Comment on the Show by Sending Mark a Text Message.Beneath the pristine white coats and confident demeanors of hospital physicians lies a growing mental health crisis that threatens not only the wellbeing of doctors but potentially the care we all receive. Drawing from alarming 2024 survey data showing that 6 in 10 physicians experience burnout and over half know colleagues who have contemplated suicide, this episode exposes the dangerous reality of physician mental health in hospital settings. When physicians reach their breaking point and seek support, many hospital employers respond not with compassion but with discrimination. Through examination of real cases, including physicians being told to "take your baggage elsewhere" or facing termination after requesting accommodations, we uncover a disturbing pattern of hospitals prioritizing profit over the mental wellbeing of their most valuable assets. The episode delves into the powerful stigma that prevents physicians from seeking help - with nearly 8 in 10 acknowledging this barrier exists within medicine. We explore how the relentless demands of 24-hour shifts, sleep deprivation, and emotional exhaustion create perfect conditions for mental health deterioration, while fear of professional consequences keeps doctors suffering in silence.Beyond identifying the problem, we discuss practical solutions for hospitals and physicians alike, emphasizing the legal protections available under the Americans with Disabilities Act and similar state laws. The message becomes clear: physicians are employees with rights, deserving of reasonable accommodations and compassionate support. This conversation matters not just for healthcare professionals but for every patient who expects quality care. After all, wouldn't you want to know that the physician treating you is working in an environment that supports their mental health rather than pushing them to the brink? Listen now to understand this hidden crisis and what can be done to address it.Association for Academic Surgery: Removing the Mask with Dr. Carrie Cunningham speech2024 The Physicians Foundation SurveyNational Suicide Prevention Lifeline If you enjoyed this episode of the Employee Survival Guide please like us on Facebook, Twitter and LinkedIn. A staggering 15% report having contemplated ending their lives at some point in their careers, with 6% having such thoughts within just a single year. The juxtaposition is jarring – these talented professionals represent the pinnacle of medical achievement, yet many battle profound personal demons in silence. Through Dr. Carrie Cunningham's courageous presidential address to the Association for Academic Surgery, we witness this paradox firsthand. Despite her Harvard professorship, research grants, and leadership positions, she openly shared her struggles with depression, anxiety, and substance use disorder. Her vulnerability challenges our assumptions that external success guarantees internal well-being.The factors driving this crisis run deep. Medical training itself plants the seeds, with studies showing one-third of interns develop clinical depression. The surgical culture's emphasis on perfectionism, combined with sleep deprivation and high-stakes decision-making, creates tremendous pressure. Add to this the startling revelation that 45% of physicians experienced serious trauma before even entering medicine, and we begin to understand the perfect storm threatening our healers. Most concerning is the pervasive fear preventing surgeons from seeking help. Many go to extraordinary lengths – paying cash for therapy, traveling to distant cities for treatment, self-medicating – all to avoid potential career repercussions. Physician Health Programs exist in every state with impressive 90% success rates, yet many doctors don't know about these resources until they're in crisis.True progress requires fundamental shifts: moving beyond superficial "wellness" initiatives to address genuine mental health conditions, fostering cultures where vulnerability is seen as strength rather than weakness, and creating environments where seeking help doesn't jeopardize careers. The Dr. Lorna Breen Act represents a step forward, named for an emergency physician who died by suicide after working on the pandemic frontlines.Have you noticed signs of struggle in a colleague or friend? Reaching out could save a life. What small step might you take today to create a more supportive environment for those battling silently around you?National Suicide Prevention Lifeline If you enjoyed this episode of the Employee Survival Guide please like us on Facebook, Twitter and LinkedIn. I, your host, found myself pondering over this existential question as I sat down with Larry Schwarm, a man touched by the tragic loss of his wife, Dr. Peg Bicker, a revered physician who sadly took her own life in 2019. Driven by the weight of his loss, and motivated by the memory of Peg's enduring commitment to her patients, Larry and I embark on a poignant exploration of the taboo topic of