RACHEL MOORE All right, good morning PT on ICE Daily Show my name is Dr. Rachel Moore I am here this morning. It is Monday morning. That means it is our pelvic day here on this podcast So I'm here with the pelvic division and I am super excited to talk to you guys this morning We're gonna be kind of diving into varicoceles and varicosities, vulvar varicosities, and the way that those are actually incredibly similar in our treatment approach, whether we realize it or not. Before we dive into that though, if you missed it, we've officially rolled out all of our certifications here at ICE. So we have certifications, we've had them before in the clinical management of the fitness athlete division and an MMOA, but now we have new ones. So we have orthopedic, we've got dry needling, we've got an endurance athlete, and then what we are super excited about in the pelvic division is we have our pelvic cert as well. So this is three courses, two online, a level one and a level two, and then one live course. If you're looking to get in on that certification, our upcoming courses, we actually have one this weekend in Bear, Delaware. That's going to be with myself and Alexis Morgan. So super excited about that. Still, time to hop into that if you want to buy yourself a plane ticket and get out there. And then we also have one on December 2nd. VARICOSITIES AND VARICOCELES Let's dive into our topic for the day. So a lot of times those of us in the pelvic space if we are maybe majority see women a lot of us tend to maybe start there and then maybe branch off into seeing men but if we are in this kind of blinders-on situation where we're like no no I only see women sometimes we may be uncomfortable or unsure if somebody gives you a call and asks about a certain diagnosis or maybe you have a friend or somebody that you know that is struggling with something and asks for advice on the pelvic space and you're trying to figure out how to get them into your clinic. And so I wanted to draw a parallel this morning between two diagnoses that we see as fairly common that actually are very similar in the way that we treat them. So that is going to be varicoceles and varicosities. So in utero, the reproductive tissues of males and females begin developing similarly. If you guys remember that from PT school, when we were learning about the brief amount that we cover these types of topics, once testosterone starts being released, that's when the reproductive organs shift and either develop into male organs or continue on the path of female organs. And so if the testosterone is there, then the tissue that is becoming the scrotum becomes the scrotum. But if the testosterone is not there, then that tissue continues on to turn into the labia. So when we think about our tissues and our anatomy, we often talk about how male and female anatomy really aren't that different. It is similar parts arranged differently and maybe to different sizes and proportions. But when we look back all the way in utero, we can see that developmentally these things start the same and there's a certain point where things branch, but we have these kind of analogous, um, uh, tissues within males and females. So, We know that the tissues are similar between the scrotum and between labia. When we're talking about varicosities, this is important for us to know because these are two diagnoses that we tend to see come up fairly frequently. VARICOSE VEINS IN THE PELVIS So before we dive into the specifics of varicose veins in the pelvic area, let's talk about what varicose veins are. Varicose veins, if you're not familiar with them, are enlarged twisted veins. So oftentimes this comes from damage to the valves in the veins. So our veins have one-way valves that help push blood up and prevent backflow back down. If there is damage to the inside of the vein and the valves are damaged somehow or maybe are not operating at the capacity that they need to be operating, we can see kind of a backlog of blood and that can lead to this kind of inflamed or swollen look to the veins and that blood just kind of pulls in there. The causes of the damage, quote-unquote, Inside of the vein can be known. So this can be something like high blood pressure or it can be unknown Things that increase your risk for developing varicosities are gonna be things like being female So that's always fun when gender is one of the top things can't control for that genetic predisposition so if you have a family history of varicosities then this might be something that you're really keeping an eye on and older age as we get older maybe those valves within the vein become a little bit less competent increased body mass and then in pregnancy we'll dive into that here in just a second and then also interestingly having a history of blood clot that's really important to kind of keep in mind on our radars not only in our post-surgical patients but we're starting to see blood clots kind of popping up more and more um and so if you have somebody who might be not hitting any of these other risk factors but has a history of blood clots it's still something that we want to kind of keep on our radar varicose veins aren't a medical emergency by any means but they can cause some like uncomfortable unpleasant symptoms like heaviness aching pain and then swelling. VARICOSITIES Let's