The acting head of the NIH reportedly pushed back against legal guidance to resume grant funding, leaving federal workers in the lurch. Also, though fungi make up a tiny part of the human microbiome, they play an important role in both the prevention and development of many diseases.Cuts And Conflicting Directives Sow Confusion For NIH WorkersJust over a month after President Trump's inauguration, federal science in the US is in a state of disarray. Executive orders to halt grant funding at the National Institutes of Health (NIH) have faced court challenges. Last week, a federal judge extended a restraining order on a proposal to cap NIH grant funding for indirect costs, costs that experts say are critical to their work. But according to new reporting, staff within the NIH have been left without clear guidance about moving forward with those grants, with the NIH's acting director reportedly pushing back against legal guidance from the agency's lawyers to restart distribution of the funding.Host Flora Lichtman is joined by Katherine J. Wu, staff writer for The Atlantic, who reported on the turmoil at the NIH. They also talk about other science news of the week, including an update on the measles outbreak in Texas, how to see every planet in the solar system this week, and how scientists think runner Faith Kipeygon could be the first woman to break a four-minute mile. You've Heard Of The Microbiome—Welcome To The MycobiomeYou've heard of the microbiome, the community of bacteria, viruses, archaea parasites, and fungi that live in our bodies. But that last member of the group, fungi, get a lot less attention than the others. And perhaps that's unsurprising. After all, bacteria outnumber fungi 999 to 1 in our guts. But now, scientists are beginning to piece together just how important fungi truly are. Disruption in the fungal balance can play a role in the development of Crohn's disease, irritable bowel disease, celiac disease, colorectal cancer, some skin diseases, and more.Host Flora Lichtman talks with Dr. Mahmoud Ghannoum, microbiologist and professor at Case Western Reserve University's School of Medicine, who has dedicated his career to studying the fungi in our bodies, and coined the term mycobiome over a decade ago.Transcripts for each segment will be available after the show airs on sciencefriday.com. This week's guest is Professor Mahmoud Ghannoum. For over four decades, Dr. Ghannoum has been exploring a critical but neglected inhabitant of the human body, the fungus. Born and raised in Lebanon, Dr Mahmoud Ghannoum is the current Director of the Center for Medical Mycology at Case Western Reserve University. He began his scientific journey at Loughborough University in England where he studied the fungus candida and its health associated diseases. Coupling this work to his curiosity about the whole area of intestinal microorganisms in the human body, he has become one of the leading researchers in the world in this space. His discoveries include the knowledge that fungal organisms constitute an essential part of the microbiome. In fact, in 2010, Dr Ghannoum was the first scientist to identify over 100 native species of fungi in the oral cavity and that they are mostly friendly to us. Like with bacteria, there are good fungi as well as bad fungi. And just as it was startling to discover that we need positive bacteria in our guts, most people today are shocked to learn that their health depends on flourishing colonies of helpful fungi. Symbiosis in all things seems to be the flavor of the day and history has proven this to be what we should have always assumed. He is widely published in top journals as well as being the founder and director of Biohm, a company dedicated to microbiome analysis and management from the perspective of fungal and bacterial communities. He is the author of Total Gut Health. This week, Dr. Ghannoum and I look critically at the microbiome of humans from the well trafficked bacterial road to the less travelled fungal road. Enjoy, Dr. M

Lay of The Land's conversation today is with Afif Ghannoum, a biotech attorney by background and the CEO of BIOHM Health — which he co-founded with his father, Dr. Mahmoud Ghannoum, a leading microbiome researcher and recognized for naming the mycobiome (the body's fungal community). With over 30,000 citations and 500 published papers, Dr. Ghannoum is one of the top 0.5% of cited microbiologists and the top 2% of all scientists across all disciplines and has been continuously funded by the National Institutes of Health (NIH) since 1991, including just receiving his latest NIH grant at age 72.Leveraging his own background launching and selling consumer products in over 27,000 stores and licensed IP to companies around the world in addition to holding several biotech patents, Afif partnered with his dad to commercialize his research and science into world-class consumer products through their company, BIOHM Health. With state-of-the-art science and one of the largest microbiome datasets in the world that combines bacterial and fungal sequencing, BIOHM was the first to address the critical link between fungi and bacteria in gut health. Over the last year, BIOHM has expanded its role with some of the largest global ingredient companies to offer a world-class suite of