Watch the Devotion Based on Acts 2:21 Remember the Fallen with Resurrection Hope Last month I was in our nation’s capital, and I took some time to walk around the National Mall. When I reached the Vietnam Veterans memorial, I walked around the statue of the three service members. As I stood behind the statues, I noticed that their gaze is fixed on the black granite wall upon which are etched the names of their fallen comrades in arms. Just then a group of men in wheelchairs rolled up. They were a group of Vietnam veterans on their Honor Flight. Some took pictures. Others just sat there with their hands folded, remembering…  This made me think ahead to this weekend. On Memorial Day weekend we remember those who have died in service to our country. But we do not grieve the dead the way that the rest of the world grieves because we have hope. We know that Christ is risen and that he lives. Those who died while serving our country and died clinging in the risen Lord Jesus, they also live!    The apostle Peter quoted the prophet Joel in his Pentecost Day sermon in Acts chapter 2 and he said this: “Everyone who calls on the name of the Lord will be saved” (Acts 2:21).  My prayer for you is that when you take time to remember this weekend – whether you do that on Monday at 3:00 PM local time, the National Moment of Remembrance, or you do that by sticking a flag in your yard, or hanging it on your garage, or laying a wreath on a tombstone – my prayer is that your soul is comforted with Jesus' resurrection promise that “everyone who calls on the name of the Lord will be saved.”   Jesus promised the one who was crucified with him, “Today you will be with me in paradise.” This promise confirms that those who died in service to our country clinging to Jesus as their Savior – their souls are with Jesus right now in heaven. I pray that you are comforted with the knowledge that because Christ's tomb is empty that one day their tombs and their urns will be empty, too. Their bodies will rise from the dead and they will live forever remember. The one who died and rose again never to die again did so, so that those who call on his name will never die and that includes you. Prayer: Lord Jesus, as we remember those who gave their lives in service to our nation, comfort hearts that still carry grief and loss. Remind us that death is not the end of those who call on your name. Because you died and rose again, your people have the sure hope of eternal life. Strengthen those who remember the fallen with the promise of your empty tomb. Lord, through the blood and sacrifice of those who have perished you continue to bless our nation with freedoms, most notably, the freedom to proclaim your truths. Prevent further shedding of blood. Memorial Day observances are scheduled throughout Haverhill in the lead up to the holiday itself, next Monday.Jeffrey Hollett, Haverhill's veteran services director, gave an overview of upcoming Memorial Day-related happenings during a recent appearance on WHAV's “Win for Breakfast” program.He said this year's theme is “What Does Memorial Day Mean to You?”“I would like to see Memorial Day Week become a week of events every single day of the week prior to,” Hollett said.This Friday, May 22, students at the John C. Tilton School on Grove Street will hold the school's Annual Memorial Day Ceremony at 1:30 p.m. Veterans, families and friends have been invited to the tribute which includes a student concert.On Saturday, May 23, veterans, scouting and other youth groups, services clubs and allies are gathering at 9 a.m., at the Haverhill AmVets Post 147, 576 Primrose St. The post is the staging area for those who are placing small American flags on veterans' graves buried within Haverhill's 17 cemeteries. Hollett said for many this rite has become an annual tradition.“We've purchased over 8,000 flags this year. We have plans to put out over 8,000 flags. We can't do that without volunteers,” Hollett said.On Sunday, May 24, church bells across the city will ring at noon as part of the national “Ring For the Fallen” tribute. Hollett notes this ritual has been adopted by the National Fallen Firefighters Foundation as a way of honoring firefighters who have died in the line of service as well.Haverhill's Annual Memorial Day Parade step off Monday, at 10:45 a.m., from the Haverhill Firefighting Museum, 75 Kenoza Ave. Those marching are asked gather at 10 a.m. The parade will march to Linwood Cemetery, 41 John Ward Ave., where the Memorial Day ceremony will take place. The AmVets host lunch following the ceremony at the post, 576 Primrose St.The city's Memorial Day activities conclude with the National Moment of Silence at 3 p.m., Monday, May 25. Anyone with a bell