More than 100 years ago August Krogh received the Nobel Prize for showing how oxygen is transported from the lungs through the blood into the small capillaries in the muscles. Details of the mechanism and how it is regulated are still central topics of research a hundred years after and understanding respiration is still a matter of life or death. In this interview science journalist Jens Degett had the opportunity to talk to, one of the world's leading physiologists professor Christopher Ellis from the University of Western Ontario (Canada) about how August Krogh's 1920 Nobel Prize greatly influences physiological research even 100 years later. In addition to Nobel Prize class research, August Krogh founded one of the first biomedical companies in Denmark. The year 2020 marked the centenary of August Krogh's Nobel Prize in physiology. August Krogh received the award for showing how oxygen is transported from the lungs through the blood into the small capillaries in the muscles. Details of the mechanism and how it is regulated are still a central topic of research a hundred years after and understanding respiration is still a matter of life or death. CardioNerds Amit Goyal, Dr. Colin Blumenthal, Dr. Kelly Arps and Dr. Justice Oranefo discuss mechanical stroke prevention in atrial fibrillation with Dr. Christopher Ellis, cardiac electrophysiology lab director and director of the left atrial appendage closure program at Vanderbilt University. There has been a significant increase in the number of patients undergoing left atrial appendage occlusion (LAAO). This trend is expected to continue with current and upcoming clinical data on this topic. In this episode we dive into the rationale behind LAAO and explore several historical facts. We then proceed to the current state of practice including currently available options, appropriate indications, post op care, and potential complications. Notes were drafted by Dr. Justice Oranefo. Pearls and Quotes - Atrial Fibrillation: Mechanical Stroke Prevention in Atrial fibrillation Surgical or catheter based left atrial appendage occlusion results in mechanical exclusion of the left atrial appendage, which is the most common source of thrombus leading to embolic events in patients with non-rheumatic atrial fibrillation. Surgical LAAO should be considered in patients with atrial fibrillation and CHA2DS2VASC score ≥ 2 undergoing cardiac surgery for other indications. Endocardial LAAO devices such as WATCHMAN FLX and AMULET are approved for stroke prevention in patients with atrial fibrillation with a CHA2DS2VASC score ≥ 2 and have an appropriate reason to seek a non-drug alternative to anticoagulation therapy. Appropriate patient selection and post-operative anticoagulation and imaging strategy are crucial for prevention and management of complications related to LAAO. Notes - Atrial Fibrillation: Mechanical Stroke Prevention in Atrial fibrillation What are the types of LAAO device? Left atrial appendage occlusion devices can be divided into epicardial closure and endocardial closure. Epicardial techniques/devices include surgical ligation, Atriclip, and Lariat. These techniques require pericardial access (either by open thoracotomy or thoracoscopic access). The goals are complete exclusion and ischemic necrosis of the LAA. LARIAT device Atriclip device Endocardial techniques include WATCHMAN FLX and AMULET devices. These techniques require the use of nitinol-based devices which are delivered into the LAA via a transeptal approach. These devices become endothelialized over time resulting in occlusion of the LAA. AMULET device WATCHMAN FLX Who is the ideal candidate for surgical LAAO? Several studies have evaluated the efficacy of surgical LAA occlusion. The most prominent being the LAOS III trial which randomized 4770 patients with atrial fibrillation and CHA2DS2VASC ≥ 2 undergoing cardiac surgery for other reasons to surgical LAAO vs no LAAO (3,4). The primary outcome of ischemic stroke or systemic embolization occurred in 4.8% of patients in the LAAO group vs 7% of patients in control group over an average ...

The crime happened back in 1990. Assistant Hofstra football coach Joe Healy was killed while sitting with his friends outside a fast-food restaurant. Police said it was a botched robbery. His death shattered the community and led to a lot of media attention. Tax expenditures seek to accomplish many of the goals of direct government expenditures, but they distribute money indirectly, through tax refunds or reductions in taxable income, rather than direct payments on goods and services or benefits. Showcasing the struggles of contemporary life, blending elements of Alternative with Rap and R&B, EFFY’s debut brings her first single ‘Lets Go Back’ which has been described as “A snapshot of a diary page from an artist who cherishes honesty”. She is currently working on other projects, which will follow her debut, taking a gentler vocal approach. But it’s not identity politics as we’re used to thinking about it. In Uncivil Agreement: How Politics Became Our Identity, Lilliana Mason traces the construction of our partisan “mega-identities”: identities that fuse party affiliation to ideology, race, religion, gender, sexuality, geography, and more. These mega-identities didn’t exist 50 or even 30 years ago, but now that they’re here, they change the way we see each other, the way we engage in politics, and the way politics absorbs other — previously non-political —spheres of our culture. In making her case, Mason offers one of the best primers I’ve read on how little it takes to activate a sense of group identity in human beings, and how far-reaching the cognitive and social implications are once that group identity takes hold. I don’t want to spoil our discussion here, but suffice to say that her recounting of the “minimal group paradigm” experiments is not to be missed. This is the kind of research that will change not just how you think about the world, but how you think about yourself. Mason’s book is, I think, one of the most important published this year, and this conversation gave me a lens on our political discord that I haven’t stopped thinking about since. After receiving his Masters degree in Healthcare Management in 2011, Chris worked as a practice management consultant for nearly five years before starting his own dental staffing startup. Chris is fascinated by the intersection of startups, technology, and dentistry which is what led him to quip, where he currently works as the Dental Marketing lead. Elisa is a RDH who graduated from NYU with her degree in hygiene in 2015 and her bachelors in 2016. She practiced clinically in private practice in Phoenix, AZ for three and a half years. She now works for quip, an oral care company based in New York, as their Dental Community Manager.

The rich have more tools to influence politics and policy than the rest of Americans, but what about the poorest citizens? In an age of increasing economic inequality, who, if anyone, represents their views and their interests in Congress? Kris Miler finds that Members of Congress in high poverty districts are not the champions of the poor. Instead, Democratic women and minorities from urban districts tend to introduce bills about poverty but have trouble getting them passed, leaving the poor without effective representation even in times of rising poverty. Christopher Ellis finds that Members of Congress are usually more responsive to the opinions of the rich than the poor in their districts, but moderates and Democrats in competitive districts with unions do represent the opinions of the poor. Low-income constituents are only sometimes visible and have a hard time holding their representatives accountable.
