Many people regard dementia as a fate worse than death, in large part because it strikes at the essence of our humanity — our memories, identity, and relationships with others. Unlike diseases that primarily afflict the body, dementia erodes the mind, leading to a gradual fragmentation and loss of self and autonomy. The burden of this disease on caregivers also cannot be understated. Not only does dementia require comprehensive, long term care that addresses the afflicted individual's cognitive, behavioral, and physical issues; witnessing a loved one's slow and irreversible decline often exerts an immense emotional toll on the caregiver. Additionally, the pervasive stigma and isolation associated with dementia can leave caregivers feeling unsupported and alone. Our guest on this episode is Dasha Kiper, a clinical psychologist who works with caregivers to people with dementia. She's the author of Travelers to Unimaginable Lands: Stories of Dementia, The Caregiver, and the Human Brain (2023). The book explores the complex relationship between caregivers and dementia patients, which are frequently rife with heartbreak, guilt, frustration, helplessness and shame. 163. A Love Story Rewritten by Alzheimer's with Michael James

Author Michael James and his wife Judy's life together took an unexpected turn when she was denied long term care insurance because she couldn't pass the ‘mini mental', the test given that revealed cognition issues.  Ultimately, she was diagnosed with Mild Cognitive Impairment that developed into Alzheimer's. Michael, faced with this challenge, knew he needed to be thoughtful, calculated, and proactive while continuing to provide Judy with the daily care she needed. As an academic and researcher, he joined several online support groups and searched for books to help him navigate this new territory. Therapy: He observed in the support groups that many caregivers' downfall is mixing their emotions with responsibilities. Realizing he needed more than anecdotal help from friends, he found a therapist who helped him separate the reality of the present moment and need to seek answers to the practical day to day challenges, while also preparing for the future. Daily Care & Move to Senior Living: Michael talks about his strategy to integrate additional support and resources into their lives. They began with in home care (which she initially rejected) by starting with a couple of hours a week and by the time they reached 30 hours per week it was time for something different. The next phase was a day program that Judy enjoyed. When that grew to 7 days a week and a few other key factors at play, he knew it was time for her to move into the memory care community. Ultimately, multiple factors influenced this decision. Judy had become fearful of being alone and hallucinations had begun. Michael was struggling to get a full night's sleep and her incontinence issues were increasing. These were layered with safety concerns because of several incidents that had occurred, and he knew it was time for the move to memory care. Plan Ahead: Michael emphasizes the importance of getting out of denial and talking about getting older. He recommends to adult children not to brush their parents off when they want to talk about planning for the future and for when they die. We all need to be more comfortable talking about aging and death. He recommends not waiting until you are in crisis! Planning ahead will help you be successful because many memory care communities have waiting lists. A new drug, Donanemab, has been hailed as a turning point in the fight against Alzheimer's after a global trial confirms it slows cognitive decline. One trial was shown to have “significantly slowed” the progression of the disease—by 35%. Earlier this year, Lecanemab, the first drug to slow the destruction of the brain in Alzheimer's, received regulatory approval in America. Lecanemab was shown to slow the rate of cognitive decline by 27% in an 18 month study involving participants in the early stages of Alzheimer's. Although not a cure, charities say the results in the journal JAMA mark a new era where Alzheimer's can be treated. The drug works in Alzheimer's disease, not in other types of dementia, such as vascular dementia. But the new drugs are not risk-free treatments. Brain swelling was a common side-effect in up to a third of patients in the Donanemab trial. The World Health Organisation forecasts more than 150m people around the world will be living with dementia by 2050. Until recently, we've been told that there are currently no approaches that have been proven to prevent Alzheimer's disease and related dementias. But are we beginning to see a future where we can make dementia a chronic condition, one you live with and die with but don't die from? Are we inching closer towards a treatment for dementia? Can we ultimately prevent or cure the disease? In the battle against dementia, is the end in sight?
