Multiple Sclerosis (MS) is a complex neurological condition that impacts an estimated 2.8 million people worldwide. This week we explore the science behind MS, treatment options, and the lived experiences of those navigating life with this chronic illness. Multiple Sclerosis (MS) has been called "the disease of 1000 faces" for good reason - it manifests differently in almost every person it affects. Understanding MS: The Science Behind the Disease Dr. Zied Tayeb, neuroscientist and CEO of MyelinZ, explained the fundamental mechanism of MS in terms anyone can understand: "The brain communicates using electrical signals. These signals travel from one neuron to another through axons, which are protected by myelin sheets that help accelerate the transmission of these signals. In MS, the immune system mistakenly attacks these myelin sheets, leading to demyelination.” This disruption in communication between neurons explains why MS symptoms can vary so dramatically - the location of demyelination determines which functions are affected. "It could be the motor cortex, resulting in mobility issues, or the visual cortex, causing problems with vision," Dr. Tayeb noted. While the exact cause of MS remains unknown, researchers have identified several factors that may contribute to its development. According to our panel, MS is significantly more prevalent in women, who represent approximately 65% of MS patients - possibly due to women's more active immune systems making them more susceptible to autoimmune conditions like MS. Geographic distribution also offers clues, with higher rates in regions farther from the equator such as Canada, northern United States, and parts of Europe (with Denmark having a particularly high prevalence). This pattern has led scientists to investigate a potential link with vitamin D deficiency, though studies remain inconclusive. The Patient Experience: Hassan's Journey Few stories illustrate the unpredictable nature of MS better than that of Hassan Nada, entrepreneur, journalist, and self-described "MS warrior." Hassan's first symptom appeared in 2018 when he suddenly lost vision in his right eye while on holiday in Egypt. "I woke up one day, and out of nowhere, my vision was blurry to the point where I thought I was blind," Hassan recalled. Despite seeing an ophthalmologist, his condition went undiagnosed as MS. His vision eventually returned after treatment with corticosteroids. Four years later, after moving to Luxembourg, Hassan experienced sciatic nerve pain and numbness in his lower body. His general practitioner recognized the connection between his previous episode of optic neuritis and his current symptoms, immediately suspecting MS. "She screamed at me and had me go to the ER right away," Hassan shared. After extensive testing, including MRIs, blood tests, and a spinal fluid sample, the diagnosis was confirmed: Hassan had multiple sclerosis. The emotional impact of this diagnosis was profound. "My life changed from that moment onwards, and I went into complete depression," he admitted. But through the unwavering support of his wife and access to modern treatments, Hassan has found ways to adapt and advocate for others with MS. Treatment Approaches: Managing an Incurable Condition While MS currently has no cure, Dr. Tayeb emphasized that it is treatable. "Notwithstanding the fact that MS is incurable, it is treatable," he stated. "The problem is that every patient is different. The trajectory of the progression, diseases, symptoms are different.” Disease-modifying therapies (DMTs) aim to slow the progression of MS by suppressing or modulating the immune system. These treatments vary widely in their administration methods (pills, injections, infusions) and side effect profiles. Hassan's treatment journey illustrates the often challenging process of finding the right approach. After being diagnosed, he was unable to receive his doctor's first-choice treatment due to having a virus that made it dangerous for him. He then tried a pill-based DMT that, despite its high success rate with other patients, didn't work for him. Finally, he found success with a monthly subcutaneous injection called Kesimpta. "Last week I went to the neurologist office depressed," Hassan shared. "I was like, 'I know it didn't work.' She's like, 'No, it actually worked. There are no new lesions. You can now make long-term plans in life.'" Innovative Approaches: The Brain's Virtual Gym Dr. Tayeb and Samaher Garbaya, co-founders of MyelinZ, are taking a different approach to MS management. Their "virtual gym for the brain" uses neurotechnology to promote neuroplasticity - the brain's ability to form new neural connections. "We're trying to promote neuroplasticity in the brain," Dr. Tayeb explained. Their platform uses games to stimulate specific brain regions while measuring brain activity. Using machine learning, they analyze six different digital biomarkers to create personalized brain exercises. This approach draws inspiration from research showing that activities like playing musical instruments or singing can activate certain brain areas, potentially slowing disease progression by strengthening neural pathways. The Role of Support: Community