Medicine is often framed as a meritocracy, where intelligence, hard work, and dedication dictate success. Yet, institutions of medicine are shaped by histories of exclusion, bias, and systemic inequities. And for clinicians coming from marginalized backgrounds, the journey is not just about learning the science. It's also about learning an entirely different set of rules — rules that are unspoken and unwritten, but deeply felt. For Damon Tweedy, MD, this struggle was deeply personal. Raised in a working class, all-black neighborhood, medicine once felt worlds away. Earning a spot at Duke Medical School was a milestone, but it came with new challenges. The paradox of being both visible and invisible; of constantly proving — sometimes subtly, sometimes forcefully — that he belonged. Dr. Tweedy talks about the paradox of striving to be “twice as good,” while still being mistaken for the janitor, turning down an invitation to play golf with faculty because he simply did not know the game, and realizing that for some of his classmates, medicine was not a leap into the unknown, but simply an inheritance. Beyond race, this episode is also about identity, resilience, and what happens when personal history collides with professional expectation. It's about how trust in medicine is built or broken not just for doctors, but for patients. John Gravino is a Catholic author who explores the intersection of health and religion and other big questions of life. He is the author of The Immoral Landscape of the New Atheism, which was the topic of a health and spirituality seminar at Duke Medical School. In his book, Gravino argues that the real problem with the world is not religion but human nature, and the problem with our human nature is located in our minds. The key to making the world a better place, therefore, depends on the healing of the human mind. But how can this be accomplished? The world of science has been amazingly successful in so many areas, but the one glaring exception to the nearly flawless track record of the sciences is in the study of mental disorder. While science has been able to find cures for so many physical diseases, a cure for mental illness has proven elusive. John Gravino offers an explanation. Science is unable to cure the mind because the mind is a fundamentally different organism. The human mind is spiritual, not physical; and thus, it obeys the spiritual laws of the universe. If you want to make the world a better place, Gravino argues that getting rid of religion—the true source of the spiritual laws of the universe—is the worst thing you can do. Gravino presents his case, based on evidence from recent advances in neuroscience, that the only thing that can save the world is true religion because only true religion has the power to heal the human mind. She mentors medical students, residents, and fellows and is a faculty mentor for the University of Colorado Cardiology Fellows “House of Cards” mentoring group. She has research funding from the NIH/NHLBI K23, NIH Ethics Grant, and Ludeman Center for Women's Health Research. Dr. Khazanie is an author on the 2022 ACC/AHA/HFSA HF Guidelines, the 2021 HFSA Universal Definition of Heart Failure, and multiple scientific statements. Episode 30 - Dr. Ritchie Shoemaker's Response to Our CIRS ConcernsDr. Ritchie Shoemaker is a 1973 of Duke University and a 1977 graduate of Duke Medical School, completing a Family Practice Residency at Williamsport Hospital in 1980. Dr. Komrad is a psychiatrist on the teaching faculty of Johns Hopkins and Sheppard Pratt in Baltimore and Clinical Assistant Professor of Psychiatry at the University of Maryland and Tulane. He earned his undergraduate degree in molecular biophysics at Yale University, his M.D. degree at Duke Medical School, and trained in internal medicine and psychiatry at Johns Hopkins. He was an attending psychiatrist on the Treatment Resistant Psychotic Disorders Unit at Sheppard Pratt Hospital for 15 years, where he continues to train residents in psychotherapy and psychopharmacology. In addition to clinical psychiatry, Dr. Komrad is a medical ethicist. He chaired the Ethics Committee and ethics consultation service for the Sheppard Pratt Health System in Maryland for over 25 years. He served on the Ethics Committee of the American Psychiatric Association for 6 years, which oversees ethics and professionalism for psychiatry in the U.S. Recently he has been speaking throughout the U.S. and internationally, also consulting to government policy makers, expressing ethical concerns related to physician assisted suicide and euthanasia, especially concern that these procedures are available to people with mental illness in some countries. Duke Medical School’s Curriculum and Admissions [Show Summary] Dr. Linton Yee, Associate Dean for Admissions at Duke University School of Medicine, shares with us the unique curriculum of the program and the thought process behind it. He also fills us in on what applicants should consider as they fill out their Duke Medical secondary applications, which will make it more likely to be invited for an interview. Interview with Dr. Linton Yee [Show Notes] Our guest today, Dr. Linton Yee, earned his bachelors and MD at the University of Hawaii. He then did his residency in pediatrics at Harbor UCLA Medical Center and a Fellowship in Pediatric Emergency medicine at Children’s Hospital in Los Angeles. From 1996 to 2007 he practiced and taught pediatric emergency medicine in Hawaii and California before taking a position