My guest today is Sarah Holiday, a long time Mother Wit mama. Sarah (and her husband!) did a lot of hard work in our time together as they worked through mental health challenges, some of which stemmed from childhood experiences, others were related to more current circumstances related to a stressful move, the pandemic and pregnancy itself. No matter what was going on for her, she always knew what she wanted and needed and was always ready and willing to do the hard and introspective work that one must do to overcome or find their way to the other side. She has always kept in touch and she reached out when she was pregnant with her second baby because she was determined to get the in person midwifery care that she deserved that first time around. She is here today to share her two experiences. I feel confident that consumers and providers alike will learn something from her story. In vitro fertilization: From feeling like a number to being seen and heard, with Star Le

My guest today is Star Lee. She had an easy time getting pregnant at 40. But after her first birth and 20 months of breastfeeding, when she didn't hit the "first ovulation jackpot" for the second time, she intuitively knew that time was of the essence if she and her partner were serious about having a second child. This is a story of being proactive by someone with a strong sense of self and a natural ability to advocate for herself and even still, it was a difficult experience that she is still sitting with and working through despite beautiful babies and joyful births. Resources- Vocabulary from this episode What are "Day 3 labs" - To be more precise, we can actually check these on cycle days 2-4. This is when we look at your estrogen levels- i.e., estradiol, as well as FSH and LH- these are some of the hormones that engage in a beautiful dance that when in perfect harmony, lead to ovulation. The early days of the menstrual cycle is when they are at their lowest and most stable so this is how we get a sense of what is going on in the first phase of the menstrual cycle- the follicular phase. Later in the cycle, we often check "day 21" labs which means we check progesterone levels to see if they rise appropriately. Day 21 is also a bit of a misnomer because it is only day 21 in a person with a 28 day cycle. Complicated huh? What is a DEXA scan? Its like an X-ray that is most commonly used to assess bone density, i.e., strength and bone loss, i.e., osteoporosis. It is also one of the best ways to understand your entire body composition (fat and muscle mass). See resources below or on my website. This episode is dedicated to the Shenoy family and to all families who have survived the loss of a child. My guest today is Divya Shenoy. She was pregnant with twins when she came to me for postpartum care. She started early because she knew the risk of preterm birth was increased for twin pregnancies. Shortly after we began working together, she learned that her cervix was shortening and that she required a cerclage- that is a stitch placed in the cervix to help it stay closed and keep the babies inside where they belong. On today's episode, I have one of the most honest stories any new parent has ever told on this show. If you have been listening for a while, you know that each season, I feature clients who are willing to share. I invite people with all sorts of varied experiences and perspectives. But I do try to shine a light on topics for which people need more information, topics that people need to learn more about and especially where I think a personal story is the best way to talk about it. So today, Patti, a client who stands out in my mind as one of the people who struggled the most with a history of depression and anxiety before, during and after her pregnancy, graciously shares her story. At times, I worried that maybe I wouldn't know how best to help her but she worked hard and was so willing to try almost anything I suggested. I found that to be such a unique quality because as you likely know, anxiety and depression can be debilitating but she was rather persistent in her pursuit of happiness. She is a gymnast, a crossfitter and does jujitsu competitively. She is a crossfit L2 coach and a Pregnancy & Postpartum Athleticism Coach (P&PA Coach). She is also a Certified Professional Midwife (CPM) who attends out of hospital births both at home and at a freestanding birthing center in Meridian Idaho at the New Beginnings Birthing Center. When she was on the show back in March she was pregnant with her first baby and suffering from hyperemesis. We talked about her experience and about why coaching and midwifery are so well aligned. Basically, we shared a soapbox for the better part of an hour. It is one of my most listened to episodes to date. So, I asked her to  join us again to share her birth story and to update us on her return to fitness. I am particularly interested to hear what might have felt unexpected, or if any given