and Family Multiple speakers emphasized that MS affects not just the individual but their entire support network. Anne Leurs, who manages the Multiple Sclerosis Society in Luxembourg, noted that MS typically appears between ages 20-40 - a critical period when people are building careers, starting families, and establishing their adult lives. "It's a very difficult period, this period of diagnosis," Anne observed. "It's a chronicle disease. This means that you will have to live your life on with this disease.” Hassan's experience underscores the crucial role of support systems. "MS either breaks a couple or it makes them the strongest couple in the world," he remarked. When first diagnosed, he told his wife she should leave. "She's like, 'It's because I love you that I'm not leaving, and we're going to make it out of this stronger than ever.'" Five years later, she remains his "backbone," managing his appointments and ensuring he takes his medications. For those without close family support, community organizations like the Multiple Sclerosis Society provide vital resources, information, and connection. Complementary Approaches: The Mind-Body Connection Mariette Lentz, a Luxembourgish soprano who specializes in the Alexander Technique, offered insights into complementary approaches that can help manage MS symptoms and improve quality of life. The Alexander Technique is "a mental technique" focused on "exploring the relationship between our thoughts and our movement," Mariette explained. While not a therapy or cure, it offers tools to help reduce stress and bring the nervous system to a calmer state - particularly valuable for MS patients who often experience anxiety related to their unpredictable symptoms. This, naturally, is a method that can help us all. The Daily Reality Hassan painted a vivid picture of what daily life with MS can entail: "Imagine waking up every day of your life with the worst hangover in the world without having to drink.” Despite this challenge, he maintains a positive outlook and has channeled his experience into advocacy work, connecting with MS communities around the world. "I'm disclosing some personal parts of my life in hopes that it's actually going to benefit people," he explained. Looking Forward: Hope in Research and Community While MS remains incurable, advances in treatment options continue to improve outcomes for patients. From traditional DMTs to innovative approaches like brain-computer interfaces and neuroplasticity training, the landscape of MS management is evolving rapidly. As our conversation demonstrated, the combination of cutting-edge medical science, dedicated healthcare professionals, strong personal support systems, and community resources creates a foundation for living well despite MS. What is electric vehicle (EV) retrofitting and why does it matter? ‘EV retrofitting' refers to the process of converting a gas-powered vehicle into an electric vehicle by replacing its gas engine with a battery pack. The battery pack is designed to fit within the vehicle's chassis—the structural frame to which its wheels, suspension, engine, and other components are attached. Typically, the battery pack fits in the space previously occupied by the gas engine and fuel tank; the available space generally varies by vehicle make and model. The battery's size and weight depend on the vehicle being retrofitted, as well as the desired range and performance. Despite these constraints, mass-producing battery packs for retrofitting a specific type of vehicle chassis with an electric powertrain is possible. Using a standard-sized battery allows for high-volume retrofits of vehicles with a compatible chassis, which can lower costs and reduce the time required for the retrofit process. Policies promoting the adoption of new EVs, such as California's Advanced Clean Cars II, can help increase the number of new EVs on the road. However, they do not address the existing vehicle fleet of gas-powered vehicles that continue to emit greenhouse gases and air pollutants. On average, these vehicles stay on the road for about 12 years in the US, and may be used even longer in low and middle income countries. Gas powered vehicles' slow replacement rate delays the increase in the percentage of EVs on the road. Retrofitting's supporters say a comprehensive approach to electrifying transportation that includes strategies for retrofitting existing gas-powered vehicles can help address the lag, particularly in low- and middle-income countries where the upfront cost of new EVs present a larger barrier to adoption.What does Shift EV do, and who is Aly El Tayeb? Shift EV is an Egypt-based company that specializes in retrofitting commercial fleets with electric powertrains. It focuses on vehicles that travel relatively short and predictable routes and return to a home base at night for charging. To make electric transportation more accessible and feasible in middle-income and emerging economies, such as Egypt, the company has adopted a strategy of retrofitting the most popular vehicle models already on the road. Dr. El Tayeb, the co-founder and CEO of Shift EV, is a chemical engineer with previous experience at a battery startup in the United