at Duke University as an associate professor in the Department of Pediatrics, Division of Emergency Medicine and as a pediatric emergency room physician. He is also Duke Medical’s new Associate Dean for Admissions, having been appointed to the post in May. Dr. Yee, can you give an overview of the Duke Medical’s highly distinctive curriculum? [2:15] The curriculum is a little different than the vast majority of US-based medical schools in that you do the basic sciences in the first year, a clinical year in the second year, a research or advanced degree in your third year, and the last year is the same as most schools, with rotations and other preparations for graduation. The curriculum has been different than most schools for the last 40 years, with the goal to produce leaders in medicine. We believe research works hand in hand with the advancement of clinical medicine. How does Duke Medical condense what many schools take 18 months or more to teach into one year? [3:42] You have to be efficient in how you are presenting material and make it relevant to how students are learning. We put our students in the clinical realm really early, seeing patients even in the first few weeks of school. A lot of it is integrating material, taking fairly complex ideas and clinical scenarios that go back to basic science in order to see relevance to the basic realm. One example I always talk to students about is shock. The definition of shock is inadequate profusion at the cellular level. If you look at that definition, how are you going to treat it? You have to reverse the profusion, so you need flow, so your carrier would be fluid, you need delivery of oxygen, and an energy source. To maximize oxygen, you need a pump to circulate what is carrying oxygen and fluids, and you need to maintain pump stability. You learn a lot of this stuff in basic biology in junior high school. You have a complex clinical scenario that you actually knew how to treat way back when, you just didn’t know how to integrate it. Can you give me a few examples of how students spend their 3rd year at Duke Medical? [11:52] The goal of the third year is to choose their own direction. Most of their academic life to this point has been pre-determined, so allowing them to choose is key in determining their thought process and ability to think critically and objectively about things. Some examples are we have a scholarship to Singapore to do infectious disease research. A lot of students do work in Tanzania as well. People have gone to Geneva to the World Health Organization, or gone to the London School of Economics for a masters there, and people go all over the place for research opportunities - it is pretty much an open book. We have people do MBAs, Divinity degrees, or an MPH. Not too many people go the JD route but every now and then we do have a student that does that. Let’s turn to medical school admission, your secondary application is one of the more thorough and demanding secondary applications. This year, old questions 5 & 6 were removed and Duke added several – from what I can see ( 1,2,7, 8 and 9).

Key Interview Takeaways Dr. Albert’s success stems from the fact that he identifies a need and creates a tool to meet that need. He is a pioneer in the digital health space, leveraging available technology to build tools like the iPhone ECG system. Dr. Albert’s viral YouTube demo of the smartphone ECG was happenstance. When he uploaded the video, Albert clicked ‘share with LinkedIn connections’ and within a few days the video had 150,000 views – which resulted in calls from several media outlets, venture capitalists, the FDA and Apple. The key to Dr. Albert’s success with the mobile ECG was validation. It took money and time to get people to publish studies, to prove that the tool worked, and to gain acceptance and adoption from doctors ‘in the trenches.’ It is possible to get people to buy medical devices and use them on their own. Alivecor’s Kardia Mobile ECG had 45,000 unique users in March, and that number has grown every month for five years. Digital health has the potential to fill in the gaps in continuity of care. Tools like Kardia Mobile ECG track patients and tell the story of what is happening when the patient is outside the clinical setting. Digital health has been held back by the fact that its implementation requires proof – which takes time. Investors must be willing to stick with it through the process of clinical adoption. As AI becomes a powerful force, technology will help us make better decisions and personalize health, yet doctors will never be obsolete due to the ‘wet’ nature of biology and the emotional quotient required. In 1977 he earned a Ph.D. in Psychology from Saybrook Institute, the leading Humanistic Psychology school. And he received a Doctor of Science degree from Ryodoraku Institute. He then spent 3½ years at Western Reserve Medical School, where his research led to both Dorsal Column Stimulation and Transcutaneous Electrical Nerve Stimulation (TENS™), both now used worldwide to help control pain. In 1966 he became Chief of Neurosciences at Gundersen Clinic, one of the 10 largest clinics in the U.S. In 1971 he founded the first comprehensive pain and stress management clinic in the world. Norman Shealy entered Duke University at age 16 and Duke Medical School at 19. He interned in Internal Medicine at Duke and then had a year of General Surgery at Banes Hospital, followed by a five year residency in Neurosurgery at Harvard’s Massachusetts General Hospital.