is asked to ring it for 21 seconds beginning at 2:59 p.m. Then, at 3 p.m., the moment of silence commences for 21 seconds.Hollett is also encouraging veterans to take part in the Memorial Day Creative Sanctuary. The online program encourages anyone inspired by the solemnity of the day to submit an essay or poem to Mission Belonging's website. The organization's “Writing in the Arena” provides a place “to explore identity and to find purpose and meaning following military service through creativity and shared experience.” Those interested in participating may visit MissionBelonging.org.Hollett also asked that other city organizations planning an observance let him know by calling his office at 978-374-2351, ext. 3932.Support the show

Take time today to remember those who sacrificed Memorial Day is a U.S. holiday that's supposed to be about mourning the nation's fallen service members, but it's come to anchor the unofficial start of summer and a long weekend of travel and discounts on anything from mattresses to lawn mowers. Iraq War veteran Edmundo Eugenio Martinez Jr. said the day has lost so much meaning that many Americans "conflate and mix up Veterans Day, Memorial Day, Armed Forces Day, July Fourth." Social media posts pay tribute to "everyone" who has served, when Memorial Day is about those who died. For him, it's about honoring 17 U.S. service members he knew who lost their lives. "I was either there when they died or they were soldiers of mine, buddies of mine," said Martinez, 48, an Army veteran who lives in Katy, Texas, west of Houston. "Some of them lost the battle after the war." Steve Merando, who has marched in Cold Spring's Memorial Day parade since he was 10 years old, agreed. "People forget that Memorial Day is supposed to be a memorial to those who were killed in action while serving their country," said Merando, who served with the U.S. Navy Seabees from 1969 to 1973, including in Vietnam and Thailand. He played Little League baseball with Keith Livermore, one of three Philipstown residents killed in the Vietnam War. In Memoriam: Philipstown and Beacon Here is a look at the holiday and how it has evolved: When is Memorial Day? It falls on the last Monday of May, which this year is May 26. In Cold Spring, a parade will begin at 9 a.m. at Stone and Main streets and progress to Cold Spring Cemetery in Nelsonville for a ceremony. Hot dogs and refreshments will follow at the American Legion. Rain or shine. In Beacon, a ceremony will be held at 11 a.m. at 413 Main St. It will include the dedication of a plaque to mark the 100th anniversary of the Veterans Memorial Building, which was completed in 1925. Why is Memorial Day celebrated? It's a day of reflection and remembrance of those who died while serving in the U.S. military. The holiday is observed in part by the National Moment of Remembrance, which encourages all Americans to pause at 3 p.m. for a moment of silence. What are the origins of Memorial Day? The holiday's origins can be traced to the American Civil War, which killed more than 600,000 service members - both Union and Confederate - between 1861 and 1865. The first national observance of what was then called Decoration Day occurred on May 30, 1868, after an organization of Union veterans called for decorating war graves with flowers, which were in bloom. The practice was already widespread. Waterloo, New York, in Seneca County, began a formal observance on May 5, 1866, and was later proclaimed to be the holiday's birthplace. Yet Boalsburg, Pennsylvania, traced its first observance to October 1864. And women in some Confederate states were decorating graves before the war's end. David Blight, a Yale history professor, points to May 1, 1865, when as many as 10,000 people, many of them Black, held a parade, heard speeches and dedicated the graves of Union dead in Charleston, South Carolina. A total of 267 Union troops had died at a Confederate prison and were buried in a mass grave. After the war, members of Black churches buried them in individual graves. "What happened in Charleston does have the right to claim to be first, if that matters," Blight told the Associated Press in 2011. When did Memorial Day become a source of contention? As early as 1869, The New York Times wrote that the holiday could become "sacrilegious" and no longer "sacred" if it focused more on pomp, dinners and oratory. In an 1871 Decoration Day speech at Arlington National Cemetery, abolitionist Frederick Douglass said he feared Americans were forgetting the Civil War's impetus: enslavement. "We must never forget that the loyal soldiers who rest beneath this sod flung themselves between the nation and the nation's destroyers," Douglass said. His concern...

