Postpartum is the time that you need to rest, replenish, and recover!     Welcome to episode 38, where I am STILL baffled about the knowledge gap, the misinformation, lack of information, contradictions, the high morbidity and mortality rates for pregnant and postpartum people, and all other failures in our healthcare system.  We have to do better.Today, I talk with Tanya Tringali, another badass individual trying her best to provide real care and support to our postpartum and pregnant women. Tanya is the founder of Mother Wit Maternity Services. She is a Certified Nurse Midwife with 20 years of experience in Maternal-Infant health. She is also a NASM Certified Personal Trainer, Crossfit Level 1 and Pregnancy & Postpartum Athleticism Coach. Tanya grew frustrated with the medical system and decided to do better for her clients. She stepped out of the system and created a holistic care program that closes the postpartum gap (let's be real, "void" might be a better word) by providing education, advocacy, and a comprehensive program of postpartum care. We talk about what to do during the first 2 weeks postpartum, what the 6 week exam is (and is not), what doctors know (and do not know), what informed consent really means, and how to wade through the sea of misinformation that's out there to find the answers and the care team that is right for YOU.We also mention the IRTH app - Birth without the Bias, it's like Yelp for maternity care.     Rose Schieck did all the right things to prepare for her pregnancy, but what she didn't prepare for was motherhood and postpartum. Sleep deprivation, anxiety, and depression came full force once she jumped back into work 2 months after becoming a new mom.After a year of self-doubt, guilt, and way too little sleep, Rose decided she needed to do something for herself.     The first 3 months after childbirth are a tricky time (understatement of the year, I know). You want to feel fit and strong again, but are also immensely tired. Your body is sore, squishy, stinky, sweaty... you feel anything but sexy. You hear conflicting advice - should I rest? Exercise more? What's the best, fastest, safest way to feel at home in my body again? Who can I trust to guide me?     It's December, which means we're heading into the season of colder temps and less daylight, and it's a time I feel called to hibernate and store up some energy for these long winter months. I've already put up lights, candles, and a tree to create a cozy den for myself! It's something I've done for years, as an antidote to the bustling Holiday Season, where everyone is busy, over extended, and staying up late. That frantic energy during the darkest month of the year just doesn't sit well with me - it doesn't feel good to be out of sync with the seasons.  Ancient medicines like Ayurveda and Traditional Chinese Medicine describe Winter as a season of cool, wet, slow, feminine and quiet energies.     No More Trauma: An Anesthesiologist's Perspective on Creating a Positive Birth Experience with Dr. Tracey Vogel

    It's time the standards change for all birthing experiences.There's a whole lot of trauma in a person's life that never gets the attention it deserves and is too easily brushed under the rug. How did we let this become the new norm?When it comes to birth, let's ask ourselves; is basic survival enough?NO! The patient's needs are not being heard. The providers don't have the time to really listen, nor are they taught how to empathize in medical school. The side effects of birth trauma are more significant issues than we make them out to be. Something CAN be done if we stop being quiet and nice and start to give give real, honest feedback. No more: "Everything is fine." - "I'm okay." - "The baby is healthy, and that's all that matters."Instead, we need to empower women to talk about their feelings and advocate for  themselves. That's why I was so happy to invite Dr. Tracey Vogel, a practicing OB anesthesiologist and a certified Sexual Assault Counselor, on to talk about what can be changed in the labor and delivery units so that birthing people feel seen, heard, and supported.Tracey truly is the definition of the Postpartum Revolution. Dr. Vogel is fighting to bring attention to this trauma, and she is a strong advocate for consistent feedback to the medical system. Dr. Vogel gives women a voice to make a change. Tracey learned many of her skills for trauma informed care while becoming a rape counselor. She's combined her experience and knowledge as an anesthesiologist with obstetrics and has some unique ideas for the healthcare world. In today's episode, we talk about the importance of informed consent and how the patient's needs should be discussed ahead of time. What-if scenarios should be a real discussion with actual back-up plans. "Part of fear is lack of understanding and lack of knowledge."There is no black and white answer to the issues.     Female hormones are finally getting their time in the sun - and everyone wants to talk about "how to balance our hormones"... but is anyone asking WHY our hormones are out of balance? During early postpartum, a period of hormonal imbalance is just a part of the deal - childbirth changes our bodies and our hormones are no exception. Our systems are designed to find balance again, but the conditions must be right... this is why ancient wisdom teaches that rest and  restoration during the 4th trimester is so vital. But here in modern day America, motherhood is anything but rest... we are overworked, underfed, and overtraining all in the name of "health". Enter Erin Holt. Erin is a functional medicine nutritionist who takes the term "nutrition" to a whole new level... it's not just what we eat, it also includes how we are (or aren't) nourishing ourselves holistically, and her  "unique" approach to hormone health addresses the whole picture.We talk about how...