About our Guests: Dr. Emily Olson is a pulmonary and critical care medicine fellow at Northwestern Feinberg School of Medicine. She attended medical school at the University of Wisconsin School of Medicine and completed her internal medicine residency at Mayo Clinic in Rochester, Minnesota. In addition to her work on gender disparities in procedural training, Dr. Olson is interested in clinical feedback and transitions in medical education. Dr. Lekshmi Santhosh is an Associate Professor of Medicine in the Division of Pulmonary/Critical Care Medicine and the Division of Hospital Medicine at UCSF. She practices in the MICU, neuro ICU, on the Internal Medicine teaching wards, and at the Pulmonary Outpatient Faculty Practice at UCSF. Although Chinese marriage law states that men and women are equal, implementation of the law in divorce cases reveals gender disparities. In divorce lawsuits, women are often left without remedies for domestic violence and mothers lose custody of their children. Rural women face the additional disadvantage of lacking resources to help them navigate the divorce process. On this episode of JHLT: The Podcast, the JHLT Digital Media Editors explore two studies from the May issue of The Journal of Heart and Lung Transplantation. Digital Media Editor David Schibilsky, MD, a cardiothoracic surgeon from Freiburg, Germany, hosts this episode.   First, Dr. Schibilsky and Digital Media Editor Erika Lease, MD, interview their first guest, Helen A. Hannan, the podcast's first-ever undergraduate pre-medical guest, from the University of Michigan. Helen was the lead author on the study “Racial and Gender Disparities in Transplantation of Hepatitis C+ Hearts and Lungs.” The study noted that prior research in utilization of kidneys from donors with Hepatitis C had shown disparities due to gender and education—and wanted to see if this was the case in heart and lung donors as well.   Drs. Schibilsky and Lease chat with Helen about the findings of the study, including some of the interesting differences shown between heart and lung recipients, and the ramifications for better patient discussions at the clinical level.   Next, Dr. Schibilsky and Digital Media Editor Marty Tam, MD, interview their next guest, Lauren Truby, MD, of the University of Texas Southwestern Medical Center in Dallas. Dr. Truby is the podcast's first return guest, having appeared on the show near the end of 2021. Let's make this summer your most productive season yet! TRANSCRIPT  Three time wasters for mom founders and how to replace that time with more productive activities. I don't know about you, but I have three children. I am looking at the summer season and I cannot say that I'm going to have more time to devote to my business and my practice where I'm teaching mom founders like you to leverage the art of strategic networking for profit. So what I have to do every single season right before is really map out The most optimal use of my day hours. So I wanted to give you a sense for some of the things I've noticed, some patterns I've noticed when I've been coaching mom leaders throughout these last 12 years. So here's the first time waster that I have noticed that are stealing money straight out of your pockets. Okay. So time waster number one, second, guessing yourself. I'm just going to say it like it is. You are having sex with your self doubts, and apparently it seems to be something that feels very good because we do it quite often, and I include myself in this as well. Oftentimes, it is hard to really almost take the wheel back of our days when it is us who is captivated by This idea that no, I don't know how to sell myself or no, I shouldn't talk to that person because I don't have a warm intro and what happens is essentially you're just there stuck looking at a long list of human beings that most likely will want their problems solved. Buy the person that's gonna call them and you do nothing you do nothing and the reason why I share this is because I was talking to a mom founder for example she literally has warm leads in her linked in right now we're talking about people. Who have raised their hands and said to this person that they want to connect with her and potentially they want to do business with her. Certainly they would like to make money with her. And yet there's this hesitation. There is this hesitation to do the deed and reach out to people that are. Literally warm leads in your LinkedIn platform. And that is a time waster. This idea that we're looking at something multiple times in a day and wondering to ourselves, should I do it? Can I do it? How will I do it rather than starting to take the steps necessary to actually reach out to those people and follow up with every single one. So I wanted to just share a potential solution. This is going to sound so obvious, but I want you to actually take action on this one. I invite you to set a timer. It could be 20 minutes. We could consider the Pomodoro process where it's like 20 minute increments and then some breaks. Set a timer and get that list of warm leads in your LinkedIn platform done. It sounds simple. It totally does. But I want you to ask yourself, like, why haven't you done it yet? Like if you actually have people, again, who have raised their hands in your LinkedIn said, I want to do business with you. Certainly at minimum, I would like to make money with you. Why have you not actually gone and done this follow up? Right? So that's the first