Robert Ritch, MD, FACS Glaucoma pearls from the world’s expert. 10.28.17 It used to be said the one of the biggest fears of many is public speaking. But now as humanity is living longer than ever, the hugest terrors are the fear of going blind. Glaucoma is a leading cause of irreversible blindness worldwide. But it is greatly misunderstood. It is actually a large number of illnesses, some of the of the eye, and some of them systemic. Dr. Robert Ritch is a leading glaucoma expert. Dr. Ritch has developed many procedures now in place by glaucoma specialists around the world. He has also defined exfoliation syndrome, which he says many eye doctors are not adequately trained to identify. Dr. Ritch says that any cogent physician, reading and understanding the literature, must embrace nutritional intervention as much as procedures and medications. There are diverse forms of glaucoma. Some are eye diseases and some are brain diseases. Some affect mostly the eye site, others affect vulnerability to other diseases. High-pressure glaucoma is an “eye disease” that makes the internal eye pressure of the eye elevate (intraocular pressure) causing optic nerve damage. Normal or low-pressure glaucoma is more often a “systemic disease”. It occurs more frequently in thinner women with various attributes: astute sense of smell, tendency to low blood pressure, cold extremities, driven personalities, and tendency to various cardiovascular disease issues that do not necessarily shorten their life span. Exfoliation syndrome is when pigment comes off (exfoliates) from the iris. The iris gives your eye its color. Blue eyes come from an iris with bluish pigment. Your iris helps control the diameter and size of the pupil and thus the amount of light reaching your retina. When pigment sloughs off from the iris, it can clog the ability of the eye to drain. Fluid builds up. This increases the inner eye pressure and ultimate can start to damage the optic nerve. The optic nerve is part of the brain. As it damages, you can loose vision. This is called visual field damage. ·      High-pressure glaucoma is an eye disease secondary to elevated eye pressure. ·      Normal or low-pressure glaucoma are diseases of the optic nerve that can’t withstand pressure variations or elevations, and often involve brain and whole body health. Transforming growth factor beta1 (TGFB1) is a protein, called a cytokine, that when elevated, can promote dangerous inflammatory changes, especially within the eye. There are a large body of peer review published data linking elevated TFGB1 to open angle normal pressure glaucoma.   TGFB1 is elevated in both normal pressure glaucoma and exfoliation syndrome patients. It is not often elevated in high-pressure glaucoma. Specific nutrients and herbs have been shown, in peer review science, to normalize TGFB1 levels.When is eye damage the worse? The worst window for eye damage is potentially during the night. Blood pressure tends to lower slightly in all health people as we sleep. But if blood pressure goes too low, or there is sleep apnea present, the eye pressure moves in the opposite direction and dangerously elevates. If this happens, eye damage can be relentless while you sleep. If you have normal or low tension glaucoma, to avoid nocturnal eye damage, raising the head of the bed by 20% (you can buy sleeping wedges at Bed Bath & Beyond), salt loading before bed with a glass of tomato juice, improve sleep apnea with machines, all which can reduce eye damage while you sleep. Nocturnal eye damage can be to the optic nerve or to other structures of the eye. It is not limited to the optic nerve. Not many doctors, even eye or sleep doctors, are aware of the huge link between sleeping and eye health. Sleeping on one side may enhance optic nerve damage in the eye on that side, in glaucoma patients.  It’s a good idea for glaucoma patients or high risk patients, to train themselves to sleep on their back. Nutrients are a must. Dr. Ritch recommends many nutrients such as pigments from flowers that help preserve eye structure thickness, Meriva turmeric, Gingko Biloba, specific minerals like zinc and magnesium, citicholine oil (Italian studies on humans with eye drops with this show reversal of visual field damage but only supplements of this are available in the US), fish oils, and much more. When still a fellow in 1978, he performed the first laser iridotomy in New York and initiated the first course on laser treatment of glaucoma at the American Academy of Ophthalmology. He is a world leader in exfoliation syndrome, which affects 80 million people, and has started a global consortium to work on preventing, reversing, and even curing this disease. Dr. Ritch has co-authored or edited nine textbooks and over 1800 medical and scientific papers, book chapters, articles and abstracts. He has presented over 750 lectures worldwide, including 50 named lectures and has received over 60 national and international awards and medals. In 1985, he founded the Glaucoma Foundation and has served as Secretary, Medical Director, and Chairman of the Scientific Advisory Board. In 1994, he initiated the annual Optic Nerve Rescue and Regeneration Think Tank, which has attracted numerous successful researchers from other fields into glaucoma research.