dive a little bit deeper into varicosities of the pelvic region so in our biologically female counterparts we see vulvar varicosities this is varicosity that develops on the vulva so anywhere along the outside of the vagina so that tissue of the vulva It can happen on labia majora, labia minora. It can be going towards the inner thigh, more into that groin area. Really just kind of depends on the area that is affected. The risk factor for this specifically is pregnancy. So we see this come up in pregnancy for a few different reasons. One reason is that we have an increase in blood volume during pregnancy in order to support the baby. So that increase in blood volume means that our veins have to work harder to push more blood up. we also know that we see relaxin circulating and that does have an effect on all tissues and then we have an increase in pressure so we have increased pressure from both the weight coming down of baby placenta amniotic fluid and all the things but then if we also think about like the anatomy of a pregnant belly as people progress through pregnancy get into this maybe anterior pelvic tilt their belly maybe drops low it can cause some congestion or some backup within that system which then leads to less efficient drainage. This is something that we see pretty often in the clinic really and you might be familiar with this if you're in the pelvic space. but what we tend to not really think about is how this parallels varicose seals. So a lot of times we're pretty confident and comfortable with vulvar varicosities, but then somebody comes in with a little bit different anatomy, and we kind of get thrown for a loop. So a varicose seal is a varicose vein that's located within the scrotal sac. This can actually develop during puberty because blood flow to the genitals increases during puberty. As those tissues are maturing things can just get a little thrown off, but it can also happen as a result of surgeries So think about vasectomies even though those are like minor office procedures surgeries vasectomies or trauma to the scrotum They're surprisingly common, especially in the adolescent puberty side of things. And just because you have a varicocele doesn't necessarily mean you'll even know it, aside from feeling it, potentially. So the biggest way or hallmark of this is called the bag of worms. because within the skirt sack that varicocele feels like a thick ropey worm and so as people are feeling around checking testicles for different things then you might feel that bag of worms type sensation or that that feeling with your fingers and other than that you may not have any idea However if you have a varicose seal that is causing problems We can see swelling pain and heaviness as I talked about earlier and if this is left alone and becomes severe it can actually impact fertility in men because it can lead to decreased sperm in the ejaculate and so it can be something that if it happens in adolescence and somebody is trying to conceive later on in life with their partner and they're struggling, it's an area to look at. Just like vulvar varicosities, we see an increase in symptoms when we're standing for prolonged periods, but uniquely to this population, we can see potential pain with ejaculation. So with vulvar varicosities, we might see pain with intercourse because of the pressure on the outside of the vulva during intercourse. But with this population, it's going to be more so during ejaculation that there is pain. WHAT TO DO ABOUT VARICOSITIES AND VARICOCELES We have our person in front of us, male or female, who comes into your clinic, some varicosity of some sort going on. What are we supposed to do? Jess actually did a really fabulous episode on this topic. It's episode 1198, so if you want to go back and listen to that, she talks specifically about varicosities during pregnancy, and those same concepts can be applied to varicoceles in men. So I highly recommend giving that a listen. We're going to dive in just really briefly touch on some of those topics and then I'll let you guys really dive into justice. External support can be a game changer for these folks, especially those with varicose heels whose anatomy is already putting things in a gravity, um, disadvantageous position for drainage. So giving some type of support, whether that is like when you're getting up and moving using your hand to support or getting some type of support garment. There are specific support garments that are made both for males and females for varicosities. soft tissue massage and when we think about this we're really thinking like mimicking lymphatic drainage I talk about this all the time with breast tissue and engorgement but the same thing we're thinking about this like congestion within the pelvic region and so we want to think about clearing more proximally up Towards the iliac vein so that we can kind of promote that drainage and then work our way down Rather than coming down to the bottom and just shoving everything up and causing more congestion Superiorly, so we're starting closer to the midline Draining quote-unquote that area. So if you're watching on Instagram, we're saying we've got a guy in and he's got varicose heels maybe we're starting here and then we're working lower and then working lower and until we get to that most distal tissue. From