Why do female singers sometimes sound like they've been gargling gravel? In this episode of Join the Docs, our melodious maestros, Professor Jonathan Sackier and Doctor Nigel Guest, strike a chord as they riddle on  raspy rappers and discuss distressed divas. Why might sopranos struggle to sing soulfully instead sometimes sounding far from sonorous? . They kick things off with a tuneful tour of the larynx and vocal cords, those tiny but mighty instruments that can make or break a performance. With a sprinkle of humour and a dash of puns, they explain how hormonal cycles can turn a diva's dulcet tones into a croaky chorus.Ever wondered why your favourite singer suddenly sounds like they've been shouting at a football match? The Docs delve into the various medical conditions that can cause hoarseness, from the common cold's pesky cousin, laryngitis, to the sneaky saboteur, acid reflux. They paint a vivid picture of how these ailments can transform a nightingale into a screech-owl, hooting away in distress. With anecdotes that hit all the right notes, they remind us that if hoarseness lingers longer than a three-week encore, it's time to seek medical advice. So, how do famous singers keep their voices in tip-top shape? The episode is peppered with references to iconic vocalists who have battled the dreaded hoarseness. Sackier and Guest serve up a smorgasbord of star-studded stories that add a touch of glamour to the medical mumbo-jumbo. Their witty banter and playful puns ensure that even the most complex concepts are as easy to swallow as a spoonful of honey. Is laughter really the best medicine? As the conversation crescendos, The Docs strike a perfect balance between education and entertainment. They sprinkle in just the right amount of humour, making the medical discussion as engaging as a front-row seat at a Broadway show. So, why not tune in and let Professor Jonathan Sackier and Doctor Nigel Guest serenade you with their insights on hoarseness? Thankfully without either of our two Docs bursting into song. Whether you're a singer, a fan, or just someone who loves a good laugh, this episode of Join the Dots hits all the high notes, ensuring you walk away with a smile on your face and a song in your heart.—--DISCLAIMER: The views and opinions expressed on Join the Docs are those of Dr. Nigel Guest, Jonathan Sackier and other people on our show. This key-rrific episode of Join The Docs features the ever-curious Professor Jonathan Sackier flying solo, sitting down without his usual sidekick Doctor Nigel Guest who is taking his own medicine and having a holiday.. This time, he's joined by the legendary John Oates. Yes, you heard it right—John Oates, the moustached marvel from the iconic duo Hall & Oates. Not only is he in the Rock and Roll Hall of Fame, but he's also a multi-talented musician, songwriter, and all-around cool guy. They sit down to talk all things music and medicine, with a hint of laughter. What sparks the conversation? Oates, dives straight into the nitty-gritty of his sixth solo album,  “Reunion." This isn't just any album. Inspired by a whirlwind of personal life events, "Reunion" is a rollercoaster of emotions, melodies, and collaborations with some of the music industry's finest. Oates regales us with tales of  jam sessions, spontaneous songwriting when the radar is alerted by life events - check out the story of a girl in cowboy boots and a pink tutu!At the heart of the episode, the emotional story behind the album's title track. Oates opens up about his ageing father, painting a vivid picture of a man who has seen it all and then some. The song is a poignant reflection on life's horizons, filled with realisations that hit harder than a double espresso on a Monday morning. Oates' storytelling is so vivid, you can almost see the twinkle in his father's eye and feel the weight of the years gone by.What's Oates' secret to maintaining his vocal health? John shares his secret weapons, gargling, humidity, yoga and not talking to our resident Professor!. And let's not forget the physical challenges he's faced—two rotator cuff surgeries, folks! Oates recounts the gruelling physical therapy sessions with a humour that makes you almost wish you were there, cheering him on from the sidelines. Hip Hop, Hooray!Wait a minuet!  Why is the healing power of music so important to Oates? To rap it up, Oates passionately discusses how strumming a guitar or belting out a tune can mend a broken spirit faster than any doctor-prescribed remedy. Oates means treble, emphasising the importance of physical therapy post-surgery, sharing anecdotes of his therapist who, according to Oates, has the hands of an angel and the motivational skills of a drill sergeant. He also tells us of the people he has  worked with, a veritable who's who of the music business. And talking of Who, his recent duet with Roger Daltrey was to help those living with teen cancer.