted almost half a billion dollars from the federal government to begin developing vaccines using the new technology. This JCO Podcast provides observations and commentary on the JCO article, "Treatment of Childhood Nasopharyngeal Carcinoma with Induction Chemotherapy and Concomitant Chemoradiotherapy: Results of the Children’s Oncology Group ARAR0331 Study" by Rodriguez-Galindo et al. My name is Suzanne Wolden, and I am the Director of Pediatric Radiation Oncology at Memorial Sloan Kettering in New York City, USA.  My oncologic specialty is Pediatric Radiation Oncology. This important manuscript summarizes the results of Children’s Oncology Group protocol ARAR0331 for childhood nasopharyngeal carcinoma.  The study enrolled 111 patients, of whom 75% were male and 47% were African American, with a median age of 15.  Eligible patients had AJCC stage IIb-IVC disease and received three cycles of induction chemotherapy with 5-fluorouracil and cisplatin followed by concurrent cisplatin and radiotherapy.  Three patients had progressive disease during induction chemotherapy, eight were removed from the study due to physician or parent preference, and another three were not evaluable.  This left 97 patients evaluable for response and concurrent chemoradiotherapy.  Responses and radiotherapy treatment plans were not centrally reviewed but were left to the judgement of the treating institution.  All patients received 45 Gy to comprehensive head and neck fields encompassing the nasopharynx and bilateral level I-V lymph nodes.  A boost to the nasopharynx and residual gross nodal disease was prescribed based on response.  The full dose of 70.2 Gy indicated for stable disease was given to 18.6% of patients while 77.3% received the protocol specified dose for complete or partial response of 61.2 Gy or lower.  Another 4.1% of patients received intermediate non-protocol doses of 63.9-66.6 Gy.  The trial was amended to reduce the cycles of concurrent cisplatin during radiotherapy to two from three after unacceptable rates of renal and gastrointestinal toxicities were reported.    This trial was limited to patients age 18 years and younger and consisted primarily of American patients.  It is notable that the demographics of nasopharynx cancer in these young patients differ significantly from adults with this diagnosis.  Most patients are teens since nasopharynx cancer is vanishingly rare in younger children.  The majority were male, and nearly half were African-American.  Nasopharynx cancer is quite rare in teens of Asian descent, in stark contrast to the adult population.  In international series, it has been noted that teens in countries around the Mediterranean also have a relatively high incidence of this rare cancer.  The study illustrates that pediatric patients are much more likely than adults to have advanced disease and WHO type III, Epstein Barr virus-associated histology. Despite a lack of central review, the response rate appears to be quite high to induction therapy as one might expect with WHO type III carcinoma.  It is surprising that any patients had progressive disease, and I suspect that the rate of partial and complete response may have been even higher than what is inferred from the radiation boost doses given.  Nonetheless, the strategy of induction therapy was highly successful in allowing a large majority of patients to receive reduced doses of radiation.  It is important to note that all patients received 45 Gy to initial comprehensive fields rather than the 50 Gy typically used for adults and that smaller, more forgiving fractions of 1.8 Gy were used in comparison to the standard 2 Gy for adult head and neck cancer.  While some patients received a standard adult boost dose of 70.2 Gy, 81.4% received a lower boost dose.  In most cases, this was the protocol specified 61.2 Gy, a 13% dose reduction.  Even with these significantly lower doses, local control was exceptionally high, with only 5.5% of patients experiencing local failure with or without a distant relapse. The successful reduction in radiotherapy doses in this study is incredibly important and by far the most newsworthy aspect of this paper.  Even full or nearly grown teens experience dramatically more severe late effects of radiotherapy compared to older adults.  The severity of soft tissue fibrosis 10 or more years after full doses of radiation to the head and neck is so great that many patients require feeding tubes and tracheostomies.  The rate of second malignancy is also much higher in young patients.  The rate of distant relapse of 10.5% was low, considering that patients with metastatic disease at diagnosis were enrolled.  It is possible that with more advanced staging scans such as FDG-PET, metastatic lesions can be more easily detected and defined.  