an exercise intervention standpoint, the pelvic floor muscles, of their functions are a sump pump. So when they contract and relax, they push fluid out of areas. So teaching our patients how to do pelvic floor contractions, how to lift up and contract into the attic, relax down and go into the basement, get that pumping mechanism going, and then teaching them belly breathing on top of that to help facilitate that as well. Finally, from a positional standpoint, we can have our patients if at the end of the day, they're super symptomatic and they're feeling rough after being on their feet, laying on their back, propping their legs up on the couch, or on a wall to get some passive decrease in gravity pressure on the pelvic region, and we can even take that a step further, have them plant those feet on that surface and do some bridging where they're squeezing their glutes, maybe adding in that pelvic floor contraction, layering that in, so we've got gravity coming down, we've got our muscles contracting and relaxing, really everything helping to push that fluid up and out into the drainage system to go bring that blood back to the heart. So, if you have somebody come in your clinic tomorrow, and you are a pelvic floor PT who traditionally treats females, and a guy walks in and he's like, I have a varicose seal, I don't know what to do. I hope that you can put your cap on, thinking cap on, and realize like, you got this, you know what to do. At the end of the day, we have to remember that our males and our females, although the anatomy is arranged a little bit differently, and proportions are a little bit different, they are similar tissue. Check out today's questions:  Judy: I'm hoping you can help me! I suffer from chronic headaches - one per month that last for 48 hours. They usually occur around the same time each month. They used to be more spread out - maybe 2-3 per year. I was diagnosed years ago with sinus headaches - had an MRI done which did not show any signs of anything sinus related. Then the Dr said migraines and to avoid red wine, cheese, etc. That didn't help. I've been seeing a chiropractor and get massages regularly thinking it may be spine or muscle tension. But they keep occurring. I have had two babies in the last 3 years so I know nursing has made my posture terrible - along with sitting at a desk working for the last 16+ years! I even went to a neurologist 2 months ago and that was a joke! I listen to your podcasts and really love the information you provide to your listeners. I'm hoping you can help me! Daniela: Hi, First, I'd like to say I am so grateful that we have you! Thanks for all you do. I did the starter kit lab tests about one year ago or so. I had high levels of mercury and aluminum and the organic acids test showed no clear signs of candida or bacteria (but the health coach did say there were some backup markers indicating possible yeast or bacteria). So I did the Equilibrium Nutrition Heavy Metal Detox in October 2018, followed by the Intestinal Cleanse and Clean Gut Probiotic. I noticed that pretty soon after I started the HMD, I started having foul-smelling gas, which was very rare for me. It has been about 10 months or so and I find that I still have foul-smelling gas quite often. Any ideas what could be causing this?  Thanks! Savana: Hi dr. Cabral. I Love the podcasts, I listen to them almost every single day! Question for you...I’ve been getting these weird head pains. Out of nowhere ,a very sharp pain and pressure will hit like a lightning bolt. I do get headaches and migraines as well but these pains only come one at a time and are quite painful. I get them probably 5-10 times a day. I havnt had a head injury so I’m not sure what could be causing this...any thoughts? Thanks Anonymous: LONG Question warning: I stopped birth control April 2018, after 10 years consecutively taking it, and went from May - December without a cycle and in December had 2 barely noteworthy cycles. I haven't had one since. Overall symptoms below, both before stopping the pill and after. I have been traditionally allergy tested (skin prick) and am allergic to basically everything outdoors (trees, grass, weeds, pollen). I have been since birth. Plus beef, pork, dairy, mold, mildew.  I was retested last year and have developed new allergies to pineapple (biggest reaction), peaches, goat, cabbage, squash, cantelope, and rice but only mildly. I got tested because for the last 6 years before that avocado killed my guts. These new allergies do not react respiratorily, they just make my guts feel like they are twisting in knots for HOURS. It doesn't move along quickly in either direction. It just sits and hurts. I didn't test for this but now almonds, walnuts, and quinoa are doing the same thing. But not 100% of the time I eat them. It's so hard to do an elimination diet when it constantly changes.  My regular annual blood work shows that my cholesterol and other numbers seem to be improving with my regular exercise and whole food nutrition diet. I did have super low Vitamin D-OH Total (results were 25) and was on a weekly 50,000IU for 1 month and now take 1,000 IU per day. My Phospahte as phosperous was high but just on the top of the recommended range 4.6. TSH is around 1-1.5 between the last two years. MCV this year was 99. Sodium 142. Creatinine 0.93. A/G Ratio 2.1. ALT 26. Iron 186. B12 733. Fasting glucose 79. Calcium 10.2 Magnesium 2.2. Albumin 5.1. I emailed my bloodwork to your support email.  