In this episode Melissa Pollock, CHESS Director of ACO Compliance and Regulatory affairs has a conversation with Megan Reyna, System Vice President of Population Health Midwest for Advocate Health about how they approach value-based care through wrap around services for the provider allowing better care for the patient.Well Megan thank you so much for joining us on the move to value podcast and we're really excited to talk with you today about the great work that you're doing at the population health at Advocate Health so, can you tell us a little bit about your role at Advocate Health and what you guys do?Yes, thanks for having me Melissa. So I'm assistant vice president with population health in the Midwest region with Advocate Health. We have recently merged, I should say combined, back in December of 2022, and you know population health and moving to value is a strong tenant of Advocate Health and we've been doing this work for some time and excited to share our journey with you.Great! So I know that you kind of mentioned there's been a few changes in the past year or so with Advocate Health, so can you explain a little bit about what's happening at the population health level?Yes. So we are coming together across both the Midwest and the southeast region. I will talk - so Advocate, which is the legacy Advocate Aurora in the Midwest and then Atrium in the southeast - what I'll speak to today is specifically the Midwest, that's really where my history is from and the work that we've been doing there.So can you tell me a little bit about Advocate Aurora the areas that they came from? I know it's two separate states and I always I tend to get them confused so I'm just curious Sorry about that so that's a problem it's Illinois and Wisconsin. We have about 6500 physicians that are participating in value-based care. Wisconsin is mainly an employed Medical Group with some independent tins that that participate in value and then Illinois is a strong pluralistic model with both a Medical Group and independent, what we call aligned physicians, that participate in a large clinically integrated network. We have about 1.3 million lives participating in value-based care contracts across both Illinois and Wisconsin, and we've been doing this as I said for some time. The clinically integrated network in Illinois has roots back to 1995 and became a clinically integrated network, one of the first in the country in the early 2000s. So currently have over 40 different value based care contracts that we participate in. We do have, we've moved a lot towards risk, so we have $1.2 billion in capitation risk that we currently take and that's actually where we we started a lot of our journey taking that risk, so you know and then have moved on to other programs as well within shared savings etcetera. So I'm curious you talked about capitation. What does that journey look like for providers? For your independent providers versus your employed providers? Has that been difficult? Just curious about that that journey for Advocate?So we started in capitation in 1995, so we've been at this for for quite some time, probably a different story than others across the country who have started with the shared savings platform and then moved to capitation or are looking to move to capitation. We really started with full risk and then also as the as the country started to take on more value based care contracts um started to get into those as well. So we have always looked at a support model that has wrap around services to our physicians participating in value based care contracts and how we really help support them be successful, and it's a true partnership between operations at the local level and...
In this episode of the Move to Value Podcast we catch up with Colleen Hole, Vice President of Clinical Integration in Population Health at Atrium Health to learn about the new partnership with the retailer Best Buy, the impact of the merger of Atrium Health and Advocate Aurora Health on the Hospital at Home program and Colleen's experience as a presenter at the global Hospital at Home Congress held in Barcelona Spain.You know, the last time we talked, we talked about the Hospital at Home program, and you gave some great information. A back story. And so, how's it going? Is it still a benefit to the community? Is it still being used in the ways that it's supposed to be? I mean, how is it? How are things transpiring?I would say we're continuing to gain momentum in the program. So, as we talked about several weeks ago, it was born out of the COVID crisis if you will. For that, I am grateful for the pandemic because in most large somewhat risk averse organizations these things don't happen very easily. You tend to meet and meet and meet and then finally maybe put together a proforma and do a small pilot. We bypassed all of that. We did a big pilot, um, out of necessity. So, what we're doing now really is pivoting from COVID, which is now less than 10% of our patients, to other diagnoses which I think I mentioned last time: heart failure, COPD, various infections, but then going into oncology, neurology, surgical trauma. Other patient categories that even some established programs I think are not pursuing. Bottom line is we've not been diagnosis specific in this program. We've been more general clinical eligibility first, by clinical condition, and then what diagnosis fit in it. And then obviously once we've got a clinical clearance, you got to look at the social determinant of health and all social determinants of health and all those other factors that play into whether the patient would be successful. But no, it's going great. We have every intention to scale as far as we need to scale to continue to decompress our hospitals. And, your point, it is making a difference in the community. We get a lot of letters and feedback from patients that say please don't ever make me go back to the hospital, I was so much more comfortable here, I feel safe here, I got to be with my dog. You know, that sounds small but it's not small. So again, as we find our population aging with more and more chronic conditions in their senior years, hospitals can be pretty risky places for those patients who are often disoriented, tend to fall at a higher rate, they're at risk for infections, they don't typically eat as well, sleep as well, and they don't move. They tend to stay in their bed with the door shut. Who wants to go down the hall in a hospital gown? So, all of these reasons in most cases make the home a better place for healing.So, we got some big news that hit the media about the partnership with Best Buy and Advocate Health Atrium. How did this come about and how does it work? It's fascinating.Two very large health systems coming together to be one. And looking for the synergies that happen when you do that. Now there are naysayers out there that say stop the madness, health systems shouldn't be merging. They are in Milwaukee, Chicago Illinois market. We're down here in the South, in North Carolina, South Carolina, Georgia, and a tad bit of Alabama. So, we're not in, you know, competing overlapping markets. They're almost identical in size.So let me back up a tad. Advocate and Aurora merged four years ago to become Advocate Aurora. Now that has merged with Atrium Health. So, the national name is Advocate Health, but each market will retain their brand that is known in that community. So, we're still Atrium Health. What I've been involved in is just integration work around nursing. So, how do we align nursing around standards both...
