A new research paper was published in Aging (listed by MEDLINE/PubMed as "Aging (Albany NY)" and "Aging-US" by Web of Science) Volume 15, Issue 6, entitled, “Knockout of AMD-associated gene POLDIP2 reduces mitochondrial superoxide in human retinal pigment epithelial cells.” Genetic and epidemiologic studies have significantly advanced our understanding of the genetic factors contributing to age-related macular degeneration (AMD). In particular, recent expression quantitative trait loci (eQTL) studies have highlighted POLDIP2 as a significant gene that confers risk of developing AMD. However, the role of POLDIP2 in retinal cells such as retinal pigment epithelium (RPE) and how it contributes to AMD pathology are unknown. In this new study, researchers Tu Nguyen, Daniel Urrutia-Cabrera, Luozixian Wang, Jarmon G. Lees, Jiang-Hui Wang, Sandy S.C. Hung, Alex W. Hewitt, Thomas L. Edwards, Sam McLenachan, Fred K. Chen, Shiang Y. Lim, Chi D. Luu, Robyn Guymer, and Raymond C.B. Wong from Royal Victorian Eye and Ear Hospital, University of Melbourne, St Vincent's Institute of Medical Research, University of Tasmania, and The University of Western Australia report the generation of a stable human RPE cell line ARPE-19 with POLDIP2 knockout using CRISPR/Cas, providing an in vitro model to investigate the functions of POLDIP2. “We conducted functional studies on the POLDIP2 knockout cell line and showed that it retained normal levels of cell proliferation, cell viability, phagocytosis and autophagy. Also, we performed RNA sequencing to profile the transcriptome of POLDIP2 knockout cells.” Their results highlighted significant changes in genes involved in immune response, complement activation, oxidative damage and vascular development. They showed that loss of POLDIP2 caused a reduction in mitochondrial superoxide levels, which is consistent with the upregulation of the mitochondrial superoxide dismutase SOD2. In conclusion, this study demonstrates a novel link between POLDIP2 and SOD2 in ARPE-19, which supports a potential role of POLDIP2 in regulating oxidative stress in AMD pathology. “In summary, we have generated a POLDIP2 knockout ARPE-19 cell line using CRISPR/Cas9 and studied the biological functions of POLDIP2. Grandon Professor of Health Policy, at the Jefferson College of Population Health.He also serves as a Special Assistant to the Chief Physician Executive of Jefferson Health. He is a prolific author, with a forthcoming book, “How Covid Crashed the System: A Guide to Fixing American Health Care.” Dr. Nash received a Bachelor's in Economics from Vassar College, a medical degree from the University of Rochester, and an MBA in Health Administration from the University of Pennsylvania. Key Insights:Dr. Nash analyzed the pandemic like an investigation into a flight crash. The black box revealed a variety of misaligned incentives in our current healthcare system. Heroism is Counter-productive. Dr. Nash pushes back on the habit of calling doctors “heroes.” It reinforces the mythology that doctors can fix and control everything in healthcare. While individual doctors did their best, in reality it was the American health system that failed during the pandemic. (27:41)Pay-vidors. Dr. Nash points to the benefits of joint venture or ownership models, where payers and providers are economically aligned. This type of model incentivizes prevention, wellness, and upstream care. Kaiser Permanente and Humana are models of this. (34:58) How to Fix Healthcare. Dr. Nash has core recommendations to fix American Healthcare: improve medical education, realign healthcare payment incentives, refocus leadership on the patient, improve connectivity with consumer, and make digital health more inclusive. Black History Month series Raymond returns to the podcast in this live conversation during the Northwest Scottish Fiddlers annual event. We celebrated Dr. Martin Luther King Jr. with our special guest Raymond. Raymond C. Mobley is an artist based in Olympia, WA. He composes, records and produces R&B, hip-hop and jazz music. He is a veteran who has proudly served his country until retirement. As a black American in a bi-racial marriage, Ray talks about how he comes to terms with the challenges in society's racial injustices and how he rises above it thanks to his upbringing and color-free view on life. Ray released his debut album "Breathe" on July 17th with Green Monkey Records. In Raymond's words: "This album is a reflection of my life. I wrote, performed, recorded and edited the whole thing. I used tools from all areas of my travels and experiences to do so. It took me a while to realize that I could do all of these things if I simply applied myself. This album has been one of many ups and downs. This album took a while to complete as I struggled to find who I really was as an artist, even though I started at a young age. My siblings often ask me for advice and the one thing I always tell them is to be true to themselves without comprises. Today, Breathe is a perfect representation of who I am. Robin chats with Dr. David Nash, the Founding Dean and the Raymond C. and Doris N. Grandon Professor of Health Policy at the Jefferson College of Population Health in Philadelphia, about the intersection of population health and precision or personalized medicine. Nash, who helped create the first college of population health, says that population health must involve better outcomes, especially since the country spends 20 percent of its GDP on health care. Eventually, he explains, we need to get to a “no outcome, no income” world, which although it sounds “harsh,” will ultimately lead to better outcomes. And we need to get rid of waste in health care by “shutting off the faucet instead of mopping the floor.” The bottom line, according to Nash, is that population health management, combined with precision medicine, can make a difference in our health care system. Zip code, personal behavior, and genetic makeup are the significant determinants in personal health, which is why population health and precision medicine are complementary, and not competitive concepts. Gene therapy and cellular therapy both are innovative strategies for optimizing the intersection of population health and precision medicine. An example of optimizing population health, Nash notes, occurs when a patient is at risk for a particular disease and his or her physician can order the right diagnostic and screening tests for that person’s specific genetic makeup. In general, he says, we need to go from volume to value and improve individual health, achieve optimal outcomes from unsatisfactory results, and get rid of unnecessary waste to improve per capita spending, all goals of population health.