—--DISCLAIMER: The views and opinions expressed on Join the Docs are those of Dr. Nigel Guest, Jonathan Sackier and other people on our show. In the trees, through the water, and under the dirt: Snakes evolve faster than their lizard relatives, allowing them to occupy diverse niches. Also, researchers are working to understand just how baleen whales are able to produce their haunting songs.Snakes Are Evolutionary SuperstarsLove ‘em or hate ‘em, new research shows that snakes deserve our recognition as evolutionary superstars. The study, published last week in the journal Science, found that snakes evolve faster than other reptiles, allowing them to thrive in a wide range of environments. It shouldn't be too surprising: Many of the nearly 4,000 snake species occupy extremely specialized niches in their ecosystems. The blunt-headed tree snake, for example, eats through batches of treefrog eggs in Central and South America. Pythons, which can grow to 20 feet long, can take down large mammals like antelopes.Joining Ira to talk about the evolutionary speed of snakes is study co-author Dr. Daniel Rabosky, evolutionary biologist and curator of the Museum of Zoology at the University of Michigan.Whale Song Is All In The LarynxWhale songs can be both beautiful and haunting. But the exact mechanism that the 16 species of baleen whales, like humpback and minke whales, use to make those noises hasn't been well understood. The finer points of whale anatomy are hard to study, in part because the soft tissues of beached whales often begin to decompose before researchers can preserve and study them. And until the relatively recent advent of monitoring tags that can be attached to individual whales, it's been hard to associate a given underwater sound with any specific whale. For a recent study, published in the journal Nature, researchers took advantage of several well-preserved beached whales to investigate the mysteries of the baleen whale larynx and its role in whale song. Dr. Coen Elemans of the University of Southern Denmark joins Ira to discuss the work, which included a MacGyveresque contraption involving party balloons and exercise bands that blew air at controlled pressures through preserved whale larynx tissues. The researchers found that there are limits to both the frequencies these whales can produce, and the depths at which they are physically able to sing.Transcripts for each segment will be available after the show airs on sciencefriday.com. Ironically it is very prevalent in the phrase ‘tip of the tongue' (and not heard as much in those who speak with a glottal stop in words such as ‘butter')Lingua-palatal – a really rare sound when you say ‘r' noises (such as in “really rare”) when the tip of the tongue nearly touches the roof of the mouth.Lingua-velar – if you have done phonics with your child you will be familiar with the ‘kicking k' – and this is how you make that ‘kicking k' sound: with the back of the tongue up against the soft palate (the velum) at the back of the roof of the mouth, and the tip of the tongue on the mouth floorLip mic – a microphone designed to be used very close to the mouth to cut out extraneous noise, for example at a sports event Hosted on Acast. Treating laryngeal cancer early and precisely can preserve patients' ability to speak and swallow. Bharat Panuganti, M.D., discusses the innovations in transoral laser microsurgery (TLM) that have made it an alternative to radiotherapy for treating early-staged laryngeal cancer. He discusses his use of the potassium titanyl phosphate (KTP) laser in this procedure, which targets hemoglobin in the cancerous tissue and limits damage to non-cancerous tissue. Learn more about the current importance of surgeon experience in performing TLM successfully, as well as the research Dr. Panuganti is conducting to possibly use robotics to close that experience gap.

Hello, and welcome to Beauty and the Biz where we talk about the business and marketing side of plastic surgery, and how Burke Robinson, MD went from a 40-surgeon practice to solo. I'm your host, Catherine Maley, author of Your Aesthetic Practice – What your patients are saying, as well as consultant to plastic surgeons, to get them more patients and more profits. Now, today's episode is called "40-Surgeon Practice to Solo — with Burke Robinson, MD". I am fascinated by surgeons' stories that relay their jagged paths from fellowship to where they are today. Nobody's path was a smooth one and nothing went scheduled as planned.  That's life. It's full of surprises, twists and turns and the secret is to adapt to these challenges and grow (or give up and settle for less). This week's special guest Dr. Burke Robinson knows this well and decided to grow. He is a board-certified facial plastic & reconstructive surgeon with 30 years of experience in private practice in Alpharetta GA. Dr. Robinson had a tough childhood start but managed to become a surgeon in spite of it, and then he ended up in a 40-surgeon ENT practice for years.  Like others who had the calling for “more”, he finally spread his wings and went out on his own.  We talked about the challenges he faced, what it took for him to finally make the move to private practice and pearls learned along the way.  He also gives a really good tip for buying a laser ;-) Visit Dr Robinson's Website P.S. Today we talk to laryngologist Declan Costello who, during the Covid-19 pandemic, helped coordinate research on the perceived dangers of aerosols and singing. A singer himself, Mr Costello is stationed at Wexham Park Hospital in Berkshire as a consultant ear, nose and throat surgeon specialising in voice disorders. KEY TAKEAWAYS Pollen allergies have been quite rife this year and many have experienced symptoms for longer. The issue is that when you have an allergy you develop a reaction that causes the lining of the nose and the lining of the throat to swell. If somebody comes into the studio with an allergy and they sound a little bit nasal, by and large, it's okay to carry on singing – if the problem is isolated to the nose and irritation. The concern is if somebody's got an audible difference in their voice. If their voice is husky or if they've got difficulty with their range, then that makes you worry that maybe the vocal cords themselves are swollen. If you