Many physicians today choose to pivot their careers and apply their clinical knowledge to the innovation and digital transformation side of healthcare. Dr. Rasu Shrestha was an early pioneer of this trend and as he explains, went from “the dark side of radiology to the dork side of informatics” more than two decades ago.In addition to spanning both technical and clinical roles, Dr. Shrestha brings a worldly view of medicine to his work as EVP and Chief Innovation and Commercialization Officer at Advocate Health. He attended college in Malaysia and medical school in India before becoming a visiting fellow in biomedical engineering at Imperial College London and a research fellow in informatics at the University of Southern California, where he also earned his MBA.Among many other positions, Dr. Shrestha also served as Chief Innovation Officer at the University of Pittsburgh Medical Center, and Chief Strategy and Transformation Officer at Atrium Health. After the December 2022 merger of Atrium Health and Advocate Aurora Health, Dr. Shrestha landed in his current role at the newly formed Advocate Health, now one of the nation's largest nonprofit health systems.In an earlier Healthcare is Hard episode, Scott Powder provided his perspective on the megamerger from the Advocate Aurora side. With health systems facing some of their worst financial years ever and confronting an urgent need to transform in fundamental ways, Dr. Shrestha shares his ideas about how they can think differently. He says health systems should move beyond the idea of coopetition to the notion of strategic partnerships done right. He urges health systems to embrace the headwinds and tailwinds of the of the pandemic, while staying grounded in their mission and the reality that the old way of doing things will no longer work.Examining AI with a critical eye. There's unprecedented excitement around artificial intelligence and generative language models like Chat GPT, but issues like hallucination bias raise serious concerns. As Dr. Shrestha explains, large language models do not really answer questions; they create responses that look like answers. These responses always sound authoritative, which means they always look right, even when they're wrong. While these technologies will undoubtedly have a role in the future of healthcare, they must be carefully examined and regulated first. Inoculating against the shiny object syndrome. Dr. Shrestha discusses how important it is to separate the hype from the hope in the era of AI and startups promising other breakthrough innovations. To remain grounded, he stays focused on three areas that he explains: moving from transactional to experiential, from a patient-centered to a person-centered approach, and from episodic care to always on 24/7 care.Realizing the benefits of scale. Despite a lot of M&A and market consolidation, Dr. Shrestha talks about how there hasn't been much success yet realizing the benefits of scale. Advocate Health is the nation's fifth largest nonprofit health system, operating 67 hospitals and more than 1,000 sites of care to generate revenues topping $27 billion.This new entity was formed by combining two like-minded, not-for-profit health systems in December 2022: Midwest-based Advocate Aurora Health and Southeast-based Atrium Health.While this was among the biggest mergers ever in the nonprofit healthcare ecosystem, it wasn't the first for Scott Powder. In the early 1990s, Scott began working for Evangelical Health System, a pioneer of horizontal integration, which later became Advocate Health Care. Over the next 30+ years serving in various strategy and planning roles, including overseeing the 2018 merger of Advocate Health Care in Illinois and Aurora Health Care in Wisconsin, Scott had a front row seat to the growth and development of the healthcare ecosystem.Scott is now President of Advocate Health Enterprises, where he is responsible for advancing Advocate Health's whole person health strategy by investing in solutions that complement the health system's core clinical offerings and broaden its business portfolio.In this episode of Healthcare is Hard, Scott talks to Keith Figlioli about the strategy behind creating Advocate Health, and the market forces driving it. They discuss topics including:The shifting mindset on geography. One of the most unique things about the new Advocate Health is its geographic footprint. While Illinois and Wisconsin are neighboring states, the company now also serves communities much further away in North Carolina, South Carolina, Georgia and Alabama through the merger with Atrium. Scott talks about the traditional mentality that all healthcare is local and how he doesn't believe that is the case anymore. He discusses the role geography still plays in high acuity care, and how technology is enabling so many more elements of healthcare to be delivered practically anywhere. The debate over scale. There's concern in the market about some health systems becoming too big, and a debate about whether or not these organizations are truly optimizing the value of their scale – or if they ever will. But Scott points out how scale is relative, especially in