Thanks!For the past 10 months, the legal world has been transfixed by the Pauline Newman saga. At 96, Judge Pauline Newman is the nation's oldest active federal judge. Last March, her longtime colleague, Chief Judge Kimberly Moore, initiated an effort to remove Judge Newman from the Federal Circuit.The complaint against Judge Newman was initially based on her supposed “cognitive decline” and “paranoid and bizarre behavior,” but it later morphed to focus on her unwillingness to cooperate with Chief Judge Moore's investigation. Judge Newman said she'd be happy to cooperate with an investigation—as long as it's conducted by a neutral party, namely, the judicial council of another circuit.As I have written repeatedly, I agree with Judge Newman on her due-process argument. It's routine for circuit judges to transfer an investigation of a fellow circuit judge—as opposed to, say, a district, magistrate, or bankruptcy judge—to another circuit. And there are some interpersonal issues between Chief Judge Moore and Judge Newman, which I might write about in the future, that make it completely inappropriate for Moore to be leading this investigation.I was agnostic, however, on Judge Newman's mental capacity. I read, along with everyone else, the gossipy details in Chief Judge Moore's various reports that made Newman sound, well, totally out of it. But I also read and heard accounts from other sources—such as journalists who visited Newman in chambers, and lawyers who saw her speak at conferences—stating that she's just fine.On January 4, I met with Judge Newman and her clerks in chambers, for about four hours. Last Friday, I interviewed Judge Newman on my podcast, for another hour. I'm now of the view that she's completely lucid and sane—and I have reason to disbelieve or at least question much of what I've read in the takedowns of her. Bob Mitchell, an integrated marketing and event strategist with a background in the television industry, believes virtual-event tech platforms are scrambling for business now because the entire business model of the past two years was based on false assumptions. Most planners did not want to be digital marketers, he says, which created a bifurcated market for in-person and digital. In this discussion with Northstar Meetings Group executive editor Michael Shapiro, Mitchell explains what he sees as the central disconnect between the tech platforms and meeting planners, the misguided ways in which the technology was targeted over the past two years, and how recent data should make it clear where virtual events can and should still play a significant role. In this wide-ranging discussion in which both Mitchell and Shapiro steadfastly avoid using the 'h' word, Mitchell addresses the need to blend in-person and digital content and reach a target audience despite the challenges. Pearls - Icosapent Ethyl Eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) are two major Omega-3 fatty acids found in fish oil. While both have been shown to lower triglycerides, only purified EPA formulations have been shown to reduce ASCVD risk.Mechanisms of triglyceride (TG) lowering by icosapent ethyl are multiple and include reduction of hepatic VLDL production, stimulation of lipoprotein lipase activity, increased chylomicron clearance, reduced lipogenesis, increased beta oxidation, and reduced delivery of fatty acids to the liver.There was only modest reduction of triglycerides in REDUCE-IT and JELIS despite association with significant reduction in cardiovascular outcome events, suggesting likely mechanisms outside of triglyceride lowering that may contribute to ASCVD reduction.While there was an increased signal for peripheral edema and atrial fibrillation associated with icosapent ethyl in prior trials, overall side effect rates were very low.Icosapent ethyl is considered to be cost-effective based on cost-effective analysis. Show notes - Icosapent Ethyl EPA and DHA have differing biological properties that may explain differences in ASCVD risk reduction observed in cardiovascular outcome trials 1.The REDUCE-IT trial, which enrolled secondary prevention and high-risk primary prevention patients with elevated triglycerides who were on statin therapy, showed significant reduction of major adverse cardiovascular events in the icosapent ethyl group compared with a mineral oil placebo2. Only modest reductions of TG were seen in the REDUCE-IT and JELIS trials despite association with significant reduction in events 2,3.  Potential mechanisms contributing favorable effects of EPA on ASCVD risk reduction include inhibition of cholesterol crystal formation, stabilization of membrane structures, reversal of endothelial dysfunction, inhibition of lipoprotein and membrane lipid oxidation 4.Pleotropic effects of EPA include influence on platelet aggregation, lower thromboxane activity, increased prostaglandin level, and effects on blood pressure, insulin resistance and inflammation.Triglycerides are a surrogate for triglycerides-rich lipoproteins, which are likely causally associated with ASCVD 5.There is increased signal for bleeding, lower extremity edema, and atrial fibrillation with icosapent ethyl but overall side effect rates are very low 2.In order to ensure higher rates of medication access and adherence, clinicians must be cognizant of the cost to the patient. In this episode, I speak to musical theatre composer and lyricist Michael Gordon Shapiro. Michael's insight into the genre is expansive. We discuss the method and elements of a musical, what functions songs play, and the traps to avoid. We also delve into compensation, publishing, and finding opportunities in the realm of musical theatre. I very much enjoyed recording this episode and hope you find it as informative as I did. Michael Shapiro is an active composer for musical theatre. His children's show Super Sidekick: The Musical is published by Samuel French/Concord Theatricals and has been performed in front of young audiences throughout the United States, including Off-Broadway in 2012. His family musicals Gideon and the Blundersnorp and The Bully Problem have won numerous awards and nominations, including (for Gideon) Best Musical at the 2021 Hollywood Fringe Festival. The Venice Ghetto was founded in 1516 by the Venetian government as a segregated area of the city in which Jews were compelled to live. The world's first ghetto and the origin of the English word, the term simultaneously works to mark specific places and their histories, and as a global symbol that evokes themes of identity, exile, marginalization, and segregation. Dr. Michael Shapiro currently serves as the Director of Behavioral Medicine at Duke Medical School's Southern Regional Area Health Education Center. As a member of the SR-AHEC faculty, he participates in the education and training of physicians going through their 3-year residency in Family Medicine. In addition, he provides direct clinical service to patients in the SR-AHEC family medicine clinic and at Cape Fear Valley Hospital. Dr. Mary Shapiro is the Director of the ADHD Clinic at the same facility, where she specializes in the diagnosis and treatment of ADHD, behavior disorders, and other childhood conditions.For the 30 years prior to moving to North Carolina, they were in private practice in Athens, Georgia. They both completed PhDs in Educational Psychology at the University of Georgia in 1984 and interned in the Department of Neurology at the Medical College of Georgia. Together, they have lectured extensively (both domestically and abroad) on topics related to child psychology, parenting, marriage, adoption, and mental health issues in a Christian context.
