Virology vanguard Dr. Bob Gallo is far from ready to slow down. Now in his seventh decade as one of America's top scientists, he could easily sit back and enjoy the fruits of his numerous and pioneering achievements. His groundbreaking work began in the 1970s with research into human retroviruses, including the discovery of the T-cell growth factor (IL-2) and the identification of the Human T-Cell Lymphotropic Virus (HTLV-1) in 1980, earning him his first Lasker Award, often referred to as “America's Nobel Prize.” His subsequent research led to the identification of HIV-1 as the cause of AIDS, securing him a second Lasker Award. In the 1980s, he was the world's most cited scientist. Along with his team, he developed the first HIV blood test, crucial for understanding the spread of AIDS and managing HIV patients. In the mid 1990's Gallo and his collaborators discovered chemokines, naturally occurring compounds that were essential for understanding how HIV infects cells. Dr. Gallo later founded the Institute of Human Virology at the University of Maryland School of Medicine in 1996 and co-founded the Global Virus Network (GVN) in 2011 to enhance global virus detection and management. Recently, Dr. Gallo and his team moved to Tampa, the new global headquarters for GVN, where he now serves as director of the University of South Florida (USF) Virology Institute and Head of the Microbial Oncogenesis Program at the Cancer Institute at Tampa General Hospital (TGH). In the first part of this wide-ranging interview, Dr. Gallo shares insights into his entry into virology and his initial research into the etiology of certain cancers. This work included crucial discoveries around T cell growth factors, paving the way for identifying HTLV-1. He discusses his collaboration with CDC epidemiologists, which led to recognizing AIDS as being caused by a retrovirus. Once the HIV virus was identified as the cause, creating the first blood test for HIV had profound impacts on the epidemic and patient care. In the second segment, Dr. Gallo discusses the origins of the HIV virus and its early global spread. He also reflects on the COVID-19 pandemic, why the focus on its origins is irrelevant and reflects on how to rebuild public trust in science and medicine, which may have been damaged during the pandemic. In the concluding segment, Dr. Gallo talks about his reasons for joining USF Health and TGH and the research areas he finds most promising going forward. He speculates on the prospects for an HIV vaccine, the impact of artificial intelligence on virology, and why he doesn't necessarily worry about the threat of the next global pandemic. Finally, Dr. Gallo opens up about how the early loss of his young sister deeply affected his life and his desired legacy. Shortly after starting his job as Dean of the USF Health Morsani College of Medicine in 2014, Dr. Charles Lockwood was presented with the exciting opportunity to help design a new medical education building. In contemplating requirements for the new space, he raised the challenge to colleagues that when the facility was completed in 2020, medical knowledge would be doubling every seventy-three days. “That fundamentally altered the way we thought about the building, and we accelerated the process of curricular reform that we had started,” he explains to Raise the Line host Shiv Gaglani. That reform included dramatically reducing the amount and length of lectures, substantially increasing active and problem-based learning, introducing more assessments and requiring students to learn how to be good researchers to help them discern what's credible within the ever-growing amount of information available. But Dr. Lockwood also sensed a need to address the way students manage the rigors of a medical education and clinical practice. “What we've tried to do is to stress the need for grit and for embracing the classic stoic teaching about taking on obstacles and challenges as wonderful opportunities to grow and become more resilient.” Lockwood credits this perspective for the institution performing well through the worst of the pandemic. “I think the most surprising thing was the relative lack of burnout at the end of it. We're moving on to new challenges, but it didn't leave us hollowed out.” Tune in for a fascinating conversation about adopting an “anti-fragile” mindset, the dangers of overreliance on heuristics in making clinical decisions, and the implications of generative AI for medical education and patient care. Haru Okuda, MD, FACEP, FSSHExecutive Director, Center of Advanced Medical Learning and Simulation Associate Vice President Interprofessional Education and PracticeChief Executive Officer, Health Professions Conferencing Corporation University of South Florida Health Dr. Okuda is the Executive Director and CEO of USF Health's Center for Advanced Medical Learning and Simulation (CAMLS), Health Professions Conferencing Corporation (HPCC). In this role, he has oversight of a 90,000 ft2 state of the art, advanced training facility, with the mission of creating and providing experiential learning that improves clinical skills and patient care in the community and around the globe. In addition to this role, Dr. Okuda also serves as USF Health's Associate Vice President of the Office of Interprofessional Education and Practice (https://health.usf.edu/ipep) focused on creating interprofessional learning opportunities from early healthcare training to clinical practice. He is Professor at the Morsani College of Medicine and practices clinically in the emergency department at Tampa