point. Second, guessing yourself. It is often very, very enticing, very, very alluring to just think about thinking about and thinking about thinking about. And I've noticed this so, so often, especially with a lot of mom founders who are hella educated. Because I think we had the luxury when we were in business school or law school in grad school to really reconsider and re argue and recreate and reinvent the wheel where right now the market moves quite quickly. And I don't think we have that luxury as a mom founder, you don't have that luxury unless you have a lot more time than me, then my God, please. Tell me, how do you have more time? So that's the first point. Here's a second time waster for mom founders. And again, how you might want to replace that time with more productive activities. So there's this idea and I hear this time again, and it frustrates me just because there have been so many opportunities and moments in my life where I could have said the same thing. And even had the same thoughts. Here's this idea. The idea that you are not getting investment dollars as easily as men who can at minimum just spit an idea on a napkin and somehow magically conjure up millions of dollars in investments. I want to ask you for real, for real, how does having that thought help you? Now, if you need that chip on your shoulder, if it drives you crazy and it makes you fight harder than amazing, but if you're wasting your already stretched out mental bandwidth on using this excuse or thinking about it, Then, again, you are wasting your time. You really, really, really are. And I believe that as moms, our superpower is having a fertile imagination. Just the other day, I was interviewing one of the most influential people in the book industry, Zibi Owens. And her imagination is wild and amazing and wonderful. And so she's building out her business. Now if she had to just think to herself, my goodness, all these guys that are at the helm of leadership and other organizations, they have it so easy. Maybe I know from the people with whom I've interacted, let me not even try. Then I just can't imagine that she would be one of the most influential people in publishing. I just can't see it. Instead of thinking about that idea, that whole, like, drawing a napkin on the table. It must be so easy for a guy. Instead of that, why don't you find a guy, quote unquote, it doesn't even matter if they are a guy. Why don't you find a person that you believe literally just drew on a napkin and sold their idea for millions of dollars and ask that person to mentor you. You're going to think that is wild and crazy, but you know what? I've done similar things in the past and I've approached strangers in libraries. If you're one of those strangers in the libraries, yes, it's me. And I have absolutely figured out a way to build rapport with strangers, connect with them, turn them into connections. And also add value by my way as well, not just me taking from their insights and wisdom. And so again, if this is your thought, okay, great. How can we leverage that idea in your favor and take action? So you stop wasting your mental energy unless you have extra energy. I don't know. I don't know. There are some moms that have like A lot of energy, but I just want to invite you to imagine this idea that you can actually step up to a person that you think just drew on a napkin and got millions of dollars, and you could just ask them to teach you how to draw. You could go to a supermarket, buy a whole, Oodle of napkins and just start drawing like a mofo. And let's see what comes up for you. Certainly something will come up. That's going to be more productive than just thinking in your mind that I wish I were a single guy. Okay. That's the second time guzzler third one for mom founders. And again, how to replace this time guzzler or waster with more productive activities. So spreading yourself and your efforts across a  wide array of activities. And I want to be really nuanced here, specifically doing this, spreading yourself across a wide array of activities, because that one activity, that one activity that you originally were thinking about doing, because you knew in the heart of hearts that that activity would turn around your business, such as having B2B conversations. Whereas in years before you've only had. B2C conversations, right, where that one game changing, I've never done it before idea started to get hard because you were actually starting to take the steps and doing that big, crazy, unnatural for you idea. Let me be super clear when I say that. So let's just imagine. That you have always sold to individuals. You have an organization. You've always sold to individuals your product. And now you're trying to gain distribution at big box stores. Let's just imagine. And you know, like, okay, like if I crack that code, then I'm going to make a Real progress towards my dreams. Then I'm going to have a business that is like a really kick ass amazing business here. Right. And so that idea, it sounds wonderful. Like a fantasy. It sounds amazing. Like a goal. It sounds amazing. Like even a napkin drawing, but then once you start taking steps and you hear rejections, or you're not getting callbacks or. People are questioning, well, how many units have you sold? And what is the, what was it? I remember when I worked at Procter Gamble, the whole like shelf, uh, velocity, right? Like how quickly can it fly off the shelf idea? When those numbers just don't