Samer Al-Homsi, MD, MBA, is the System Chief of Blood and Marrow Transplant and Cellular Therapy at Northwell Health Cancer Institute and serves as Director of Faculty and Academic Affairs in Medical Oncology. He is a Professor of Medicine at the Zucker School of Medicine and of Cancer Research at Feinstein Institutes of Medical Research. Previously, he was the Executive Director of Blood and Marrow Transplantation at NYU Grossman School of Medicine. Dr. Al-Homsi graduated from Damascus Medical School and trained in Hematology and Medical Oncology in France. He completed his residency at Advocate Health Care and fellowship at the University of Massachusetts. He has led programs in Malignant Hematology and Blood and Marrow Transplantation at several institutions, including NYU Langone Health. His research focuses on preventing graft-versus-host disease (GvHD), developing innovative approaches such as the CyBor and CAST regimens, particularly in haploidentical transplants. Dr. Al-Homsi aims to address healthcare disparities due to donor shortages among minority groups. Dr. Al-Homsi is also the President of the American Arab Assembly of Cellular Therapy and Transplantation (AAACTT), promoting collaboration among its members. About Dr. Shernan Holtan Dr. Holtan is a clinical/translational investigator. Her work focuses on increasing resilience, both at the tissue level to prevent and treat GVHD, and at the whole person level, designing exercise programs to mitigate cancer therapy-associated aging. In GVHD, her early work focused on epidermal growth factor and pregnancy hormones to facilitate tissue repair in GVHD, which led to a successful phase II study for the treatment of high-risk acute GVHD and discovery of the GVHD biomarker amphiregulin. More recently, she has led and published two prospective clinical trials regarding the use of PTCy as GVHD prophylaxis, the results of which are changing practice around the globe. Dr. Holtan has also been a competitive powerlifter, setting a national record in 2019. She has leveraged her knowledge regarding strength training into translational studies and clinical trials that are helping to reverse some of the damage done by high-dose chemotherapy and radiation. Through her multidimensional accomplishments, Dr. Holtan has demonstrated a unique and powerful intersection of medical research and physical resilience, redefining standards of care in HCT, and innovatively combating the deleterious effects of cancer treatment.

Advocate Health is the nation's fifth largest nonprofit health system, operating 67 hospitals and more than 1,000 sites of care to generate revenues topping $27 billion.This new entity was formed by combining two like-minded, not-for-profit health systems in December 2022: Midwest-based Advocate Aurora Health and Southeast-based Atrium Health.While this was among the biggest mergers ever in the nonprofit healthcare ecosystem, it wasn't the first for Scott Powder. In the early 1990s, Scott began working for Evangelical Health System, a pioneer of horizontal integration, which later became Advocate Health Care. Over the next 30+ years serving in various strategy and planning roles, including overseeing the 2018 merger of Advocate Health Care in Illinois and Aurora Health Care in Wisconsin, Scott had a front row seat to the growth and development of the healthcare ecosystem.Scott is now President of Advocate Health Enterprises, where he is responsible for advancing Advocate Health's whole person health strategy by investing in solutions that complement the health system's core clinical offerings and broaden its business portfolio.In this episode of Healthcare is Hard, Scott talks to Keith Figlioli about the strategy behind creating Advocate Health, and the market forces driving it. They discuss topics including:The shifting mindset on geography. One of the most unique things about the new Advocate Health is its geographic footprint. While Illinois and Wisconsin are neighboring states, the company now also serves communities much further away in North Carolina, South Carolina, Georgia and Alabama through the merger with Atrium. Scott talks about the traditional mentality that all healthcare is local and how he doesn't believe that is the case anymore. He discusses the role geography still plays in high acuity care, and how technology is enabling so many more elements of healthcare to be delivered practically anywhere. The debate over scale. There's concern in the market about some health systems becoming too big, and a debate about whether or not these organizations are truly optimizing the value of their scale – or if they ever will.   