a fast-evolving healthcare market. For example, he raises the point that even if the five largest nonprofit health systems in the country were combined, they would still only generate half the revenue of a company like CVS Health or United Health Group, and only a fraction of the revenue of players like Amazon. He also talks about ways health systems can create scale outside of traditional M&A, such as joining forces around issue-specific consortiums.Dual transformation. Scott compares the difficult decisions facing healthcare to other industries like automotive, where companies have made commitments to move away from the core of their business – the internal combustion engine – in favor of investing in electric vehicles of the future. He talks about the capital-intensive demands of operating a core clinical care delivery business, and how challenging it is to divert money from those operations to invest in other areas. But he says it's the only way incumbent health systems will survive.Care in the home. A lot of Scott's focus at Advocate Health Enterprises is around a thesis that a person's home will be a center for care delivery in the future. Our conversation today centers on the humility it takes to actually listen to a different perspective than your own. Maybe listening with an open mind is something we can practice. Rev. Terrance Thomas believes that God is on the side of the oppressed – those that experience injustice because of race, gender, or sexual orientation. Ultimately, though, he believes the pattern he’s seen in the scriptures: that God will step in to help/heal/bless AFTER we’ve gotten up ourselves. So, that’s what he encourages us to do first. Then there's the practice of Black joy and Hope as resistance. The whole idea of oppression is to keep people down. So when people continue to shine and live fully, it is an act of resistance to find happiness and comfort despite tribulations. Meet Rev. Terrance Thomas The Reverend Terrance L. Thomas is an Itinerant Elder with the African Methodist Church and Senior Pastor of Bethel AME Church in Champaign, IL. In addition to his pastor role, he has worked as a hospital chaplain with Advocate Aurora and Carle Health. He sits on several community boards including Champaign Country Christian Health Center, CU Trauma and Resiliency, and HV Neighborhood Transformation. He is also the Executive Director of the Bridgewater-Sullivan Community Life Center. A dedicated liberation theologian and unapologetic Black Nationalist, Rev. Thomas received his undergraduate degree in Inner City Studies with a minor in Sociology from Northeastern Illinois University and a Masters of Divinity from Garrett-Evangelical Theological Seminary. In his spare time, Rev. Thomas enjoys theater, jazz, the sultry soul of 70's music, video games, political discourse with a sip of whiskey, baseball, professional wrestling, and community service projects. He is a member of Phi Beta Sigma Fraternity Inc; a proud father to five beautiful children; and he loves to quote Zora Neale Hurston when she says, “If you ain't got it, you can't show it. If the merger goes through, the combined company would have over 64 thousand employees and 22 hospitals in the state. This is Beaumont's latest attempt to merge with another hospital company, with a stated goal of completing the merger by October first. That's less than one year after the company tried and failed to merge with Illinois based Advocate Aurora health.

And I oversee all things kind of employment brand recruitment, marketing, mostly for our staff level recruiting, but that also do support leadership, recruiting, and a little bit of clinician recruiting as needed. So we had an interesting session today, it was two hours we did two show and tell some two very large organizations that have done some really great stuff from a branding and delivery standpoint. And then we did a little bit of an exercise and each of those focused on internal attraction, and marketing and external attraction and marketing with some undertones of focus being around DE&I and this big wave of attrition and challenge of attraction that we see so swelling in front of us. So let me ask with with all of the stuff that we covered in a very rapid two hours. Jesse, was, was there a big takeaway, or an aha moment for you that you that you'd want to share with the industry people should be aware of? Jessica Summerfield, Advocate Aurora Health 3:18Well, one thing that I've been thinking about that answers your question, but also it doesn't, but I just wanted to put this plug in is, we talked a lot about burnout, I was part of the retention group and the attrition group. And because we work in health care, like it's been especially bad, I think, just we've just been so busy, even from a recruiting perspective, and you're trying to hire for all these different areas, and I myself have been feeling burned out and coming to this session, even just for two hours, like helps to refresh a little bit, you know, getting away from the doldrums of like creating flyers and Facebook posts and talking with other industry professionals and kind of feeling recognized in that it's helpful.