Imagine you’re in the pub and you’ve just bumped into your friend Mario Roxas. Mario happens to be a naturopathic doctor and the director of research and development at Thorne, the Rolls Royce of supplement companies. Wouldn’t you want to know what supplements Mario takes? Me too, and so I couldn’t resist making that my first question. But what is it that makes Thorne Research different? Why not buy your supplements from Costco? Two reasons: research and quality. Thorne employ a team of medical and naturopathic doctor scientists that spend their time studying the scientific literature on the lookout for evidence supporting the decision to manufacturer a specific supplement. Secondly, there’s quality. Thorne cater to the sensitive customer likely to experience allergic reactions. The raw materials used to make a supplement originate from trusted sources, but Thorne validate the content of all their supplements no less than three times during the manufacturing process. Surprisingly, the FDA laws only require a supplement manufacturer to list the ingredients they added to the supplement, and there’s no guarantee that one or more of the raw ingredients used are free of potential allergens like lactose even though they’re not listed on the label. Are big supplement companies any different from big pharma? Allopathic medicine is often accused of being bought, but what makes alternative any different? Are the naturopathic doctors simply prescribing Meriva in place of Humira? I would argue no, absolutely not, and the proof is in the name supplement. As Mario points out, supplement is the perfect word. You can’t supplement your way out of a bad diet or lifestyle, and so the supplement is but one tool in the extensive chest of a good functional medicine practitioner. Three years ago I had never taken a nutritional supplement. Now I have a cupboard full of them. The reason is simple: they work. I used to take regularly chlorpheniramine for allergies. Killing off overgrowths of opportunistic pathogens and parasites with herbs like oregano and noni got me most of the way there, now I stay symptom-free by regularly supplementing with methyl-folate. The supplement didn’t just mask my symptoms; it corrected a deficiency and as a result, my red blood cells have become normal sized. Adverse events? Do you know of someone who had a serious adverse event as the result of taking a nutritional supplement free of contaminants? I don’t think this happens very often, if at all. Curcumin is a powerful antioxidant derived from turmeric root used heavily in traditional Indian cuisine.  Turmeric is a close relative of ginger which I use quite a bit to improve digestion and reduce inflammation. Curcumin was first used about 4,000 years ago in Chinese and Ayruvedic medicine. Curcumin has been studied extensively in human clinical trials at the M.D. Anderson Cancer Institute in Houston, Texas.  These studies found that curcumin was safe for patients under-going chemotherapy and does not need to be avoided.  In fact, it was found to enhance the effectiveness of chemotherapy. Curcumin and other plant-derived compounds have been shown to work synergistically with Vitamin D3. How should you take curcumin? Unfortunately, curcumin is poorly absorbed however it’s absorption can be enhanced significantly if it is combined with phosphatidyl choline.  It is also very unstable but this can also be remedied by combining it with lecithin. I recommend CurcuSelect from Moss Nutrition which is formulated to contain the phosphatidyl choline and lecithin in the patented Meriva form. I recommend taking curcumin with a meal that contains fat or with fish oil to further support absorption.  500mg twice a day with meals is the most effective therapeutic dose which would be two capsules of CurcuSelect twice a day during acute health problems. A maintenance dose would be 250mg twice a day with meals which would be one CurcuSelect taken twice a day. Curcumin is not recommended during pregnancy, if you are taking blood-thinners, or with gall stones.  If you iron levels are low and you are trying to improve your ferritin levels then curcumin may inhibit your progress due to it’s chelating ability of iron.

Con la prima generazione di Meriva, Opel ha inventato le monovolume compatte. Con la nuova Meriva rilancia sulla flessibilità e l’originalità. A cominciare dal sistema di apertura delle porte posteriori controvento denominato FlexDoors. Se si esclude “sua maestà” Rolls Royce, la nuova Meriva è l’unica vettura che offre la facilità d’accesso principesca che solo l’apertura a doppio battente può consentire. Oltre all’innovazione pratica delle porte, i progettisti Opel hanno allestito una vettura che offre davvero molto in termini di immagine, qualità, comfort ed efficienza dei propulsori. Il frontale ripropone il family feeling introdotto con Insignia e Astra. Se già all’esterno l’upgrade stilistico è evidente, gli interni compiono un deciso balzo in avanti. Tutto, dalla scelta dei materiali all’ergonomia, passando per le finiture da vettura premium, contribuisce alla sensazione di benessere dei passeggeri. La versatilità e la modulabilità degli spazi la fanno da padroni. Un originale sistema FlexRail a doppio binario collocato tra i sedili anteriori permette di collocare oggetti e di far scorrere il bracciolo anteriore in base alle esigenze. 32 vani portaoggetti consentono di stipare tutto quello che può servire ad una famiglia con bambini, l’utenza principale di questo genere di vetture. Le motorizzazioni sono ad alta efficienza e consentono mediamente un incremento di potenza del 15% ed una riduzione delle emissioni del 10% rispetto al vecchio modello. I tre benzina sono tutti da 1,4 litri con un aspirato da 100 CV e due turbo da 120 e 140 CV. L’offerta Diesel comprende il 1.300 CDTI da 75 CV e il 1.700 da 110 CV. Altri tre diesel arriveranno più avanti. Su strada si apprezza il grande comfort e la silenziosità di marcia, anche ad alta velocità. Unico neo, la posizione un po’ troppo inclinata del volante multifunzione. Infine una piacevole sorpresa: i prezzi. A parità di equipaggiamento sono inferiori a quelli della Meriva precedente, con una completa entry level che parte da 16.250 Euro. La motorizzazione top di gamma parte a meno di 18.000 Euro.