But Scott points out how scale is relative, especially in a fast-evolving healthcare market. For example, he raises the point that even if the five largest nonprofit health systems in the country were combined, they would still only generate half the revenue of a company like CVS Health or United Health Group, and only a fraction of the revenue of players like Amazon. He also talks about ways health systems can create scale outside of traditional M&A, such as joining forces around issue-specific consortiums.Dual transformation. Scott compares the difficult decisions facing healthcare to other industries like automotive, where companies have made commitments to move away from the core of their business – the internal combustion engine – in favor of investing in electric vehicles of the future. He talks about the capital-intensive demands of operating a core clinical care delivery business, and how challenging it is to divert money from those operations to invest in other areas. But he says it's the only way incumbent health systems will survive.Care in the home. A lot of Scott's focus at Advocate Health Enterprises is around a thesis that a person's home will be a center for care delivery in the future. He partners with world-class external research experts to develop and conduct some of the most comprehensive and important studies on food insecurity—studies that are guiding changes in how our country addresses domestic hunger from the ground up. Prior to Feeding America, Tom served as the North America President of Convergence CT, where he worked collaboratively with health care providers, life science companies and payers in using technology to leverage real-world data to improve outcomes across the healthcare ecosystem. Before that, Tom served as the Vice President of Research & Innovation at Advocate Health Care, where he led a department that managed over 600 research studies at any given time and produced over 200 publications annually. Tom has spent over 20 years utilizing research and program evaluation as vehicles to create collaboration and partnership among organizations, to make community programs more efficient and effective, and to demonstrate impact in the lives of people. Scientific evidence suggests that the positive effect spirituality has on physical and mental health is as essential to disease prevention and recovery as regular fitness and healthy eating (Walker, 2002). Personal prayer and meditation are tools many use pertaining to spirituality. Another study of 2,679 baby boomers found the rates of psychopathology, including depression and other mental illnesses, were half as much in people of faith that frequented church services than those who had infrequent church attendance.  High blood pressure was lowered more in men with a strong religious commitment in another study comprised of 400 participants (Advocate Health Care, 2005). Anti-Depressants “These results suggest that some people are more responsive to the intention to treat their depression, and may do better if psychotherapies or cognitive therapies that enhance the clinician-patient relationship are incorporated into their care as well as antidepressant medications,” Zubieta says. “We need to find out how to enhance the natural resiliency that some people appear to have.” https://www.nih.gov/news-events/nih-research-matters/placebo-effect-depression-treatment   Knee Surgery Patients with osteoarthritis of the knee who underwent placebo arthroscopic surgery were just as likely to report pain relief as those who received the real procedure, according to Baylor College of Medicine - study published in the July 11 New England Journal of Medicine. https://www.sciencedaily.com/releases/2002/07/020712075415.htm Placebo Fitness Research Mind over milkshakes: mindsets, not just nutrients, determine ghrelin responseTwo groups were drinking milkshakes, both which were 380 calories but the shakes had different labels on them. One read 620 calories and one read 140 calories. After drinking the shakes, the researchers monitored their ghrelin levels (hunger hormone, which often triggers more hunger) and satiety signals. The participants who drank the 620 calorie labeled shake had lower ghrelin levels and reported feeling full and satisfied. The group with the 140 calorie labeled shake had the opposite! Even though both shakes were in fact the exact same, nutritionally speaking. Anabolic steroids: the physiological effects of placebos Researchers took 15 lifters and had them train for seven weeks, telling them that the people who get the best results would be given free steroids. In 7 weeks, the lifters put a combined total of ~22 pounds on their bench, military press, seated press, and squat, on average. They then took 6 of the participants to join the “steroid” trial. They told them they were taking 10mg/day of Dianabol, but they were really just taking placebo pills. They trained for another 4 weeks and put a combined total of ~100 pounds on those same 4 lifts, meaning their rate of progress increased almost 8x just because they thought they were on steroids. The Weight of a Guilty Conscience: Subjective Body Weight as an Embodiment of GuiltYou may actually feel heavier if you allow yourself to feel guilty or shameful from eating bad food. Likewise, when you dread working out or view it as a punishment - it will be one!   