The Health, Labor and Welfare Ministry is considering creating a new framework to support elderly people who live alone and have no relatives they can depend on. The framework would cover hospital admission, entry into nursing homes and post-mortem procedures such as funerals. The ministry aims to launch the program in fiscal 2027 with an eye toward revising the Social Welfare Law next year. Their collaboration—and the nearly 100 interviews with families, doctors, and whistleblowers—revealed this crisis is widespread, persistent, and demands urgent reform. Processing her own trauma, Susie found motivation in community and collective action. Nursing Homes Ireland is warning the sector faces deepening fragmentation, widening inequities and unsustainable pressure on residents, families and staff. A new Insights Paper identifies 10 thematic priorities, including human rights, safeguarding, equitable access and workforce planning. It says a person-centred approach must shape all future policy decisions. To discuss this further, Derrick Lynch was joined by Maurice Harvey, Clare Older People's Council Chair and Madge O'Callaghan, Kilbane resident.

In our third week with our couple in their 80s, we consider options to obtain the most LTC benefits from annuities. If you already own annuities, check for a Chronic Illness rider or LTC rider. If there is one, read it carefully as different rules apply to access enhanced benefits for care. If you need help understanding your rider, ask me for guidance Some pay the LTC benefits tax-free and some don't. Some require your need to be permanent, some don't. Some will enhance benefits for both cognitive and physical needs, and some don't. If you own an annuity (non-IRA) with a good amount of earnings, it can be repositioned to an annuity that can pay potentially three times the account value of the annuity, or more, tax-free when used for care. This eliminates all the taxable gains your current annuity has earned over the years. If your annuity IS your LTC plan, there are wiser ways to use it than simply withdrawing the earnings and paying taxes on the gains. Schedule with me to learn more

Lauren welcomes Gulrukh Mehboob for a discussion about the impacts of relaxed staff training and licensing requirements on nursing homes during the COVID-19 pandemic. Gul is a Fulbright PhD scholar at the University of Iowa College of Public Health, studying health workforce policy, particularly in long-term care settings. • Between March 2020 and September 2021, 19 states reduced CNA training and licensing requirements in an effort to address staffing shortages in nursing homes. • Gulrukh's study found that these policy changes did not improve CNA staffing levels in nursing homes, even though some scholars had argued that strict training and licensing requirements were a barrier to entering the field. • Gulrukh suggests that improving wages, benefits, and working conditions for CNAs may be more effective than reducing training and licensing requirements for addressing staffing shortages in nursing homes. • Her future research will explore the impact of other workforce policies, such as incentive-based programs and wage increases, on staffing and quality of care in nursing homes. Nearly 40 years ago, after repeated reports, articles, and studies highlighting dismal conditions in nursing homes, Congress passed the comprehensive Nursing Home Reform Act, raising the standard of care and expectations for residents of nursing homes across the country. While we are still advocating for the promise of the law to be experienced by all residents, the importance of implementing the provisions is as necessary today as they were when the law was passed. The federal spending bill calls for a reduction in Medicaid spending by an estimated $930 billion over the next decade. The shared federal-state health program currently provides insurance coverage to more than 70 million Americans with low incomes. Effects of the cuts will not only be felt by low-income Pennsylvanians, but through the state's hospitals and even nursing homes. The Trump administration is holding up nearly $7 billion dollars in federal funding that Congress had already approved for educational programs. Pennsylvania and other states were slated to get the money on July 1st. The State Department of Labor and Industry continues to offer support to workers impacted by the idling of the Cleveland-Cliffs Steelton plant. Assistance has included transition information and a job fair. Two Pittsburgh universities are among those working to understand the impact artificial intelligence is having on jobs. The study aims to get ahead of technology shocks that may change how people work. Developers in Pennsylvania who want to skip the line for certain environmental permits can apply through a new program from the state Department of Environmental Protection called Streamlining Permits for Economic Expansion and Development, or SPEED. A Dauphin County judge collapsed while riding his bike and was taken to Penn State Hershey Medical Center, where he was pronounced dead last Friday. Judge Michael J. Smith was a Magisterial District Judge of 20 years. In Centre County, efforts to strip control of a troubled rural water company from its longtime family owner have hit another roadblock, again delaying a permanent fix to years of unreliable service. Rock Spring Water Company and owner J. Roy Campbell recently rejected a $65,000 offer from nearby State College Borough Water Authority to buy the private system, according to reporting by our partners at Spotlight PA. Nine people were rescued from the Swatara Creek by the Hershey Volunteer Fire Department on the 4th of July. It happened near the Hershey Dam, a low-head dam operated by Pennsylvania American Water. We're getting bad news from what seems like every direction these days. Governments and media are all talking about wars and destruction. But this is not new. We've been through these kinds of events before, too many times, and we're still here and moving forward. We can't let panic, wars and market crashes control our choices. These thnigs happen without our input. We can control our clarity, courage and conviction to stay on course. If you've been considering planning for future extended care needs, or if you've been working on your plan, don't stop now. Too many find a reason to pause and then forget their reasons for planning in the first place. Then, 10 or 20 years go by, and something happens to them or someone close to them, and they restart their search. The need for better palliative care in nursing homes is significant. Consider this: the majority of the 1.4 million adults residing in U.S. nursing homes grapple with serious illnesses, and roughly half experience dementia. Many also suffer from distressing symptoms like pain. In addition, about 25% of all deaths in the United States occur within these facilities. Despite these substantial needs, specialized palliative care beyond hospice is rare in nursing homes. Furthermore, only about half of nursing home residents nearing the end of life receive hospice care. So, how can we improve palliative care for individuals in nursing homes?
