Three decades ago, Esther Gokhale created the Gokhale Method to apply primal posture to modern pain. Gokhale emphasized small improvements in everyday movements to prevent and heal back pain and musculoskeletal damage. Now, more than 20,000 people have learned to walk, sit, bend and sleep more efficiently by adopting Gokhale's primal posture approach. Dr. D.J. Kennedy, professor and chair of the Department of Physical Medicine and Rehabilitation at Vanderbilt University Medical Center, told us how he sees the Gokhale method work for his patients in this episode. “I think she paints a picture that makes a lot of sense intuitively. And she's also going about trying to get the data to prove it,” said Kennedy. The Gokhale method is about to be tested in a randomized clinical trial at Stanford University. In this MBO episode, we learn how Esther Gokhale developed the method in part due to her own suffering.  Pregnant with her second child, Gokhale was experiencing severe back pain even after a corrective surgery. Gokhale tried physical therapy, chiropractic, acupuncture, massage, yoga, stretching, strengthening, and “checking her head.”  Nothing helped. Thanks to an upbringing in India studying classical dance and observing the posture of local laborers, Gokhale knew that “something was wrong with this picture.”  In the quest for relief, Gokhale built on those childhood observations and her own scientific background to create a new theory of movement based on traditional practices. Soon after, she published Eight Steps to Pain Free Back, which has sold over 300,000 copies.  Today, the Gokhale Method may be poised for even wider acceptance. In addition to online courses and a nationwide network of teachers, the Method now offers a Posture Tracker app to help students reinforce better posture. You deeply love your sport and the community of those who play it. But your body, through pain and injury, is telling you it's time to move on.  At some point in our adult years, we all deal with this transition. And in this episode, our protagonist, Mark, still in his early thirties, talks about his own transition from high level Ultimate Frisbee player (aka Ultimate Mark) to lower risk recreational soccer goalie (aka post-Ultimate Mark) accelerated by the spinal fusion he had back in high school.   In our Season 1 episode, Ultimate Mark described the intensity he brought to the game of Ultimate disc. “I kind of just developed this persona as the person who will get hurt multiple times per game,” he told us. But, as Dr. Mohamad Bydon of the Mayo Clinic explains, the threat of adjacent segment disease made Ultimate an increasingly risky proposition for Mark.  Mark needed a change of sport but feared losing the friendship he'd made through Ultimate. “I dare you to find a better community of athletes anywhere in the world than the ultimate frisbee community,” Mark said.  Can Mark move beyond his Ultimate identity? How can he leave the community loves? Dr. Michele Kerulis of Northwestern University describes the deep emotional attachments we all form to the sports we play. What if back-pain relief was just a few steps away? Esther Gokhale's work with posture and movement finds that just changing the way you sit, stand, lie down, and walk can eliminate your back pain.  Esther grew up in India helping her mom treat orphaned babies and she studied biochemistry at college. Later, she became a licensed acupuncturist. So, when her own pain and herniated disc issues emerged, it motivated Esther to study how people move.  “There were some seeds already sown in my brain that ‘there's something going on with people who live close to the ground that makes sense and that works.' And so, when this horrible thing happened to me, it resonated for me to study techniques that changed the way you use your body,” Esther says. Esther developed and trademarked the Gokhale Method, an eight-step plan to adjust your back's curvature and movement from an S or C-shaped curve to a J-shaped curve. Esther found this technique by studying the common thread between ancient cultures and today's primal cultures: “If you go to your museums and you look at the Greek statues, they're doing exactly what these Ubong guys are doing, they look the same. And so why would that be? Well, the simplest explanation is that it is natural. That's why they converge on the same form. For Dr. Stuart McGill, surgery should be the last resort. In fact, for many patients who don't properly address their pain, surgery is, at best, a temporary option. Consider this: following a disc hernia procedure, many patients, including those Dr. McGill sees, usually fall back into old habits and run the risk of re-injuring their damaged spine. “So you are going to have to change the way that you move [and] become more efficient in purposefully stimulating the adaption that you need in your body to become robust again,” he says. The problem, Dr. McGill says, is that most orthopaedists don't do a full exam of the whole patient to find the source of the pain. That can take two to three hours.  “The assessment, if it's thorough enough, will always give you the answer,” says Dr. McGill. And that answer is rarely putting a patient under the knife. What if back-pain relief was just a few steps away? Later, she became a licensed acupuncturist. Not quite. 1. How much protein can one absorb per meal? [20:09] Leslie says: I am a 70-year-old female.  Paleo and low carb for many years. Avid podcast listener and book buyer - love all of yours!  