He is a plank owner, meaning he was an original crew member stationed aboard the USS Ronald Reagan (CVN-76). He was preparing for an upcoming training deployment on the USS Theodore Roosevelt (CVN-71) when the September 11th attacks took place. That training deployment turned into a combat deployment, and Nate served on the first carrier battle group deployed after the September 11th attacks. This deployment set a post WWII Record (and pre-pandemic record) for most consecutive days at sea, without a port call at 159 days.After returning to the USS Ronald Reagan, Nate was again asked to deploy aboard the USS Theodore Roosevelt, also known as America's Big Stick. The USS Theodore Roosevelt was the first carrier battle group surged, ahead of her deployment schedule, for the beginning of Operation Iraqi Freedom. The Theodore Roosevelt was a launching point for air assaults at the beginning of the war in Iraq.Nate was transferred to shore duty in 2004 and served at Naval Air Station Lemoore California until his honorable discharge in 2007. After leaving the Navy, Nate spent years working in the energy industry in multiple capacities and in law enforcement, while also attending school. We're honored to have on this week's episode of "Straight Outta Health IT" the dynamic and unconventional Dr. Kameron Matthews, MD. This amazing clinician has dedicated her life's mission to the equitable delivery of healthcare and the adoption of health policies impacting underserved communities. Dr. Kameron Leigh Matthews is the Chief Health Officer of Cityblock Health, a transformative, value-based healthcare provider integrating medical, behavioral, and social services for Medicaid and dually eligible and low-income Medicare beneficiaries. A board-certified Family Physician, Dr. Matthews has focused her career on underserved and vulnerable communities, having held multiple leadership roles in correctional medicine, federally qualified health centers, and managed care. Most recently at the Veterans Health Administration, she led transformational efforts focused on integrated, Veteran-centered models of care including the implementation of the MISSION Act of 2018 and the EHR modernization effort. She is a member of the National Academy of Medicine. In addition to other non-profit and advisory board positions, she serves as the Vice-Chair of the board of directors of the National Quality Forum. As a passion outside of work, she co-founded and co-direct the Tour for Diversity in Medicine, an initiative seeking to bring premedical enrichment activities to underrepresented minority high school and undergraduate students across the country. In this episode, she shares her journey into healthcare advocacy after taking a detour from med school to earn a law degree which sparked her passion for helping underserved communities. Social determinants of health like zip code, education, and income really do affect access to care. Dr. Kameron reflects on how she started working at Federally Qualified Health Centers and for her, being a physician of color absolutely made the difference. Diversity, equity, and inclusion are not just buzzwords according to Dr. Mathews. A diverse healthcare workforce can directly impact the quality of care and the equitable treatment of patients of color. Her passion now is leading CityBlock Health. Friend of the show Bruce Carruthers returns to the show to update us on the status of VA Health Care and the good news that the AIR Commission has been derailed. The Asset and Infrastructure Review (AIR) commission was part of the Mission Act and was scheduled to review the recommendations coming from the non-veteran Secretary of Veterans Affairs to close or restructure more than 30 VA facilities. Thankfully a bi-partisan group of Senators derailed the commission by not seating commissioners. So good news but the fight continues to halt the push to privatize the VA.
A dozen senators reject the Veterans Affairs Department's plans to reshape its health care real-estate. The senators say they won't proceed with the President's nominees to serve on the Asset and Infrastructure Review Commission. The commission, part of the 2018 MISSION Act, is supposed to review the VA's recommendations on how it expects to right-size its real-estate portfolio of medical facilities across the country. But VA officials say their plan to close old facilities and build new ones won't stop here. For an update, Federal News Network's Jory Heckman joins the Federal Drive with more.

Building up the addiction medicine workforce and integrating key aspects of addiction medicine into medical education is important. Anthony P. Albanese, MD Dr. Albanese is a gastroenterologist, hepatologist and addiction medicine specialist who has over 25 years of experience in clinical medicine and medical education. He works with VA medical centers and their university affiliates to enhance interprofessional faculty education and develop, expand, and enhance Graduate Medical Education (GME) in accordance with the Veterans Access, Choice, and Accountability (VACAA) and MISSION Act initiatives. Dr. Albanese is a Clinical Professor of Medicine at the UC Davis School of Medicine and is Chief of the Hepatology Division for the VA Northern California Healthcare System based in Sacramento. He is President of the California Society of Addiction Medicine and Chairman of the State Chapters Council for the American Society of Addiction. Albanese attended Oral Roberts University for undergraduate and medical school training. He trained in internal medicine in New York at St Luke's Roosevelt Hospital and continued for gastrointestinal and addiction medicine fellowship. Biden has included $18 Billion in his infrastructure package for upgrading the VA, However the AIR Commission (Asset and Infrastructure), part of the Mission Act is being used by critics to delay that funding and enhance the chances of VA medical center closures and the increase in VA privatization. VA has a variety of resources for you as a VA patient to help you in urgent care or emergency situations. You can call and talk with a nurse or make arrangements to go to an urgent care center that is privately operated but in the VA network. Listen in to get the phone number to call to talk with a VA nurse who may be able to help you without your going anywhere or assist you in knowing what to do so as not to run up a large bill for services that if handled properly will be paid by the VA. The whole premise of the mission act, to provide veterans with health care services outside of the VA system, won't work if providers don't get paid. But reports of that very phenomenon keep coming in. That's prompted demands for action by the Veterans Health Administration from members of Congress. Among them, the chairman of the House Veterans Affairs Committee's oversight and investigations subcommittee, New Hampshire's Chris Pappas who joined the Federal Drive with Tom Temin to lay out his concerns.

