On December 7, 2023, our host Michelle’s grandson was rushed to the hospital in an emergency situation; unbeknownst to his family, the ten-year-old had a circulatory malformation in his brain. This arteriovenous malformation (AVM) had burst, bringing him dangerously close to death as blood flooded his brain; after multiple surgeries and a 40-day hospital stay, he is now home recovering. Michelle turned to TikTok to learn about the issue and seek guidance from others and found Kristi Kelley. Kristi’s AVM ruptured while she was gardening in her 40s. Now, years later, she might have partially lost her eyesight, but in that struggle, she has gained a positive attitude, gratitude for the life and the time she has, and a passion for connecting with others struggling with the impacts of AVM. Abstract: Cerebral Cavernous Malformations (CCMs) are vascular lesions that predominantly form in blood vessels of the central nervous system (CNS) upon loss of the CCM multimeric protein complex. The endothelial cells (ECs) within CCM lesions are characterised by overactive MEKK3 kinase and KLF2/4 transcription factor signalling, leading to pathological changes such as increased EC spreading and reduced junctional integrity. Concomitant to aberrant EC signalling, non-autonomous signals from the extracellular matrix (ECM) have also been implicated in CCM lesion growth and these factors might explain why CCM lesions mainly develop in the CNS. Here, we adapted a three dimensional (3D) microfluidic system to examine CCM1 deficient human micro-vessels in distinctive ECMs. We validate that EC pathological hallmarks are maintained in this 3D model. We further show that supplementing the ECM with distinct forms of Hyaluronic Acid (HA), a major ECM component of the CNS, alters CCM1 biology, independent of KLF2/4. This study provides a proof-of-principle that ECM embedded 3D microfluidic models are ideally suited to identify how changes in ECM structure and signalling impact vascular malformations. Abstract: Pathogenic ACTB and ACTG1 gene variants, encoding the actin isoforms {beta}CYA and {gamma}CYA, respectively, are frequently associated with the Baraitser-Winter-CerebroFrontoFacial syndrome (BWCFF-S) that includes malformations of cortical development. Here we explore whether cerebral organoids grown from BWCFF-S patient-derived induced pluripotent stem cells can provide insight into the pathogenesis underlying the cortical malformations of these patients. Cerebral organoids expressing either an ACTB or an ACTG1 gene variant, each with a point mutation resulting in a single amino acid substitution, are reduced in size, showing smaller ventricle-like structures with a thinner ventricular zone (VZ). This decrease in VZ-progenitors is associated with a striking change in the orientation of their cleavage plane from predominantly vertical (control) to predominantly horizontal (BWCFF-S), which is incompatible with increasing VZ-progenitor abundance. The underlying cause appears to be an altered subcellular tubulin localization due to the actin mutations that affects mitotic spindle positioning of VZ-progenitors in BWCFF-S. Something happened to Jean one day and when she awoke in the hospital 5 days later Jean's life was changed forever. Jean was diagnosed with something called AVM (arteriovenous malformation). Jean was left-side dominant before the accident but after she couldn't move anything on the left side. Jean had to learn all over again, this lady absolutely will not quit and has come a long way. Going from not being able to talk to getting her licence back and driving, this is one story that will impress you. The 2013 ARUBA trial concluded that medical management alone is superior to medical management with interventional therapy for patients with unruptured brain arteriovenous malformations. A study was conducted to analyze AVM treatment and epidemiologic response to the ARUBA trial. Following ARUBA, there has been a stark decrease in interventions for uAVMs. Nationwide rAVM discharge incidence is inversely correlated with uAVM intervention rate. In general, the primary difference between disinformation and misinformation is intent. Both are forms of fake info, but disinformation is created and shared with the goal of causing harm. Usually, misinformation falls under the classification of free speech. On the surface, disinformation and misinformation have a lot in common: They're both types of false information. In fact, misinformation can sometimes be disinformation, and disinformation can give way to misinformation. But despite their similarities, the two terms aren't exactly interchangeable.