Don't want any more sarcopenia than I already have.  Followed your advice and went on the Ketogains macro calculator that recommends about 100 gm protein per day.  But I'm also working on lowering insulin by only eating 2 meals per day.  Searching tells me that bodybuilders think they can only absorb 30 gm per meal.    I eat berries, collagen, protein powder (grassfed!), goat kefir and powdered multi vitamins mixed together along with 4 oz meat and 2 eggs for breakfast around 11 am.  For dinner I eat 4 oz meat with some Amy's organic soup and sometimes a salad.  I am not aiming for ketosis, just low carb. I could stand to lose a few pounds, but at 5'4" and 135 at my age I am OK but would like more muscle or at least not lose any more.  Should I add a third meal for some of the protein or perhaps redistribute the protein amounts?  Thanks! I do this workout about once a week, https://www.youtube.com/watch?v=dBbPsJKq9uM     1 hour of Zumba about 3-times per week and miscellaneous other exercises.  Before Covid I did more, including some blood-flow restriction  which seems to work on the same principle as the video I linked.  I feel like it takes about a week for me to recover enough to want to do the video workout again.  I don't do the lifting-the-bag part of the workout - my back isn't perfect.  I also recently added Esther Gokhale's daily workout (1,2,3 move) and should add the associated weight workout as well.  I liked Doug McGuff's concepts and have listened to the IHMC podcasts as well as Peter Attia's stuff.  Like I said, I am an avid podcast listener!! 2. Thru hiking, backpacking, and paleo/ketogenic diet [25:08] Matt says: I have been following a paleo diet for 5-6 years now, and transitioned to a ketogenic approach at the beginning of this year.  Since then I've been able to maintain a much lower  body weight than ever before in my adult life and my endurance and fitness levels (and to a lesser degree strength levels) have been better than ever before.  These past couple years I have gotten more into thru hiking and backpacking and have been planning some longer hikes and a long term goal of doing the entire appalachian trail.  My question is; to your knowledge is there anyone developing programs or templates on planning long hikes primarily utilizing ketogenic and paleo foods?  Everywhere you look when planning thru hikes the advice is largely the same (granola, oatmeal, instant mashed potatoes, and candy for 'energy'), but it seems obvious that 6 months on a trail requires primarily oxidative phosphorylation, and KD or paleo seems to be an obvious choice.  Information on the topic seems pretty slim, especially when it comes to planning and packing for at least a weeks worth of sustenance that would be necessary when out in the wilderness.  With as many ultra runners and endurance athletes beginning to tout the benefits of KD, I am wondering if there are any thru hikers doing the same thing? I'm into my fourth week and feel I'm doing relatively well.  I've got a spreadsheet where I track all my food, stay on track well although I do have a Perfect Keto Whey shake for breakfast everyday.  Not ideal but helpful given my vocational and avocational commitments.  Four weeks in  and I've lost 8 pounds and am in nutritional ketosis based on measuring blood ketones.  One of the cookbooks I've picked up mentions recalculating your macros periodically.  I've done that and it drops my macros slightly based on weight loss to date.  Seems reasonable to me, but is this something you also recommend doing? More people with platforms like yours need to speak out. The hypocrisy, the nonsensical rules (bars are open but kids can't go to school), the suppression of seemingly effective CHEAP widely available prophylaxis and treatments....its all SO INFURIATING and it is so good to hear both of you question what is happening on your platform. THANK YOU. I've unfortunately had to distance myself from a few health "influencers" that I once really respected who have gotten very SJW and holier than thou this past year, so I am thrilled that I finally started listening to your podcast and seem to have found some kindred spirts. Definitely makes me want to join the Healthy Rebellion too, which brings me to my question... I am 37, work out moderately (strength training 2-3 times a week, trail running or walking 1-2 times a week), get lots of outside time, pretty good sleep, and eat 80-20 primal (stick to whole foods, but will indulge in ice cream, white rice, or other no-nos on occasion). The primal eating with a focus on protein and fat has done wonders for my hunger and "hangry" symptoms, which used to be a constant plague (who knew that a breakfast of Cheerios, skim milk, and a banana did not a satiating meal make). However, I've always been about 20 lb overweight, and still feel the need to eat every few hours to avoid irritability, brain fog, and shakiness. This tells me I'm definitely not metabolically healthy, which is my ultimate long-term goal (although weight loss would also be nice). However, I'm currently 6 months pregnant with my second child. I plan to breastfeed this little guy for around 2 years like I did my first, who I weaned when I was about 10 weeks pregnant. In other words, I've been either pregnant or nursing now for the last 3.5 years, pretty much ever since I discovered "Wired to Eat" and a more paleo lifestyle. Healthy pregnancy and breastfeeding are my top near-term