States. He founded the company in response to the lack of accessible EV options in middle-income and emerging economies, such as Egypt. He is best known for his novel Season of Migration to the North, considered to be one of the most important novels in Arabic literature and one of the all-time great works of fiction. More and more athletes from all over the region are taking up the sport. Equally, Saudi Arabia is seeing a booming interest in sports, especially triathlons, and our guest today has and continues to play a key role in developing it in the region. This unintentional trailblazer is Dr. Dina Al Tayeb, who began competing in triathlons back in 2005 when no one was doing it and there really wasn't any tri community to fall back on. Dina became the first Arab and first Saudi national to complete an Ironman distance triathlon. She is also the first Arab woman and Saudi National to qualify and complete the Ironman World Championships in Kona. And she is the first Saudi National to qualify and compete in the 70.3 World Championships. Feats and titles she never set out to accomplish as she was simply looking to challenge herself in sports. In our chat with Dina, she details her start into the world of triathlons when it was largely unheard of, particularly for a Saudi woman. Along with being a triathlete, she is a certified Ironman Coach, an entrepreneur, dentist and mother. She also is one of the founding board members of the Saudi Triathlon Federation. Dina's impact cannot be overstated and in fact, the day after our chat with Dina, she featured in the New York Times' weekend magazine. Analysts say many investors remained cautious ahead of the outcome (結果) of this week's policymaking meeting of the U-S Federal Reserve and the central bank's comments on America's economy. **Vice President of Italian Senate Arrives in Taiwan ** The vice president of the upper house of the Italian parliament has arrived in Taiwan for a six-day visit. The visit to Taiwan this week by Gian Marco Centinaio is the highest-level Italian parliamentary trip to Taiwan since 1970. Centinaio is heading a three member delegation that includes one senator. According to Taiwan's Ministry of Foreign Affairs, Centinaio and his delegation will meet with President Tsai Ing-wen for formal (正式的) talks. The foreign minister says the delegation is seeking to gain a deeper understanding of Taiwan and explore opportunities for bilateral collaboration in the fields of politics, trade, technology, and culture. **Rain Helping Further Ease Drought in South ** The Water Resources Agency says the heavy rain that has lashed parts of the island over the past few days has contributed some 23-million cubic meters of water to major reservoirs. According to the agency, the Xinshan, Feitsui, Shihmen, Second Baoshan reservoirs in the north have received an estimated 7.1-million cubic meters of rainfall. In central Taiwan, the Techi Reservoir is now at 74-per cent of capacity, and the Liyutan Reservoir has reached 73.6-per cent of its capacity. While in the south, the Tsengwen and Wushantou Reservoirs have now accumulated more than 100-million cubic meters of rain water in total. The Water Resources Agency says levels are expected to continue to rise as the current plum rain front is forecast to last until June 18. **Japan Investigating Defense Force Shooting ** Japan's Self Defense Force is launching an investigation after a trainee shot and killed two others at a shooting range on Wednesday. The 18-year old was arrested and all shooting ranges (射擊場, 靶場) across the country shut down following the incident. Chris Gilbert reports from Tokyo. **Myanmar Army and Resistance Clash Leaving Dead ** Reports say recent fierce fighting between Myanmar's army and resistance fighters in an area east of the country's capital has killed at least two dozen civilians. The fighting has taken place in Shan State's Pekon township, a hotly contested (有爭議的) area of the armed struggle that arose after the army's February 2021 seizure of power from the elected government of Aung San Suu Kyi. A local resident said at least 26 people were killed, either shot dead in their homes or when they went out onto the street, or killed by artillery strikes. **Pope and Imam Call for Human Fraternity ** Pope Francis and a leading Sunni imam have made calls for peace as the U.N. Security Council met to discuss the importance of human fraternity. The pope sent a statement calling for an end to war. Sheikh Ahmed al-Tayeb, grand imam of Al-Azhar in Cairo, said in a virtual briefing that human fraternity was the key to global peace. After their appeals, council members adopted a resolution recognizing that hate speech, racism, xenophobia, intolerance and gender discrimination can contribute to conflict. he resolution urges all countries to condemn (譴責) these acts. It was adopted unanimously even though some of the council's 15 members have been accused of some of the same actions they are condemning. Episode 159: Mobility, Memory, and the performance of Bousaadiya in Libya In this podcast, Dr. Leila Tayeb, Assistant Professor in Residence in the Communication and Liberal Arts Programs at Northwestern University in Qatar (NU-Q), explores