A Texas anesthesiologist, Dr. Raynaldo Ortiz Jr., 61, has been sentenced to 190 years in federal prison for injecting drugs into patients' IV bags, resulting in at least one death and causing multiple cardiac emergencies. Northern District of Texas Chief Judge David C. Godbey described Ortiz's actions as "tantamount to attempted murder.” During the sentencing hearing, emotional testimonies were presented. One victim's son told the court that his 10-year-old child no longer trusts doctors because "a doctor tried to kill Pops." Another family member recalled the horror of seeing Ortiz's "dead fish stare" on surveillance video. In April, Ortiz was convicted on multiple counts, including four counts of tampering with consumer products resulting in serious bodily injury, one count of tampering with a consumer product, and five counts of intentional adulteration of a drug. "Dr. Ortiz cloaked himself in the white coat of a healer, but instead of curing pain, he inflicted it," U.S. Attorney Leigha Simonton said in a statement following the conviction. "He assembled ticking time bombs, then sat in wait as those medical time bombs went off one by one, toxic cocktails flowing into the veins of patients who were often at their most vulnerable, lying unconscious on the operating table. We saw the patients testify. Their pain, their fear, and their trauma were palpable in that courtroom." The unexplained medical emergencies began in 2022 at Baylor Scott & White Surgicare North Dallas, just two days after Ortiz was notified of a disciplinary inquiry stemming from an incident where he allegedly deviated from the standard of care, according to the criminal complaint. Ortiz, who had a history of disciplinary actions, reportedly complained to colleagues that the center was trying to "crucify" him. Surveillance footage presented during the trial showed Ortiz repeatedly retrieving IV bags from a warming bin and then returning them shortly after. The videos also depicted him mixing vials of medication and observing as emergency responders assisted the victims. Doctors testified about their confusion when patients' blood pressure suddenly skyrocketed after new IV bags were administered. Upon reviewing medical records, they noticed a pattern: all the emergencies occurred immediately after hanging new IV bags. Patients recounted waking up unexpectedly intubated in intensive care units, experiencing pain and fear for their lives. The surgical center reported five emergency transfers in August 2022 alone, matching the facility's total for the entire previous year. Approximately a month after the unexplained emergencies began, a fellow anesthesiologist at the facility died while treating herself for dehydration using an IV bag. This incident heightened suspicions about the IV bags being tampered with. In August 2022, suspicions were further raised when an 18-year-old patient undergoing sinus surgery experienced severe symptoms, including high blood pressure, cardiac dysfunction, and pulmonary edema. She was rushed to the intensive care unit in critical condition. Tests on her IV bag fluid revealed a dangerous mix of bupivacaine (a nerve-blocking agent), epinephrine (a stimulant), and lidocaine (an anesthetic). Authorities also found a puncture in the bag. Investigations concluded that Ortiz had injected drugs into the IV bags, placed them back into the warming bin, and waited for them to be used in surgeries conducted by his colleagues. Surveillance footage corroborated these findings, showing Ortiz manipulating the IV bags and observing as patients suffered complications. Citing records from the Texas Medical Board, Ortiz had also been arrested on allegations of abusing women and shooting a pet dog. Additionally, local NBC affiliate KXAS reported that he owed millions of dollars to the IRS. His defense attorney, John Nicholson, argued that prosecutors had targeted the most convenient person without thoroughly investigating other medical staff who handled the IV bags. Despite the defense's claims, the jury found Ortiz guilty on all charges. TOrtiz's sentencing brings a measure of closure to the victims and their families, but the trauma inflicted remains. As U.S. Attorney Simonton emphasized, "He exploited his position to harm patients at their most vulnerable moments. Today's sentence ensures he will never harm another patient again." Her background began in acute short-term care but is currently part of the day neuro team. She explains the difference between a case manager and a social worker. Karol's greatest need is more resources - there often