Dr. Craig Joseph, Chief Medical Officer at Nordic Global, highlights the concerns and challenges healthcare professionals face when using health IT and the clinician burnout related to the administrative burdens of electronic health records.  He addresses the obstacles to integrating IT into healthcare workflows, including cost, risk aversion, and resource limitations. He identifies areas where AI can have an immediate impact in reducing errors and biases. Craig elaborates, "In the past, we lived in a paper-based world, so it was pretty flexible. You could fill out a form or not fill out a form. You could give as much detail or as little detail. Often, especially physicians really couldn't read what they were writing anyway, so it was less important. All of that's changed now in the last 15 or 20 years since the advent of electronic health records." "What we've seen with large language models, like Chat GPT and other modern AIs, is the ability to do a lot of things with voice now, to be able to communicate directly with a patient, not have to look at the screen to either get information or to write a progress note. Lots of physicians have complained deservedly that they have to spend pajama time, time after they put their kids to bed at night, where they're not getting paid and writing their progress notes and office visit notes." "I think AI, like LLMs are going to really do a lot to help to rehumanize the interactions that we have. Although, there will be downsides. For that to work, there has to be a microphone in the exam room and it has to be listening to what you and the doctor are saying to one another. Folks are going to have to be comfortable with that to be able to leverage some of those things." #NordicGlobal #AI #MedicalAI #Hospitals #HealthIT nordicglobal.com Download the transcript here

OVERVIEW:  Obesity is a disease. It's not just a lifestyle issue. Jason A. Duprat, MBA, CRNA, Healthcare Practitioner, and Host of the Healthcare Boss Academy podcast, sits down with Katherine Saunders, Co-Founder of Intellihealth, Diplomat of the American Board of Internal Medicine and the American Board of Obesity Medicine, and Host of the Weight Matters Podcast. Join us in this episode as we debunk the long-standing stigma surrounding obesity and learn more about Katherine and Intellihealth's mission to bring accessible medical obesity care for all.   EPISODE HIGHLIGHTS  Katherine's mentor had a predecessor company which she and her husband transitioned into Intellihealth, wherein they aim to democratize effective obesity treatment. Serving as a Primary Care physician, Katherine was overwhelmed with her patients' weight-related diseases which inspired her to pursue Obesity Medicine. Physiological and pathophysiological changes occur with excess weight that make it very difficult to lose weight and easy to gain weight. Obesity Medicine is a newer field of medicine. There are only under 6,000 physicians who are certified in Obesity Medicine, which is astoundingly low considering how rampant obesity is in America. The cornerstone of the field is dietary strategies, physical activity, and behavioral change. Medications come in only after comprehensive evaluations and needs assessments. Katherine mentions the American Board of Obesity Medicine, Obesity Medicine Associate, and conferences available to learn more about the field. She works closely with Software Developers in Intellihealth, combining two vastly different fields to make Obesity Medicine more accessible for all. One of their goals is to break the stigma, to educate people that obesity is a disease and not dependent on willpower alone.   TWEETABLE QUOTES  “Obesity is a disease. This episode is sponsored by Charm Economics– data-driven solutions for digital health & MedTech businesses. Dr. Block and his guest, Dr. Ken Milne, delve into the concept of knowledge translation and the time it takes for new information to be integrated into clinical practice. They reference a famous paper that found it takes an average of 17 years for clinically relevant information to reach the patient's bedside.  However, the hosts believe that this time frame can be shortened with the help of social media and evidence-based medicine. They emphasize the importance of evidence-based medicine but also note that it should not be the only factor in decision-making. The hosts also discuss their checklist for evaluating the validity of studies and how they use their podcast to share new information with their audience. Overall, the hosts believe that shortening the time it takes for new information to be integrated into clinical practice can lead to improved patient outcomes. The episode also touches on the potential benefits of vitamin C in treating sepsis patients. One study conducted at Dr. Paul Merrick's Institution showed a significant mortality benefit in patients who were given a cocktail of thiamine, Vitamin C, and hydrocortisone. However, the study was not a randomized control trial and was before-and-after observational, leading