Investors in the seniors housing and care space have always seemed to make skilled nursing a second-thought customer. Now, however, the demographics have become so positive, even skilled nursing is enjoying more time in the spotlight — and it should for a long time to come. That's according to Bill Kauffman, a senior principal at the National Investment Center for Seniors Housing & Care, who keeps a close eye on skilled nursing matters. “The optimism is certainly here, for sure,” he told McKnight's Long-Term Care News Executive Editor James M. Berklan at the recent NIC spring conference in San Diego. “Especially over the next 10 years.” The US Department of Housing and Urban Development has traditionally been a strong capital provider for skilled nursing operators, Kauffman noted. But now that most major pandemic concerns have subsided, real estate investment trusts — including public ones — are also becoming more active. With interest rates remaining high, the cost of capital remains the big question mark, Kauffman explained. But, he quickly added, he thinks “it's just a matter of time” before skilled nursing funders take advantage and build more to accommodate the historic crush of baby boomers. “You have a supply and demand situation for private pay seniors housing and have a supply and demand situation for skilled nursing — and they're both positive,” he explained. Listen in to hear more on what Kauffman thinks the key factors are and what moves providers must make to get more capital — and what they must spend at least some of it on to succeed.

Skilled Nursing care, also known as Post-Acute Rehabilitation, is for those who need short-term care following an injury, surgery, or illness. The goal with this level of care is to successfully transfer patients from hospital to home, or senior living community, by providing the tools and resources for each phase of recovery. A stay at a skilled nursing, rehabilitation, and post-acute care center is meant to be a transitional period to help patients recover and return to their everyday lives. Our Co-Host Ron had shoulder surgery recently and needed this type of care until he could regain use of his "good-arm".  As you probably know from an episode we did (again with Ron as the guest) very early in our podcasting to discuss his life with a disability.  You see, Ron was hit be a motor vehicle as a child and lost the use of one of his arms. In caring for our communities, a carefully designed Care Continuum Blueprint becomes the roadmap to enhanced population health outcomes—a testament to the profound impact of integrated care and strategic coordination.  There has never been a more compelling time to adopt a system of care based on population health management. The COVID-19 pandemic revealed substantial health disparities and compels us to take action. The population is aging, and the Medicare insolvency crisis is looming. Now is the time to move away from fee-for-service care and toward an approach that prioritizes quality, outcomes, and affordability for all populations. In this week's episode, we interview Dr. Mark Angelo, a senior administrator of a large accountable care organization and a leader in population health and palliative medicine.  He is the author of the new book Caring for Our Communities: A Blueprint for Better Outcomes in Population Health, that provides tactical guidance for developing effective population health programs and explores value-based care models. Dr. Angelo is an inspirational leader to the health value movement, providing a road map for creating an equitable, outcomes-focused system, using the right resources to nurture the health of our communities. Dr. Mark Angelo currently serves as CEO and President for the Delaware Valley ACO (DVACO). In this role, he oversees clinical strategy and operations, including quality, population health pharmacy, clinical integration, care coordination, post-acute networks and practice transformation. In addition to serving patient communities as an executive with the ACO, he is a practicing palliative care doctor who continues to see patients.  DVACO has participated in the MSSP since 2014 and also works with commercial and Medicare Advantage payers in an effort to grow and expand the mission of value-based care in the Greater Philadelphia area. The Race to Value Podcast