land in terms of the ones that you offer to the person that is in charge of distribution, like that is when all of a sudden you'll say to yourself, you know what? Let me just do more of B2C. Let me just do more of B2C in all these different ways. Let me just do anything other than this brand new thing that I set out to do. And that is where you start to lose focus and the impact that you can have on your business really is diluted for sure. So what would be the best way to actually replace that time waster with something that is a lot more productive? So something that will be a lot more productive is to go back to the original strategy and find someone who executed precisely on that strategy, maybe ask them to mentor you, ask them for templates, pay for their templates, hire them as a coach, and do it in differently, right? So if whatever it is that you're doing is not producing the results that you imagine, that's one side of it. The other side of it must be or could be that the sales cycle is going to be longer. And so that's the other side of it. So if you intend to actually make some progress in that way, it's important to just go back to the drawing board, but not like erasing the whiteboard, but go back to what you originally intended to do, what you know in your heart to work. Okay. And figure out how to crack that nut or code or whatever it is that you're selling or trying to accomplish.  If you are living any of these three time wasters, right? And let's just imagine that your cash runway is running out. Then I want to invite you to imagine this scenario. This is a crazy idea that I am so excited about. Because This is how I support my mom founder clients. So picture it. Yes, we are going to Sophia Protillo from the Golden Girls. That's what she used to say. Picture this. A power packed three hour zoom session right from the comfort of your home. We do it over zoom. So you don't have to sacrifice time from your business or your family. And during this intensive session, I am your guide. We literally hop on zoom, share screens and start diving into the trenches together, reaching out to potential investors, maybe podcast guests or partners or media giants in real time. So you know that list of warm leads? Yeah, we're going to execute on it real time together so that this way your spirit and my spirit are co creating some happy flow of progress. And we are not stopping to let you think over, think, rethink, or re imagine. So, This idea of a three hour session with me, it is called a VIP day. It is a VIP day that you will be very prepared for because I'll provide a questionnaire up front. You will have an opportunity to really think through. How we will leverage every single minute, because every single minute counts for a mom founder world, we'll have the opportunity to, again, before we hop on that three hour call, I will know what your business goals are, your target investors, or potential podcast guests and partners or media contacts, right? And also what challenges you are facing, whether they are more mindset or strategic or tactical, right? And we're just going to be making sure that those three hours together are maximized, just totally maximize. Then real time. Live on zoom. It's just about action. We're going to roll up our sleeves and dive into the heart of Networking reaching out following up doing everything that feels hard to do when you're by yourself Because you don't have someone telling you like do the work It's not like you have this boss, right? And we're gonna actually implement everything that I just shared with you as far as execution in real time So we're gonna leverage some of the insights from your pre work We're gonna identify the people as I said all on the spot And you're going to see firsthand how you can actually craft some compelling outreach messages and make meaningful connections that matter. And it does not have to mean you are being stopped or paused by gatekeepers. That is actually what makes my process different. I don't actually engage with gatekeepers. Fun fact. All right. So. Again, a lot of this outreach tends to be so daunting, especially when you're doing something new, exploring a new strategy. But with my guidance, you're going to get it done. I mean, whether it's through email, LinkedIn or another channel, you're going to have a clear plan of action and you're going to have a ton of confidence to execute this real time. Okay, so I think it's so important. Also, follow up. I know a lot of times, of course, like one point out, one reach out, one email is not going to be get you all these results immediately. Although, for a client, it only took an hour for her to get On the calendar of someone in an insurance company, but let's just imagine we're reaching out to 50 people, a hundred people, and some will not get back to you within that three hour timeframe. So I'm going to make sure that you know how to follow up with the individuals that we reached out to during that three hour, let's just call it sprint. So at the end of this VIP networking day, you're going to feel empowered. energize, and you're going to be totally unstoppable. I think it's so urgent right before the summer lets out that you have optimized your time in terms of outreach efforts, that you have absolutely done as much as possible to put the ball in your court so that things start moving towards the direction of your biggest ambitions. Women and minorities continue to be underrepresented in