August 16, 2024Mark, Scott, and Ray talk about G2211 and 99459 updates and two CPT description questions that came into the PRS Communities:I have received a denial for px 50545 stating the adrenal gland was not removed, therefore,  50545 is not supported.  PX description:  Laparoscopy, surgical; radical nephrectomy (includes removal of Gerota's fascia and surrounding fatty tissue, removal of regional lymph nodes, and adrenalectomy). I recall reading a PRS article regarding  what constitutes a a radical nephrectomy (of course, currently I can not find my reference). But if I recall correctly, the main criteria for radical is the removal of the Gerota's facia.  The nodes, adrenal and fatty tissue were not the determining factor, and did not have to be removed to classify as radical  50545.   Can y'all please help direct me to the correct understanding? Does the adrenal gland have to be removed in a radical nephrectomy? Thanks for any input y'all can provide.   The CPT book does not define limited vs complete for penile duplex scans the way it does for other vascular studies. How do we choose between 93980 and 93981, and what do we look for in documentation to determine whether it is complete or limited If it is not a follow up? I have never had a provider perform this procedure before and want to make sure i am understanding the codes correctly. You heard it here first! Listen up for a well designed prospective cohort study analyzing what happens when smaller trochars are used during prolapse repair in women who are part of a surgical registry. Dr. Allison Edwards, a urogynecology fellow from the University of Calgary shares her interesting findings comparing trochar size, finding decreased opioid use after surgery for the smaller caliber trochar.  

In this first episode of season 2, Simone tells her own tale of infertility with the help of her husband Stuart. The journey began after 12 months of trying to conceive naturally and Stuart deciding to get his sperm tested. They found out Stuart has low sperm count and for them to conceive naturally would have a 1 in a million chance and the best option was IVF with ICSI. After 1 stimulated cycle and 3 failed transfers, Simone had a Laparoscopy. The surgery found mild endometriosis, adhesions and high natural killer cells. They did another stimulated cycle and a had a few more failed transfers (on an immune protocol) before discovering the DQ alpha gene partial match. They completed LMIT in Melbourne and went through another stimulated cycle (doing PGS this time) and a few more transfers. Using their first PGS embryo and on a tailored immune protocol, Simone fell pregnant. A year after their daughter was born they decided it was time to extend their family. In this episode Amy shares with us her long IVF journey. Amy knew from a very young age she wouldn't be able to conceive naturally as she struggled with severe endometriosis. After 5 years trying IVF, including an ectopic pregnancy and then a miscarriage, Amy followed a friend's advice and sought out a laparoscopy and hysteroscopy before her next transfer which was successful! Surgery can be scary, but especially when you have no idea how bad your chronic illness and surgery will be. This is the unique experience of pre-surgery nerves many women go through, going into their first Endometriosis surgery. If you're like me, and you had to do all of the researching, self-diagnosing, and intense advocating in order to get your first surgery scheduled--I encourage you to finally take a break from that momentum that got you to this point! I know it's so easy to lose yourself in prepping & planning, but if you've gotten this far, you've more than likely already done all the planning and prepping that is needed. Now it's just about the waiting game & not driving yourself insane in the mean time. To all the women out there navigating the medical system in regards to their Endometriosis, I genuinely wish you luck and smooth sailing on your journey to a more pain-free life. Living with worsening chronic illness is not for the faint of heart. Especially when the medical realm doesn't make it any easier for women like me, who suffer with Endometriosis, to navigate seeking treatment and diagnosis. I've struggled with the chronic illness and chronic pain my whole life, yet I am only now getting the diagnosis and treatment I need for it at the age of 25 almost 26. And what's even more disheartening is....I am one of the lucky ones. 25 is considered a pretty young age for being diagnosed and receiving surgery for Endometriosis. Too often, women with this chronic illness will go their whole lives suffering in silence with no glimmer of hope, knowledge, or guidance in sight. This is absolutely unacceptable. Even though I finally got to the point of getting my Laparoscopy & Excision surgery scheduled with an Endometriosis specialist, it took me 2 years of actively pursuing this exact result to finally get to this point. And by the tie I found myself with a treatment plan, I was already broken down and gaslit into hopeless defeat. Should I be surprised that I've faced very similar experiences within my mental health journey and getting the diagnosis I need to provide a sense of closure and deeper understanding of myself? Perhaps the answer is no, sadly. I've been talked