BACK PAIN... I'll admit, maybe not the sexiest topic, but arguably one of the MOST important topics that not enough people are talking about or are aware of. BACK HEALTH & BACK PAIN - I can almost guarantee most of you tuning in have at some point experienced back pain, or maybe you presently have back issues that just don't seem to ever correct themselves. For Dr. Stuart McGill, surgery should be the last resort. In fact, for many patients who don't properly address their pain, surgery is, at best, a temporary option. Consider this: following a disc hernia procedure, many patients, including those Dr. McGill sees, usually fall back into old habits and run the risk of re-injuring their damaged spine. “So you are going to have to change the way that you move [and] become more efficient in purposefully stimulating the adaption that you need in your body to become robust again,” he says. The problem, Dr. McGill says, is that most orthopaedists don't do a full exam of the whole patient to find the source of the pain. That can take two to three hours. “The assessment, if it's thorough enough, will always give you the answer,” says Dr. McGill. And that answer is rarely putting a patient under the knife. Other solutions, which can often include long-term therapies, will get to the source of the pain and lead to long-term healing “95% of the time,” he says. Back pain is the bane of athletes everywhere. About a third of adults deal with it at some point, and that number is even higher for us fitness folk. To make matters worse, the cause of back pain, and low-back pain in particular, is rarely obvious and notoriously difficult to diagnose. It also often strikes out of nowhere, lingers for a while, and then suddenly vanishes for no apparent reason. Or not—sometimes it sets in the for long haul. All this mystery, confusion, and frustration has made low-back pain a reliable honeypot for Internet profiteers, fake doctors, and supplement shysters who aren’t bothered one bit by cashing in on people’s pain. That’s why I wanted to talk to one of the most respected, experienced, and knowledgeable experts in the world on spinal health and function—Dr. Stuart McGill—to help cut through the noise and get an honest, practical, evidence-based take on treating back pain. Dr. McGill has spent over 30 years studying the mechanics of the spine and what causes back pain and how to safely and effectively treat it, has published several books including The Back Mechanic, and through his work has helped tens of thousands of people prevent and eliminate back pain without invasive surgery. Back pain has become the world’s leading cause of disability. Stuart McGill has been at the forefront of non-surgical approaches to addressing back pain for many years. His 2015 book "Back Mechanic: The Secrets to a Healthy Spine Your Doctor Isn't Telling You" is a wonderfully accessible account of his methods and perspectives. McGill spent 30 years as a professor of spine biomechanics at the University of Waterloo in Canada. His laboratory has become a renowned destination for everyday people as well as Olympic and professional athletes from around the world who are struggling with back pain. He is the author of more than 300 scientific publications and 3 textbooks that address issues such as lumbar spine function and injury mechanisms, patient assessment, corrective exercise prescription, and performance training. McGill also consults for many medical management groups, governments, corporations, legal firms, and elite sports teams. He has won numerous awards, including the prestigious Volvo Bioengineering Award for Low Back Pain Research. He released his landmark text, “Low Back Disorders: Evidence-Based Prevention and Rehabilitation,” in 2002. It changed the way coaches, bodybuilders, athletes and non-athletes approached core training. His new book, “Back Mechanic,” is written for a lay audience and addresses common misperceptions about back pain. It also provides a step-by-step guide of the McGill Method to fix back pain. When it comes to back health, what are the biggest misconceptions and what are the exercises that makes the biggest difference in reducing pain? On this episode, we talk to author and doctor of spine biometrics Dr. Stuart McGill and discuss the programs he built to address body deficits and help build a pain free foundation. Three Takeaways If you take away the mechanical cause of back pain and stop the trigger, you’ll reset the threshold trigger a little higher. For good spine health, walking in small doses and small intervals guarantees success. Every time you trigger pain, you trigger easier the next time. At the top of the show Stuart McGill told us about his story and how he got started in his practice. With an interest in optimal spine function, the work he has done relies on his ability to probe a patient’s pain provocation mechanism. This goes against the current medical system trend which, “now revolves around the 15 minute appointment. There is no hope, for the physician to do a thorough enough assessment to arrive at a precise diagnosis of the pain.” Next we went into detail about his work of assessing pain triggers and the book he wrote, The Back Mechanic. By helping his patients understand the nature of their pain he gives them “the liberation of knowing that you have control over your pain trigger.” We also talked about how Stuart believes in coaching people through pain instead of passing intervention. He also provided his best advice for maintaining back health - walking “in small intervals.” We went onto touch on the modern workforce and why sitting in our chairs all day puts a lot of strain on our backs. He also stated the danger of going from that to going straight to the gym and how that can do even more damage. “You can have a mobile spine but you can bear really high loads or you can be a power lifter but you have to be stiffened in order to to do that.” To aid your back in the office, avoid overloading your spine for long periods by getting out of the chair, pushing for the ceiling, inhaling and exhaling. This will give your ligaments normal spinal stability. Toward the end of the show, we discussed chiropractic manipulation and Eastern medicine. It’s very common for strength and conditioning coaches not to measure what that person’s particular body needs. Dr. Stuart McGill stresses the importance of that knowledge. It’s also necessary to remember these key points; the more you trigger back pain you get better at sensing it. After a back injury the body grows more nerves into the disc and every time you trigger pain you trigger easier the next time. Additionally, walking is necessary for back health, “That’s how our bodies were designed.” Dr. Stuart M.