Placebo Applications Avoiding information overload Avoiding program or diet hopping “Speaking things into existence” is a personal preframe that causes subconscious positive habits to be more likely to occur in your day to day life. Belief in said diet may result in said hypothesized result occurringSeed cycling, fat burners, paleo, alkaline water, etc…. IMproving your mental relationship with food and exercise, in order to get more out of it and stay motivated regularly Self affirmations repeated overtime can rewrite the truths within your mind. Your actions are dictated partially by your subconscious mind AFTER you've decided with your conscious mind Read, listen, and study things, people, or topics that you wish to act or be like. In this health care podcast, seven thought leaders talk about the areas of promise they see in health care in 2020. Seven thought leaders include: Kimberly Noel, MD, from Stony Brook Medicine Eric Weaver, from Innovista Health Solutions Suzanne Delbanco, from Catalyst for Payment Reform Sue Schade, from StarBridge Advisors Naomi Fried, from Health Innovation Strategies Joe Grundy, from Grundy Consulting Adrian Rubstein, from Merck Just a couple of comments up front here. I don’t want to further my reputation for dropping major spoilers, however, so I’ll keep this short. Many of the thought leaders today talk about AI in various contexts. Are you rolling your eyes right now? If so, let me remind everyone about the Gartner Hype Cycle. The first step is wild-eyed enthusiasm. The next step in the hype cycle is anger, the old trough of disillusionment. I’d suggest that as far as AI is concerned, we are coming out of that trough and AI—be it artificial intelligence or augmented intelligence or machine learning or deep learning or whatever you choose to call it—it is being used, for reals, for various applications. Other corroborations among our thought leaders include the importance of exalting primary care, in the form of what some may call direct primary care and Zeev Neuwirth calls complex-condition care or condition-specific care—a relationship model, if you will. Another idea that comes up in various ways is the idea of breaking down silos and getting everyone with a stake in patient health to the table and focused on achieving better patient outcomes using all the technology and wherewithal available to us in 2020. By all the stakeholders, I mean going beyond the usual suspects of providers and insurance carriers—meaning employers. Also meaning Pharma, in the sense of Pharma taking the opportunity to collaborate more deeply toward outcomes their medications can potentially confer … IRL with RWE. Today’s episode features the following guests: Kimberly Noel, MD, MPH, is a board-certified, preventive medicine physician. She serves as the telehealth director and deputy chief medical information officer of Stony Brook Medicine, where she provides leadership to all telehealth activities of the health system. Dr. Noel is also the chief quality officer of the patient-centered medical home (PCMH) for the family medicine department, working on quality improvement and population health management for National Committee for Quality Assurance (NCQA) designation. She practices occupational medicine clinically and provides digital solutions for employee wellness programs. She is an appointee the New York State Department of Health Regulatory Modernization Initiative Telehealth Advisory Committee and has won many service and innovation awards for health care. In academia, her research areas are in machine learning, risk models, and remote patient monitoring. Dr. Noel has developed several educational curriculums, including a 40-hour telehealth curriculum for the School of Medicine, as well as interprofessional educational curriculums with the School of Health Technology and Management, Nursing, Dentistry, and Social Work. Dr. Noel is a graduate of Duke, George Washington, and Johns Hopkins Universities. She is a proud graduate of the Stony Brook Preventive Medicine program, whereby she is now working collaboratively with the residency program leadership on development of a telehealth preventive medicine service. Eric Weaver, DHA, MHA, is nationally recognized for his work in primary care transformation and value-based care. As a corporate vice president for Innovista Health Solutions, he oversees enterprise strategy and technology adoption for a fast-growing population health management services organization. Dr. Weaver has been recognized for his contribution to the health care industry by receiving the ACHE Robert S. Hudgens Award for Young Healthcare