Welcome to the fifth episode of season two of Conversations in Fetal Medicine, where we talk to Dr David Coggin-Carr. David is a UK+US dual-certified obstetrician, subspecialist in Maternal and Fetal Medicine, and early career clinical academic with additional training and expertise in Integrative Medicine and medical acupuncture. He currently practices full-spectrum MFM in both Vermont and upstate New York and additionally serves as Associate Medical Director of the Birthing Center and Associate Director of Quality for Obstetrics at the University of Vermont Medical Center. His clinical interests include planned vaginal breech birth, assisted vaginal birth, fetal growth restriction and electronic fetal monitoring. His research interests are focused on the evidence-based integration of acupuncture and related techniques into conventional maternity care, as well as the exploration of novel applications of acupuncture in animal models of high-risk pregnancy. His lab is currently examining the efficacy, safety and mechanisms of action of electroacupuncture in a rat model of maternal obesity characterized by insulin resistance and uteroplacental insufficiency. He has also served as Editor-in-Chief of the scientific journal Acupuncture in Medicine (published by SAGE and owned by the British Medical Acupuncture Society) since 2015.After graduating from UCL medical school in 2004, he spent 12 years in UK postgraduate training during which time he was awarded an MSc in Western Medical Acupuncture by the University of Hertfordshire and a PhD in Fetal Medicine from UCL under the supervision of Anna David and Jacqueline Wallace at the University of Aberdeen (to which he relocated for 18 months). His thesis was entitled “Evaluation ofPrenatal Adenoviral Vascular Endothelial Growth Factor Gene Therapy in the Growth-Restricted Sheep Fetus and Neonate”. He began subspecialty training at St George's, University of London, completed the RCOG ATMs in Maternal Medicine and Advanced Labour Ward Practice and ultimately gained his CCT in general O&G, but subsequently made the decision to emigrate to the USA and retrain in O&G and MFM.He spent his first year as an intern at NYU (including a stint at Bellevue, the USA's oldest public hospital) and then moved on to the University of Vermont for a further three years' of Ob/Gyn residency followed by three years' of MFM fellowship. While finally working in a substantive post (as an attending physician and Assistant Professor), he is currently rounding out 20 years' of postgraduate training by completing a 1-year faculty fellowship in Integrative Medicine through the Osher Collaborative for Integrative Health.We have not included any patient identifiable information, and this podcast is intended for professional education rather than patient information (although welcome anyone interested in the field to listen). On Friday, June 20, 2023, Hudson Mohawk Magazine Roaming Labor Correspondent Willie Terry attended a rally at the YWCA to stop the closure of the Burdett Birthing Center at Samaritan Hospital ward in Troy. The struggle continues to save the Birthing Center. In this labor segment, Willie revisited the rally where speakers discussed the importance of the Birthing Center to the Troy community.

We will be hearing the in's and out's of how their births felt for them, what they chose and why they chose that, and experience it alongside them as they relive it for themselves! Taylor shares how positive of an experience she had at the hospital, and yet why she also chose not to return for her second birth. She chats through how calm and attentive her midwife was, as well as the dichotomy of needing to trust her body to tell her what to do during labor. She talks through the differences in her births, and why she believes the one in the birthing center actually went more smoothly. The New York state fair is in full swing and Syracuse. Fairgoers are walking the barn, seeing prize livestock and blue ribbon vegetables on display. This includes seeing the birth of a calf at the dairy cow birthing center, but that experience is meant to be much more than witnessing an "udder miracle", as it's called.We sat down in the middle of the birthing center tent with Eileen Jensen, the executive director of the New York animal agriculture coalition that organizes the birthing center.

The first story of her son's birth was drastically different from her daughter's birth. Her son was born at a birth center after being in labor for over 26 hours (and pushing for two hours). Her daughter was born in the backseat of her car, after 15 hours of mild contractions and 5 hours of more intense contractions. Last time you heard from your host, Angela Dulaney, I shared with you my experience with my OB up to that point and why I switched to a birthing center. Well, things progressed rather quickly after that. After only 2 appointments with the Birthing Center midwives, I was diagnosed with Gestational Hypertension and referred out to a High Risk OB, which would later lead to a Severe Preeclampsia diagnosis, but not before spending 2 separate stays in the hospital. In this episode, I go into detail about our birth story and the events leading up to the birth of our beautiful babygirl.

In today's episode we're talking to mama Emily Lucchino about giving birth at a birth center, being a natural mama, and how chiropractic practices factored into her prenatal care. Meet Emily, chiropractor, doula-in-training, mama to Ava Grace, wife to Joe, and lover of all things natural. Emily grew up on a farm and familiar with what it’s like to live off the land. She shares that when they weren’t feeling well in her home, they used hand-picked herbs, flowers and other natural remedies for healing. It makes sense that Emily was drawn to school for chiropractic care, being in alignment (pun intended!) with natural healthcare and living. Her husband Joe had a more modern upbringing. So when it came time to decide what to do when they became pregnant, it was a big test of their marriage. To set the scene, Emily and Joe got pregnant just after getting married, at the same time their careers were taking off. So here they are with multiple new life experiences and figuring out how to navigate it all. They asked themselves, “are we ready for this?!” Emily originally wanted a home birth. After some discussion and compromising with Joe, they decided on a birthing center. Emily takes us through the process of this decision, along with some of the deep self-work she did while pregnant. She’s excited to share her story because it’s an opportunity to re-write the script of what you typically hear around birth.