What's the difference between Veterans Day and Memorial Day? Memorial Day and Veterans Day honor the sacrifices of innumerable individuals who sacrificed themselves to preserve the freedoms all Americans enjoy, with Memorial Day remembering those who gave their lives, and Veterans Day honoring all who served in the U.S. Armed Forces. The National Moment of Remembrance is an annual event that asks Americans, wherever they are at 3:00 p.m. local time on Memorial Day, to pause for a duration of one minute to remember those who have died in military service to the United States. The time 3 p.m. was chosen because it is the time when most Americans are enjoying time off of work for the national holiday. The Moment was first proclaimed in May 2000 for Memorial Day that year, and was put in law by the United States Congress in December 2000. We Remember Our Veterans Background: Wartime veterans or their surviving spouses (or other dependents) may qualify for the Improved Pension to help them with their expenses. This pension is increased for those who are Homebound or qualify for the Aid & Attendance benefit. Topic: During May we celebrate our fallen war heroes on Memorial Day (which became a national holiday in 1971). This year it falls on Monday, May 31st. Let's take a moment to thank our heroes and make sure they know they may be entitled to the non-service connected pension. Issue: Let's make sure our non-service connected veterans, have the money to help pay for care whether at home or in a facility of their choosing. • Memorial Day: o Honoring the men and women who died while serving in the U.S. military dates back to shortly after the Civil War. During this time, various towns and cities paid spring tributes to fallen soldiers. o 1968, General John A. Logan called for Decoration Day - a nationwide day of remembrance for those who had fallen during the Civil War. o Later, Decoration Day evolved to include honoring those who had served during all of our wars. o Renamed Memorial Day in 1971 when it became an official federal holiday. o Memorial Day traditions vary from visiting the resting place of our heroes, parades, picnics and a National Moment of Remembrance at 3:00pm local time. • Difference between service-connected disabilities & nonservice connected disabilities. o Most Veterans are aware that they could be entitled to compensation for service-connected disabilities o Many Veterans are not aware of the nonservice connected pension they (or their dependents and surviving spouses) may qualify for.  This basic pension is called the Improved Pension, with additional benefits for those who are homebound or who qualify for aid and attendance benefits. • Do not have to be ‘in the VA System' • Can qualify even if denied for other benefits in the past • Qualifications can be confusing, generally a veteran should qualify: o Served active duty at least 90 days but at least 1 day during wartime but more if served after 1980) o Not a dishonorable discharge

Eight local musicians joined the 5,000 across the nation to participate in the National Moment of Remembrance on Memorial Day Monday. At 3 p.m., Gene Clarke, Cheryl Clarke, Ella Davis, Jammaris Reddit, Michael Finely, James Boyette, Tony Cooke and Chris Beverly played Taps in front of the Clarke County Courthouse. All of the players participate in the Flying Bricks Community Band which meets Monday nights at 7 p.m. in the THS band room in Thomasville. Anyone interested is invited to attend. Vaginal metformin is mentioned as an anecdote which has not been proven to work we remembered Memorial Day. Introduction: Vaginal Metformin.  By Hasaney Sin, MD, and Hector Arreaza, MD.Today is May 31, 2021.  There’s a saying that I came across on social media that has always spoken to me which I find relevant to our vocation. “The more I learn, the more I find out I don’t know”. So comes the joys (and challenges) of our chosen career. Case in point, have you ever heard of vaginal metformin? Neither have I, until today. There was a randomized clinical trial plan in 2013 at Assuit University in Egypt studying the effectiveness of vaginal metformin for the treatment of polycystic ovarian syndrome (PCOS). As primary care providers, we are very aware of the gastrointestinal side effects of metformin when taken PO. This sometimes prevents compliance with metformin. The study at Assuit University was to study the effectiveness of metformin when given vaginally in the effectiveness of treating PCOS, while also decreasing the undesirable side effects of metformin when given PO in hopes of also ultimately improving adherence. Unfortunately, the study was planned to be finished in 2014, but no results have been published thus far[1]. Stay tuned in case there is any update.Arreaza: I had to do a search because I was very curious too. There is at least one occurrence when vaginal metformin was mentioned, at least in English. It was in an online forum where a doctor recommended vaginal metformin for PCOS to a patient. This has not been evaluated or approved by any organization, so I would not recommend it. Civil war ended in 1865.   A fun fact to know, is that Memorial Day, was originally called “Decoration Day”.  