patent issuing and less often are granted credit for their innovations. We examine why this is, the impacts it has, and what can be done about it. Patents, and the protection of inventor rights, was deemed important enough that when the U.S. Constitution was ratified in 1788 it included what is now known as the intellectual property clause: Article I, Section 8, Clause 8, which reads “[The Congress shall have Power . . . ] To promote the Progress of Science and useful Arts, by securing for limited Times to Authors and Inventors the exclusive Right to their respective Writings and Discoveries.” Our guest in this episode is Lisa Larrimore Ouellette, whose latest research looks at inequality in the patent system and how that impacts innovation. The World Health Organization highlights that low- and middle-income countries such as India must increase coverage of priority health services to accomplish health-related sustainable development goals. While the majority of hospital CEOs in India are physicians, the opposite is the case in the UK and in both countries MD MBA graduates are rare. In the National Health Service, there are increasing efforts to involve physicians in senior management teams and to encourage flexible and portfolio working. Our research project findings provide valuable insights into inequalities, (quiet) quitting, and medical career crafting to understand medical leadership development and engaging physicians in institutional decision-making to improve clinical outcomes. Around the world, the coronavirus pandemic has taken an especially high toll on women and girls. From public health to education to jobs and livelihoods, studies have revealed a gender disparity in the impact of COVID-19 that is particularly wide in lower- and middle-income countries. Yet for all the work that's been done, experts say there's still a lot they don't know about how these impacts are being felt across different communities. To help address this problem, the Center for Global Development recently launched a new initiative to analyze the gendered impacts of the pandemic and study policy responses around the world with the aim of addressing the long-term causes of gender inequality. UNESCO DONATES TO EDUCATION: A UNESCO donation of some $18,000 to the gender and education divisions of government last week marked the official start of a project, which seeks to explore gender disparities in education in Saint Lucia and make policy recommendations to close any existing gaps. The project is known as the Gender Dynamics in Academic Attainment of School-Aged Children in Saint Lucia Before and During COVID -19 and has as its main objectives to compare the academic performance of male primary school children with that of their female counterparts and to explore the extent to which there are gender disparities in the instruction, assessment and discipline of primary and secondary school students in saint Lucia. The project, according to Secretary General of the national Commission for UNESCO Marcia Symphorien, compares gender differences in performance of school aged children in face-to-face and remote instructional/learning settings and provide policy and future research recommendations to close any gender gap in educational attainment on the island. Minister with responsibility for Gender Relations Hon. Virginia Albert-Poyotte, who was an educator by profession, says gender inequality remains one of the greatest threats to sustainable development. She highlighted male underachievement as a very disturbing concept requiring well-grounded research that is open and asks the right questions, while at the same time noting the plight of women facing increasing rates of unemployment during the covid pandemic. The expected target groups include primary school boys and girls from grades four and six, primary school teachers and principals in four educational districts and top 2019 Common Entrance Examination results placings in selected Districts. Permanent Secretary in the Department of Education, Innovation and Vocational Training Michelle Charles says no child should be left behind, and that the policies and programmes that administers education in Saint Lucia must reflect that in ensuring there is equal opportunity and nurturing for both male and female. On today's episode, Dr. Pamela Kunz, director of the Center for Gastrointestinal Cancers at the Yale School of Medicine, and vice chief of Diversity, Equity and Inclusion for Medical Oncology at Yale, discusses compelling sessions from the 2021 ASCO Annual Meeting that addressed gender disparities in the global oncology workforce and sexual harassment experienced by oncologists.   Transcript: ASCO Daily News: Welcome to the ASCO Daily News Podcast. I'm Geraldine Carroll, a reporter for the ASCO Daily News. My guest today is Dr. Pamela Kunz, an associate professor of medicine in the division of oncology at the Yale School of Medicine where she also serves as the director of the Center for Gastrointestinal Cancers. Dr. Kunz also serves as the vice chief of Diversity, Equity, and Inclusion for medical oncology at Yale. Today, Dr. Kunz will highlight strategies to dismantle gender disparities in the global oncology workforce featured at the 2021 ASCO Annual Meeting. She will also tell us about the first study in oncology to systemically characterize the incidence of sexual harassment experienced by oncologists. Dr. Kunz reports no conflicts of interest relating to our discussion today and her full disclosures are available on the transcript of this episode. Dr. Kunz, welcome back; it's great to have you on the podcast again.  