physician suicide, the stigmatization of mental health in the medical field, and the relentless issue of physician burnout. Grappling with the complex, unforgiving landscape of the medical practice, we dissect the systemic issues that contribute to physician burnout - inadequate reimbursement for patient care, intricate coding and quota systems, and the onerous burden of electronic medical records. We delve into the poignant narrative of Peg's battle with depression, revealing the unseen side of a woman who, despite her struggles, was deeply committed to her patients. Larry's experience provides a window into the toll these systemic issues can take on physicians like Peg and opens up a conversation on the need for significant structural changes. As we navigate the intricacies of this topic, we underscore the urgency for improved professional training for physicians and the unforeseen benefits of a business background in running a medical practice. Our discussion serves as a clarion call for systemic change - a plea to the healthcare industry to safeguard its physicians from the hardships Peg had to endure. Through the heartache of Peg's passing, Larry and I hope to inspire necessary conversations and vital changes. Join us on this journey and let's work towards creating a support system that truly serves our physicians, both professionally and personally. You matter. Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR   Behind the white coats and stethoscopes, there lies a profound truth - doctors, too, are human. Joining me again are I.C.F.-Certified coaches on our team at TheHappyMD.com - Pam Pappas MD and Penelope Hsu MD, to continue the three-part series on recognizing and preventing physician suicide. In part one, Dr. Pappas and Dr. Hsu bravely shared their own experiences with suicidal ideation. In part two, we discuss what to do if you suspect a colleague is having suicidal thoughts and how to reach out to express our concern. We also explore the barriers to seeking help including imposter syndrome, shame, and guilt.  While we may not be able to prevent every instance of physician suicide because of their never-show-weakness programming, we have the power to make a difference through our compassion. Whether we're colleagues, friends, family, or just part of a tribe, we can reach out and give support to someone who might be in a tough spot. Our job is to look out for each other and make sure we have each other's backs even if they tell you to go away and leave them alone. You'll never know when a simple yet genuine gesture of empathy can save someone's life. “If you do reach out, you're not going to get rebuffed. If you can hold on somehow through all that darkness, the love that you would give is right there for you. They're going to be there. You just have to say something.” - Penelope Hsu MD “An antidote to shame is the self-compassion. If we're the safe place, then people feel safe to tell their story and somehow the shame starts to ebb away.” - Pam Pappas MD   Episode Highlights: Social isolation and the theory of suicide Fostering a culture of kindness, compassion, and community Trust your instincts and reach out to show you care Breaking down the wall of impostor syndrome and cracking the shell of shame   Resources: Preventing Physician Suicide Part 1: Recognizing You Are At Risk - The First-Person Experience of Suicidal Ideation https://physiciansonpurpose.libsyn.com/70-preventing-physician-suicide-part-1-recognizing-you-are-at-risk-the-first-person-experience-of-suicidal-ideation   Physician Support Line https://www.physiciansupportline.com/ 888-409-0141   Connect with Dr. Hsu: Book a Discovery Session with Dr. Hsu: https://www.thehappymd.com/physician-coach-penelope-hsu-md Website: https://drpenelopehsu.com/ Linkedin: https://www.linkedin.com/in/penelope-hsu-3796395/ Connect with Dr. Pappas: Book a Discovery Session with Dr. Pappas: https://www.thehappymd.com/physician-coach-pam-pappas Website: https://drpampappas.com/ Linkedin: https://www.linkedin.com/in/pampappasmd/   Watch this on Youtube: https://www.youtube.com/watch?v=zks9w0cU3hk   Learn more about Dr. Dike and The Happy MD: https://linktr.ee/dikedrummond Contact Dr. Drummond Email: support@thehappymd.com  Phone: 206-430-1905 Web contact form: https://www.thehappymd.com/contact