Dr. Les Waite of Cleveland VA Medical Center is a national expert on gambling addiction of active duty and veteran population. He discusses the problem, its co-morbidity with other issues (depression, PTSD, TBI, etc) and its treatment options. It is estimated that 56,000 active-duty troops have a gambling addiction. Veterans tend to be 2 to 4 times more likely than the general population to be addicted to gambling. Vietnam veterans are as high as 10 times greater than the general public. Few people get treated and resources are limited at VA. However, The Mission Act may give greater treatment options in the community. First through the Choice Act and then the MISSION Act, Congress and the Department of Veterans Affairs have been trying to expand veterans' access to private sector medical providers since 2014. As of June, those choices now extend to urgent care clinics. VA now has 5,000 of those walk-in clinics under contract to treat veterans with relatively minor illnesses and injuries, and said it's adding more every day. Dr. Kameron Matthews is the deputy under secretary for Health for Community Care. American Warfighter Radio Episode 5: "WeCare" with Shane Whitecloud and Boone Cutler Robert Kimmel, MD Dr. Kimmel is the Chief of Community Care Service, leading the implementation of the Mission Act. He was formerly the Chief of Ambulatory Care Service. He is a primary care physician and continues to have a panel of Veterans he cares for in primary care. He is a super nice guy who is able to translate complicated Mission Act law requirements into plain language for the providers and patients of the VA Sierra Nevada Health Care System. He is steadfastly working on internal processes with all of the changes to ensure the transition is smooth for our fellow Veterans. He is practical and very Veteran-focused. Even though he transitioned into management some years ago, he has continued to keep his schedule seeing patients as a provider. Joining us on the phone for news, Chris Buccleuch "Bullethead". John Milton Peterson will be calling in to discuss vetted media outlet listings of the top 50, a study of 15 or so social media outlets and ideas and published work on how to be safer and avoid targeting online. We also have an official representative of the US Coast Guard calling in to discuss the Texas police chief/retired Army paratrooper, Chris Reed, who went missing and eventually was found after being thrown overboard his fishing vessel when it was hit by the wake of a passing vessel in Galveston Bay. The Coast Guard searched more than 650 square miles over the course of 40+ hours. A new deadline at the Department of Veterans Affairs is right around the corner. VA will start next week to implement new criteria for veterans to access community health care. The new access standards are just one piece of the VA MISSION Act. VA said it's ready to meet the June 6 deadline. But other parts of the MISSION Act are falling behind. The Department of Veterans Affairs has less than two months now to stand up a new consolidated community care program under the MISSION Act by June 6. At least some lawmakers and veterans service organizations say VA isn't ready. And they're urging the department to delay the rollout of the new program. IT delays and staffing concerns are putting a wide variety of major projects at the Department of Veterans Affairs at risk. VA is months away from kicking off a new consolidated community care program under the MISSION Act in June. But senators say they're worried VA's decision tool that's supposed to help the department decide whether veterans are eligible to visit a private sector doctor won't be ready in time. An automated IT system that's supposed to process payments to newly eligible families who take of care of veterans may not be ready by its deadline either. Budget debates for 2019 just ended and congressional appropriators are on to 2020. Lawmakers looking at the Department of Veterans Affairs see an agency still reeling with change. VA Secretary Robert Wilkie said the agency is in the middle of one of the greatest transformations in the department's history. As part of the VA Mission Act, Congress told the Veterans Affairs Department to consolidate its various private-sector care programs into one. But a new report by the Government Accountability Office finds VA hasn't done a great job interacting with private sector providers up to this point. Senate Majority Leader Mitch McConnell canceled much of the August recess so that members could finish appropriations bills on time and vote on pending presidential nominees. But senators have a few more items on the to-do list. They are working through a long list of amendments to the defense authorization act. Some of those would affect federal employees if enacted. There's also a few outstanding items on the VA MISSION Act. The president signed it into law last week.