services, including product development leveraging its industry-leading database, bioinformatics, testing, and clinical trial support — their first BIOHM FX probiotic supplement was designed to specifically help reduce digestive plaque found in the gut.BIOHM is currently embarking on a clinical extension directly for medical practitioners and is quickly expanding into retailers across the country, as it not only manages one of the most comprehensive gut health datasets, but also has access to one of the largest collections of fungi strains, second only to the Centers for Disease Control and Prevention (CDC).In late 2022, Afif secured $7.5 million in funding for BIOHM to support their broader vision. Health begins in the gut. If you're a gardener, then you know that healthy soil is crucial to growing healthy plants. Think of your microbiome as the garden from which your overall health grows. The microbiome is the community of microorganisms, including fungi (the mycobiome), bacteria and viruses, that exist in our digestive tract, skin, mouth, and vagina. These microorganisms change in response to a host of environmental factors, such as stress, exercise, diet, medication, toxins, and other exposures.I would say that over 90% of the people that I work with experience some level of GI distress. Initially, they may come to me for weight loss, hair loss, low energy, or a diagnosis of an autoimmune disease, but, low and behold, it always circles back to their gut health. When our gut is inflamed, due to chronic stress, autoimmune diseases, poor diet, food sensitivities, and taking medications, especially antibiotics, we experience negative health consequences. Health consequences could look like skin disorders, such as acne or psoriasis, chronic fatigue, migraines, brain fog, weight gain, mood disorders, and acid reflux. Some negative health conditions not talked about as much when it comes to gut health are erectile dysfunction, hair loss, and asthma. Think about it, if our gut is inflamed, and we are eating an optimal diet full of bioavailable nutrients, our gut cannot properly digest, break down, and absorb the nutrients. If we don't absorb and utilize nutrients, we can't make ATP, our energy currency, inside our cells. How is our body going to perform all of the biochemical functions necessary to sustain optimal life if we don't have enough energy? Our body's job is to keep us in a state of homeostasis and when it's not, certain decisions have to be made. We don't need hair to survive, and growing hair requires nutrients and energy. A healthy gut is imperative for good health and most people have no idea how to make that happen for themselves. Our microbiome directs how we look, feel and think.Afif Ghannoum is co-founder of the first total microbiome company, BIOHM Health, where he has commercialized consumer probiotics and microbiome testing kits that target the dual role that fungi and bacteria play in the digestive tract. Could a saliva swab detect cancer presence? Researchers are closer than ever to using such noninvasive biomarker tests as diagnostic tools. Why? Well, it's all about balance, even at the smallest level. Researcher Mahmoud Ghannoum joins Richard to discuss the interactions of the human microbiome. Listen and learn What he's found through studies on cancer and digestive health regarding microbiome species abundance and richness, Why biofilms result from a particular microbiome combination and how his research has found ways to inhibit these films for Crohn's disease patients, and How these findings can be used to generate probiotics and bio therapeutics to address health and disease and a healthy microbiome for cancer therapy. Mahmoud Ghannoum is a professor in the Department of Pathology and Dermatology with the School of Medicine at Case Western Reserve University. He also started a company called Biohm Health, which uses microbiology to develop therapeutics for human microbiome health based on gut microbiome research. Microbiome communities live all over us and inside of us, he says, and he researches how they impact our health. He explains that the microbiome is made up of bacterial and fungal communities. The balance between pathogenic and healthy species and the interactions between each community has dynamic health implications, which is why scientist are connecting healthy microbiomes and cancer prevention. When these communities are out of balance, scientist call this dysbiosis. His extensive studies on tongue cancer and head and neck squamous cell carcinoma showed a connection between cancer and the microbiota. The species diversity and richness of bacteria and fungi both decreased in tumor groups. In the case of head and neck squamous cell carcinoma, he was also able to look at metabolites and found high levels of 2-hydroxyglutarate, which means this chemical can serve as a biomarker of disease, enabling noninvasive diagnosis. In 2016, he also published a well-known study showing how pathogenic bacteria and fungi in microbiomes interact and secrete polysaccharides that form biofilms in the guts of Crohn's disease patients. He's developing a therapy to remove these biofilms by inhibiting the hyphae growth of a candida species. He continues to research effective ways to bring health through microbiome balance.
