Scottish composer and conductor James MacMillan intertwines the political and the spiritual in his work. He draws on Catholic and Scottish musical traditions, while addressing liberation theology. His composition ‘Tuireadh' (Lament) is dedicated to the 167 victims of the 1988 Piper Alpha disaster. Piper Alpha was an oil platform that exploded and collapsed under the effect of sustained gas jet fires in 1988. Mcmillan's lament for the dead premiered on June 25, 1991, at St Magnus Cathedral, Kirkwall, Scotland. Nazanin Noori and Andrea Belfi have reworked ‘Tuireadh', incorporating drums, rhythmic percussion and modular electronics, as a tribute to the cries of mourners of human cost. The piece was commissioned by The Lake Radio as part of our Works for Radio 2024 with support from Koda's Cultural Funds and was selected by an international jury consisting of representatives from Seydisfjordur Community Radio (IS) and Retreat Radio (SE).

In 2021, opioids were involved in nearly 75% of all drug overdose deaths The rise of synthetic opioids like fentanyl, which is much more potent than heroin or prescription opioids, has played a major role in the increase in overdose deaths What is Narcan AKA Naloxone? Competitive opioid antagonist. It sits on the receptor but doesn't activate it. When do we give Narcan? Respiratory rate less than 8-10 breaths per minute Should you check the pupils? An opioid overdose classically presents with pinpoint pupils BUT… Hypercapnia from bradypnea can normalize the pupils Taking other drugs at the same time like cocaine or meth can counteract the pupillary effects Basilar stroke could also cause small pupils, so don't anchor on an opioid overdose How does Narcan affect the body? Relatively safe even if the patient is not experiencing an opioid overdose. So when in doubt, give the Narcan. What if the patient is opioid naive and overdosing? Use a large dose given that this patient is unlikely to withdraw 0.4-2 mg every 3-5 minutes What if the patient is a chronic opioid user Use a smaller dose such as 0.04-0.4 mg to avoid precipitated withdrawal How fast does Narcan work? Given intravenously (IV), onset is 1-2 min Given intranasal (IN), onset is 3-4 min Given intramuscularly (IM), onset is ~6 min Duration of action is 60 mins, with a range of 20-90 minutes How does that compare to the duration of action of common opioids? Heroine lasts 60 min Fentanyl lasts 30-60 min, depending on route Carfentanyl lasts ~5 hrs Methadone lasts 12-24 hrs So we really need to be conscious about redosing How do you monitor someone treated with Narcan? Pay close attention to the end-tidal CO2 to ensure that are ventilating appropriately Be cautious with giving O2 as it might mask hypoventilation Watch the respiratory rate Give Narcan as needed Observe for at least 2-4 hours after the last Narcan dose Larger the dose, longer the observation period Who gets a drip? If they have gotten ~3 doses, time to start the drip Start at 2/3rds last effective wake-up dose Complications Flash pulm edema 0.2-3.6% complication rate Might be from the catecholamine surge from abrupt wake-up Might also be from large inspiratory effort against a partially closed glottis which creates too much negative pressure Treat with BIPAP if awake and intubation if not awake Should you give Narcan in cardiac arrest? Short answer no. _About the Lecture_ This event was recorded live on September 8, 2022 at The Institute of World Politics in Washington, D.C.. With one year going by after the Taliban's seizure of Afghanistan, the fundamental question remains in the minds of the people most affected: is the Taliban able to bring stability to the nation and form a legitimate government in the country? A short introduction covering The Bonn Agreement (Post-9/11 and the U.S.'s sacrifices and achievements in Afghanistan) and The Doha Agreement (Failures of the peace process, the withdrawal of American forces, collapse of the Afghan government, and violent return of Taliban) will begin the lecture. The first part of this lecture will focus on “a blind spot in the world and a voiceless nation,” addressing questions of what happened to the government, the democratic institutions, and values that led to the culminating point of the Taliban's takeover and their rule thus far? How has the Taliban behaved toward women; ethnic, religious, and political groups; and former members of government and ANDSF in the past year? Topics of unemployment, poverty, and the current security situation including the resurgence of ISIS and other terrorist groups' activity over the past year will be discussed. The second part of the lecture will focus on the reaction of the Resistance forces in opposition to the Taliban, which includes former government members, and defense and security forces in their ranks. The final part of the lecture will address the salient question of whether the Taliban has fulfilled its end of the Doha Agreement's commitments. These commitments can be categorized into three parts: the Taliban and its connection to international terrorist organizations; the establishment of a genuine Intra-Afghan peace process; and the Taliban's (as a would-be legitimate governing entity) relationship with the surrounding region and its actors. The lecture will conclude with suggestions and solutions for the failings of the Taliban and their governing system in Afghanistan. _About the Speaker_ *Mr. Mohibullah Noori* is a modern and moderate politician resisting on behalf of democracy and regional integration within Afghanistan. He is the leader of the Heart of Asia Nations Integration Movement and former Policy Director at the National Security Council of the Islamic Republic of Afghanistan. Mr. Noori has written several books on regional integration policy and has been an advocate for this idea since 2001. He is known in Afghanistan for his ideas on the Fajristan Civilizational Region, Heart of Asia Regional Order, and Government of Peace. Chef Hamidullah Noori is the owner of The Mantu restaurant in Richmond, Virginia. He began his chef's career at age 18. In 2003, he joined the Intercontinental Hotel in Kabul, Afghanistan and soon after, in 2004, joined the Kabul Serena Hotel, a five-star hotel in Afghanistan. After his training, he returned to Kabul Serena Hotel as the Executive Chef. He was featured in several cooking show episodes on TOLO TV in Afghanistan and was later given the Executive Chef position at the USAID.