- Hormones are not the chief problem - hormones are RESPONDING to the chief problem- No one has cured anybody by correcting a lab value: lived experience first, labs second. - Thyroid meds without a life that supports your thyroid doesn't cut it. -  Our culture  has no idea how to handle postpartum depression and grief (the grief associated with loss of self), so we easily fill the hole with trying to "fix" - dieting, etc.- We've been taught that our bodies are something to fear, to not trust, and we've divorced ourselves from our bodies. So, lab values and supplements aside, how do we heal hormones?     Postpartum has it all - depression, anxiety, rage, joy, fear, love, frustration, happiness, grief... yep it's the full spectrum of emotions, sometimes all in one day. We hear about it sometimes, whispers of mental health struggles, but like the rest of the mental health world, it's not talked about openly. So how the heck do we deal with that? How do we know if we're okay? How do we ask for help?That's what I talked about with maternal mental health counselor, and new mom herself,  Mary Sanker. How can we navigate this postpartum period and still feel like ourselves? How do we find support when the world we live in offers no village?     As a pelvic PT, I get asked the same questions a lot, so I thought I'd record an episode talking about the most common things postpartum people ask me. So, here's what I want you to know about your pelvic floor:- It needs time and rest to heal during the first 6 weeks postpartum- A happy pelvic floor lives in a calm and rested body, so why are we so resistant to rest?- Leaking pee is not normal (and while I'm at it, a word about the word "normal)- Painful sex is not normal- Constipation is not normal- Prolapse happens, and it doesn't have to be scaryWe don't know our pelvis very well, due to lack of information and proper education... and unfortunately we humans tend to fear what we don't know. The purpose of my career, and this podcast, is to educate people. Because education is the antidote to fear and anxiety. Sure, we can be in a crappy place, but if we understand why and how to get ourselves out of it, we're ok. I want you to feel confident and empowered to take your postpartum healing into your own hands, ask for what you need, and find the team to support you, so you can ride off into the sunset like the badass mother that you are.     Wait, I thought this wasn't a parenting podcast?? (It's not.) Think back to the sex education you received as a kid... chances are, it wasn't all that informative or empowering. And we all muddled through our lives, probably ashamed of our bodies in some way, scared of sex (but also intrigued), figuring out our boundaries as we went. Hopefully we made it to adulthood unscathed (sadly, this is unlikely - 1 out of every 6 American women has been the victim of an attempted or completed rape in her lifetime, according to RAINN). Ok, now here we are, entering our own parenting journey... will we let the cycle continue? The shame, inadequate knowledge, fear... I guess we made it through ok, so let's just hope for the best? I wouldn't judge you if you did... how can we teach what we don't know, or model behaviors we've never seen, all while possibly facing some major demons from our own past? It's overwhelming. But I have great news...Caiti Courtier saw these struggles and created a solution. She saw how the kids in her Health classes were asking the same basic questions about sex, about their bodies, about love, about relationships... how they were hungry for knowledge and just begging for someone to give them the honest truth, no frills, no judgement. And she did the best she could in the classroom, but public education systems are so diverse, it's not possible to "get it right" when talking about anything other than human anatomy and function.     What is vaginal steaming - aka pelvic steaming - aka yoni steaming? Well, if you have a pelvic floor, steaming is for you! This ancient practice is gaining popularity in the US, and is something that can support women throughout the lifespan - first menstruation, fertility, labor prep, postpartum recovery, menopause and post-menopause. I talk with Adrienne about how steaming promotes a healthy menstrual cycle, aids with labor preparation (which MUST be done under the guidance of a practitioner!), and can help with postpartum pelvic floor healing. We talk through the 4th Trimester Steam Study, a foundational study that showed promising results when it comes to many markers of postpartum healing. We also talk about the importance of self-care, rest, and how steaming promotes those things with a non-invasive, healing practice.     Today I talk with Dr. Allyson Shrikhande about how to holistically to heal your pelvic floor during postpartum using a multidisciplinary approach. We talk about some of the "common but not normal" symptoms that postpartum people experience, including bladder urgency, frequency, and incontinence, painful intercourse, painful bowel movements, constipation, hemorrhoids, fissures, and UTI symptoms that won't go away. Dr. Allyson also talks about the importance of building a team of healthcare providers postpartum, and why it's best get in "early and often" for pelvic floor care (it's easier to fix when it's not a chronic problem!).
