In this forward-looking episode of the SCCM Podcast, Daniel F. McAuley, MD, explores how the clinical and research communities are rethinking acute respiratory distress syndrome (ARDS), shifting from a one-size-fits-all model to a focus on identifying and targeting modifiable traits. Building on his Thought Leader Session at the 2024 Critical Care Congress, Dr. McAuley unpacks the major thematic shift toward precision medicine in critical care. Instead of treating ARDS as a single, homogenous condition, researchers are increasingly identifying biologically distinct subgroups—especially hyper- and hypoinflammatory phenotypes—that may respond differently to therapies. These insights are fueling a new generation of trials that aim to prospectively apply this knowledge to treatment strategies. Central to this evolution is the Precision medicine Adaptive platform Network Trial in Hypoaemic acutE respiratory failure (PANTHER), of which Dr. McAuley is a team member. PANTHER is a Bayesian adaptive platform randomized clinical trial studying novel interventions to improve outcomes for patients with acute hypoxemic respiratory failure. Designed to be adaptive and biomarker informed, PANTHER will test therapies such as simvastatin and baricitinib, based on real-time phenotyping of patients with ARDS. Throughout the episode, Dr. McAuley reflects on how advances in machine learning and biomarker identification are making precision treatment more feasible. He discusses the importance of maintaining evidence-based supportive care, such as lung-protective ventilation and prone positioning, while integrating new targeted therapies. Discover the latest investigations into potential therapeutic agents—including mesenchymal stromal cells, statins, and extracorporeal carbon dioxide removal—as Dr. McAuley aims to translate early findings into tangible improvements in patient outcomes. This episode offers critical insights into the changing landscape of ARDS research and patient care, as Dr. McAuley articulates a hopeful vision for the future—one in which targeted, individualized treatments can improve outcomes for patients with one of critical care's most challenging conditions. Dr. McAuley is a consultant and professor in intensive care medicine in the regional intensive care unit at the Royal Victoria Hospital and Queen's University of Belfast. Robin McAuley has built a legendary career spanning decades, establishing himself as one of rock's most enduring and powerful vocalists. Best known for fronting McAuley Schenker Group in the late ‘80s and early ‘90s, his soaring voice became a signature of melodic hard rock. Horror is punk.Though there seems to be a particular empathy between punk music and horror, the concern could be, if you were to collect a bunch of horror stories themed to punk rock songs, wouldn't they all just be the same thing - more or less?Not when William Sterling and his band of scribes get together, because one of my favorite things about this anthology is how unique of an approach each author took to the assignment. A particular standout is Liz Kerin's story Down By the Water (inspired by the PJ Harvey song?) In which a girl rebels against her controlling parent - which seems pretty foundational punk - only the whole story seems set on some fairy tale seaside community with witches and sea monsters (maybe). Such a creative approach to the theme of the book! There are fourteen more songs beside, and I'll let you discover them on your own. Suffice to say, I really enjoyed the book, and loved to have William Sterling and Brian McAuley on to give some insight into the book's inception through it's upcoming release.I'll plug once more ordering from one of the above links directly from Truborn. Robin McAuley is widely recognized as one of the great singers of a golden period in hard rock music. His voice can be heard on albums from Grand Prix and Far Corporation, and of course, on the chart-topping MSG records 'Perfect Timing,' 'SaveYourself,' and 'MSG.' Through these releases, he was introduced to hard rock fans around the globe, who no doubt recognized the inherent talent and skill in his vocal abilities. Once he departed MSG, McAuley released one solo record in 1999, then joined AORlegends Survivor. In this episode, we have the pleasure of hosting Mark McAuley, a deep brain stimulation (DBS) patient and the CEO of Astronomy Australia Limited, and Dr. Susan Boehnke, an Associate Professor at Queen's University in Canada and director of the Neurotech Microcredential Program and the Neurotech Discovery Lab. Together, they've been part of remarkable efforts that not only focus on the practical and ethical aspects of neurotechnology but also engage students in real-time research. Mark brings over thirty years of experience in research and development, with a remarkable track record of securing $300 million in Australian Government grants for major research infrastructure projects. Holding degrees in astrophysics, ancient history, and an MBA—where he was awarded the Vice-Chancellor's medal as the university's highest-achieving postgraduate student—Mark was diagnosed with Parkinson's disease in 2010 and received a DBS implant in 2020. Post-surgery, he's been a passionate advocate for better patient care and improved DBS programming to enhance clinical outcomes. Dr. Susan Boehnke completed her PhD in Neuroscience at Dalhousie University and has an extensive background in auditory neuroscience and primate neurophysiology. She led the creation of one of the first non-human primate models of Alzheimer's disease and established Canada's first non-human primate biobank. In response to the explosion of interest in neurotechnology, she's now pioneering a micro-credential program in neurotech and exploring the ethical issues surrounding it. She's also leading the Training Committee for Connected Minds, a significant research initiative between York and Queen's Universities. At the recent Neuromodec conference in New York, Mark and Susan presented their work, which emphasizes patient inclusion in research—a theme Dr. Boehnke passionately advocates for—and provides transformative learning experiences for students. Today, we'll dive into their journeys, the science behind their projects, and explore potential insights into the future of patient-led research and education in neurotechnology.

This week's special podcast is a look down memory lane at arguably the best boxing gym and stable ever to come out of Ireland, the famous Barney Eastwood stable of the 1980-1990's. Barney Eastwood was born in 1932 in Cookstown, Tyrone. He was a talented Gaelic footballer, winning the All Ireland Minor title in 1948. His love of boxing would begin after watching US soldiers box at the US Army base in Tyrone back in WW2. Barney would start his own bookmakers business in the 1950's and after building an empire, he would turn his attention to his love of boxing. Barney would build a gym and stable of fighters/coaches over the next two decades that would compete with any gym in Europe. Barry McGuigan, Hugh Russell , Herol Graham and Dave McAuley would win World, European and British titles during the 1980's. Who can forget the special nights with Barry McGuigan and the amazing relationship they built together which unfortunately would turn sour years later. By 1992, Paul Hodkinson, Crisanto Espana and Victor Cordoba would also join Dave 'boy' McAuley as World champions. Countless other champions would make their mark in the gym including Ray Close, Sam Storey, Noel Magee, Crawford Ashley as well as future World champions Eamonn Loughran, John Lowey and Steve Collins. Barney Eastwood would end his love affair with boxing by the late 1990's and eventually sell his successful Eastwood's bookmakers chain in a huge multi-million pound deal. He would retire to his home in Holywood with his wife Frances until his sad passing in 2020. He legacy lives on and he will forever be regarded as the 'Godfather of Irish boxing'.