to skepticism about the conclusions. Over the next few years, properly designed randomized control trials were conducted and found that vitamin C had no significant impact on sepsis patients. Therefore, while there was initial interest in the potential benefits of vitamin C in treating sepsis, further research has shown that it is not an effective treatment. Looking for something specific within the show? Here you go! Eosinophilic esophagitis (EoE) is an increasingly prevalent, chronic, T2 inflammatory disease triggered by food and/or environmental allergens. Due to underlying tissue inflammation, patients suffer diminished quality of life due to symptoms such as weight loss, difficulty swallowing, and heartburn. Patients can experience these symptoms for a long time before receiving a proper diagnosis. Once diagnosed, treatment options have historically been limited. Fortunately, therapies that treat other type 2 disorders are in development or are approved to treat EoE. At a recent live event, experts used patient cases to discuss the diagnosis, underlying causes, and the newest treatments for EoE, including biologic therapies. Watch this on-demand version now! Upon completion of this activity, participants should be better able to: Distinguish signs and symptoms of eosinophilic esophagitis (EoE) among children and adults to enable an earlier diagnosis and minimize complications due to uncontrolled disease; Recognize underlying mechanisms of EoE, including the role of key cytokines such as interleukins -4, -13, and -5 as drivers of type 2 inflammation and how this relates to emerging treatment; and Apply the latest expert recommendations and clinical evidence to the treatment of patients with EoE, particularly as novel and emerging agents become available.

One of my greatest frustration with practicing medicine is poor patient compliance. You spend time educating the patient and discussing the treatment plan. When you see them back in follow up you discover they haven't followed through. If you are lucky, they are the same. Unfortunately, sometimes they are worse.Using FDA-approved devices, remote monitoring remote patient monitoring (RPM) and remote therapeutic monitoring (RTM)  patients  can alert physicians to any concerning  data between visits. That way, physicians can intervene before things spiral out of control.  The goal behind remote monitoring programs is to improve patient care, decrease healthcare spending by being proactive and improving patient compliance. And while I'm sure that we are all for improved patient care and outcomes, I know that nobody wants to do more work. Or do it for free. Here's the good news: Implementing RPM/RTM does not require more physician time. When set up correctly, you only have to write the order. You can use your staff to do the rest. RPM/RTM can significantly increase your revenue. In this episode I'll tell you the key differences between RPM and RTM and how to utilize them in your practice. If you'd like to hear more tips on how to start, run and grow your practice and related medical businesses, please sign up for my newsletter at https://www.thepracticebuildingmd.com.   Be sure to join my FB group, The Private Medical Practice Academy.Enroll in my course,  How To Start Your Own Practice and get the step-by-step process for opening your doors.Or join The Private Medical Practice Academy Membership for live group coaching, expert guest speakers and everything you need to know to start, grow and leverage your private practice.

Obesity is a disease. It's not just a lifestyle issue.   Jason A. Duprat, Entrepreneur, Healthcare Practitioner, and Host of the Healthcare Entrepreneur Academy podcast, sits down with Katherine Saunders, Co-Founder of Intellihealth, Diplomat of the American Board of Internal Medicine and the American Board of Obesity Medicine, and Host of the Weight Matters Podcast. Join us in this episode as we debunk the long-standing stigma surrounding obesity and learn more about Katherine and Intellihealth's mission to bring accessible medical obesity care for all.   EPISODE HIGHLIGHTS  Serving as a Primary Care physician, Katherine was overwhelmed with her patients' weight-related diseases which inspired her to pursue Obesity Medicine. Katherine's mentor had a predecessor company which she and her husband transitioned into Intellihealth, wherein they aim to democratize effective obesity treatment. Science and research show that obesity is a disease and not just a lifestyle issue. JOIN OUR GIVEAWAY! By considering biomedical and patient-reported outcomes when conducting research and measuring care quality, we can enrich our understanding of patient experiences and identify opportunities to improve patient health status by aligning treatment goals with issues important to patients.    In this episode of GMO Dialogues, Dr. Jennifer Flythe, Associate Professor of Medicine at UNC School of Medicine, discusses Patient Related Outcome Measures and how they can improve the quality of patient care.