are keen to know precisely what you're allergic to, then there are various blood tests and other kinds of tests that you can do to know exactly what it is you should be avoiding. Most people who have hay fever are allergic to a whole number of things. So it's not just a single allergen. BEST MOMENTS ‘General hay fever and stuffiness wouldn't necessarily mean I would suggest stopping singing' ‘Colds are viral. They will do what they're gonna do' ‘We didn't go into it in order to open up singing again. We had to go into it completely dispassionately and saying, we don't know what the dangers are' EPISODE RESOURCES Guest Website: voicedoctor.co.uk Social Media: Twitter: @Voicedoctor_uk Relevant Links & Mentions: Neil Med Nasal Rinse: https://www.neilmed.com/unk/ Sterimar: https://www.sterimarnasal.co.uk/ Advised Nasal Spray: Dymista   ABOUT THE GUEST Declan Costello is a consultant ear, nose and throat surgeon specialising in voice disorders (a laryngologist).  He studied music at St John's College,Cambridge, where he was a choral scholar, and went on to study medicine at Imperial College. As a singer himself, he has a particular interest in treating voice disorders in performers. Having worked for ten years in Birmingham as a consultant, he moved to Wexham Park Hospital in Berkshire in 2018. He has published a number of books and has written many chapters, including the chapter on "Larynx" for the 42nd edition of "Gray's Anatomy”. In the middle of 2020, at the height of the coronavirus pandemic, he helped to coordinate research on the perceived dangers of singing and aerosols. This research led to the UK government opening up performing after a prolonged period of enforced silence.  ABOUT THE PODCAST BAST Training is here to help singers gain the knowledge, skills and understanding required to be a great singing teacher. Tara continues part two of delving into some basic definitions and terms that make up parts of singing. This week she focuses on the larynx itself—the voice box. She breaks down each part to give singers more confidence in exploring their own voice and the physical parts of their body that make up the voice.   My vocal course is NOW OPEN! Click here for access: https://tarab.vipmembervault.com/products/courses/view/12   Today we look at the Larynx—what is it? It is also called the Voice Box and sits in the anterior neck.   The epiglottis is the valve that closes over the trachea when swallowing is needed. It lifts when breathing and phonation is done.   (From TEACHMEANATOMY.info) The layrnx is made up of extrinsic and intrinsic muscles. What are the extrinsic muscles? The suprahyoid and the infrahyoid group sets make up these muscles and attach to the hyoid bone. Ligaments bind these to the larynx itself, causing it to move as one whole.  The suprahyoid raises the larynx and the infrahyoid muscles move it down.   What are the intrinsic muscles and what do they do? They control the shape of the opening (glottis) and the length and tension of the vocal folds themselves.   Cricothyroid—stretches and causes tension on the vocal ligaments—increases pitch   Thyroarytenoid—Relaxes the vocal ligaments—loosens and lowers pitch   Transverse and oblique cricoarytenoid—closes the arytenoid cartilages during                           exhalation—they act in a sliding motion. (The area at the middle of the vocal folds in the voice box looking down.)   Lateral cricoarytenoid—The adductors of the vocal folds—closes them—exhalation.           (Also how whispering is produced.)              Posterior cricoarytenoid—the abductors of the vocal folds—opens them—inhalation.   “It's more important if you simply remember the things that they (these intrinsic muscles) actually do.” Here is a link to see the video that shows these parts:   https://youtu.be/b89RSYCaUBo   Vocal folds themselves: They are attached at the front to the thyroid cartilage and at the back to the arytenoids. They contain the vestibular folds and the vocal folds.   Vestibular folds: (false vocal folds) They are the vestibular ligament covered by a mucous membrane and are pink. They are fixed folds. Vocal folds: They are white in color and are controlled by the muscles to open/close, relax/tense.   “The beautiful part of this is that all of these muscles and ligaments are working together to create all the pitches and the actual sound of what's being produced.” Another video to watch to help you see how these work together: https://youtu.be/f62dqc-L36o On all the parts of the voice box and vocal tract combining together: “When they do work together…it becomes effortless. It feels effortless.”   Please share this episode to your social media—either Instagram or Facebook. Guest: David G. Lott, M.D. Host: Amit K. Ghosh, M.D. (@AmitGhosh006) About 60,000 Americans have had their vocal cords removed due to disease or trauma necessitating a laryngectomy. Post-laryngectomy, patients are unable to speak, are at risk for aspiration, and many suffer from significant anguish resulting in depression. Fortunately, there are several treatment options available, the first being laryngeal transplant. While laryngeal transplantation allows patients to smell, taste, swallow, and communicate, it has drawbacks. Currently only a small number of larynx transplants are done in the U.S., and only a select few patients with laryngectomy qualify for larynx transplant. In this episode, Dr. David Lott, division chair of laryngology at Mayo Clinic's Arizona campus, discusses the role of regenerative medicine, which has shown promise in using 3D printing and a patient's stem cells in creating a larynx.
