In this episode, I sit down with Wendy McSparren, licensed clinical social worker, group practice owner, and co-founder of Admirra, a CRM built specifically for mental health practices. Wendy's story is one of my favorite kinds, she didn't wake up one day and decide to build software. She was living inside the chaos of running an insurance-based group practice during the pandemic and simply needed a better way to survive it. When the tools on the market didn't fit the reality of how therapy practices actually operate, she and her husband (a software engineer) built what they couldn't find. If running a group practice feels like herding highly educated cats, this episode is going to feel like someone finally put words to what you've been living. I'm joined by Amanda Esquivel, a Texas group practice owner who's scaled to 50 clinicians across seven locations—and she's breaking down why most “staff problems” aren't actually staff problems. They're leadership-role problems.Amanda names the four roles practice owners bounce between when growth accelerates: babysitter/manager, bottleneck, firefighter, and mentor-leader. And she's honest about where she's had to course-correct—because when everything runs through you, you don't just get busy. You get stuck. Your team gets stuck too.We also talk about what employees are really reacting to when they complain about low pay, lack of tools, unrealistic expectations, and “promises” that don't materialize. Amanda shares the transparency practices she uses to educate rather than defend—like showing where the money actually goes, tracking attendance metrics without shaming, and building a training hub so clinicians aren't reinventing the wheel. The throughline is simple: your job isn't to be everyone's emergency contact. Your job is to build people and systems. If you've been living in manager mode, exhausted, resentful, and constantly “on,” this conversation will help you shift back into leadership that develops your staff instead of babysitting them.In this episode, you'll learn:The four roles group practice owners cycle through—babysitter/manager, bottleneck, firefighter, and mentor-leader—and how to identify which one is running your practice right now.How to shift out of constant firefighting and bottlenecking by building systems, metrics, and delegation structures that develop staff instead of controlling them. What staff complaints about pay, tools, expectations, and follow-through are really communicating—and how transparency and education reduce resentment and turnover.Practical leadership tools that support retention and accountability, including structured one-on-ones, attendance and caseload metrics, shared training hubs, and role clarity. If you're ready to move out of manager mode and into grounded, sustainable leadership, start by grabbing the bonus resources that support this kind of systems-based growth. If you're still clarifying how your practice should be structured, the free practice guide will help you name what needs attention next. For supervisors and practice owners who want clearer expectations and less daily friction, the Supervisor Checklist gives you a concrete place to begin. And if you want ongoing CE credit, leadership support, and real-world training you can actually apply, the Step It Up Membership is where this work deepens over time. Get your step by step guide to private practice. Because you are too important to lose to not knowing the rules, going broke, burning out, and giving up. #counselorsdontquit.

In today's Ask Allison, we're answering a common question: How do you increase your income in private practice without starting a group or building new programs? There's a lot of talk about "passive income," but most of those models require ongoing marketing and a real love for creating content. So instead, we focus on the most straightforward path for solo clinicians: raising your fees to premium rates. I'll walk through how private-pay pricing can significantly increase your annual income without adding hours or managing a team. By the end, you'll have a clear, sustainable way to grow your income while keeping your practice simple and aligned. Sponsored by TherapyNotes®: Looking to switch EHRs? Did you work your ass off to hit a huge milestone, and now you feel more stressed, tired, and overwhelmed than before? The success hangover is real, and it's stopping ambitious group practice owners and mega successful solo practitioners like you from enjoying what you've built. You finally scale to multiple six figures, fill your calendar, buy that dream home, and instead of celebrating, you're buried under more responsibility, wondering, "Wait, is this it?"In this episode, I'm breaking down why this happens and what you actually need to do about it. We're talking about the systems and delegation that will save your sanity, the difference between actual rest and just numbing out on your phone, and how to stop being trapped in that buzzy, exhausted cycle even after you've "made it." Chats with group practice employees are more fun when you're not their practice owner! Maybe a bit more revealing, too. Such is the case with my guest, Sierra Waller, LCPC, an early-career therapist working at Health In Tandem, a small group practice in Chicago. Apologies if you've landed here hoping for a gripe-and-gossip session. Instead, Sierra and I dig into her experiences as a newer therapist in a group practice setting. This trajectory from grad school straight into private practice has become increasingly common. Subsequently, there's a learning curve to be aware of on both sides of the owner/therapist dynamic. GUEST BIO Sierra Waller is a Licensed Clinical Professional Counselor at a small group practice in Chicago. She primarily works with clients navigating anxiety, stress, life transitions, and relationship issues. Guided by the belief that mental health deserves the same care as physical health, Sierra fosters a collaborative, non-judgmental space where clients can explore patterns, identify strengths, and take meaningful steps toward growth. I'll start by stating the obvious. Being a group practice owner or group practice therapist in "unprecedented times" is a challenging endeavor. Our people and profession are struggling under the pressures of late-stage capitalism, antagonistic politics, and rage-bait algorithms —the same forces that affect our clients. I'll follow up the obvious with a hard truth. Some of the ills are coming from inside the house, er, practice. I believe in the good and essential aspects of the group practice model. I also believe that the only way out is through. And together. As does Gabrielle Juliano-Villani. She's a business mindset coach and consultant who helps therapy practice owners scale, sell, and thrive without burning out. The solution to all (okay, most) of the challenges facing group practices is embedded, quite cleverly, in the name. Owner, admin, or therapist, your individual participation informs the altruistic and financial goals of the entire practice. And also… Groups are powered by people––real, live humans with individual expectations, convictions, experiences, and traumas. Samantha Perry, LCSW, encourages folks on both sides of the "old school/new school" divide to get comfortable with transparency and vulnerability. She also recommends committing to intrapersonal healing work, especially around our faulty strategies for addressing shame (excessive caretaking, aversion to feedback, etc) or how those strategies sabotage the work environments we're trying to create! GUEST BIO Samantha Perry (she/her) is an LCSW, working at LifePath Therapy Associates in Chicago as the Coordinator of Group Therapy and Training. She is a certified NARM therapist, and loves supporting folks who want to build their capacity for connecting to themselves and others, create a new relationship to their body, or develop freedom and flexibility around their experiences of shame. She specializes in working with folks healing from religious and complex trauma. When she's not learning or working, you can find her quilting, dancing, or writing, where she is learning to embrace her own perfectionist parts. *** Join the Group Practice (R)evolution! Good people create great group practices. Eventually. I tacked that adverb on the end not to undermine anyone's ability to craft cultures of transparency and inclusivity, but to acknowledge that the whole shebang requires constant work. Amy Zajakowski Uhll, LCPC, founder and director of Chicago Center for Integration and Healing (CCIH), is an excellent example of an accomplished leader and eternal learner, the latter contributing so much heart to the former. Amy and I discuss her 30-year career evolution from nervous newbie to seasoned practice owner.