experience challenged her beliefs or changed her perspective. Because, while I don't believe we must have babies in order to be good at this work, it does provide a perspective that you cant get any other way.Enjoy the show.Resources and shout outsBlog Post that summarizes Rachel's tips on birth prep and return to fitness from this episodeExpecting Amy (Documentary about Amy Schumer's pregnancy/experience with hyperemesis)Dr. Jennifer Anacker, Anacker Clinic of Chiropractic, Meridian and Boise, IDWant to become a Pregnancy & Postpartum Athleticism Coach, find one, or purchase a training program to help you stay active during pregnancy and postpartum?Six exercises for the early postpartum period (Brianna Battles, owner/founder of Pregnancy & Postpartum Athleticism)Support the showThriving After Birth (an online course)Comprehensive Care60 Min Consultation: Use discount code- FirstConsult10%offInstagram: @mother.wit.maternityYou Tube Channel

I think the title says it all but you will have to listen to get all the details. The biggest take away is the connection that you can literally hear and feel between these two women. Forming these real connections is one of the gifts that midwives give to our clients but it takes two and the people who choose home birth are open to and actively giving back to their midwives too. Im not trying to change anyones mind about where the right place to give birth is. The right place is exactly where you want it to be, where you feel safest. But I do hope that this episode will help to answer questions that listeners may have about home birth and dispel some myths and at a minimum, help people understand why some choose home birth and help newly pregnant people better understand all of their options.GuestsDori Scallet is a home birth mama x2, partner to Joe, a therapist and social worker (a former performer like so many of my midwife and therapist friends). Anne is a mom to two boys- James and Charlie.  Before becoming a mom she was a Pastry Chef.  After experiencing serious burn out in the restaurant industry, she shifted to coaching Crossfit.  Then, while pregnant with her first son, she became certified as a Pregnancy and Postpartum Athleticism coach because she was not getting answers to her questions on exercise in pregnancy.  She had her first son in a hospital with an obstetrician, and quickly realized that the whole experience was not what she wanted.  Three years later, she had her second son at home under the care of a midwife.  Both experiences taught her so much about herself as well as the birth world.Anne is a friend from my OG Crossfit gym in Brooklyn, shout out to Crossfit 718! I wanted to have her on the show to share her birth stories, and, more importantly, to talk us through why she had a hospital birth with her first baby and a home birth with her second. This is so important for people to hear because Anne's story is not unique. Many have a first baby in the hospital for a variety of reasons but partly if not entirely, because they were raised believing that this is the safest way to do things and they simply don't question it as a norm. But for many young and healthy women, having a baby is often the first time they are a “patient” in the hospital, and some will walk away from this experience feeling unsupported, others will feel as if they were fighting an uphill battle, some will be traumatized, obstetric violence and coercion is real. Some people have received the inaccurate memo that out of hospital birth is unsafe. Many don't know what a midwife is or exactly what we do or where we do it.Join us as Anne shares her journey with us.ResourcesChristiane McColskey, City Midwifery (Anne's Midwife mentioned in this episode)Tanya Wills, Manhattan Birth (Anne took a class with this awesome midwife to prepare for a home birth)New Yorker Article on home birth - Martine Jean Baptiste, one of the two midwives featured in this article will be a guest next season.Birth It Up By Leisel Teen, Mommy Labor Nurse, check out her podcast, episode #74 to hear our conversationOnline Childbirth Education. My guest today is Cheri Van Hoover. She has been an ardent advocate for women, families, and the profession of midwifery for more than 4 decades. She became a midwife in 1987. A native of Washington State, she lived in California for 32 years, where she received her midwifery education and provided full-scope care for widely diverse populations in varied settings, culminating as assistant clinical professor with the UCSF Faculty OB/GYN Group. Since returning to rural Washington 15 years ago, her clinical practice has focused on outpatient reproductive health and she has served as distance faculty for the Midwifery Institute of Philadelphia University, now Thomas Jefferson University. This is how I know Cheri. She was first my professor for a health policy course and then we worked together during the years that I was faculty at PhilaU. Her scholarly publications