General Hospital. Prior to coming to USF Health, Dr. Okuda was the national medical director for the Simulation Learning Education and Research Network (SimLEARN) where he established national strategy and business plans for simulation-based programs at more than 150 U.S. Department of Veteran Affairs' medical facilities. In addition to his role as national medical director of SimLEARN, Dr. Okuda served as the acting deputy chief of patient care services officer for the Veteran's Health Administration, where he was responsible for policy development and oversight of the national Office for Women's Health Care, Community and Preventative Health, Social Work and Pharmacy Benefits Management. Before joining the VA, he was assistant vice president and director of the Institute for Medical Simulation and Advanced Learning for the New York City Health and Hospitals Corporation, the largest public health system in the United States. Dr. Okuda received his Bachelor of Science degree in neuroscience from Brown University, his medical degree from New York Medical College, and his certificate in Healthcare Modeling and Simulation from the Naval Postgraduate School in California. He completed a residency in emergency medicine at the Icahn School of Medicine at Mount Sinai where he served as their chief resident, and then completed a clinical quality fellowship from the Greater New York Hospital Association. He is a fellow of the American College of Emergency Physicians and an inaugural fellow of the Society for Simulation in Healthcare Academy. He has also served as a chair or member of several medical and simulation committees; and most recently serves as President for the Society for Simulation in Healthcare. He has co-authored numerous textbooks, peer-reviewed publications and textbook chapters. Known for his passion for teaching, innovation and business, Dr. Okuda received the 2017 Distinguished Educator Award by the Society for Academic Emergency Medicine Simulation Academy for the creation of the simulation-based training program SimWARSTM; was named one of the top 25 Healthcare Leaders Under 40 by Becker's Hospital Review in 2012; was selected as one of 40 Under 40 New York's Rising Stars in Business by Crain's NY Business Magazine in 2011; and was awarded the 2017 Healthcare and Medicine Leader of the Year by i4 Business Magazine.

Karina is currently a Masters student at USF studying Exercise Science. She is a research assistant in both the Performance and Physique Enhancement lab and the Molecular Pharmacology and physiology lab at USF Health with Dr. Bill Campbell. As we'll touch on in the episode, Karina has been through her own struggles as an athlete with binge eating, competing and learning how to manage her health. From these experiences, she's been able to better understand how to navigate nutrition along the very broad spectrum - between supremely structured and tracking macros & habits down to the minutiae to eating intuitively and approaching nutrition with more flexible mindset. What I've always admired about Karina is her focus on delayed gratification within her coaching methods. Making sure you don't neglect mental health in pursuit of physical health is paramount to ensure sustainable results. He also oversees the faculty group practice of the medical school – with more than 1,000 providers, it is one of the largest multispecialty group practices in Florida. On this week's Outbreak News This Week Radio Show, I had the opportunity to spend the whole hour discussing the three most commonly diagnosed free-living amoeba species that infect humans: Naegleria fowleri, Acanthamoeba species, and Balamuthia mandrillaris, with someone with expertise in infectious diseases and the unique perspective of a parent that lost a child to one of these lethal parasites. Yersiniosis is an infection caused most often by eating raw or undercooked pork contaminated with Yersinia enterocolitica bacteria. CDC estimates Y. enterocolitica causes almost 117,000 illnesses, 640 hospitalizations, and 35 deaths in the United States every year. Infection of the bone--short and sweet, that is the definition of osteomyelitis. Transmitted by the bite of the tsetse flu (Glossina spp.), African trypanosomiasis, of sleeping sickness is a serious infection caused by microscopic parasites of the species Trypanosoma brucei. Although the infection is not found in the United States, historically, it has been a serious public health problem in some regions of sub-Saharan Africa. In 2014, 3,796 sleeping sickness cases were reported to the World Health Organization; T. b. gambiense accounted for >98% of cases. Many cases, however, are probably not recognized nor reported. I recently had a long conversation with Associate Professor of Infectious Disease and International Medicine at USF Health, Sandra Gompf, MD about a myriad of infectious disease topics from her handy book, Gompf's ID pearls and one on the topics I asked her about was African sleeping sickness.
Infectious diseases are caused by pathogenic microorganisms, such as bacteria, viruses, parasites or fungi; the diseases can be spread, directly or indirectly, from one person to another. Zoonotic diseases are infectious diseases of animals that can cause disease when transmitted to humans. Infectious disease medicine requires an extensive understanding of the way in which bacterial, viral, fungal, and parasitic infections occur in humans and how they present clinically, as well as knowledge about antimicrobial agents, antibiotic resistance, vaccines, and other immunobiological agents.
