The Rankapole family who lost their daughter in a crash which killed three learners and a driver at Ga-Mashashane in Limpopo, say they are failing to come to terms with her passing. Both vehicles, a bus and a minibus taxi were carrying learners. Several learners were injured and were hospitalised at various health care facilities. The Education Department says it has initiated psychosocial support to the affected families. The SABC News crew visited one of the bereaved families and compiled this report. A 49-year-old Nepali woman has died in a crash at Lalor, Melbourne. On Friday, 19 June, after 6 am, emergency services were called after two cars collided at the intersection of Dalton Road and Burton Street. Victoria Police say the female was treated by paramedics but died at the scene. The 52-year-old male driver of the same vehicle was arrested at the scene. Late on a quiet Sunday night in May, gardaí in Limerick arrived on the scene of what they expected would be a routine call-out to a road traffic incident.Instead they were met with an extraordinary scene.In one of the damaged cars they found a dead man on the back seat. Mark Ezzell, Director of the North Carolina Governor's Highway Safety Program, was in his early 20s when his father, Sen. Jim Ezzell, died on Capital Boulevard in Raleigh on the eve of session starting in 1991. That loss not only changed Mark's life forever, it also created an opening in the North Carolina Senate for a young House member named Roy Cooper. But this story is about more than politics, it's also about perseverance and grace. Senator Ezzell, who lived with cerebral palsy, and his wife made the intentional decision to adopt a child with a disability. At age eight, Mark was adopted from foster care and welcomed into a family that saw his potential, not his limitations. 19 year old Davion Flanagan's family and 20 year old Dominic Russo's family want to know what the hell happened that morning. How could 17 year old Mackenzie Shirilla drive her car straight into a brick building at 100 mph killing their two boys? How could she be the sole survivor? This is the case of Mackenzie Shirilla who used a high speed, intentional motor vehicle collision to end the lives of her longterm boyfriend Dominic Russo and his friend/roommate at the time, Davion Flanagan. In Lonoke County, Arkansas, a judge found that investigators' handling of evidence in Aaron Spencer's murder case was "so egregious" the prosecution had to be dismissed. The dashcam SD card that could have told the whole story was handled in violation of department policy and lost. Spencer, who killed the man accused of crimes against his daughter, was running for sheriff against the incumbent whose deputies mishandled the evidence. The court used the word "coverup."In Strongsville, Ohio, Mackenzie Shirilla's car did what she wouldn't — it recorded the final seconds before she drove into a brick building at close to a hundred miles per hour, killing Dominic Russo and Davion Flanagan. She never talked to police. On monitored jail calls, she and her mother used a coded language that investigators cracked, allegedly revealing a plan to fabricate a seizure defense.On the Carnival Horizon, the FBI built a case against Timothy Hudson using DNA that points to him with 120 sextillion-to-one certainty in the death of his eighteen-year-old stepsister, Anna Kepner. But the lead agent couldn't confirm under oath whether DNA was collected from Anna's neck — the area linked to the cause of her death. Building a double murder case without a confession, without testimony, and without a single statement from the defendant requires investigators to let the evidence speak for itself. In Mackenzie Shirilla's case, the evidence was devastating.A car's data recorder showed the accelerator pushed to full capacity with zero braking, approaching a hundred miles per hour aimed at a building. Surveillance footage captured the vehicle driving normally through a residential area before an abrupt, deliberate acceleration. Shirilla had driven that dead-end route days earlier — a road she didn't normally take. Weeks before the crash, she'd told her boyfriend, Dominic Russo, she would "crash this car right now." Russo and their friend Davion Flanagan were pronounced dead at the scene.The behavioral evidence was equally striking. On monitored jail calls, Shirilla and her mother used a coded language investigators cracked — allegedly revealing Shirilla asking if they could claim she'd suffered a seizure. That claim became the defense's central argument: that a condition called POTS had caused Shirilla to black out. No medical records or expert testimony confirmed the diagnosis.