It was 3-years after the Civil war ended, May 5, 1868, that “Decoration Day” was declared as a time for the nation to decorate the graves of those lost in war.  Graves were adorned with flowers and their lives celebrated.  Maj. Gen. John A. Logan then declared that “Decoration Day” should be observed on May 30th. It is believed that this date was chosen because flowers would be in full bloom across the country. The “birthplace” of “Memorial Day” was recognized as coming from Waterloo, New York, because Waterloo was the first to use this term to expand honor and recognition of all US fallen soldiers of war from the Civil War and from World War I. In 1971, “Memorial Day” was officially declared a national federal holiday: The National Moment of Remembrance encourages all Americans to pause wherever they are at 3:00 p.m. local time on Memorial Day for a minute of silence, to remember and honor those who have died in service to the nation. Happy Memorial Day everyone! ___________________________A1C.By Hector Arreaza, MD, and Yodaisy Rodriguez, MD.   Definition. Glycated hemoglobin (glycohemoglobin, hemoglobin A1c, or just A1c) is a form of hemoglobin that is chemically linked to a sugar. Glucose spontaneously bind with hemoglobin, when present in the bloodstream of humans.A1C refers to the percentage of glycosylation of the hemoglobin A1C chain and correlates with the average blood glucose levels over the previous 2-3 months from the slow turnover of red blood cells in the body. A RBC lives 120 days.History of A1C. Huisman and Meyering separated glycohemglobin for the first time in 1958. A1c for monitoring the degree of control of glucose metabolism in diabetic patients was proposed in 1976 by Anthony Cerami, Ronald Koenig and coworkers.A1C was first included in the ADA guidelines as a diagnostic test for diabetes in 2010. Prior to that random glucose or fasting plasma glucose were used for diagnosis.For diagnosis of diabetes, A1C testing should be done by a technique certified by the National Glycohemoglobin Standardization Program and consistent with the Diabetes Control and Complications Trial reference assay.A1C levels. A1C 6.5% is diabetes.Of note, other criteria for diagnosing diabetes: Fasting plasma glucose >126 mg/dL, 2-hour plasma glucose > 200, random glucose >200 plus classic symptoms.In patients with prediabetes, A1C should be tested yearly.The American Diabetes Association (ADA) has recommended glycated hemoglobin testing (HbA1c) twice a year for patients with stable glycemia, and quarterly for patients with poor glucose control. Use ADA guidelines to assess targets.Point-of-care A1C (POC A1C): POC is not recommended for screening or diagnosis but it is good for monitoring.A1C limitations.There are some limitations to A1C testing, and an incomplete correlation between A1C level and average glucose level in certain individuals.Nonglycemic Factors That May Interfere with A1C MeasurementFalsely lower A1C: Acute blood loss, Chronic liver disease, Hemolytic anemias, Patients receiving antiretroviral treatment for human immunodeficiency virus, Pregnancy, Vitamins E and C. Patients being treated for iron, B12 or folate deficiency, EPO, chronic hemolysis (thalassemia). Lower or elevate A1C: Hemoglobinopathies or hemoglobin variants, Malnutrition Falsely elevate A1C: Aplastic anemias, Hyperbilirubinemia, Hypertriglyceridemia, Iron deficiency anemias, Renal failure, Splenectomy.For example, when RBCs have a short life, like in acute bleeding, the A1C is falsely low. On the other hand, when RBCs live longer (history of splenectomy and aplastic anemias) the A1C is falsely elevated. It’s a good idea to do CBC with A1C.Ethnic groups: Hemoglobinopathies or hemoglobin variants can change A1C levels and may be more prevalent among certain racial and ethnic groups. A1C tends to be higher in some races/ethnic groups: AA, Hispanic-Americans, Asian-Americans.Other A1C limitations: It gives you an average, patient may be experiencing hypoglycemia alternated with hyperglycemia and result in normal A1C. Screening for diabetes.ADA: Screen for diabetes or prediabetes all asymptomatic adults, according to the ADA, who have overweight or obesity with one or more risk factor (first degree relative with diabetes, high risk race or ethnic group, history of CVD, hypertension, dyslipidemia, PCOS, physical inactivity, severe obesity, acanthosis nigricans), patients with prediabetes (every year), women with GDM (every 3 years), all other patients after 45 years of age. If results are normal, test every 3 years, patients with HIV.USPSTF: Adults aged 40 to 70 years who are overweight or obese. The USPSTF recommends screening for abnormal blood glucose as part of cardiovascular risk assessment in adults aged 40 to 70 years who are overweight or obese. (Draft: Asymptomatic adults ages 35 to 70 years who are overweight or obese) This is a Grade B recommendation. Clinicians should offer or refer patients with abnormal blood glucose to intensive behavioral counseling interventions to promote a healthful diet and physical activity. The USPSTF recommends screening for gestational diabetes mellitus (GDM) in asymptomatic pregnant women after 24 weeks of gestation. This is a Grade B recommendation.Grade