Peter M. Hoffman is the author of “Karmic Winds: Reflections from The Smartest Guy in Hollywood.”  Peter is a Yale-trained attorney who became a powerful Hollywood insider helping to steer blockbuster films such as Rambo,  Terminator 2 and Basic Instinct.  A pioneering figure during the independent film industry boon, Peter enjoyed much success.  But as the industry turned, legal battles mounted and he was dogged by a federal investigation that landed him behind bars.  There he penned this memoir examining the nature of justice, the seductions of power and the invisible forces that shape our lives.  He speaks openly about his lifelong pursuit of meaning through Buddhism and shares fascinating behind-the-scenes stories of some of the movie industry's most pivotal moments.

Contributor; Taylor Lynch, MD Educational Pearls: Thoracotomy  Thoracotomy is used in traumatic cardiac arrest to replace conventional CPR with direct access to the chest. Goals include identifying and controlling reversible causes of bleeding, prioritizing blood flow to the heart and brain, and performing open cardiac massage. Trauma categories Penetrating trauma: Gunshot wounds and stab wounds. Has a higher chance of survival because the injury may be localized and directly repairable. Cardiac stab wounds may have the highest survivability because the defect can be visualized, repaired, and treated with blood administration. Blunt trauma: Motor vehicle collisions and falls from height. Has a much lower chance of survival. Western guidelines EMS must witness the patient lose pulses. Penetrating trauma: CPR for less than 15 minutes. Blunt trauma: CPR for less than 10 minutes. Survival decreases to essentially zero beyond these time limits. Eastern guidelines Focus on the presence of signs of life in blunt or penetrating trauma. Signs of life may include: Pupillary response. Measurable blood pressure. Purposeful movement. Patient selection Thoracotomy should only be performed when the patient has a reasonable chance of survival. It is a highly morbid procedure with significant occupational risks, including needlestick injury. Appropriate patient selection and timing are essential. Procedure Begin on the left side of the chest. Cross-clamp the aorta to restrict blood flow below the heart and prioritize circulation to the heart and brain. Identify and repair visible sources of bleeding involving structures such as the heart or lungs. Perform open cardiac massage as the equivalent of CPR. ACLS medications may still be administered. Seit Oktober 2025 sind Praxen und Kliniken verpflichtet, die elektronische Patientenakte (ePA) zu befüllen. Im Alltag von Millionen Versicherten spielt die ePA-App allerdings noch keine Rolle. Was in digital gut aufgestellten Hausarztpraxen heute schon möglich ist und wo es bei der ePA noch hakt, erzählt Dr. Peter Münster aus Münster. Er gehörte zu den Testern der ePA und weiß, wie wichtig ein gutes Praxisverwaltungssystem für die Nutzung ist. Die wesentlichste Anwendung ist heute die elektronische Medikationsliste: Verordnungen und Abgaben in der Apotheke werden transparent. Der Hausarzt moniert: Krankenhäuser speisen bisher kaum Daten in die ePA ein, Betäubungsmittel-Verordnungen fehlen, Labor- und Befundübertragungen müssen stärker automatisiert werden. Und ohne Volltextsuche bleibt die ePA oft eine unsortierte PDF-Sammlung. "Bohúň by mohol pre naše dejiny znamenať omnoho viac, a to predovšetkým vďaka jeho neúnavnej snahe o budovanie hodnôt a integrovanie Slovákov a Sloveniek do širšieho kultúrneho spoločenstva. Ak sa aj štúrovské hnutie z času na čas, nie veľmi presvedčivo, vymedzilo voči dianiu „na Západe“, forma ich komunikácie, trvanie na vzdelanosti, nároky kladené na reprezentáciu a otvorenosť kritike do vlastných radov presahovali snívanie o slovanskej vzájomnosti a vypovedali skôr o hlbokom porozumení svetu budovaného Habsburgovcami." Toto sú slová kunsthistorika Júliusa Barzciho o tvorbe najvýznamnejšieho slovenského maliara 19. storočia Petra Michala Bohúňa. Ako súvisí umenie s dejinami? Prečo má zmysel pozerať sa na dejiny práve cez maľbu a vizuálne umenie? Čo Bohúňove portréty vypovedajú o ľuďoch druhej polovice 19. storočia?