cover diverse topics: health policy education, midwives in politics, midwives as palliative care clinicians, and body art. She is an active member of the American College of Nurse-Midwives and has won numerous professional awards. Cheri is the third of 4 birth professionals to share her birth story, which of course is also the birth of a mother and a midwife. ResourcesAn interview with Suzanne Arms and her website.Apgar score (ACOG)27:45 Cheri talks about dopplers. She is talking about most common tool that we use to listen to the heartbeat of a fetus. A handheld doppler allows you to hear and count the fetal heart rate. It is the same technology used with continuous fetal monitoring. Unfortunately, despite its widespread use, it has not been shown to significantly improve outcomes in low-risk pregnancies, it is associated with false positive results and therefore leads to an increased use of cesarean sections.Continuous fetal monitoring (Evidence Based Birth)Rh-negative blood type and Rhogam (ACNM)Story of Kate Bowland NOTE: The author defines lay midwives as, “Midwives who are not nurses.” Later, she defines it as, “midwives without institutional training.” Neither of these definitions are accurate today. They were at the time because the CM and CPM credential did not yet exist. Then I started to see some nuance, but I was still quite skeptical, especially as it related to supporting breastfeeding goals, but Kaela has really helped me to see the bigger picture. Of course, all sleep coaches are different and come with their own ideas of what is right or wrong and what works and doesn't work and there isn't a lot of hard science yet, but I can still get behind sound advice when framed the right way. It's not a far cry from things that I would call midwifery intuition, things that we have known in this profession for nearly all of time but have had to prove before being heard. ResourcesKaela's website: Sustainable Sleep and IG handle: @sustainable.sleepMommy Labor Nurse episode 74 (Liesel interviews me and talks about her mental health)Attachment theory vs attachment parenting (Article)Thriving After Birth: Mother Wit's comprehensive, self-paced, online postpartum courseBirth It Up By Leisel Teen, Mommy Labor Nurse, check out her podcast, episode #74 to hear our conversationOnline Childbirth Education. Robin and Kathy Herron, who you recently heard talking about painful sex in the postpartum period are the two most important midwives in my own professional journey. They took me under their wing and taught me everything they knew.Robin is here to tell her birth stories today. However, this is also the birth of a midwife. These stories are often interwoven. What a special treat it was for me to document her stories given the role she has played in my life and career.ResourcesHistorical people, places, and medical information mentioned in this episodeRabbit test Sister Angela Murdaugh Therese Dondero, a midwife who died at the age of 40. She made important contributions including starting the historic and beloved practice at North Central Bronx – A practice that remains beloved today. Please forgive the strange link to an important book called Mainstreaming Midwives. It was not easy to find something open source to share with you. Pelvis shapes and how they relate to birth experiences: Robin talks about her “platypelloid  pelvis.” In reality, most are of mixed shapes. Historically, we have talked about them as distinct. These distinctions have since been debunked. Furthermore, the research that led to the names of these 4 main pelvic shapes was racist and its usefulness is currently being questioned.ScopolamineKaro Syrup infant formulaTransient tachypnea of the newbornMaternity Center AssociationKitty ErnstNipple Stimulation for augmentationLaboyer, Gentle BirthBirth It Up By Leisel Teen, Mommy Labor Nurse, check out her podcast, episode #74 to hear our conversationOnline Childbirth Education. So, we midwives, and others, of course, squeeze this in wherever we can, and unfortunately, it can be almost imperceptible to our clients and society at large. Guest: Chantel Zapata-PrayResourcesOfficial position statement on preconception care from the American College of Nurse-MidwivesFibroidsDuchenne Muscular DystrophyTanya's favorite Continuous Glucose Monitor (CGM) By Nutrisense Use code MOTHERWIT for $25 off your purchase.Birth It Up By Leisel Teen, Mommy Labor Nurse, check out her podcast, episode #74 to hear our conversationOnline Childbirth Education. Individualization! One of my favorite things to do is educate both my clients and healthcare providers about fitness in the perinatal period. There is so much that needs to change so much about the way we approach this discussion in the clinical setting. Sometimes it doesn't happen at all and often times it is brief, inaccurate, and instills fear in the people who are trying to get moving or who want to stay moving. My guest today is Rachel Mast.
