 Najpierw zakwalifikowanie działania prezesa Trybunału Konstytucyjnego Bogdana Święczkowskiego w kwestii niedopuszczenia osób wybranych przez Sejm na stanowisko sędziów TK jako niedopełnienia obowiązków lub przekroczenia uprawnień, następnie siłowa interwencja policji – taki możliwy mechanizm dalszego konfliktu wokół TK przedstawiła w Radiu Wnet sędzia Anna Gąsior-Majchrowska. Rzeczniczka prasowa stowarzyszenia Prawnicy dla Polski zastrzegła, że jest to prognoza budowana na podstawie obserwowanych poszlak, jednak oceniła ryzyko takiego rozwoju sytuacji jako wysokie. 

Spór wokół Akademii Wymiaru Sprawiedliwości może być elementem znacznie poważniejszego konfliktu o Trybunał Konstytucyjny – ocenia Piotr Matczuk. Witam Państwa, nazywam się Jarosław Drożdż, pracuję w Centralnym Szpitalu Klinicznym Uniwersytetu Medycznego w Łodzi, skąd nagrywam podcast Kardio Know-How. W tym odcinku kontynuuję omawianie badań z ESC 2026. TARGET-CTCA dotyczy bardzo częstej sytuacji klinicznej — pacjenta z bólem w klatce piersiowej, u którego po wykonaniu EKG i seryjnych oznaczeń troponiny wykluczono zawał, ale troponina pozostaje umiarkowanie podwyższona (powyżej 5 ng/l, lecz poniżej 99. percentyla). W tej grupie ryzyko zawału lub zgonu jest większe, a około 2/3 pacjentów ma w CTCA lub koronarografii cechy choroby wieńcowej. Badanie TARGET-CTCA, opublikowane równolegle w NEJM (https://www.nejm.org/doi/10.1056/NEJMoa2608903), randomizowało 3170 pacjentów do standardowej opieki lub rutynowej angio-TK tętnic wieńcowych (CTCA). Mimo że CTCA skutecznie wykrywała i klasyfikowała chorobę wieńcową, po obserwacji sięgającej 6 lat nie wykazano istotnej redukcji głównego punktu końcowego ani zgonów czy zawałów, a rewaskularyzacji było tyle samo w obu grupach. Ważnym wyjaśnieniem może być fakt, że populacja badania była mniej obciążona ryzykiem, niż sugeruje określenie „intermediate risk”, a jedynie 9% pacjentów miało istotną dynamikę troponiny (delta-cTn ≥3 ng/l), dlatego wyników nie należy automatycznie odnosić do pacjentów z typową dławicą, dynamicznymi zmianami troponiny czy wysokim prawdopodobieństwem CAD. Zaletą CTCA jest natomiast możliwość wykrywania licznych zmian pozasercowych — łącznie istotne znaleziska stwierdzono u 9,9% pacjentów, m.in. guzków płuca, rozedmy, włóknienia, rozstrzeni oskrzeli czy powiększonych węzłów chłonnych — co może częściowo tłumaczyć obserwowany, choć statystycznie nieistotny, spadek śmiertelności całkowitej. CTCA bardzo mocno wpływała również na leczenie: przy istotnej CAD stosowanie statyn wzrastało do około 90%, a ASA do 79%, jednak ta intensyfikacja terapii nie przełożyła się na redukcję zawałów ani zgonów. Badanie pokazuje więc, że sama informacja o obecności miażdżycy nie poprawia rokowania, a „diagnostic momentum” — wykonywanie badania tylko dlatego, że można dzięki niemu coś wykryć — nie jest wystarczającym argumentem za rutynowym CTCA u każdego pacjenta. Jednocześnie CTCA okazała się bezpieczna w odpowiednio wyselekcjonowanej populacji: reakcje na kontrast wystąpiły u 0,3%, ostre uszkodzenie nerek u 0,1%, a łącznie zdarzenia związane z badaniem u 0,4% pacjentów. Najważniejszy wniosek praktyczny jest taki, że rutynowe CTCA po wykluczeniu zawału nie daje dodatkowej korzyści prognostycznej całej populacji, natomiast selektywna diagnostyka oraz odpowiednio wczesne wdrożenie leczenia przeciwmiażdżycowego i modyfikacja czynników ryzyka mogą być wystarczające — decyzję o wykonaniu CTCA należy więc pozostawić ocenie klinicznej, zamiast traktować je jako obowiązkowy element ścieżki diagnostycznej.Szczegółowy TRANSKRYPT do odcinka.Podcast jest przeznaczony wyłącznie dla osób z profesjonalnym wykształceniem medycznym.

