It is imperative that children and young people are central to the co-design and co-production of our patient safety improvement interventions. In this episode, we speak with Dr Jane Runnacles, consultant paediatrician at St. George's Hospital, and Dr Victoria Dublon, paediatric diabetes consultant at the Royal Free Hospital. Both are champions of improvement work that puts the young person and their needs first. As Jane and Victoria describe, involving children, young people and their families in improvement work improves the experience and outcome for all involved. There are fantastic examples of co-creating and co-producing safety improvements in healthcare. We discuss the practicalities of how to do this and who to involve in your healthcare setting, and we hear about some of Jane and Victoria's successes. Thank you for listening. What role can eye tracking play in building trust between pilots and the aircraft they fly? Can it help improve flight safety, or help train the next generation of pilots? These are the questions tackled in today's episode featuring Karl Tschurtschenthaler and Simon Schwerd both from the Institute of Flight Systems. Karl studied Biomedical Engineering and received his Master of Science degree in 2018. Afterward, he worked as a research engineer in the field of numerical modeling and has been working as a research associate and Ph.D. student at the Institute of Flight Systems since October 2020. His research objective is Pilot Activity Recognition using probabilistic graphical models based on real-time data (eye tracking and psychophysiological sensors). Simon studied engineering at the Technical University of Munich with a bachelor's degree and  master's degree in mechanical engineering. At the Institute of Flight Systems, he developed future cockpit concepts for military aircrafts and is pursuing his PhD in the estimation of Situation Awareness based on pilot monitoring via eye-tracking. His general research interests include Operator & Pilot Assistance, Adaptive Automation and Human-Automation-Integration. Simon and Karl talked all about their work with eye tracking in aviation, including cooperative automation and human-automation-interaction in aircraft cockpits. Major Jameel Janjua, experimental test pilot with the Canadian Armed Forces, shares ideas on how he stays appropriately "switched on" given the needs of any performance situation. His world, much like in medicine, is a high-pressure, high cost of failure performance context. Jameel provides rich insight on a range of human factors, and high-performance related topics. Among those he touches on include performing under pressure, maintaining situation awareness, the need for attentional flexibility, preparing to respond to the unexpected, and critical task prioritization. Throughout the discussion we draw parallels between the conditions he performs under and how, from a human performance standpoint, these might mirror what physicians face in their work. 08:27 Jameel describes what attributes make for a great fighter pilot The central role that passion plays No such thing as fair-weather fighter pilots Resilience matters Harmonious and obsessive passion 17:21 Cultivating an ideal focus, and performance mindset Attentional flexibility and task execution Preparation (training and simulation) drives “in the moment” focus Staying perceptive in critical AND non-critical moments Engagement: Going from 0-100 and all points in between Maintaining focus during benign performance moments 33:30 Communication and pilot focus Differences in comms between combat vs. test missions Personal “housekeeping” tasks to stay focused in flight Playing the “game of contingencies” How comms during test flights can enhance focus, performance, and safety Micro breaks to defocus and then re-focus “Buying risk”: The need to be in an optimal state 46:32 Switching on when the unexpected occurs Focus first on mission critical tasks Keeping the main thing, the main thing Train and simulate FOR the unexpected Train to your breaking point…and then go further Knowing the “attentional revisit rate” for each task/procedure 58:44 Recognizing those who have “it” and those who don’t “It” as a function of being teachable, open, humble, and driven to improve Further, “it” comprises the ability to rank, and stack, and execute important tasks and decisions in critical moments Arousal control and “it”

Balaji Ramadoss: Future of Healthcare Balaji is the Founder and CEO, Edgility, where he is enabling a cognitive Healthcare disruption. Before launching Edgility he earned a PhD and worked in Healthcare for last 15 years as CRO for Tampa General and VP of Technology Experience for Stanford Healthcare Palo Alto. Prior to leading large teams, Balaji was in Public Health and responded to Hurricane Katrina, Deploying as tech consultant where he developed the first command center based on healthcare needs. Healthcare Disruption / Transformation Healthcare is ripe for disruption, where do start looking at this big system to create disruption? Healthcare is inefficient; Inefficiency is unethical Clinicians are the most important aspect of the equation Healthcare lacks situational awareness Joy in Practicing Medicine: Physicians, Nurses and Care Providers got into medicine the patients and the joy of practicing medicine, yet we hear a lot about physician's being very dissatisfied with their careers. What do you guys see happening here? Clinicians should focus on patients. Visibility of logistics should be taken care of for them. 'I' in the middle of the triangle. 2008 -> Today We've implemented tech so fast that we didn't re-engineer the process. Help healthcare unpack transaction system of data. Think about radar for real time pings. Design Thinking. First stage, empathy. Do you understand the problem emphatically? Broke Healthcare into 15 domain areas and now 6 core offerings. Flare... Deep experience helps them to identify the domain ares. Start with empathy. Chose orders management first. Revenue cycle. Care management. Interdisciplinary communication - Dr's talking to themselves. Formed design teams. Made of clinicians, not admins and techs. Worked directly with these teams. Brought all of this back to the patient experience. Flare back up by looking at workflows, EMRs, etc. for ways to create triggers and events Situational Awareness. I've heard you talk about Situation Awareness as part of the solution. Can you explain what that means to you? How do you create that in such complex healthcare environments? Definition: At any point in time do you know what's going on? Supply Chain: Tracking packages. In Healthcare. Doctors and nurses have so many logistics to manage that it's difficult for them to see the whole picture. What's Next in Healthcare? Healthcare is not simple. Will not be simplified like Uber. Genomics are the next wave. Won't make it simpler. Will make it more complex. Without cleaning up foundation getting there will hard. Precision medicine. Fee for performance. Standard questions One piece of tactical advice you'd give our ambitious corporate audience that they can put in place today to help them change their life / career? When you have doubt, look past the curve. Where you see is where you go. (Very Zen & the Art of Motorcycle Maintenance if you ask me.) Best business book you've ever read?