the cultural politics of mobility and memory in Libya. Looking at Bousaadiya, a figure who has been performed in many iterations throughout North Africa, she offers a reading of these performance practices as a space in which Libyans enact and contest practices of belonging. Tayeb describes how performance, and specifically dance, creates a frame through which to observe political, historical, and cultural phenomena. Highlighting repetition as an important element of performance, she argues that mimesis of certain practices over time can serve to reinstantiate – or disrupt – power structures. Bousaadiya performance practices, Tayeb argues, serve as a space in which Libyans grapple with the unresolved history of the trans-Saharan slave trade which took place in Libya for centuries and persisted even after it was formally abolished. Reading Bousaadiya through these lenses allows for an excavation of this history, its legacies, and opportunities for repair. Leila Tayeb is Assistant Professor in Residence in the Communication and Liberal Arts Programs at Northwestern University in Qatar (NU-Q). She earned her PhD in performance studies from Northwestern University and holds an MA in performance studies from New York University (NYU) and an MA in international affairs from The New School. Leila is an interdisciplinary scholar of performance and politics, focusing on topics including sound and militarism in daily life, dance studies, digital intimacies, race and indigeneity in North Africa, and state-sponsored performance. Her writing has appeared in the Arab Studies Journal, the Journal of North African Studies, Communication and the Public, and Lateral. Together with Adam Benkato and Amina Zarrugh, Leila is a founding member of the editorial collective of the multilingual, open-access publication Lamma: A Journal of Libyan Studies. The article that Leila discusses in this episode, “To Follow Bousaadiya: Mobility and Memory in Libyan Cultural Politics,” is forthcoming in the Middle East Journal of Culture and Communication in English and is in the process of being translated into Arabic for subsequent publication. Dr. Dina Al-Tayeb is the first Saudi and first Arab woman to complete the Ironman World Championship, an endurance event held annually in Hawaii that takes the average competitor 13 hours to complete. It starts with an open water swim of 2.4 miles (3.86 km), then a 112-mile (180 km) bike race, and ends with a marathon (26 miles or 42 km). Dina is also a mother and a periodontist with a love of drumming and music. Despite the physical toll racing has on the body, she loves the thrill of being an athlete and has finished over one hundred triathlons. She shares her secret to success, outlook on life, and how she balances it all.

What is electric vehicle (EV) retrofitting and why does it matter? ‘EV retrofitting' refers to the process of converting a gas-powered vehicle into an electric vehicle by replacing its gas engine with a battery pack. The battery pack is designed to fit within the vehicle's chassis—the structural frame to which its wheels, suspension, engine, and other components are attached. Typically, the battery pack fits in the space previously occupied by the gas engine and fuel tank; the available space generally varies by vehicle make and model. The battery's size and weight depend on the vehicle being retrofitted, as well as the desired range and performance. Despite these constraints, mass-producing battery packs for retrofitting a specific type of vehicle chassis with an electric powertrain is possible. Using a standard-sized battery allows for high-volume retrofits of vehicles with a compatible chassis, which can lower costs and reduce the time required for the retrofit process. Policies promoting the adoption of new EVs, such as California's Advanced Clean Cars II, can help increase the number of new EVs on the road. However, they do not address the existing vehicle fleet of gas-powered vehicles that continue to emit greenhouse gases and air pollutants. On average, these vehicles stay on the road for about 12 years in the US, and may be used even longer in low and middle income countries. Gas powered vehicles' slow replacement rate delays the increase in the percentage of EVs on the road. Retrofitting's supporters say a comprehensive approach to electrifying transportation that includes strategies for retrofitting existing gas-powered vehicles can help address the lag, particularly in low- and middle-income countries where the upfront cost of new EVs present a larger barrier to adoption. What does EV Shift do, and who is Al Elayeb? EV Shift is an Egypt-based company that specializes in retrofitting commercial fleets with electric powertrains. It focuses on vehicles that travel relatively short and predictable routes and return to a home base at night for charging. To make electric transportation more accessible and feasible in middle-income and emerging economies, such as Egypt, the company has adopted a strategy of retrofitting the most popular vehicle models already on the road. Al Elayeb, the co-founder and CEO of EV Shift, is a chemical engineer with previous experience at a battery startup in the United States.