In the Race to Value, we must recognize that quality of life is the ultimate currency of healthcare, and this aim is all the more important in senior living facilities.  Transforming health outcomes for skilled nursing and senior living populations is not just a goal; it's a commitment to providing the care and dignity our elders deserve.  This week, we profile a leader in the value movement who leads a company on a mission “to improve the health, happiness, and dignity of senior living residents”. We are joined by Mark Price, CEO of Curana Health – a leader who lives by the mantra that “extreme passion” is the single most important ingredient to reform the American healthcare system. Curana Health is a provider of value-based primary care services exclusively for the senior living industry, including in nursing homes, assisted/independent living facilities, CCRC/life plan communities and affordable senior housing communities. Curana Health serves more than 1,100 senior living community partners across 30 states and participates in the MSSP ACO, ACO Reach and Medicare Advantage programs with CMS. Backed by more than $300M in venture capital funding, the organization is poised to disrupt care delivery in senior living on a meaningful scale through innovative care models and applied analytics. In this episode, you will learn about how to transform health outcomes for skilled nursing and senior living populations through extreme passion.  We cover such topics as how to leverage APMs such as MSSP and ACO REACH in the senior living setting, the performance results of Curana Health across their value-based portfolio, technology innovation, palliative care, the state of the nursing home industry, and future trends in the shift to home-based care delivery. Episode Bookmarks: 01:30 Introduction to Mark Price, CEO of Curana Health. 03:45 An estimated 27M more people are aging into the 75+ cohort through 2050, resulting in rising age and higher health acuity levels of residents moving into senior living. 05:00 Curana Health has achieved a 39% reduction in 30-day hospital readmissions and a 37% reduction in total hospital admissions among Medicare Advantage I-SNP members. 06:00 “There are many subsectors in the industry where value-based care can succeed.  The important thing is ensuring that your people have an extreme amount of passion for making it work.” 07:00 Founding story of Curana Health based on how we would want our loved ones to be cared for at the end of life. 08:45 The majority of Americans will spend some time in senior living or skilled nursing in the final years of their life. 10:00 Elite Patient Care ACO performed in the top 1% of ACOs in its first year of operation, achieving PBPY savings amount of $2,235—the highest PBPY for any first-year MSSP ACO since 2012. 11:30 Curana Health also has one of the top performing ACO REACH and risk-based MA I-SNP programs in the country. 11:45 “Our core business is not a payment model. It is a clinical model that produces health outcomes which, in turn, enables affordability as well.” 13:00 Developing a population health playbook for the senior living space. 14:00 Success in developing a level of clinical integration within a senior living facility that is now owned by the company. 15:00 MA Institutional Special Needs Plans (I-SNPs) are designed to meet the needs of people living in long-term care settings such as long-term care nursing, skilled nursing facilities, and inpatient psychiatric facilities. 16:45 Facilities are taking an ownership position of MA plans for senior living and skilled nursing residents. 17:00 Mark provides perspective on I-SNPs and how the Curana Health clinical model is achieving results to improve clinical outcomes. 18:30 Performing well by recognizing the commonality between MSSP, ACO REACH, and Medicare Advantage. 20:00 How CMS and CMMI is incorporating innovation to value-based payment models (e.g.SNF 3-Day Rule Waiver).

Sometimes after a hospital stay to recover from illness, medical incident or injury, a person isn't in position to go home yet. They are stable enough to leave the hospital, but the home setting would be too much too soon. Reasons may include, there is not anyone at home that is able to care for them, the home isn't set up yet for them yet or maybe a move to senior living is in order but someone isn't sure where to go. A short stay in rehab can allow the person some time to get stronger, while the family sorts out figuring out what is next. If the person had 3 nights in the hospital, Medicare will cover rehab as long as the person is showing improvement. In this episode I speak with an expert who shares what you can expect from a rehab stay and how to make the most of the time there. If you have questions about any level of senior living, check out my new video series HERE. Michael Greenlee and Hannah Vale are joined by Shirley Gamble. She is a Division Director at BAYADA Home Health, which offers Skilled Nursing, Home Health, Hospice, Assistive Care, Habilitation, Behavioral Health and more to patients in 24 states. She started her career in the freelancing market as an editor and proofreader. Her journey within home care began with a non-profit offering home care services in a dozen counties within Missouri. She then moved to a larger organization where she was a Director of Branch Operations for a single-site. Shirley dispels the assumption that compliance equals quality, which is not the only component to true quality. Quality also includes financial performance, client and employee surveys, and communication with clients and employees.  Shirley gives useful advice on how to assess and improve processes within a home health agency: care, reimbursement, outcomes, and reporting systems. To tackle service-related problems, administrators must first know there is a problem and use reporting systems to filter results. Training is usually the expected course of action for a service-related concern but there are other important ways to make data meaningful.  Perhaps most important is that activities should be communicated in writing or via a webcast (and that the response should be formatted and look pleasant to the eye!) It is also important to share the numbers to show the volume of responses and to show those whose input wasn't chosen why. Shirley gives simple steps an agency can use to improve employee engagement and gain feedback to help an agency increase in QUALITY!

On this revisited episode of Beyond the Balance Sheet, we are joined by guest Anna Hall, the creator of The Purpose Equation®. Throughout this episode, we discuss the importance of purpose and defining your how and your why. Anna breaks down what purpose is, and how people can go about finding their purpose through her signature equation. Anna says that  knowing your purpose equation can determine how to move through life. She  believes people's purpose(s) can shift throughout their lifetime through different milestones. Later in the episode, Anna provides excellent advice for parents supporting their children to determine the child's purpose by getting  curious about their interests. Tune in today to learn about individual purpose and how to create meaning and joy in everyday life.     IN THIS EPISODE:     [1:50] What does the word “purpose” mean?   [3:23] What is the purpose equation? What are the components?   [6:37] How does the purpose equation translate to practical things?   [13:22] Is there a natural time when people re-define their purpose?  [16:15] What is the linkage between mental wellness and a sense of purpose?  [23:53] How do families and parents talk to their children about purpose?  [28:55] What is Anna's purpose?     KEY TAKEAWAYS:     Purpose is not something that you do. What you do is an output of who you are and your why.   Everyone has a purpose. Everyone has the power within themselves to make a difference in the world.   What does your child love? A parents' job is to help unpack WHYs in the activities your children love. Focus on having your child figure out WHO they are, not what they will do. Try to remove SHOULD.     BIO:  Anna was a teenager when she volunteered at a nursing home in her hometown of Hanover, NH.
