Contributor; Taylor Lynch, MD Educational Pearls: Thoracotomy  Thoracotomy is used in traumatic cardiac arrest to replace conventional CPR with direct access to the chest. Goals include identifying and controlling reversible causes of bleeding, prioritizing blood flow to the heart and brain, and performing open cardiac massage. Trauma categories Penetrating trauma: Gunshot wounds and stab wounds. Has a higher chance of survival because the injury may be localized and directly repairable. Cardiac stab wounds may have the highest survivability because the defect can be visualized, repaired, and treated with blood administration. Blunt trauma: Motor vehicle collisions and falls from height. Has a much lower chance of survival. Western guidelines EMS must witness the patient lose pulses. Penetrating trauma: CPR for less than 15 minutes. Blunt trauma: CPR for less than 10 minutes. Survival decreases to essentially zero beyond these time limits. Eastern guidelines Focus on the presence of signs of life in blunt or penetrating trauma. Signs of life may include: Pupillary response. Measurable blood pressure. Purposeful movement. Patient selection Thoracotomy should only be performed when the patient has a reasonable chance of survival. It is a highly morbid procedure with significant occupational risks, including needlestick injury. Appropriate patient selection and timing are essential. Procedure Begin on the left side of the chest. Cross-clamp the aorta to restrict blood flow below the heart and prioritize circulation to the heart and brain. Identify and repair visible sources of bleeding involving structures such as the heart or lungs. Perform open cardiac massage as the equivalent of CPR. ACLS medications may still be administered. The Halifax brand is being retired after more than 180 years — and Charlie Nunn says artificial intelligence is the reason why.The chief executive of Lloyds Banking Group told the Big Boss Interview that the way customers discover financial products has fundamentally changed. Increasingly, people are asking AI tools and large language models to find the best mortgage or savings account, making multiple banking brands less relevant in an increasingly digital world.That shift is also reshaping the debate around bank branches. Nunn challenges one of Britain's most politically sensitive narratives, arguing that physical access to banking has never been greater once post offices, banking hubs, community bankers, ATMs and cash points are taken into account. While he acknowledges that many people feel left behind by branch closures, he says the way banking services are delivered is changing, with thousands of Lloyds colleagues now providing hour-long consultations to vulnerable customers in their own homes. Traditional branches, he believes, will survive for at least another decade, but what constitutes a "branch" is becoming increasingly blurred.Nunn also argues that Britain has become too cautious. Regulation makes it harder and more expensive to lend than in many comparable countries, he says, while the economics of housebuilding no longer work in many parts of the country. The average first-time buyer is now 34, two-thirds take out mortgages lasting 30 years and the bank of mum and dad remains central to getting on the property ladder.Some 13.5% of 16 to 24-year-olds are not in education, employment or training, a figure Nunn describes as "frightening" given the country's ageing population. If young people cannot build skills, save and buy homes, he warns, the long-term economic implications for Britain are significant. Bert Mizusawa is a retired major general in the United States Army, serving in the Army from 1979 to 2015. Mizusawa also served in the United States Senate as a professional staff member and as a Senior Executive in the Pentagon, making him one of only a handful of individuals to serve at flag rank in the military as well as in both the legislative and executive branches. Mizusawa is also an attorney and is admitted to the bars of New York, the District of Columbia, Virginia and the United States Supreme Court. Awards: Distinguished Service Medal Silver Star Defense Superior Service Medal Legion of Merit Bronze Star Medal Combat Infantryman Master Parachutist Ranger Air Assault Joint Chiefs of Staff Identification Badge Humanitarian Service Medal 1983 Soviet defector incident Mizusawa led the Joint Security Force in a historic firefight against North Korean forces. Mizusawa was awarded the Silver Star for “exceptional valor and gallantry in action” while serving as the Commander of the Joint Security Force (JSF) Company at Panmunjom, Korea on 22 and 23 November 1984. His citation reads “In reaction to thirty attacking North Korean soldiers in pursuit of a Soviet defector, Captain Mizusawa's outstanding leadership and aggressive actions in leading his company while under fire were instrumental in defeating the enemy. Additionally, he personally led the defector to safety while under fire and deliberately, at great risk to himself, exposed himself to the enemy in front of his own troops to ensure the success of his company's combat action. Throughout the intense firefight, Captain Mizusawa displayed a complete disregard for his own personal safety while accomplishing his mission.” Some have credited the successful firefight and rescue of the Soviet defector, which unexpectedly did not result in a Soviet demarche, for convincing President Reagan to hold firm in his negotiations with the Soviet Union, which ultimately led to the end of the Cold War.