Survival for glioblastoma remains very poor despite decades of research, with a 5-year survival of only 5%. Technological improvements that have revolutionized treatment of ischemic stroke and brain aneurysms have great potential in providing more precise and selective delivery of cancer therapeutic agents to brain tumors. In this podcast, JNIS Editor-in-Chief, Dr. Felipe C. Albuquerque speaks with Dr. Stephen Chen (1)and Dr. Peter Kan (2) about their paper "Perfusion-guided endovascular super-selective intra-arterial infusion for treatment of malignant brain tumors". The paper describes a technique using perfusion guidance to enhance the precision of endovascular super-selective intra-arterial (ESIA) infusions of mesenchymal stem cells loaded with Delta-24 in the treatment of glioblastoma. After extensive testing and investigations Maggie was diagnosed with a rare condition called Uterine Arteriovenous Malformation (AVM). It has since been 2 years & at the age of 28 Maggie is currently 3 weeks post op after having a hysterectomy due to ongoing complications. Maggie & Mon discuss the pressures that society places on women to have bigger family's without considering any of the post-partum issues and hope that this episode provides better education and awareness surrounding this topic. Congenital pulmonary airway malformations, also known as CPAM, are masses of abnormal lung tissue that form antenatally. They were until recently known as CCAMs, or congenital cystic adenomatoid malformation. Affected patients may present with respiratory distress in the newborn period or may remain asymptomatic until later in life. Many cases are now detected by routine prenatal ultrasound examination. Sharon's passion for horses was first noticed when she was caught riding a bush horse at 4 years of age. She would climb up the front leg and haul herself up onto the back via the mane proud as a peacock. She still doesn't know to this day how she survived, but by 14 she was working with racehorses and riding competitively in the show ring and winning up to Open Minor Royal classes by 18. Her desire to understand the biomechanics of the horse led her down the path of dissections and research and if that wasn't enough, she built horse skeletons where students could move the limbs so to have a better understanding of the leg in motion. This became a critical factor when she rehabilitated or assisted in the selection of horses for performance riders in Australia and overseas. Her keen eye for detail saw her involved with horses that became or were already the Australian Champion in 8 different disciplines, with some competing at WEG and or the Olympics. With Equine degrees and a number of other qualifications in tow, Sharon's research and published scientific papers have seen her lecture around the World as a key note or guest speaker at numerous events. So with this in mind, Sharon will be speaking on one of the current topics associated with her research - Equine Complex Vertebral Malformation (ECVM), or the Congenital Malformation of the 6th and 7th Cervical Vertebrae.

Sex disparities in acute ischemic stroke outcomes are well reported with IV thrombolysis. Despite several studies, there is still a lack of consensus on whether endovascular thrombectomy (EVT) outcomes differ between men and women. In this podcast, we discuss the Selection for Endovascular Treatment in Acute Ischemic Stroke (SELECT) study, in which women had similar discharge outcomes as men following EVT, in a propensity-matched cohort, but the improvement from discharge to 90 days was significantly worse in women, suggesting the influence of post-discharge factors. Serious complications associated with VSS are rarely reported. In this podcast, JNIS Editor-in-Chief, Felipe C. In episode 112 of The Monica Kade Podcast, I chat to Claire Rackley, an Australian Artist and Abstract Acrylic Painter. Her art intentionally provokes emotions of joy and peace through the use of bright colours and bold strokes. But she wasn't always an artist as such. This particular career trajectory came to life after undergoing brain and spine surgery in 2015. Claire used her creativity as therapy to heal and recover from the trauma, but as time progressed people began to ask to purchase her artworks. She has an interest in mental health and how colour can impact a person and their emotions. What caught my attention with Claire was her love to impact those in the waiting and recovery rooms of hospitals and medical practices with beautiful tiles on the ceiling, in the hope it would impact their state of being. This woman is changing the world and making such a profound impact that I can't wait to see how it ripples out into across the world. "Creativity brings life.