In that case, they can be targeted with radiotherapy to decrease the risk of relapse.  This approach has been shown to be effective in achieving cure in adults.  It is unlikely that an increase in chemotherapy beyond what was used in this protocol would be tolerated or worth the added toxicity.  Indeed, it is unclear whether the 3rd cycle of concurrent cisplatin was beneficial since there was only a trend for improved EFS and the cumulative toxicity is significant.  One must be mindful that increases in systemic therapy have been shown to decrease tolerance for head and neck radiotherapy.  This can be counterproductive to outcome if there are delays, breaks or failure to complete the course of radiation. This trial was so successful in its ability to decrease radiation exposure in young patients, that we should be inspired to go further.  My thoughts regarding the next trial would be to use PET scans for staging and response assessment and to push the radiotherapy doses lower.  This approach is currently being investigated for adult HPV associated head and neck cancer with doses as low as 30 Gy.  Perhaps in pediatric (or even EBV positive adult) nasopharynx cancer, 30-36 Gy would be enough for the comprehensive initial fields with boost doses in the range of 50-60 Gy for those who respond favorably to induction chemotherapy.  Dose reductions of this magnitude would make major differences in the acute and especially the late toxicities.  In addition, modern radiation technologies such as proton therapy allow improved avoidance of normal tissue exposure for these young, curable patients. In summary, this article provides evidence that response-based radiation dose reduction is possible for pediatric nasopharynx cancer.  Even with numerous limitations, the outstanding results of this trial inspire us to continue moving forward in the direction of reducing radiation exposure for these young patients.  Long-term follow up is also urged to quantify the expected benefits of these therapeutic changes. Neisseria meningitidis, ein Gram negatives pathogenes Bakterium ist eine der Ursachen für schwere Septikämie und Meningokokkenmeningitis. Nach Besiedelung des menschlichen Nasopharynx und Übertritt in die Blutbahn besteht ein zentraler Schritt in der Pathogenese der durch N. meningitidis verursachten bakteriellen Meningitis in der Interaktion der Bakterien mit Zellen der Blut-Hirn-Schranke. Die Schwere der Erkrankung scheint direkt mit der Produktion proinflammatorischer Zytokine, Chemokine und Wachstumsfaktoren zu korrelieren. Daher wurde in der vorliegenden Studie mit Hilfe eines Zellkulturmodells die Freisetzung von Tumornekrosefaktor alpha (TNF-a), Interleukin-1b (IL-1b), Interleukin-6 (IL-6), Interleukin-8 (IL-8), Monocyten-attrahierendem Protein-1 (MCP-1) und transformierendem Wachstumsfaktor beta (TGF-b) durch Gehirnendothelzellen nach Infektion mit Meningokokken analysiert. Mit ELISA und RT-PCR wurde die Freisetzung von Zytokinen und die Transkription der Zytokin-codierenden Gene von humanen Gehirnendothelzellen (HBMEC) nach Infektion mit dem Meningokokkenstamm MC58* und seiner unbekapselten isogenen Mutante MC58 siaD der Serogruppe B nachgewiesen. In Übereinstimmung mit der Zytokinfreisetzung wurde dabei ein typisches Genexpressionsmuster festgestellt. Beide Bakterienstämme beeinflußten die Transkription der Gene, die für IL-6 und IL-8 kodieren, wobei die Transkription bei den Zellen, die mit dem unbekapselten Stamm infiziert wurden, früher nachzuweisen war. Die Transkription des TNF-a Gens wurde nur nach der Infektion mit der unbekapselten Mutante nachgewiesen. Für IL-1b und MCP-1 wurde keine verstärkte Transkription festgestellt, wogegen das Gen, welches für TGF-b codiert, von infizierten wie uninfizierten Zellen gleichermaßen exprimiert wurde. Neben den intakten Bakterien führte auch die Stimulation mit Außenmembranproteinen zu einer Induktion der Zytokinfreisetzung. Die Verhinderung der Internalisierung der Bakterien in die Zellen bzw. die Blockade des a5b1 Integrin Rezeptors reduzierte die Freisetzung von IL-8 und TNF-a, nicht jedoch die Freisetzung von IL-6. Während durch die IL-6 oder IL-8 Prästimulation der HBMEC keine Veränderung des Invasionsverhaltens der Meningokokken beobachtet werden konnte, führte eine Prästimulation mit TNF-a zu einer deutlich gesteigerten Invasion der Bakterien in die Zellen. Diese Ergebnisse machen deutlich, daß der Entzündungsprozeß im Gehirn eine komplexe Interaktion zwischen Bakterium und Wirtszelle erfordert. Dabei spielen die Gehirnendothelzellen offensichtlich eine wichtige Rolle in der interzellulären Kommunikation der beteiligten Zellen, indem sie Zytokine als Immunmodulatoren freisetzen, die ihrerseits zu veränderter Expression von Adhäsionsmolekülen führen könnten.