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR We kick off a seriously important three-part series that tackles something we all need to talk about more - preventing physician suicide. This week, I'm sitting down with two remarkable guests and I.C.F.-certified coaches on our team at TheHappyMD.com, Dr. Penelope Hsu and Dr. Pam Pappas. We discuss the often-ignored topic of recognizing your own suicidal ideations and the immense implications it can bring along. Both Dr. Hsu and Dr. Pappas hold nothing back as they share their personal experiences with suicidal ideation and burnout. Their stories remind us why it's crucial for wellness leaders to show vulnerability, and to use their platforms as stages to empower others in their struggles. "I think It is far more common than we think. Isolation is the thing that keeps us from acknowledging it within ourselves, of getting support, of sharing, of leaning on anyone. My hope would be that people can hear our stories and realize it doesn't mean you're weak. It doesn't mean you're broken. It just means you're human." - Penelope Hsu MD "Things are going on inside us all the time that may look very different on the surface and how other people see us. We know that physicians are extremely well-versed in trying to look more together than they actually feel. And the code of silence around depression and suicidality is extremely strong in our culture at large, but I think even more so in physicians because there are a lot of implications." - Pam Pappas MD     Episode highlights: The link between burnout and suicidal thoughts Dr. Pam's story: burnout, trauma, and the power of a listening ear Dr. Penny's story: isolation, creative calling, and the road back to medicine Suicide statistics in medical students and physicians Resources: Physician Support Line https://www.physiciansupportline.com/ 888-409-0141   Connect with Dr. Hsu: Book a Discovery Session with Dr. Hsu: https://www.thehappymd.com/physician-coach-penelope-hsu-md Website: https://drpenelopehsu.com/ Linkedin: https://www.linkedin.com/in/penelope-hsu-3796395/   Connect with Dr. Pappas: Book a Discovery Session with Dr. Pappas: https://www.thehappymd.com/physician-coach-pam-pappas Website: https://drpampappas.com/ Linkedin: https://www.linkedin.com/in/pampappasmd/ Watch on Youtube: https://youtu.be/zC_YanMxQCk Learn more about Dr. Dike and The Happy MD: https://linktr.ee/dikedrummond Contact Dr. Drummond Email: support@thehappymd.com Phone: 206-430-1905 Web contact form: https://www.thehappymd.com/contact

In this episode, Dr Michelle Chestovich shares her beautiful and heartbreaking story of how she lost her sister to death by suicide. She has become a voice and an advocate for not only awareness but "normalizing" this conversation. She is bringing it out of the darkness that is usually hides it and illuminating it with the amazing tools she learned by certifying as a physician coach. We are so glad she is with us on this episode and join her as allies in the quest to bring physician mental health to the forefront of every wellbeing initiative. Favorite moments from Dr Chestovich: The work that you are doing is certainly helping people because I think it begins, number one, raising awareness, and number two, normalizing that we too are human and we can have problems with our brain. Just like we can have kidney stones and back problems, our brains can get sick too, and in fact, we're at higher risk for such things. And again, this is like my town cry. Not only are we all at risk, but number two, children aren't gonna save us. I mean, I get it that it's part of our culture that we so identify with our profession, particularly physicians, but maybe we can realize like we are amazing humans outside of that. She loves encouraging organizations to find more effective solutions to burnout in medicine.  And while the culture of medicine must indeed change, she loves empowering physicians to take care of their needs and mental health through coaching. You can catch her on her weekly podcast , Re-Mind Yourself, where she shares tips for physician moms who are trying to find more balance in their lives; She loves helping other physician moms realize that life doesn't need to be so hard and that they can enjoy this life they have worked so hard to create.   This show is dedicated to Michelle's younger sister, the formidable Dr. Gretchen Butler, described here in an excerpt from her obituary:  “Her strong work ethic, openness to other perspectives and eye for detail made Gretchen an excellent radiologist and entertainer extraordinaire. The only thing better tan having Gretchen attend your party was having her plan it! She made everything she did more beautiful and more fun with her infectious laugh and kind heart. Gretchen was a doting daughter, loving wife, skilled physician, elegant hostess and generous friend. While she had many accomplishments and excelled at these roles, nothing made her more proud or brought her more joy than her three children : Hudson, Harriet and Oliver. The void left in the lives of those who loved her is beyond words.”    The suicide rate among male physicians is 1.41 times higher than the general male population. And among female physicians, the relative risk is even more pronounced — 2.27 times greater than the general female population.   5 HIGH ACHIEVER TRAITS THAT PROVOKE ANXIETY: HAVE A HARD TIME SAYING NO,  HARD TIME ASKING FOR HELP,  COMPARE OURSELVES TO SUCCESSFUL PEOPLE,  LOOK SO FAR AHEAD WE'RE NOT PRESENT IN THE MOMENT, RELY ON ACHIEVEMENT to FEEL WORTHY.  