Dr. Pamela Kunz: Thank you so much. My pleasure to be here. ASCO Daily News: The theme of the 2021 ASCO Annual Meeting was Equity. Every Patient. Everyday. Everywhere.  Equity issues also apply to the oncology workforce and there were some very interesting discussions at the meeting on workplace disparities and harassment. You chaired an education session on dismantling gender disparities in the global oncology workforce. This session brought together a really interesting and diverse panel of experts  in medicine.  They discussed compelling data around gender disparities and steps to diversify leadership in medicine. They also looked at the role of male allies and how allies and advocates can support all women, and shared strategies on how to activate and empower female leaders.  Can you tell us more about this session? ("Dismantling Gender Disparities in the Global Oncology Workforce Together").  Dr. Pamela Kunz: Sure. This was--thank you for asking about that session. I think that really the theme of equity permeated so many different aspects of this Annual Meeting. And I think it was really inspiring and I think incredibly helpful to think about really reimagining how we provide cancer care. And I think I really like to think of workforce disparities as the other side of the same coin of patient disparities or inequities in patient care. I think that in order for us to provide equitable patient care, we really have to provide and create a diverse, inclusive, and equitable workforce. And so this is really one aspect of that is around gender disparities or, of course, other disparities in the workforce. And we actually do touch on that in one of the talks. So we put together a diverse panel that represents a number of different viewpoints. They were not all oncologists. In fact, I was the only medical oncologist on. Dr. Reshma Jagsi is a radiation oncologist. And Dr. Hannah Valentine is a cardiologist who was previously at the National Institutes of Health (NIH) and the inaugural director of their diversity program. Dr. Leon McDougle also spoke. He's a family medicine physician, the current president of the National Medical Association. And Mrs. Dee Anna Smith is the CEO of Sarah Cannon Research Institute. So we had this really incredibly diverse group of perspectives. And as you mentioned, we really touched on a whole variety of topics. I think it's also worth just mentioning kind of the scenes for this. This session originated well over 2 years ago. And I think that the timing of this now happening in 2021 following the pandemic I think was really incredibly important. I think we didn't really recognize it at the time. We were supposed to do this session last year in 2020. And it was really the 2020 planning committee that approved the session with Dr. Howard Skip Burris and Dr. Tatiana Prowell and Dr. Melissa Johnson. We had all these conversations of how do we get men in the room to talk about gender disparities? And we really crafted this panel to try to address a diverse audience and get everyone in the room. And then it was really so well timed with Dr. Pierce's ASCO theme of equity for every patient, every day, everywhere. It really just tied in nicely. ASCO Daily News: Excellent. What are the key takeaways here for oncologists?  Dr. Pamela Kunz: Sure. I can add some of that. I think that the first--and this was really addressed by Dr. Reshma Jagsi--is that we need to collect the data. We need to measure evidence of disparities at our institutions, in our organizations in order to really know where we're starting and in order to know how we're getting better. We have a lot of objective data already. But I think that I want to challenge all of our listeners to think about how can we be better about collecting that data in our own institutions. I think that the takeaways from Dr. Valentine's talk were some really wonderful concrete solutions to diversify the workforce. She took some lessons learned from programs she initiated at the NIH. And I'd like to specifically highlight a program at the NIH called the Scientific Workforce Diversity Toolkit. And in that, they instituted a program for cohort hiring in the Distinguished Scholars program. And this was bringing together a diverse group of underrepresented minorities and women into this scholars program. And they demonstrated really increased rates of female tenure track investigators. And I think that we can all do that in our institutions and organizations by instituting cohort hiring. From Dr. Leon McDougle's talk, he really highlighted this concept of intersectional feminism. And this term was coined by Kimberle Crenshaw. She's a professor of law at Columbia University. And it speaks to the fact that many marginalized characteristics or people who are in underrepresented groups may have characteristics that intersect. So that includes gender, age, sexuality, education, race, culture, ethnicity. And if any one person has a number of these characteristics, they may, in fact, increase the burden on that individual and may increase their risk for discrimination and for disparities. And I think it's recognizing the intersection. Intersectionality happens. And our women of color and our women who may have these other marginalized characteristics may be especially at risk. He also talked about a program at the Ohio