Executive of the Year and the Modern Healthcare “Up & Comers” Award in 2016. Prior to joining the Innovista leadership team in 2015, he was the president and CEO of Austin, Texas–based Integrated ACO—one of the more successful physician-led accountable care organizations in the country. Suzanne Delbanco, PhD, is the executive director of Catalyst for Payment Reform (CPR), an independent, nonprofit corporation working to catalyze employers, public purchasers, and others to implement strategies that produce higher-value health care and improve the functioning of the health care marketplace. In addition to her duties at CPR, Suzanne serves on the advisory board of The Source on Healthcare Price & Competition at the University of California–Hastings and the Blue Cross Blue Shield Institute. Previously, she was the founding CEO of The Leapfrog Group. Suzanne holds a PhD in public policy from the Goldman School of Public Policy and an MPH from the School of Public Health at the University of California–Berkeley. Sue Schade, MBA, is a nationally recognized health IT leader and Principal at StarBridge Advisors providing consulting, coaching, and interim management services. She recently served as the interim chief information officer (CIO) at Stony Brook Medicine in New York. She was a founding advisor at Next Wave Health Advisors and in 2016 served as the interim CIO at University Hospitals in Cleveland, Ohio. Sue previously served as CIO for the University of Michigan Hospitals and Health Centers and, prior to that, as CIO for Brigham and Women’s Hospital in Boston. On June 5, 2017, the Supreme Court decided Advocate Health Care Network v. Stapleton, which is consolidated with Saint Peter’s Healthcare System v. Kaplan, and Dignity Health v. Rollins. The Employee Retirement Income Security Act of 1974 (ERISA) requires that employee retirement plans contain certain safeguards, but exempts “church plan[s]” from these requirements. Under 29 U.S.C. 1002(33)(A), the term “church plan” means “a plan established and maintained… by a church or by a convention or association of churches which is exempt from tax….” After a controversy involving an Internal Revenue Service determination that the church plan exemption did not encompass pension plans established and maintained by two orders of Catholic sisters for the employees of their hospitals, Congress amended the statute to add subsection (C), which provides: “A plan established and maintained for its employees (or their beneficiaries) by a church or by a convention or association of churches includes a plan maintained by an organization, whether a civil law corporation or otherwise, the principal purpose or function of which is the administration or funding of a plan or program for the provision of retirement benefits or welfare benefits, or both, for the employees of a church or a convention or association of churches, if such organization is controlled by or associated with a church or a convention or association of churches.” -- Plaintiffs in this case are a group of employees who work for church-affiliated non-profits. Plaintiffs sued the non-profits, alleging that their retirement plans are subject to ERISA and that by failing to adhere to ERISA’s requirements the non-profits have breached their respective fiduciary duties. Defendants moved for summary judgment, but the district court denied the motions because it determined that a plan established and maintained by a church-affiliated organization was not a church plan within the meaning of the statutory language. The U.S. Court of Appeals for the Seventh Circuit affirmed. -- By a vote of 8-0, the Court reversed the judgment of the Seventh Circuit. In an opinion by Justice Kagan, the Court held that under ERISA, a defined-benefit pension plan maintained by a principal-purpose organization -- one controlled by or associated with a church for the administration or funding of a plan for the church's employees -- qualifies as a "church plan," regardless of who established it. All members joined her opinion except for Justice Gorsuch, who took no part in the consideration or decision of the case. Defendants moved for summary judgment, but the district court denied the motion because it determined that a plan established and maintained by a church-affiliated organization was not a church plan within the meaning of the statutory language. The U.S. Court of Appeals for the Seventh Circuit affirmed. -- The question now before the Supreme Court is whether the Employee Retirement Income Security Act of 1974's church-plan exemption applies so long as a pension plan is maintained by an otherwise-qualifying church-affiliated organization, or whether the exemption applies only if, in addition, a church initially established the plan. -- To discuss the case, we have Eric Baxter, who is Senior Counsel of the Becket Fund for Religious Liberty.
