Mental shortcuts, such as bias, are essential to our survival. Understanding that unconscious bias is a mental shortcut makes it easier to examine it in ourselves, instead of simply rejecting the idea that we might harbor it. If it's a moral failing, well, clearly, I don't want to admit that possibility about myself. If we are willing to look at ourselves, we can dig into what unconscious biases we may have. We can consider where they started. Without punishing judgment of ourselves or our early influences. My most recent guest, David Hoffman, described the article he had written with Helen Winter, called “Follow the Science: Proven Strategies for Reducing Unconscious Bias.” You can find it here: https://blc.law/wp-content/uploads/2023/04/Follow-the-Science-Proven-Strategies-for-Reducing-Unconscious-Bias-Final-2023-04-15.pdf Do you have comments or suggestions about a topic or guest? A message came through Dante and Dory's website contact form. It was detailed, critical, and specific- complaining about pricing, construction timelines, employee compensation, and the value of shopping local. But something felt off. The team ran the message through an AI detector: 100% AI-generated. In this episode, Candace unpacks the entire experience - from the initial reaction to the deep dive into what it could be (competitor? phishing probe? mass AI blasting? real person hiding?). And more importantly, she shares what Dante and Dory's is doing differently because of it. She shares:
On February 17, nine backcountry skiers were taken by an avalanche at Frog Lake, in the Northern Sierra Nevada near Truckee, California. Since then, the sense of loss and confusion over what happened has been overwhelming. This accident has impacted everyone in the Truckee and Lake Tahoe community on some level. Because of pending investigations, there are so many unanswered questions that will remain unanswered for a long time.As the community struggles to understand, people around the world continue to die in avalanches, including our friend, Stratton Matteson, who was taken by an avalanche in British Columbia a week after the Frog Lake accident. A gut punch made worse by finding out only minutes after we recorded the recap of our trip to Frog Lake. A trip to Frog Lake ending only 48 hours before the accident. Traditionally, these moments were never treated as ordinary. In this episode, Dylan is joined by Jyotishi and Ayurvedic practitioner Laura Plumb, Vaidya Dr Krishna Raju, Vaidya and medical astrologer Dr Harsha Raju, and mantra teacher Purnesh. Together they explore eclipses through the lenses of Ayurveda, Jyotish and mantra śāstra. They speak about Rahu and Ketu, the difference between solar and lunar eclipses, why digestion and prāṇa are considered more sensitive during these periods, and why eclipses have long been used as powerful windows for mantra and inner practice. 🧭 REBEL Rundown 📌 Key Points 🧠 We don’t know what we don’t know: Low experience can inflate confidence; true expertise usually brings humble certainty.🏥 ED relevance is universal: From central lines to transvenous pacing, over- or under-confidence shows up at every level—intern to seasoned attending.🧩 Metacognition matters: Accurate self-assessment is a clinical skill; reflection + feedback loops keep us calibrated.🛠️ Practice beats bravado: Skill decay is real; deliberate practice and HALO (high-acuity, low-occurrence) refreshers protect patients.🤝 Psychological safety ≠ niceties: “Confident humility” enables questions, feedback, and better resuscitation decisions—especially under uncertainty. Click here for Direct Download of the Podcast. 📝 Introduction Welcome to REBEL MIND—Mastering Internal Negativity during Difficulty. In this series, we turn the same critical lens REBEL EM uses for literature inward—into mindset, leadership, and psychological safety—so we can deliver better care outward to patients and teams. In this episode and blog post, hosts Mark Ramzy and Kim Bambach (Assistant Professor of Emergency Medicine, The Ohio State University) explore a deceptively simple question: How accurately can we assess our own performance? The answer hinges on a classic cognitive bias that touches all of us in emergency medicine. 🧾 Paper Kruger J, Dunning D. Unskilled and unaware of it: how difficulties in recognizing one’s own incompetence lead to inflated self-assessments. J Pers Soc Psychol. 1999 Dec;7 PMID: 10626367 Cognitive Question How accurately can we assess our own performance? 💭 What is the Dunning-Kruger Effect? The Dunning–Kruger Effect is a cognitive bias where:Lower-skill individuals tend to overestimate their competence, andHigher-skill individuals often underestimate theirs.Translation for the busy clinician: early on the learning curve, confidence spikes (“Mount Stupid”) because we don’t yet see the complexity. As experience accrues, confidence dips (“Valley of Despair”) with growing awareness, then rises again—grounded in nuance and humility.Key insight: True expertise ≠ louder certainty; it’s often quieter, more curious, and more collaborative. How It Applies to the Emergency Department Procedures (e.g., central lines, TVP): Watching a 5-minute video creates “I got this” energy—until the wire won’t pass, the patient thrashes, or you hit carotid. Competence includes troubleshooting in context.Skill Decay is Inevitable: If