In this clinically rich episode, host Dr. Ted Lain sits down with board-certified dermatologist and global skin cancer expert Dr. Todd Schlesinger — AAD Board of Directors member, Mohs surgeon, clinical assistant professor at George Washington University School of Medicine, and medical director of the Clinical Research Center of the Carolinas — for a deep dive into systemic and targeted therapies for non-melanoma skin cancer (NMSC). The doctors begin with a thorough breakdown of the hedgehog signaling pathway (PATCHED, Smoothened, GLI-1 transcription) and how mutations in this pathway drive basal cell carcinoma (BCC) growth. They compare the two FDA-approved hedgehog pathway inhibitors (HHIs) — vismodegib (Erivedge) and sonidegib (Odomzo) — covering their mechanisms of action, volume of distribution differences (16–18L vs. ~9,000L), indications for locally advanced and metastatic BCC, how to define "locally advanced," and complete vs. partial response rates. Dosing strategies are addressed in detail, including alternate-day dosing and treatment breaks backed by the MIKIE study and STEVIE safety study. For managing the most common adverse events — muscle cramps, dysgeusia, weight loss, and fatigue — Dr. Schlesinger shares his clinical protocol using L-carnitine supplementation (1,500–2,000mg liquid, started 2–4 weeks before therapy) along with calcium and CoQ10. The conversation then moves to PD-1 and PD-L1 checkpoint inhibitors for locally advanced and metastatic cutaneous squamous cell carcinoma (cSCC) and BCC, covering cemiplimab (Libtayo), pembrolizumab (Keytruda), and nivolumab (Opdivo). The hosts explain the immune checkpoint mechanism using a memorable analogy, discuss how UV exposure upregulates PD-1 on tumor cells, and explore the practical realities of dermatologists prescribing infusion-based immunotherapy — including multidisciplinary care team logistics, buy-and-bill considerations, and when to partner with oncology. The episode closes with an exciting look at the pipeline: intralesional therapies for nodular and superficial BCC from companies including Verica, iViva, PHIO, and Feldan, red light PDT for superficial BCC nearing FDA approval, and the broader question of where these drugs fit as neoadjuvant, adjuvant, or primary therapies — and what complete response benchmarks (80–95%) dermatologists should expect before adopting non-surgical primary options. Steve Schlesinger is a Fellow at the Century Foundation in New York City. Mr. Schlesinger authored "Act of Creation: Founding of the United Nations," and he is an expert on the UN and international issues.  The League of Nations was the forerunner of the UN, which was created in 1945 where over 60 million died. The UN has been vital in preventing World War 3, providing humanitarian assistance to hundreds of millions of refugees, developing partnerships with groups such as Rotary International to eliminate the scourge of polio, and sending peacekeeping troops into dangerous areas to restore stability. Other basic UN services are to develop international agreements to safely move aircraft, ships, telecommunications, mail and weather information worldwide. The Trump administration has drastically cut US donations to the world body, which is damaging the US and the world. The US benefits more from the UN than probably any other country.

Why Should We Care About the Indo-Pacific?

Why Should We Care About the Indo-Pacific?

Japan's February 8th snap election delivered a historic result: Prime Minister Sanae Takaichi's ruling Liberal Democratic Party (LDP) secured a rare two‑thirds supermajority in the powerful lower house, giving her the strongest mandate any postwar leader has enjoyed to date. That will sharply increase what she can do on economic policy, defense readiness, and how Japan responds to pressures from both the US and China.Jake Schlesinger - President and CEO of the US‑Japan Foundation and a longtime Japan-watcher - joins the show to explain why this election was a potential turning point in a political system often seen as stable to the point of stagnation. He argues that while Japan's politics have frequently been defined by weak and short-lived prime ministers and cautious consensus, this vote creates the conditions for unusually decisive leadership.Schlesinger describes an electorate focused less on the LDP's history of political ethics controversies and more on daily economic pressure combined with a sense that Takaichi herself represents a fresh face. The episode unpacks how Japanese voters can be skeptical about big fiscal promises while still rewarding the leader who seems most willing to speak directly to pocketbook issues like inflation, wages, and household strain. The conversation shifts to geopolitics, as Schlesinger highlights how China's recent threats and coercive signaling appeared to backfire, strengthening public support for a leader who stands up rather than backing down. He frames this as a meaningful change in Japan's public mood: a country once inclined to avoid antagonizing neighbors is increasingly prepared to accept friction if it's tied to national security - particularly around Taiwan, a Chinese invasion of which Takaichi has warned would pose an existential crisis for Japan. The episode also examines what this election mandate could mean for the US‑Japan alliance. While people-to-people ties remain strong, Schlesinger notes that Tokyo is navigating a complex era of “America-First” politics - simultaneously hugging the US closer while hedging its bets with other regional partners like Australia, India, the Philippines, and South Korea. Finally, the discussion touches on constitutional constraints on Japan's military and whether this supermajority could finally open the door to a formal revision.

In this “Teabag” episode of Tea with Dr. D, host James Q. Del Rosso, DO, tackles questions submitted by dermatology clinicians across the country, offering evidence-based insights across several challenging clinical topics. Dr Del Rosso begins with hidradenitis suppurativa (HS), highlighting the importance of recognizing comorbidities and lifestyle factors that contribute to disease onset and progression, including smoking and elevated body mass index. He advises a phased management approach: prioritizing symptom control and patient comfort before introducing discussions about weight loss or smoking cessation. Depression, diabetes, metabolic syndrome, and cardiovascular disease are also important comorbidities to address over time. On initiating biologic therapy for HS, he explains the 3 pathologic phases, inflammatory, destructive, and burnout, and emphasizes that biologics such as adalimumab, secukinumab, or bimekizumab should be started early, in the inflammatory phase, to prevent irreversible scarring and sinus tract formation. He also shares procedural pearls, including the use of high-dose intralesional triamcinolone, incision and drainage, and deroofing for focal or persistent lesions. The discussion then shifts to photodynamic therapy (PDT) for nonmelanoma skin cancers. Referencing a study by Schlesinger et al, Dr Del Rosso reviews the efficacy of red-light PDT with 10% aminolevulinic acid gel for superficial BCC, highlighting improved clearance rates and favorable cosmetic outcomes over vehicle. Finally, Dr Del Rosso reviews new insights into topical nonsteroidal options for lichen planus, including off-label use of roflumilast and ruxolitinib creams. Both agents, with mechanisms targeting PDE4 and JAK pathways, respectively, have demonstrated reductions in inflammation and pruritus in some reports. Tune in for a concise, high-yield discussion featuring Dr Del Rosso's practical guidance and clinical pearls for today's dermatology practice.

To say we’re excited by the prospect of new music from seminal ‘90s/2000s dreampop band Ivy would be an understatement. Traces Of You, the erstwhile project’s first new release in twelve years has been built from the ground up using demos and fragments which date from 1995 to 2012. After losing founding member Andy Schlesinger to COVID in 2020, surviving bandmates Dominique Durand and Andy Chase thought that Ivy was done forever. But as the pair prepared for back catalog reissues, they discovered a treasure trove of reel-to-reel tapes and old hard drives with unfinished songs. They handpicked the best and carved out an album’s worth of goodies for this new collection, complete with contributions from Schlesinger. “Heartbreak” is a 2010 demo that showcases their twinkling magic.