I am almost positive I have either thyroid and/or adrenal issues, leaky gut or something, but I can't afford to test right now. What can I do? Symptoms:  Insomnia fall asleep ok, but wake up a lot Restless all the time at night discomfort  Fatigue, I feel heavy eyed all the time even if my brain won't shut offBrain fog Memory problems Low sex drive (problem for at least 10 years)Hair falling out, breaking, brittle eyebrows thinning dry skin/dry hairRinging in ears Hands and feet always cold Lack of excitement, I don't even have hobbies anymoreInability to lose weight in lower body. Like I look skeletal in the chest and my upper body but my legs are very heavy tree trunk looking still. I wanted to start off by saying that I am so excited to be a part of your IHP level 1 and 2 training programs. Before i ask my question(s) let me start with a brief background of my current challenge. I have been on the fence for quite some time now. I have 9 amalgam fillings. I want to take them out before having my second child but am afraid to disturb them preconception. My now almost 5 years thankfully is fine . I breastfeed for 2.5 years and I am afraid that as these amalgams fade they will continue to leach and I am afraid of conceiving again with them in. I am worried for both mine and future baby's health. Keeping them or removing them preconception. Ihave received varied opinions from Natropaths, Functional medicine doctors (that I trust) and my dentist who happens to also be a biological dentist office. I am a researcher and I have done a lot of research and couldn't come up with any compelling evidence that shows no harm to baby if amalgams are taken out before conception, however know how unhealthy it is to keep them in. As part of our Community Health Partnership with Valley Health, we were in the studio today with Dr. Matthew Borkon, a vascular surgeon with Valley Health Vascular Surgeons for a conversation about varicose & spider veins. Dr. Borkon joined Valley Health Vascular Surgeons in November 2016. He serves as Medical Director and Chief of Vascular Surgery, and Chairman of the Vascular Clinical Council at Winchester Medical Center. In addition to his Winchester office location, Dr. Borkon sees patients in Valley Health's multi-specialty clinics in Front Royal, VA, and Martinsburg, WV. Dr. Borkon explained the cause, risk factors & symptoms for both varicose and spider veins. Today is National Leg Wear Day and it made me wonder how many of you ladies out there take advantage of this soon to be forgotten fashion accessory? Which type do you like to wear? Are you the traditional type wearing only solid black, navy or nude? Or, are you more adventurous type wearing wild patterns, checks, prints and vibrant colors? As a professional image consultant I've learned that leg wear can make or break an outfit. Also, it is imperative to wear them if the occasion calls for it. If you notice, more and more women are steering clear of wearing them. I still consult my clients and tell them the importance of wearing a pair, especially if it's for an important meeting or if you're say, speaking on stage. When I teach my “What's My Style” fashion class, I help women and men learn what their fashion styles are – there are 7. Fall has arrived and now's the time to take advantage of the season by adding some cool and fun leggings as well as tights, pantyhose and other fun legwear by incorporating them into your daily fashion routines. Have you ever thought about wearing a pair of fishnets to the office? Some folks may think that's a little bit much, but, you most certainly can wear them as long as they're the right webbing. For instance, the larger the weave, the less “sexy” they look. I found several pair years ago that I love to wear to work, they're beige, white and purple. I've worn them to office meetings, network events and speaking engagements. They actually look fun, exciting and personable. Here are a couple you can try: Fishnet Stockings  If you like more of the creative styles, you may like to add some of these fun and whimsical ones: Fun & Whimsical Tights Not only do these tights make your legs look more fashionable, up-to-date and pulled together; you'll also be able to camouflage some of those not-so-lovely things that you may not like about them such as spider and varicose veins, scars or stretch marks AND the best part is that you may even be able to skip a couple of leg shavings (no one will know but you and possibly your significant other).   