Whether you like it or loathe it, videos of chiropractic adjustments get lots of views on social media. On YouTube, there are multiple chiropractors with over a million subscribers! There is even 1 with over 2 million subscribers. Then there are the accounts on Instagram and TikTok with massive followings. All this means that it's an increasingly common experience to have someone come in for their first visit and say something like "I want to get cracked like I saw on Instagram" or "I want the y-strap adjustment like I saw on YouTube". It's a challenging situation because they are often coming in with expectations that are VERY different to the way most of us practice. In this episode, Martin breaks down exactly how to handle this shituation, including the exact questions to ask. Most chiropractic communication approaches are front-loaded – they focus only on what to do in the first few visits. Maybe you have a workshop to explain the 'big picture' of chiropractic, but there is no (or little) focus on what to do on adjustment visits to drip feed the key concepts. It sets you up for failure if your vision for your practice is lifetime care but you expect people to remember the concepts you explained to them months, years, or decades earlier. A key part of creating a wellness or performance culture is to create a system for communicating it EVERY visit. A core component of the approach that Martin teaches in the "Daily Interactions" workshop is using closing statements. In this episode, Martin explains what they are, how to use them and what closing statements to not use! Most chiropractors focus WAY too much on new patients/ clients/ practice members! Their procedures focus on what to do on a first and second visit, new clients are treated like visiting royalty, and they tell themselves that what they 'need" to grow their practice is to get MORE new people! It often creates a trap for them where they always 'need' new people! The alternative is to focus on long-term relationships. Part of this is to put as much focus and effort on reactivations. How can you make a remarkable experience for people who drop out of care and then come back? Caring for patients with acute myeloid leukemia (AML) depends on many factors and can vary as much as the patients themselves. As such, shared decision-making—those collaborative conversations between patient and physician that guide the treatment trajectory—are critical to align goals and help determine what's best for each patient. Due to the challenges COVID-19 presents for immune-compromised individuals, clinicians are having more complex discussions with their patients diagnosed with AML—including how to proceed with the COVID-19 vaccine and booster shot. Listen NowWhile it appears obvious a person's health status is directly related to their life circumstances the health care industry has been slow to recognize an individual's bio-psycho-social factors or characteristics in planning and delivering an individual's care.  This critcism is typically phrased as clinicians being over attentive to the "patient" and under attentive to the "person".  For various reasons having in part to do with utilization/cost, reimbursement and population health concerns this is changing.  That is the health care industry is developing a greater appreciation and more sophisticated understanding of the non medical predictors of health care risk.  During this 21 minute interview Dr. Alkema discusses why the health care industry has been slow to adopt socioeconomic factors in care planning and delivery, non-medical factors that correlate with higher care utilization, how these factors or characteristics can be used for predictive purposes and related related issues.  Dr. Gretchen Alkema currently serves as Vice President of Policy and Communications for The SCAN Foundation.  Prior to joining SCAN Dr. Alkema was the 2008-09 John Heinz Health and Aging Policy Fellow serving in the office of Sen. Blanche Lincoln.  Dr. Alkema earned her PhD at the University of Southern California's Davis School of Gerontology and and completed her post-doctoral training at the VA Greater Los Angeles Health Services Research and Development Center of Excellence.  Her academic research focused on evaluating innovative models of chronic care management and translating effective models into practice.  She is a Licensed Clinical Social Worker and has practiced in government and non-profit settings including community mental health, care management, adult day health care, residential care and post-acute rehabilitation.Listeners will recall in August 2013 Dr. Alkema discussed the relationship between Medicare utilization and cost and beneficiary (declining) functional status.  For more on predictive analytics related to high-risk Medicare beneficiaries see: http://www.thescanfoundation.org/sites/thescanfoundation.org/files/identifying_high_cost_benefits_fact_sheet_1_1.pdf This is a public episode.