Każda duża afera ma moment, w którym przestaje być luźnym zbiorem nazwisk, spotkań i przelewów, a staje się opowieścią o państwie. Dla Zondacrypto nadszedł on w piątek. Donald Tusk wszedł na sejmową mównicę z protokołami przesłuchań i połączył rozrzucone elementy w jeden, wyjątkowo ponury obraz. Według zeznań cytowanych przez premiera Przemysław Kral miał umówić się ze Zbigniewem Ziobrą na 2 mln zł w zamian za rozwiązanie swoich problemów z prawem. Były minister miał znać akta jego spraw i obiecywać, że po powrocie do Ministerstwa Sprawiedliwości „ukręci łeb" prowadzonym sprawom. Pieniądze miały trafić do fundacji Ziobry, a szczegóły miała ustalać Patrycja Kotecka. Ziobro przekonuje, że chodziło o legalne, bezinteresowne darowizny.  Ot, człowiek z poważnymi kłopotami daje niemal pół miliona byłemu prokuratorowi generalnemu i niczego w zamian nie oczekuje. To z pewnością trzyma się kupy.  Na tym historia się nie kończy. Kral miał usłyszeć od tajemniczego „pana X", że po zwycięstwie Karola Nawrockiego będzie można załatwić mu prezydenckie ułaskawienie. Zapłatą miały być pieniądze na reklamy, kongres CPAC w Rzeszowie i imprezę w USA. Nawrocki odpowiada, że jego podpisem nikt handlować nie może. Zapewne. Dlaczego zatem ludzie krążący wokół prawicy sądzili, że mogą obiecywać bezkarność w imieniu przyszłego prezydenta? Zondacrypto pokazuje cały mechanizm: pieniądze otwierają drzwi do polityki, a znajomości rodzą obietnice ochrony. Kral zaczął mówić i ten mechanizm pęka razem z jednością prawicy. Morawiecki atakuje Kaczyńskiego za związki jego ludzi z Zondą. A Nowogrodzka przekonuje, że aresztowany szef PKOl Radosław Piesiewicz to człowiek byłego premiera, choć my go znamy jako zdeklarowanego sasinowca.  Tusk wykorzystuje ten moment, by otworzyć drugi front. Gdy PiS liczy straty po zeznaniach Krala, premier wyważa drzwi do Trybunału Konstytucyjnego i przeprowadza przez Sejm kandydaturę swego współpracownika Macieja Berka. Jeszcze niedawno rządzący obiecywali odpolitycznienie TK. Teraz wysyłają tam człowieka prosto z rządu, bo tłumaczą, że sytuacja jest wyjątkowa. Nawrocki rozważa odmowę odebrania ślubowania — na razie szuka pretekstu. Wie, że zaślubiny Berka mogą doprowadzić do odbicia Trybunału z rąk ludzi PiS.

Sprawa Zondacrypto nabrała tempa w tym tygodniu, ale przyszły ma być jeszcze gorętszy. To co wiemy teraz, wskazuje na to, że szef Zondy Przemysław Kral w swoim biznesie bardzo mocno stawiał na finansowanie otoczenia PiS. Wpłaty na fundację Ziobry, spotkania w Budapeszcie i na Ibizie, negocjacje z Tomaszem Sakiewiczem w sprawie kupna części telewizji Republika, do tego finansowanie prawicowych imprez wyborczych. Chociaż jest też kilka awantur o Romana Giertycha i jego zaangażowanie w sprawę.    Rząd przyjął budżet i sytuacja wygląda tak - Trybunał Konstytucyjny ma dostać Macieja Berka i nie dostać kasy. Ponieważ prezes TK nie chciał gadać o pieniądzach z rządem to rząd powiedział, "a figę dostaniesz". W budżecie nie ma ani złotówki na Trybunał. Za to jest Berek, którego Tusk wysyła do TK. Koalicjanci na razie się trochę na ten pomysł obrazili, ale jak przyjdzie co do czego, to wszyscy grzecznie zagłosują.