Rick is out of town for the weekend so last night I decided to revisit this set only to realize I made a glaring omission; I left out "Make You Feel My Love." The song was originally written by Bob Dylan but never released until much later. When the original surfaced, Billy Joel, Trisha Yearwoood and Garth Brooks all did remakes of the song. To this day, I hold the Garth Brooks version very close to my heart. Listening to the version by Adele, brought back all those old feelings of betrayal, resentment and revenge; feelings that I have managed to suppress in sour relationships since then because it makes me appear petty, if not foolish. It came out in 1998, a time when my relationship with Pete was on the rocks as I talked about in the DIVA Tribute to Erasure. I would go to this dive billiard hall and have a couple of Bohemia's (Mexican beer) and play that song on the jukebox over and over while drowning in my own tears before heading to the gay bar for the rest of the evening. Knowing that Pete would be there, it aloud me to clear my head in advance, lest I do something stupid. I've included two versions of the song; one a dance version and the other a more chilled version that keeps up with the tempo of the original. The dance version is really good but I just wish they had slowed it down some. If you don't like the additions, I suggest you keep the original set as the newer version is replacing the older one which will no longer be available. English singer Adele (Adele Laurie Blue Adkins) won two Grammy's for her debut album 19 in 2009 and it's been nothing but uphill since then. After the success of that album, if you still didn't know who she was, you couldn't avoid her after her sophomore album release 21. Her amazing performance of "Rolling In the Deep" on the Letterman Show further solidified her name and went on to win an additional six Grammy's including Album of the Year. By the time 25 landed, all bets were off when she won a contentious battle of the Grammy for Album of the Year beating out another one of our favorite divas Beyoncé. While heartfelt, her acceptance speech may have been slightly over the top but all is forgiven because she was (as in past interviews) genuinely sincere if not surprised by her win. While lacking the vocal gymnastics of her predecessors, Adele has put herself in a position to wrangle the mantle away from Celine Dion who stole it from Barbra Streisand. She has the qualities of Celine's overly genuine and heartfelt gratitude and sincerity with Barbra's street smarts, honesty, and use of frosty language to express her views. Combined with her outstanding beauty and talent, she IS the ultimate DIVA. This appointment caps a 20-year tenure on the faculty of Thomas Jefferson University. He is also the Dr. Raymond C. and Doris N. Grandon Professor of Health Policy. In 1995 he received the top recognition award from the Academy of Managed Care Pharmacy. He received the Philadelphia Business Journal Healthcare Heroes Award in October 1997, and was named an honorary distinguished fellow of the American College of Physician Executives in 1998. In 2006 he received the Elliot Stone Award for leadership in public accountability for health data from NAHDO. In 2009 Dr. Nash received the Wharton Healthcare Alumni Achievement Award. In 2012, he received both the Joseph Wharton Social Impact Award and the Philadelphia Business Journal Medical Education Innovator of the Year award. Repeatedly named to Modern Healthcare’s list of Most Powerful Persons in Healthcare, his national activities cover a wide scope. He serves as an Industry Advisor for the VHA Center for Applied Healthcare Studies and he is a member of the Board of Directors of the Population Health Alliance (formerly CCA). Dr. Nash is a principal faculty member for quality of care programming for the American College of Physician Executives in Tampa, Florida, and leads the academic joint venture between ACPE and the JSPH. Dr. Nash is a consultant to organizations in both the public and private sectors. He has chaired the Technical Advisory Group (TAG) of the Pennsylvania Health Care Cost Containment Council (HC4) for more than a decade and he is widely recognized as a pioneer in public reporting of outcomes. In December 2009 he was named to the Board of Directors for Humana Inc., one of the nation’s largest publicly traded healthcare companies. He recently retired from the Board of Endo Health Solutions, a publicly traded pharmaceutical company headquartered in Dublin, Ireland. In October 2013 he joined the board of Vestagen, a privately held advanced medical textile company in Orlando, FL. He is on the Board of Main Line Health – a four hospital system in suburban Philadelphia, PA. From 1998–2008, he served on the Board of Trustees of Catholic Healthcare Partners in Cincinnati, Ohio where he chaired the Board Committee on Quality and Safety. Through publications, public appearances, his “Nash on the Road” blog, and an online column for MedPage Today, Dr. Nash reaches more than 100,000 persons every month. He has authored more than 100 articles in major journals. He has edited 22 books, including Connecting with the New Healthcare Consumer, The Quality Solution, Governance for Healthcare Providers, Health Care Quality: The Clinician’s Primer, Population Health: Creating a Culture of Wellness, and most recently, Demand Better. From 1984 to 1989 he was Deputy Editor of Annals of Internal Medicine. Currently, he is Editor-in-Chief of four major national journals including American Journal of Medical Quality, Population Health Management, P&T, and American Health and Drug Benefits. Dr. Nash received his BA in economics (Phi Beta Kappa) from Vassar College; his MD from the University of Rochester School of Medicine and Dentistry and his MBA in Health Administration (with honors) from the Wharton School at the University of Pennsylvania. While at Penn, he was a former Robert Wood Johnson Foundation Clinical Scholar and Medical Director of a nine-physician faculty group practice in general internal medicine.