I recommendation (insufficient evidence): Asymptomatic pregnant women, Before 24 Weeks of Gestation. The USPSTF concludes that the current evidence is insufficient to screen for GDM in asymptomatic pregnant women before 24 weeks of gestation.A1C Targets.A1C goals can range from 6.5% to 8%. Target is individualized based on life expectancy, disease duration, presence of complications, CVD risk factors, comorbid conditions and risks for severe hypoglycemia. Sometimes your goal can be independent of A1C, for example, your goal can be to avoid complications. As a fun fact, A1C is not used in veterinary medicine.Conclusion.By Hector Arreaza, MD. Now we conclude our episode number 54 “A1C”, three characters that may not mean much for most people but for patients with diabetes, it is a very important number to remember. Remember to check the A1C in all your patients with poor control of diabetes every 3 months, or every 6 months in patients with good control. A1C has its limitations but it certainly is the best way to assess your patients’ glycemic control. We all know that Memorial Day is an American holiday that is observed the last Monday of May.  But do you know why we celebrate this holiday?  Memorial Day's purpose is to honor the men and women who died while serving in the U.S. military.   It is a very somber day to me as I remember my family members, fellow veterans, and military members who have lost their lives while serving this great country.  Also, I also remember those that served in the military.  Many veterans may have made it back from military conflicts but they did not come back whole.  It is those veterans that need to be kept in our thoughts and prayers as well. I remember growing up in a small town in upstate, New York.  (Yes, New York does have small towns.) Each Memorial Day, our city would honor each former resident that lost their life while serving in the U.S. military. As a youngster, I did not grasp the significance of this day until after I served in military conflicts. History Memorial Day, which was initially known as Decoration Day. Our ancestors started remembering service members killed in action after the Civil War.  While this holiday was celebrated, it did not become an official federal holiday until 1971.  I have wondered why it took our Government so long to create this holiday.  There were many wars or conflicts before 1971 that would have brought this to their attention before this date.  But better late than never. For more information on the history of Memorial Day. Birthplace of Memorial Day I never realized that Memorial Day had a birthplace, but it does. In 1966, the U.S. Government declared Waterloo, New York as the official birthplace of Memorial Day.  The reason Waterloo, New York?  Well, because each year that city hosted an annual, community-wide event.  During this event, local businesses were closed, and residents decorated the graves of military members with flowers and flags. Maybe the U.S. Government realized that many communities have independently held memorial gatherings to remember our military members and decided that we needed to memorialize the day. National Moment of Remembrance  For those that were not aware, each year on Memorial Day, a national moment of remembrance takes place at 3:00 p.m. local time.  Join me for a moment of silence as we remember these brave individuals.  Traditions Americans have many cultures to celebrate the day.  Some people will visit cemeteries or memorials.  Others will hold family gatherings and participate in parades.  Still, others will travel to the great outdoors and camp, fish or ride UTVs. Me? I prefer to stay home and reflect on those that made the ultimate sacrifice and gave their lives to defend our country.  I believe that each of us celebrates the holiday based upon our traditions or past experiences.  It is a day that needs to be remembered for that I am sure.  I hope that those of you with young children will come up with your tradition so that every American understands the significance of this day. Does Our Younger Generation understand the importance of Memorial Day? We need to teach the younger generations the importance of our history and why we have the traditions that we do.  I am not sure if our more youthful generations know why we celebrate this day.  I cannot answer that question. But I hope that they will learn the reasons why and carry on the tradition.   Don’t let Memorial Day mean you just the start of the summer season. This Monday we celebrate Memorial Day as an offical day to honor those who died fighting for our country. One of our former Presidents passed a memorandum in May of 2000 and called it the "National Moment of Remembrance". In this he decided it was time to pause and consider the true meaning of this holiday. Therefore each Memorial Day there is to be a moment of silence and 3:00 p.m. local time as a simple and unifying way to commemorate our history and honor the struggle to protect our freedom. Paul in writing to the saints at Ephesus seeks to strengthen their remembrance also by his prayer that "the eyes of your understanding being enlightened; that you may know ..." Never forget the grace of our Savior.