aren't enough for her to do her job. She urges patients to be persistent in recovery and their families to relentlessly help them find means of doing so. The following question refers to Section 9.5 of the 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure. The question is asked by Keck School of Medicine USC medical student & former CardioNerds Intern Hirsh Elhence, answered first by Vanderbilt Cardiology Fellow and CardioNerds Academy Faculty Dr. Breana Hansen, and then by expert faculty Dr. Javed Butler. Dr. Butler is an advanced heart failure and transplant cardiologist, President of the Baylor Scott and White Research Institute, Senior Vice President for the Baylor Scott and White Health, and Distinguished Professor of Medicine at the University of Mississippi The Decipher the Guidelines: 2022 AHA / ACC / HFSA Guideline for The Management of Heart Failure series was developed by the CardioNerds and created in collaboration with the American Heart Association and the Heart Failure Society of America. It was created by 30 trainees spanning college through advanced fellowship under the leadership of CardioNerds Cofounders Dr. Amit Goyal and Dr. Dan Ambinder, with mentorship from Dr. Anu Lala, Dr. Robert Mentz, and Dr. Nancy Sweitzer. We thank Dr. Judy Bezanson and Dr. Elliott Antman for tremendous guidance. Enjoy this Circulation 2022 Paths to Discovery article to learn about the CardioNerds story, mission, and values. Question #31 Mrs. Hart is a 70-year-old woman who was admitted to the CICU two days ago for signs and symptoms consistent with cardiogenic shock. Since her admission, she has been on maximal diuretics, requiring greater doses of intravenous dobutamine. Unfortunately, her liver and renal function continue to worsen, and urine output is decreasing. A right heart catheterization reveals elevated biventricular filling pressures with a cardiac index of 1.7 L/min/m2 by the Fick method.   What is the next best step? A Continue current measures and monitor for improvement B Switch from dobutamine to norepinephrine C Place an intra-aortic balloon pump (IABP) D Resume guideline directed medical therapy Answer #31 Explanation The Correct answer is C – Place an intra-aortic balloon pump. This patient is between the SCAI Shock Stages C and D with elevated venous pressures, decreased urine output, and worsening signs of hypoperfusion. She has been started on appropriate therapies, including diuresis and inotropic support. The relevant Class 2a recommendation is that in patients with cardiogenic shock, temporary MCS is reasonable when end-organ function cannot be maintained by pharmacologic means to support cardiac function (LOE B-NR). Thus, the next best step is a form of temporary MCS. IABP is appropriate to help increase coronary perfusion and offload the left ventricle. In fact, for patients who are not rapidly responding to initial shock measures, triage to centers that can provide temporary MCS may be considered to optimize management (Class 2b, LOE C-LD).    The guidelines further state that in patients presenting with cardiogenic shock, placement of a pulmonary arterial line may be considered to define hemodynamic subsets and appropriate management strategies (Class 2B, LOE B-NR). And so, if time allows escalation to MCS should be guided by invasively obtained hemodynamic data via PA catheterization. Several observational experiences have associated PA catheterization use with improved outcomes, particularly in conjunction with short-term MCS. Additionally, PA catheterization is useful when there is diagnostic uncertainty as to the cause of hypotension or end-organ dysfunction, particularly when the patient in shock is not responding to empiric initial measures, such as in this patient. There are additional appropriate measures at this time that are more institution-dependent. An institutional shock team would be very helpful here as they often comprise multidisciplinary teams of heart failure and critical care specialists,