I remember when I had my ankle tendon repair surgery. It was back in 2004 and I had this terrible looking scar that ran from one end of my ankle to another (I fell in a sink hole outside my home in Brooklyn) and wearing tights and dark color pantyhose make me feel so much better about the appearance of my legs. It covered up the scar until it was lightened where I could go back to wearing au-naturel They were my saving grace. They not only kept the scar from being seen, it also kept me from constantly looking at it as a reminder of the fall. If you've never been one to wear tights, you might want to begin slowly. I'd say introduce a pair that might have a tiny tight weave or small light pattern. Nothing too flashy or revealing. But, if you're looking to go from “drab and dreary to a vixen” then try on some thigh highs. Here are a couple of my favorites:  Thigh Highs You can't help but immediately feel sexier once you slide on your first pair and, another bonus, your partner will be drooling all over you once they find out what you're wearing. If you want to push the envelope, add a garter belt to match. Here are a couple of combination hosiery: Garter Belts with Stockings  The last thing I want to share with you is that the legging or even   Jeggings (jeans/legging combo) is one of the quickest ways to enhance your casual work pieces which you can add to your wardrobe that will not only flatter just about every leg length out there, but, they're quite easy to wear, comfortable as hell and you can wear them year-round. Some of my favorites are Lulumon just add your favorite pair of ballet flats wear a long tunic or man-tailored blouse, tie a sweater over your neck add a couple of necklaces, a chunky cuff bracelet, cool funky earrings and viola instant chic. Imagine, if you will:  Candlelight, soft music… Your lover gazes softly at you from across the table.  The kids are at Grandma’s house.  The atmosphere smells of hormones.  Pupils are dilated.  A gentle touch, the promise of soon being much… much closer…  Then it hits!  The dreaded “H-word.”  You know the one.  HEMORRHOIDS.  They know no bounds of race, creed, or economic status.  They care not to which political party you subscribe or what country club you’re a member of.  The all too familiar burning, itching, irritation begs even the most dignified among us to stealthily conceal creams and wipes and pads promising freedom from this most undignified situation amongst the cereal and pasta in the bottom of the shopping cart.  The problem is—as with most of our crisis care based healthcare system—that those creams and wipes and pads are really only focused on reducing symptoms… for a time.  They do nothing to address the reason behind (no pun intended) the symptoms.   Well, what if I told you the reason behind the symptoms is ridiculously simple to treat?  Inexpensive?  Non-invasive?  Doesn’t involve humiliation or pulling or cutting or bleeding from the bum in front of a room full of medical professionals? Is your interest piqued yet?   First let’s be clear on exactly what hemorrhoids are.  Hemorrhoids, in the most basic sense, are varicose veins of the lower rectum.  Well, what are varicose veins (just to be completely clear)?  Veins, just like every other part of the body, have something called “tone.”  This tone is the tightness, the resilience, the resistance of the walls of the veins to the pressure of the blood moving through them.  When the walls of the veins become lax, weak, flabby—just like when other parts become flabby, like the belly or the thighs or the chin—they begin to stretch out, sag, and lose the elasticity that allows them to rebound, causing the blood to pool and the vein to balloon from the pressure.    Now, sure, these varicosities are unsightly and sometimes uncomfortable, but why should we care?  Varicose veins are an early indicator of something run amok with the vascular system as a whole, and the vascular system is the body’s delivery/pick-up highway that begins and ends with the heart.  So we’re not just talking about an embarrassing little problem that nobody wants to acknowledge.  We’re talking about the organ that pumps blood to every cell and organ and system in the body.  Pretty important if you ask me!   Enter stone root—Collinsonia Canadensis—brought to King George by his botanist, Peter Collinson, introduced to it by the American Indians in 1600.  This tough little herb is known for its tonic (toning up) effect on everything it comes in contact with, with applications for everything from bruises to urinary incontinence to a congested liver to… yep, you guessed it… even hemorrhoids!  It can take time, and obviously the effects are greater with the compounded efforts of a nourishing diet, but this amazing plant is able to literally retract and pull up those distended veins.    So take heed!  Listen to that forbidden doctor within you.  If it’s sending you warning signs about the health of your vascular system, get hold of some of this life-giving stone root, and treat the real reason for those oh-so-blushworthy symptoms.  You won’t regret it!  What you will learn in this episode: What the foundational reason for hemorrhoids is.  Why hemorrhoids are not talked about openly, and why doing nothing about it until it is too late is not worth it.  Why hemorrhoids are not just a senior problem anymore- and why we are seeing an increasing number of younger people with degenerative problems like it. Everyone that goes into the miracle in a bottle Market ultimately winds up in worse conditions, Surgeries or other affects. A lot of the time they don't realise that it came from there use of the miracle in a bottle. Don talks about his childhood, Sunday school, his first taste for Pulse and the start of his life long quest looking for Pulse After 17 years and no pulse in sight Don realised enough was enough and quit his search. Don was sitting eating an apple one day and something happened to him like never before.  