priorities, therefore I don't think doing something like going full keto would be right for me, but I would love to start the journey towards better metabolic health. Where should I start? And, related to the Healthy Rebellion, I'd love to join but am not sure it will be worth it in the near-term given my current maternal journey. From the time we are born, we are conditioned to sit, sleep, walk and stand in a certain way. We are directed to “sit up straight” and to stick our “chest out” when we slouch. These ideas are actually counterproductive and do not correspond with how our ancestors sat, stood, and moved. The result? Back pain, digestive issues, shoulder and neck impingement, and more. Esther Gokhale knows this firsthand. She had excruciating back pain and resorted to several surgeries before she discovered the root of the problem: poor posture. Introduction 4:05 Specialises in yoga/yoga therapy for low back pain/sciatica. She has experienced these for a long time. Developed back issues as an adult. Researched for solutions. In Canada in 2003/04 she began to find  yoga intensives for specific conditions. Found Feldenkreis and took trainings because it helped her back pain. Yoga was  too generalised for the senior classes she was teaching. 7:34 Pulled together different modalities to help with back pain. She found PNF (proprioceptive neuromuscular facilitation) to be helpful which stretches and strengthens, tones and relaxes muscles. Applied to her senior’s classes. Found Viniyoga with Robin Rothenburg – yoga for low back pain course. Also interested in what Dr. McGill could add to the problem. Put all of these modalities together to develop a workshop and classes. 12:36 Major causes of low back pain a) the L5-S1 area very vulnerable to wear and tear, b) bulging disk (not herniated discs)  which usually results in mild or moderate pain, c) breakdown of facet joints – repetitive movements over time cause wear and tear, d) slouching – more load on the spinal discs. Need to lengthen through the spine, improve posture through strengthening back muscles. L5-S1 needs to be in slight anterior tilt. 16:15 Most students are over 50. Jo offers a continuum of classes, one on one sessions to work on their low back pain. Usually can go into a class with mild pain may need one on one sessions for more moderate pain. Asks questions and does an intake to place them in the correct class/session. 18:16 Joanne asks yoga teachers to think about: “I ask teachers to consider this: we are teaching a 21st century body & mind a practice that was designed thousands of years ago. A practice designed to prepare the body for meditation and for thin male figures.  The spine itself has changed shape in the last century alone and we have become much more stationary.” 19:19 Esther Gokhale found that the spine shape has changed in the last century because of changes in the design of furniture as well as slouching. The spine normally has a J curve which has slight curves throughout its length until the bottom of the spine. But we now have an S curve spine that is much more curved than before. And the spine has less support. Do students really need backbends and forward bends if they have back pain? Are they properly prepared for these asanas. Telling people to roll up from a forward bend does not engage the back muscles; the spine is doing all of lifting. 23:15 Many more cases of back issues with the change in the shape of the spine. J curve is less curvy with more support. 28:00 Students know what feels good and what doesn’t. Students asked when back pain occurs – bending forward, bending backward, what feels good or bad? Starts with really gentle postures and notices how students move. She may decided to take change and change the postures if students seem to be in pain. Students will not always follow directions to come out of a posture or pick a more appropriate modification even though invited to do so. Courtney’s exposure to biomechanics and rehabilitative exercise started in her early childhood, as an elite level gymnast, kayaker, and ballet dancer. At age 16, she left home to pursue her professional career with The Houston Ballet, where she remained for 5 years until a severe ankle injury forced her to retire.  After retirement, she transitioned immediately into a two-year course of study in rehabilitative exercise and Pilates, under the supervision of nationally renowned Pilates instructors Bryan Peters and James Harren, who had trained with the original colleague of Joseph Pilates himself. She then earned her certification through the Pilates Method Alliance and became a top-rated trainer for the following 6 years.  Following the birth of her first child, however, she began to suffer from injuries related to pregnancy and childbirth, none of which her prior core training was able to resolve. This realization led her to study with Thomas Myers, Yamuna Zake, Esther Gokhale, and Stuart McGill, all prominent leaders in the field of spine health and pelvic stabilization.  