Dr. Shibuya earned his Doctor of Podiatric Medicine degree from Temple University and Master of Science degree in clinical investigation at the University of Texas Health Science Center at San Antonio.  Dr. Shibuya is an Editor-In-Chief of the Journal of Foot and Ankle Surgery and Chairperson of the Council of National Registry Management for the American College of Foot and Ankle Surgeons (ACFAS). Prior to UTRGV, he served as Chief of the Podiatry Section under Surgical Services at Central Texas VA Health Care System, where he also served as a member of the Podiatric Surgical Advisory Board for the National Surgery Office, National Podiatry Field Advisory Committee for the Office of Patient Care Services under Veterans Health Administration, Chief Podiatry Consultant for VISN 17, and VA Site Director for the Baylor Scott and White Residency Program.  Academically, he was a professor of surgery at Texas A&M University, College of Medicine prior to UTRGV, where he trained hundreds of students, residents, fellows, and colleagues. He served as a Director of the Continue Medical Education Committee for the Texas Podiatric Medical Association and Chair of the Annual Scientific Conference Committee for the ACFAS. He is also a faculty of AO North America.  In research, he specializes in health services research, epidemiology, and large data analysis, leading to over 100 peer-reviewed publications, textbook chapters, and consensus statements. Clinically, he is a fellowship-trained, board-certified surgeon by the American Board of Foot and Ankle Surgery in Foot, Rearfoot Reconstructive, and Ankle surgeries. Dr. Jensen and Dr. Richey emphasize the importance of research in podiatric medicine and encourage students to pursue research opportunities. Dr. Shibuya shares his own journey into research, starting during his residency program when he had the opportunity to do a research project in San Antonio. This experience sparked his interest in research and led him to pursue a fellowship with UT San Antonio, where he focused on research and clinical work. According to Dr. Shibuya, research is crucial in advancing the field of podiatric medicine. He believes that the profession needs to move towards evidence-based medicine and that research plays a vital role in providing the evidence needed to support clinical practices. Dr. Shibuya also emphasizes the importance of questioning established practices and seeking the truth through research. For students interested in research, Dr. Shibuya advises finding a mentor with research experience. Mentors can provide guidance and support throughout the research process, including study design, data analysis, and publication. He also suggests utilizing resources like YouTube to learn more about research methods and statistics. In terms of research opportunities in podiatric medicine, Dr. Shibuya suggests that diabetic foot research is an important area to focus on due to its impact on a large number of people. Pearls and Quotes - Atrial Fibrillation: Situational Assessment of Stroke and Bleeding Risk In patients with persistent atrial fibrillation with tachycardia induced cardiomyopathy, timely restoration of normal rhythm is important. In patients not on established oral anticoagulation one option is to wait 3 weeks on oral anticoagulation prior to considering cardioversion. Another option is to pursue TEE prior to cardioversion as TEE is currently the gold standard imaging modality to exclude a LAA thrombus. Following cardioversion (chemical or electrical), anticoagulation must not be interrupted for 4 weeks due to atrial stunning. This is especially true for patients who have been in atrial fibrillation for an extended period of time. Individualizing assessment of stroke and bleeding risk is imperative when determining perioperative anticoagulation (AC) management. ACC has a helpful app (ManageAnticoag App) to make this easier. When considering AC in triggered atrial fibrillation (e.g., pneumonia, sepsis), it is important to consider the substrate that made the patient susceptible to developing atrial fibrillation. AC is favored in patients with high CHA2DS2-VAsC score and many traditional risk factors for atrial fibrillation as they are at high risk for future development of atrial fibrillation. Atrial fibrillation is a marker of poor outcomes in patients who have undergone coronary artery bypass graft (CABG) surgery. It is unclear if patients should be started on long-term AC for new onset atrial fibrillation after CABG regardless of risk factors. This is currently being investigated in the PACES trial. Notes - Atrial Fibrillation: Situational Assessment of Stroke and Bleeding Risk How do we choose an imaging modality for excluding LAA thrombus exclusion prior to cardioversion? TEE is the gold standard. It also provides other information that is important for management of atrial fibrillation (e.g. LA size/volume, presence/degree of mitral regurgitation/stenosis, ejection fraction). Gated cardiac CTA may have a growing role for evaluation of LAA thrombus. What is the data behind the recommendation for uninterrupted AC following cardioversion and what is atrial stunning?