State where he is on faculty entitled Advocates and Allies. And it's a National Science Foundation-funded program that trains men how to be advocates and allies. And then lastly, Miss. Dee Anna Smith spoke about creating a tapestry of allyship. She had this beautiful visual metaphor of really bringing together not just mentors. It's sort of modernizing the idea of mentorship and to really thinking more about allyship and how our trainees need to bring together, yes, perhaps mentors, but that allies really can represent an alternative to mentorship and a tapestry meaning that you need more than one person to serve as an ally for you. So I think those were--it it truly was--I moderated. I think these folks did all of the work in presenting. But it was really inspiring and I think very solution focused. ASCO Daily News:  Well, you were also the discussant of session that addressed a new study, Abstract 11001 on sexual harassment of oncologists. Now, few studies have used comprehensive validated measures to investigate the incidence and impact of workplace sexual harassment experienced by physicians and none, according to the authors of this study, by oncologists. So this is really important. What can you tell us about it? Dr. Pamela Kunz: Yes, absolutely. And I think the points that you made already really make this important and validate it. And I think the findings then in and of themselves are quite striking. So this group of authors led by Dr. Ishwaria Subbiah conducted a study. It was a cross-sectional survey of ASCO's research survey pool. And they then used the sexual experiences questionnaire, which is a validated questionnaire, as you mentioned. And this is really I think a real strength of the study. And they examined various aspects of sexual harassment. I think it's important for our listeners to understand the definition of sexual harassment. So this includes gender harassment, unwanted sexual attention, and sexual coercion. And gender harassment includes things that if we use the iceberg analogy, which they included in their presentation and was so nicely described in the NASEM, the National Academies of Science Engineering and Medicine report from 2018, the iceberg really underneath the surface contains many of these aspects of gender harassment that go unnoticed and unrecognized and include things like microaggressions. And in this study, they evaluated four downstream domains impacted by workplace sexual harassment including mental health, job satisfaction, sense of safety at work, and turnover intentions, meaning if an individual planned on leaving that specific job. And they looked at incidents of sexual harassment both by perpetrator, so institutional insiders or patients and families, and then by type of sexual harassment. So they received about a 30% response rate. They had 304 practicing oncologists access the survey link. And 273 provided responses. And I'll just hit some of the take-homes. So I think what I was struck by is the high rate of sexual harassment when the perpetrator is an institutional insider. So those are peers or supervisors. 70% of physicians reported one or more incidences of sexual harassment. This was higher in women. So, 80% of women reported sexual harassment compared to 56% of men. So, that was statistically different. But I was really struck by the fact that men were experiencing this as well. And then in terms of sexual harassment incidents when the perpetrator was a family or patient, 53% of physicians reported one or more incidences of sexual harassment. And this was 67% for women and 35% for men, also statistically significant. In terms of that difference. And then really a significant downstream impact from these experiences both for physicians who experienced this harassment from institutional insiders or from patients and families. And I think that we saw that really across the board for mental health, workplace safety, job satisfaction, and turnover intentions. And I think the take home for our listeners is that this can really lead to a significant loss of talent. And I think that if we are really hoping to--see, this is me editorializing. We are hoping to improve the diversity of our workforce because we know that that leads to better patient care and better patient outcomes. This is really important for our workforce to try to tackle and solve this problem of sexual harassment. ASCO Daily News: Absolutely. Women comprise nearly half of all US medical students but they remain underrepresented in academic practice within surgical specialties.  To see if this occurs in otolaryngology departments, the authors analyzed faculty listings from academic departments and compared academic rank and h-index by gender.  The h-index was chosen to reflect scholarly productivity, since it reflects the number of manuscripts published by the author that have been cited at least that many times (e.g., an h-index of 10 means they published 10 articles cited by others 10 or more times each).  Men had higher h-indices than women at junior academic ranks, but the productivity rates of women increased and equaled, or surpassed, those of men later in their careers.  Women academic otolaryngologists of senior rank had higher h-indices than their male counterparts.  Despite this higher productivity women are underrepresented in positions of leadership and seniority, which is likely generational since the proportion of female otolaryngology residents has only recently exceeded 20%.