over, misunderstood, patronized, and misdiagnosed every single time I sought out help from a professional in some capacity. The road to seeking out the diagnosis for my ADHD wasn't easy, but when I finally received the formal diagnosis & began receiving treatment for the ADHD my life drastically changed for the better. Had I not advocated for myself, had a clear understanding of my symptoms, and trusted my intuition--I'd be receiving treatment for a personality disorder that I did not have. And now I am in the beginning stages of the journey into seeking out a formal diagnosis for Adult Autism. As I continue to navigate this aspect of my life, I have a stronger sense of what I may come across in regards to ignorance from medical health professionals & how to navigate that. I will continue to seek out necessary help from those who are qualified to do so, and I will not stop until I receive that answers I need to help me understand myself more deeply. So what is Secondary care and what should you expect to happen?   Well, we want to make sure you understand this, as we don't want you to get to the end of the year and find there is actually a problem because you have been dismissed along the way.  There are better tests that can't be done in Secondary Care that can't be done in Primary Care, which means at the GP and we've discussed them with our resident expert Dr James Nicopollus.   Seeing a gynecologist: One with a special interest in fertility is key. Women often go to a fertility specialist later, because there is often a delay in referral from GP to Secondary care and then there can be a delay from Secondary Care to IVF. This has been heightened by the Pandemic.   Ideally referrals should happen through the NHS - should come from the GP to a fertility center.   What happens at the referral?   Clinics are trying to do a one-stop-shop. People will have done a semen analysis but will be asked for a report to make sure it is accurate. GP's will have tested FSH to check egg reserve and whilst this is OK, it varies between months as well as other variables. The AMH test is more reliable, as it shows the more follicles you have which is better fertility indicator. Tubal Patency checks tubes Important to rule out pelvic issues such as thyroid, polyps, or endometrioma which might impact the outcome.  If sperm and egg reserve is OK, are you ovulating regularly, with a progesterone check if you haven't had one?  Laparoscopy looks inside the tummy with a camera, however, this is done by general anesthetic and this is more likely if there are symptoms of endometriosis  Most people have a HyCoSy or HSG - inserting dye to screen the fallopian tubes   STI's such as chlamydia can be silent so these tests are important to exclude them A complicated appendix procedure could have caused adhesions or any other significant pelvic surgery can cause issues.If there is any suggestion of painful intercourse or painful periods this should be done.   Next, if these tests are clear, in the absence of sub-fertility then an assessment of Sperm DNA fragmentation is needed which looks at the sperm genetic material which can impact natural fertility, IUI, IVF, and ICSI.   Urologists are still not working as much with clinics as they should be   If all these tests are inconclusive and all investigations have been done and you don't have a diagnosis and you keep trying and it isn't happening, then the next stage is to go to the fertility clinic for fertility treatment. You should then have your options explained: Continuing naturally, IUI, IVF with the success rates explained    A lot of people are often well informed, other times incorrectly by Dr. Google, which is why we want to always guide you. The reason people's experiences vary so much in terms of the tests they get access too, is because different clinics and trusts offer different services, and the Doctors in clinics might not map things out correctly. There might also be limitations in funding. These are all ways in which you might feel you aren't being guided properly and this is why it is important to become a fertility advocate. You should always go into a consultation with a list of questions and never feel you shouldn't ask them. In the past, surgery was widely used to correct pelvic anatomy.  But how successful is surgery as a way of correcting abnormalities of the uterus? Has IVF changed in terms of the types of surgeries done today? In this episode, Prof Chapman talks about endometriosis and how it affects fertility. Laparoscopy (keyhole surgery) is very helpful in removing the endometriotic cysts found in the ovaries.  Tune in to find out whether surgery is effective in eliminating endometriosis and whether the burning of the cysts affects the number of eggs. You'll also learn so much more about the benefits vs risks of the surgery in increasing tubal function. What are fibroids? Do they have any effect on fertility especially if they're found outside of the uterus? Do they need to be removed? Laparoscopy (Keyhole Surgery) In this episode, Prof discusses a keyhole surgery, also known as laparoscopy. Many infertile patients are advised to have laparoscopy by their specialist. Several causes of infertility can be detected by laparoscopy. Laparoscopy isn't without its problems though. For one, it requires a general anesthetic.