https://www.psychologytoday.com/us/blog/anxiety-in-high-achievers/202011/5-anxiety-provoking-habits-among-high-achievers Physicians are stuck in a Catch-22:  the high self-expectations, intense professional pressure, and prevailing culture of the medical profession contribute to the rise in mental health issues among physicians, but these factors also discourage struggling physicians from seeking help. The Medscape National Physician Burnout & Depression Report 2018(www.medscape.com) showed that  66% of male physicians and 58% of female physicians who reported burnout, depression, or both had never received professional help, were not currently seeking professional help, and did not plan to seek professional help.   Barriers to Seeking Help Through her research at the University of Michigan Depression Center in Ann Arbor, Gold seeks to understand why physician mental health issues remain largely untreated(www.ncbi.nlm.nih.gov). “There's definitely a stigma inside medicine around mental health,” she says. She has found that physicians as a group tend to hold themselves to high standards, so a mental health issue is often perceived as a weakness. “Physicians don't want to appear as if they can't do their job,” says Gold. “We tend to be great masqueraders.” “Gretchen was a perfect example of what we need more of in medicine: We want people who are both driven and empathic, perfectionists who also care about helping colleagues learn and grow. Now we need to create a system to support this kind of doctor.”    - Dr. Michelle Chestovich   So what are some of the solutions? Changes within healthcare culture Work hour restrictions- not just for residents but also attendings Standardized maternity leave policies   Making Coaching standard of care  If you're a physician or med student  in need of help, please call the  Physician Support Line: 1 (888) 409-0141, or visit: https://www.physiciansupportline.com/

Dr. Richard Shaw discusses his new book, Shame No More - A Framework For Healing With A Message of Grace.Dr. Richard Shaw is a Licensed Mental Health Counselor in Washington State and a Licensed Marriage and Family Therapist in Oregon. In addition, he is an ordained minister in the Foursquare Church and an Associate Professor of over 25 years in the Graduate School of Counseling at George Fox University. He has been married for over 30 years and has two adult children. As shocking as this may sound, it is estimated that physicians end their lives 1.8x more than the general population. It is close to our missions and dear to our hearts that we discuss the definition of Shame and how we might ask for help in order to care for others. The suicide rate among physicians is the highest of any profession. What factors contribute to this and how can we help the healers?   In 2018, Amandip Sidhu lost his brother, Dr. Jagdip Sidhu, a cardiologist, to suicide. Because of extreme overwork, feelings of shame and failure, and burnout, Dr. Jagdip chose to end his life as the only solution to his misery.   In the wake of this tragedy, Amandip realized the importance of addressing the mental health and well-being of healthcare professionals.  He then founded the charity Doctors in Distress to support physicians in their struggles, with the vision of preventing further cases of suicide.   Listen to know more about the story that inspired the founding of a charity for doctors.   He wanted to send a very clear message: “Look what's happened to me, look what the system has done to me.” So that really inspired me to really try and think how do I carry on that message.  I then sort of started to think I wanted to create something to make sure primarily that other people don't go through a similar phenomenon. - Amandip Sidhu   Highlights: (02:05) Dr. Jagdip Sidhu's medical career (04:47) Beginning of burnout (11:36) Tragic loss (13:51) Founding of a charitable mission (21:27) The culture of medicine (27:03) Two prime directives (33:33) Stigma and taboo in healthcare (37:10) Doctors in Distress' services (43:07) Dame Claire Gerada   Resources: Doctors in Distress: Website: https://www.doctors-in-distress.org.uk/ Facebook Page: https://www.facebook.com/DoctorsinDistress Linkedin Company Page: https://www.linkedin.com/company/doctors-in-distress/ Twitter: https://twitter.com/DoctorsDistress   Connect with Amandip Sidhu: https://www.linkedin.com/in/amandipsidhu/   Watch on Youtube: https://youtu.be/YzQTYCBsAe4   Learn more about Dr. Dike and The Happy MD: https://linktr.ee/dikedrummond   We would love to hear your feedback.  Send us your review on Apple Podcasts/Itunes, or in other directories through this link: https://www.podchaser.com/podcasts/physicians-on-purpose-1546320

According to the American Foundation for Suicide Prevention, the rate of suicide among male physicians is 1.41 times higher than among men in the general population. For women, the risk is 2.27 times greater. If we prioritize the mental health of medical professionals who are caring for some of our most vulnerable patients, and encourage help-seeking behaviors for mental health concerns and substance use disorders by reducing stigma, increasing resources, and having open conversations about mental health- maybe we can change the culture.