From there, Courtney became certified as a personal trainer through National Academy of Sports Medicine and soon after was granted their advanced certification as a Corrective Exercise Specialist. She also carries a Prenatal and Postpartum specialization from NASM and multiple certifications in pelvic rehabilitation, injury prevention, and activated release therapy. Her unique method as taught in MommaStrong is the product of this comprehensive background, helping mothers all over the world finely tone their body’s through clinically approved, and feasible workouts that instigate an adventurous life through strength that matters. Back Spasms and Problems 101   "Most back spasms are from either not stretching and warming up or from sitting all day. You really can't escape back spasms. Even if you're physically active, they're impossible to avoid. When we think of back pain, we usually imagine someone suddenly throwing out their back. But back pain can happen day to day and even the simplest of movements can cause it to happen. Recently, Devon experienced a horrible back muscle spasm the day after a dance class and we had the idea to talk about back spasms on the show. It's a great group to talk with other people about health, share tips, and encourage each other to keep living a healthy lifestyle.   Good Posture 101 This week, the Posture Guru of Silicone Valley aka Esther Gokhale of the Gokhale Method, joined myself to talk about the importance of good posture.  Have you ever wondered how you can improve your back's posture at your desk or whether a standing desk would be better for you? Esther answered a ton of our questions and gave us some good, basic stretches to begin our way to healthier and stronger bodies.  Primal Posture vs. Posture Today Unlike today, our primal ancestors were living a certain lifestyle by hunting and gathering each day. Overtime, they developed specific movements and postures fit for that lifestyle. Now, times have changed and today we're doing different things with our bodies by sitting at a computer desk rather than squatting and bending over to do different activities like picking up sticks for firewood. We've lost a lot of kinesthetic wisdom in first world cultures. Yet there are many cultures out there, especially in Africa, that continue to teach younger generations how to hold their posture as far as how to hold babies, work in the fields to grow and gather crops, lying down to sleep, bending over to cook etc. All of this without fatiguing themselves and running out of breath when we're having difficulties simply bringing groceries from the car into the kitchen.  Keep in mind, learning how to have good posture is not that simple. As Esther put it, just like a language, good posture takes time and practice to learn. In order to see great improvement in your posture, hans-on assistance and coaching is usually necessary.     What's Better? A Sitting Chair or  a Standing Desk? Both are good because we need change throughout the day, but we also need plenty of rest. If you find yourself standing all day (like older generations did when they worked and stood in factories), you will have many back and muscle pains from your poor posture.  Compared to standing, sitting also helps a person have deeper thoughts and better motor skills.   Improve Shoulder Posture Esther's tips for good shoulder posture: One at a time, bring one shoulder forward a little bit.  Bring your shoulder up a little bit. Move your shoulder back and relax. Repeat with the other shoulder. You will experience better circulation, breathing, and feel more comfortable overall after doing this simple exercise.  How to do Stretchsitting Sit with your bum back in the seat. Hinge away from the back rest. Lengthen your back with a crunch by folding your rib cage forward. Maintain your forward fold while slowly using your hands to push your top half and lengthen more (push on arm rest if they're there). Hinge back towards the back rest (use a towel or the Gokhale Method chair) to help stick/hook yourself to the back of the chair. Relax and your weight will be attached to the towel and you will have traction against the chair. Feel free to do a shoulder roll afterwards. You're done! Stretch sitting can be done anywhere and at anytime. Mix things up by standing up, walking around, or do some other light stretching from time to time (every 20-30 minutes).       About Esther Gokhale Esther Gokhale (Go-clay) has been involved in integrative therapies all her life. As a young girl growing up in India, she helped her mother, a nurse, treat abandoned babies waiting to be adopted. This early interest in healing led her to study biochemistry at Harvard and Princeton and, later, acupuncture at the San Francisco School of Oriental Medicine. After experiencing crippling back pain during her first pregnancy and unsuccessful back surgery, Gokhale began her lifelong crusade to vanquish back pain. Her studies at the Aplomb® Institute in Paris and years of research in Brazil, India, Portugal and elsewhere led her to develop the Gokhale Method, a unique, systematic approach to help people find their bodies’ way back to pain-free living. Gokhale practiced acupuncture and taught posture, dance, and yoga in her Palo Alto wellness center for over twenty years. The Gokhale Method continues to grow as qualified Gokhale Method teachers now teach the method worldwide. Gokhale's book, 8 Steps to a Pain-Free Back, has sold over 100,000 copies and has been translated into eight languages. Author, teacher and therapist Esther Gokhale explains how our society's view of a healthy spine has drastically changed over the past 100 years due to fashion and industrial design, and why back pain has become a huge problem. Gokhale's international insight crosses all social boundaries to look at the human being in motion; what healthy movement is, and how we can recover our ability to stand, sit or bend and feel great.