Don litterally could not breath out but he could only take air in, he started blacking out and thought he was dying.  All of sudden this big breath came out of him again.   2 thoughts crossed his mind then and there, 1 was that this is embarrassing and 2 you are through looking and it was time to quit this and get a life.    Our body (Abode) is sacred and it was divided vertically into 3 levels, from the bottom of the feet to the navel is the Basement.  The navel to the throat is the Main floor and the throat to the top of the head is the Upper rooms (the Heavens).  Then the heavens were divided into 3 levels - chin to nose, nose to brow and brow to the top of the head.  The top of the head has an eye that always sees in all ways, which is called the minds eye, eye of imagination or the third eye = Insight.  There were 12 lights (the Apostles) that lit the dome of the heavens, just like the sun, we are a Microcosm of that greater macro measurement out there.   In ancient times (the doctrine of Signatures) they knew that the offerings that should come into the body to support its Vitality, Health and Longevity which came from Plant foods that grew in the basement of the Earth, the main floor of the earth and the heavens that you had to reach up and pick. (e.g. grains, grapes, nuts, fruits) Carrots support Vision.  Tomatoes are one of the most profound Heart foods you can eat. Grapes target the entire Cellular structure of the Heart. Walnuts look like the brain and they are Brain food.  Avocados help a woman's body Balance the Hormones and Prevent Cervical cancers, complication and delivery. Bell peppers target the Heart.  Passionfruit you can see a signature pattern of the male testicle's and of the Ovaries of the female. Peanuts are the source of the arguing that is called Viagra.  Bananas and cucumbers are self explanatory.  Mushrooms target specifically the Thyroid.  Anything which is called a 'head' of eg. Cauliflower, Broccoli or Cabbage targets different functions of the Brain. Celery stalks, lettuce's etc target the making of new Blood and the delivery of it throughout the body.  Grapefruit targets and clean the Arteries.  Berries protect you and were used to Vaccinate yourself.  If you have big Varicose veins you should take leafy head plants like these ones, crush them to release the juices, put them on like a pulsate and wrap them in gauze for 4-5hours at a time over 3-4weeks, and your condition will remit. Don is thrilled about people beginning to gain enough knowledge and look at things with a different set of eyes and a questioning mind to everything that's going on and what their being told.       In the USA after 55 years of pharmaceutical, 3 trillion dollars just fighting one Disease and over half a million weapons of war, their's a quarter of a million supplements.  Yet we are the sickliest Generation of American's to of ever lived.   The USA has the highest infant mortality of any country.  In the USA of the 250 thousand food poisonings that occur from people eating at public restaurants and 231 supposedly can be tracked to a Plant food.  However it was sitting next to the Fluids of a piece of animal that was being cut up.  It is going to get more and more intense in the next 3 -5 years in the attacks of Whole foods, they attacked spinach and killed the market in the USA.   Farmers lost their farms and then large agri businesses came in and bought them up and then just kept growing, like a disease. This has happened to onions and apples also. More studies are showing that colours of the food targets your Emotions. eg. Red is positive for feelings of love, power and desire and the negative is anger and rage.  In Don's book 'The Pharmacist Desk Reference' has an entire section about the anatomy and the emotions that they represent and where those Positive and negative Molecules go and the foods that support that emotion or eliminate it if it is a Negative emotion. If you project a blue or blue green colour, before you go to bed you should eat 1 whole food offering of the colour that you projected and saw.  You make that an offering to the temple for Remising the molecules of that emotion. eg. If you dont feel love and want more love in your life then eat Red foods.  if you want to be brighter, mentally eat Yellow foods.  If your lacking curiosity and wonder eat Orange foods.

Edgar Cayce said that if we could control these aspects of the human form, we could extend life to whatever we chose, that's how important these two principles are. Assimilation Assimilation is defined as the measure of the ability of any body to utilize foods we eat. Some people think that having massive doses of food will give them all they need. They could not be more wrong: it is all right shoving nutrition in but it is quite another thing if the body can absorb it and use it in that form. Typical assimilation rates might vary from 20% to 80% and nutrition supplied in some forms will not be absorbed at all. That means out of every 454 grams of food you might only absorb 20-80% of that. What happens to the food after you swallow it depends to a large degree on the other three principles of your body, namely circulation, relaxation and elimination. Circulation is crucial if assimilation is to be maintained at high levels. Circulation is helped greatly by some important factors; exercise, especially cardiovascular. Apparently something