­­In this new episode of Out of the Clouds, host Anne Muhlethaler interviews Nadine Kelly (https://www.instagram.com/yogimdnet/?hl=en),  a retired pathologist, experienced yoga instructor, American Council of Exercise certified Health Coach and Senior Exercise Specialist. She is also the founder of YOGI M.D. and host of the YOGI M.D. Podcast. ­Anne and Nadine are Akimbo almumni, they met in 2020 on the Podcasting Fellowship, an online course for budding podcasters hosted by Seth Godin and Alex di Palma. In this conversation, Nadine candidly reveals her journey leaving her medical career due to depression to finding her path as a yoga teacher, and later on as a health coach. Anne and Nadine discuss the link between yoga and mental wellness, and how Nadine's calling to teach seniors who she warmly refers to as her ‘wise women'. They talk about their learnings from coaching, particularly around limiting beliefs, and the value in Nadine's motto: no strain, no gain. The two of them found tremendous value in Seth Godin's Akimbo workshops, which Nadine calls ‘the classroom she always wished she'd had' and touch on the importance of good teachers. Still, every Sunday since May 22nd, 2006, I have published a podcast here. It was actually a no-brainer when I thought of who (and what) I would like to discuss for this week’s 700th episode. I wanted to discuss podcasting, and I wanted to discuss it with the one and only Seth Godin. I’ve known Seth for a long time. He’s been a guest on this show more than ten times over the years. Publishing my 30th episode is a marker and a cause for celebration. I started to learn to host and produce a podcast in fall, 2018. The Podcasting Fellowship was a great experience. I published a trailer in December, 2018 and then began publishing weekly in January, 2019. It’s a blast. Podcaster Joanna Penn has been at it for 10 years. I heard her anniversary episode in March, in which she spoke of the 30th published episode as an indicator that a podcaster is committed. The idea stuck with me. And here we are: episode 30!

Capacity for best health ebbs and flows. That's life. Some call citizen capacity - "patient engagement." But that's too weak. Growing and sustaining citizen capacity is citizen power. Our personal and community health depends on it. Four Stories I'm going to tell you four stories: I'm unsteady when tired. I get tired more often. I'm more likely to fall when tired. My capacity diminishes. I resort to mobility aids to increase my capacity. First one cane, then two canes, now occasional wheelchair.  Preventing injury maintains my capacity. Mobility aids build my capacity. At the same time maintaining and building capacity sustains hope in me. Hope creates capacity. It's reinforcing.  I am one person building my capacity. When I was 16 (I know, I'm old), I started preparing for the draft. I went to a church for draft counseling and enrolled in a course that trained people to be draft counselors. It started me on a career of obtaining profound knowledge of the law and regulations to manage within them.  I ended up counseling young men for almost two years. It served me well in my own mission not to be drafted. I built personal capacity, personal power, and guided others to exercise their power. I'm podcasting now. I joined a community - The Podcasting Fellowship. The sponsors of the community, Seth Godin and Alex DiPalma, created a seven-week course about podcasting and a platform for about 330 international aspiring podcasters to learn together and support each other. About 90 of those original participants are still meeting, six months later, learning and supporting each other. Somewhere between 90 and 250 of the 300+ are actively podcasting. Sustainable community capacity building. Garrett County, MD Health Department (population 30,000) created a platform for promoting the collaboration of almost 40 groups of local residents working together to solve the problems afflicting all communities, most with larger populations and resources – drug misuse, loneliness, immunization, food insecurity, oral health, medication management, maternal/fetal health, chronic disease management, transportation, homelessness. Community capacity building. Leveraging their resources! Build Citizen Power for Best Health Synonyms for capacity include competence, effectiveness, efficiency, potential and POWER. As in life, capacity ebbs and flows. Power ebbs and flows. I care about citizen capacity and citizen power. Some call it engagement, as in patient engagement. But that's too weak. Building citizen capacity adds to the base of community capabilities and sustains it. Where do we need to build citizen capacity? First and foremost - Habits for best health (personal and community) Coordination of care Policymaking Planning care Caregiving Informed decision-making Paying for healthcare Team building Leadership Storytelling, blogging, podcasting, vlogging, writing Advising governance, design, operations, and learning across healthcare Community services Community accessibility  Lot’s of opportunity for building citizen capacity Success Factors for Citizen Power What does it take to sustain citizen capacity building? Sustaining capacity is cultural, habitual, and repetitive. Here's my partial list of success factors: Community (any definition) self-identifies a problem and is dying to solve it Common values, common language, trust Content in byte-sized (and bite-sized) pieces sensitive to people's most comfortable learning style Small support groups, mentoring Doing, more than listening Any level of participation works Continual learning Ongoing synchronous (face-to-face or voice-to-voice) and asynchronous (participate when convenient) sharing and feedback Transparent governance - how the capacity building process is managed New blood welcomed Have fun doing it Yes! Citizen Power-Good for what ails you. Closing

Alex and I met in 2018 when I was a student in The Podcasting Fellowship, an online course she co-created and runs with business legend Seth Godin, who will factor prominently in our conversation. It’s everywhere, it’s everywhere. I can’t help but see almost everything I do in my advocacy through the lens of health equity.  Whether it’s making decisions about our health and medical care, managing pain, young adults with complex medical issues transitioning from pediatric to adult medicine, men in caregiving, data sharing, patient access to data. Everywhere. I’m defining equity as people having the same opportunity to achieve best physical, mental and spiritual health no matter their social circumstances, biology, genetics, or physical environment. I wanted to take a look at bias, inclusion and equity from outside healthcare. So, I interviewed Ame Sanders and we talked about our own biases, inclusion or lack thereof in our communities, measuring bias, and taking action to reduce inequities. Ame caught my eye with her podcast, Equity Warriors, and her company www.stateofinclusion.com. See the show notes for further information. We decided to interview each other. It can be social media. It can be the LGBTQ community. It can be people that you identify with. But for the work that I do, largely, the way I define community is either city or county. And I'll give you a reason because my mission for my work is to work with communities to help them become more inclusive. I believe you can change the world one community at a time. and I work with a lot of data. There is a certain level of data that's required to do this work. And it's mostly available at the county level sometimes even not at the county level. Measuring equity -race It is a bit of a conundrum because in one hand you don't want to ask people that question because it shouldn't matter, right. We should be able to go through life and never be asked that because race is an artificial construct in any case. It's not a factual thing. It's an artificial construct and in some cases, as you pointed out, it’s how people perceive themselves and how they define themselves. Knowing better and doing better There’s a quote from Maya Angelou that says, ‘when you know better do better.’ So each of us, I think the most important thing in this work is that each of us own very responsibly. The need to know better and that includes knowing better about ourselves.  And doing better. Establishing trust Then I think that the next thing to work on is dialogue and trust. Because communities only change in this area if you can establish a basis of trust. And that's really important. There’re probably organizations in every Community. You can wake up in the morning and love where you're going to (novel idea, right).   Thriving is when we realize the beauty of all that we GET to do (not have to must do or should do)   We must be careful not to trap ourselves in constant go, go, go and starve ourselves from personal one-to-one time.