you haven’t done a chest tube or a TVP in months, you’re not as sharp as last time. Without deliberate refreshers, you drift below the safe-performance line.Everyone’s a Novice Somewhere: New disease entities, evolving algorithms, new tools (POCUS, decision support) mean even attendings routinely re-enter novice zones.Feedback Blind Spots: Lower performers can both overestimate their skills and resist feedback—while many high performers (particularly women, per discussed literature) undervalue their abilities.Culture is Clinical: The ED demands decisive action amid uncertainty. Psychological safety + confident humility lets teams surface alternative diagnoses, challenge momentum, and correct course fast. ⏩Immediate Action Steps for Your Next Shift Run a 60-second debrief on two casesWhat went well? What would I do differently next time? Write one improvement you’ll test today.Play “What if the opposite were true?”Anchored on “lumbosacral strain”, Ask, What if fever/incontinence appears? How does that change my path?Solicit 360° micro-feedbackAsk a nurse, resident, and peer: “One thing I did well; one thing to improve.” Say “thank you,” not “but.”Schedule a HALO refresher this weekPick one high-acuity, low-occurrence procedure (TVP, cric, thoracotomy). Do a 10-minute mental model + equipment walk-through; book sim time if available.Adopt a pre-procedure pauseIf X goes wrong, I’ll do Y. Name two likely failure modes (e.g., “wire won’t advance,” “delirium/agitation”) and your first corrective step.Language shift on shiftSwap “I’m sure” → “I’m reasonably confident, here’s my plan B.” Invite input: “What am I missing?” Conclusion The Dunning–Kruger Effect isn’t a moral failing; it’s a predictable human pattern that every clinician rides—often multiple times per day in the ED. The antidote is metacognition: routine reflection, explicit debiasing, deliberate practice, and feedback within a psychologically safe culture. 🚨 Clinical Bottom Line Competence is quiet and curious. The more we know, the more we recognize what we don’t—and the better we become at caring for patients and each other. Further Reading Dunning D, Kruger J. Unskilled and Unaware of It (1999). Luke 18:9-14 - He [Jesus] also told this parable to some who trusted in themselves that they were righteous, and treated others with contempt: “Two men went up into the temple to pray, one a Pharisee and the other a tax collector. The Pharisee, standing by himself, prayed thus: ‘God, I thank You that I am not like other men, extortioners, unjust, adulterers, or even like this tax collector. I fast twice a week; I give tithes of all that I get.' But the tax collector, standing far off, would not even lift up his eyes to heaven, but beat his breast, saying, ‘God, be merciful to me, a sinner!' I tell you, this man went down to his house justified, rather than the other. For everyone who exalts himself will be humbled, but the one who humbles himself will be exalted.”

Episode 28 with Boston-based artist Adam O'Day. Adam paints across genres and scales up to building sized murals all across the region! He shows his work widely and in 2014 his painting was chosen for the “portrait of a city” competition in Boston! We recorded this episode from his studio in an old shoe factory in Abington, Massachusetts, and we cover its potential haunting, his move from naval engineer to full time artist, the role of collaboration, where his interest in shanty vernacular comes from as well as talk about the relationship between his painting and music. We talk about Metal Aesthetic, the weird and inclusive family of metal heads and his band MOLLUSK… a doom/sludge metal band. We reminisce about both spending our formative years growing up in Michigan and local midwestern nostalgia as well as the practice of what it takes to turn a representational view into a real place and a finished painting. Loved this conversation and can't wait for you to hear! As always, please go give Adam O'Day a follow on Instagram, find out when his next concert will be and check out the show notes for a ton of links. In today's episode, we're diving into the three steps of "showing up" in your relationships - whether it's with your partner, your kids, your family, or your friends. Are you willing to embrace who you are right now, to own your past, and realize it doesn't have to dictate your future? We explore the first step - "looking inward" - and discuss the importance of giving yourself love, accepting responsibility for your actions, and aligning them with your intentions for how you want to show up in this world. Then, we uncover the significance of "looking forward" and why it's essential to start acting in the direction of your desires, even if you're not yet fully there. Finally, we tackle "looking together" - where we communicate, share our intentions, and operate as a team in our relationships. We emphasize the profound vulnerability and the crucial role of communication in truly understanding and supporting each other. We also shed light on the delicate balance of grace and gratitude in navigating life's messiness and cultivating growth together. So, are you ready to step into who you want to be and show up in your relationships fully? Tune in to uncover practical insights and strategies for thriving together, embracing growth, and fostering deep connection.