like 50 or so times as many blood vessels open up when exercising than just sitting. We have about 60,000 miles of vessels, mostly capillary type, so you can imagine the effect of increased blood flow. Massage, hydrotherapy treatments and osteopathic adjustments help because the blood stream carries the rebuilding forces to the body in these channels. Background: Chronic Venous Disease (CVD) is one of the most frequent illnesses in the industrialised countries. Its prevalence within adult population exceeds in some European countries 50%. This accounts for 1.5 till 2.0 % of health care expenditures. Varicose veins (vv) and CVD decrease the quality of life of patients and their families. Venous wall degeneration is common primary cause of CVD. However, the pathophysiologic role of degenerative venodilation has not yet been finally established. In contrast to venous obstruction and venous reflux, venodilation has not been acknowledged by experts as a substrate of CVD. The aim of this study was to examine the role of dilation of Common Femoral Vein (CFV) as possible risk factor for CVD. Methods: In this cross-sectional study a total of 249 right legs (including 181 women and 68 men) were analysed. According to the CEAP classification study population was limited to primary aetiology (EP). DVT (PO) and other secondary and congenital causes of CVD (ECS) were excluded. Subjects with clinical stage 0, 1 or 2 (C0-2) were considered as non-diseased, otherwise (C3-6) as diseased. Right (n=249) and left (n=242) legs were analysed separately. Multiple logistic regression analysis was performed with the aid of SAS System. Results: The risk of CVD was around 10-fold higher in women with CFV diameter exceeding 14.0 mm as compared with those having diameter of 14.0 mm or less (adjusted for age, vv, and CFV-reflux). Based on Receiver-Operating-Characteristics curves, the ability to discriminate between diseased and non-diseased legs was higher for CFV dilation then for CFV-reflux. The curve of association between the CFV diameter and risk of CVD rose monotonically and revealed exposure-effect pattern. In women two threshold values were found for CFV diameter: at 14.0 and 17.5 mm; in men only one: at 17.5 mm. Final logistic model (women) included four covariates: age, vv, CFV-diameter and CFV-reflux duration. A probability of CVD was estimated using these parameters. Increasing body weight and height, BMI and body surface area (BSA) were associated with higher risk of CVD. However, in contrast to body weight and BMI that showed non-linear association, BSA predicted the risk of CVD in a linear manner and the association showed biological gradient. Conclusions: (1) CFV diameter is an independent risk factor of CVD of primary aetiology. Higher diameter values correspond to higher risk of clinically overt CVD. (2) CFV diameter was shown to possibly indicate the risk of CVD progression. (3) A given diameter of CFV should be interpreted in both sexes differently as the normal limits are higher in men than in women. The predictive value of CFV dilation is higher in varicose-free subjects as compared with those with vv. (4) The risk of CVD increases consistently with enlargement of CFV-diameter. In women with CFV diameter exceeding 14.0 mm the risk of CVD is markedly higher as compared with those having CFV of 14.0 mm or less. Thus, in women diameter values greater than 14.0 mm should be considered pathological. (5) The role of venodilation seems not to be restricted to predisposing to development of valvular incompetence. For making a diagnosis of CVD in women more information can be derived from the measurement of CFV diameter than from the measurement of CFV reflux. Venous valve insufficiency and ambulatory venous hypertension seem not to be necessarily involved in development of early stages of CVD. (6) Estimated probability of CVD can be utilised in phlebological evaluation of subjects, and as health index for assessing the extend of disease in the general population. This can be best estimated using four parameters: age, vv, CFV-diameter and CFV-reflux duration. These four predictors bear complementary information about risk of CVD and should be assessed together. Other parameters that have been examined in the study (including refluxes in other venous segments) do not further enhance the accuracy of diagnosis of CVD. (7) BSA is associated with both the risk of CVD and the diameter of CFV. It is hypothesised that the biological mechanism explaining the role of BSA as risk factor of CVD rely on its determining blood-volume and blood-pressure load of peripheral venous system; over time increased venous load may lead to venodilation, venous stasis and in consequence to CVD. BMI and body weight seem to represent those factors less appropriately and are related to the risk of CVD partly due to confounding by age, sedentary lifestyle, social class, immobility or hormone disorders. The risk of CVD in women depends on changes of body weight beneath the value of 80 kg, BMI beneath the value of 25 kg/m2 and body height above 160 cm. Beyond these limits, changes in these parameters do not affect the risk of CVD. (8) For the purpose of research on CVD, material should be analysed separately for each of the genders because of possible differences in the pathophysiology of CVD between the sexes.
