Zach and Kevin welcome Alan back to the show by commiserating over those moments where you look at your schedule and wonder why you (or your past self) agreed to a specific procedure. Kevin's #16 Crown Prep: Kevin recounts the dread of seeing a crown prep on a third molar (#16) at 8:00 AM. Despite the technical difficulty and the "Is this a joke?" feeling, it ended up being successful with a rubber dam. Zach's "Self-Extracting" Molars: Zach shares a cautionary tale from his early career working with his father. He attempted to crown a second molar that was tilted so far distally it was practically trying to extract itself. The result? A lost contact, a lost crown, and eventually, a lost patient. The Bioclear Pivot: Alan discusses a recent case where he planned a crown on #15 but found decay so deep it was near the crest. He pivoted to a side-by-side Bioclear composite, which was harder and more time-consuming but ultimately the right clinical choice. Categorizing the "Resurrected" Patient The main theme of the episode focuses on patients who reappear after years—sometimes a decade—away from the chair. Alan and the hosts break these down into tiers: 1. The S-Tier (The Best Case) These are patients who didn't leave because of a conflict; they simply "had a gap in life." Examples: Moving away and moving back, going through a divorce, or the "COVID-12" (people who stopped coming in 2020 and are just now emerging). The Dynamic: They still feel like "your" patients. They often have a high follow-through rate because they are relieved to be back in a familiar, trusted environment. 2. The A-Tier (The "Grass Wasn't Greener") Patients who left for a specific reason—usually insurance—and realized that "a dentist is a dentist" isn't actually true. The Insurance Trap: Many leave because the office went out-of-network, only to find that the "in-network" experience lacked the technology or relationship they valued. The Result: When they return, they often have a newfound appreciation for the higher level of service and clinical care provided. 3. The Lower Tiers (The Friction Cases) The Billing "B-Tier": Patients who disappeared because they owed money. They usually only reappear when they have an emergency and are suddenly willing to pay their old balance to get out of pain. The Personality "C-Tier": Patients who left after a confrontation or a bad review. Professional Growth & "Hard Nos" Alan reflects on how his clinical confidence has changed over the years. When a patient returns after 10 years, they aren't seeing the same dentist; they are seeing a version of Alan that is more skilled, has better tools (like new Bioclear matrices), and more experience. When is a "Resurrection" a "Hard No"? Abuse of Staff: Rudeness, yelling, or cussing out the team. Chronic No-Shows: People who are "constitutionally incapable" of making it to an appointment. Colossians 2:9-10 The Lamb of God Is God Himself When John the Baptist saw Jesus coming toward where he was baptizing in the Jordan River, he said, “Look, the Lamb of God who takes away the sin of the world!” But in what way was Jesus the “Lamb of God”? First, Jesus was a sacrifice for sin that only God himself could offer. All the lambs and other animal sacrifices brought to the temple by Jewish believers would never be enough to reconcile sinners to a holy God. Rather, they served to prepare the people for the one Lamb sent by God who alone could make full payment for our sins. This is possible because Jesus Christ, the Lamb of God, was not only sent by God, he is God. The Bible tells us that “in Christ all the fullness of the Deity lives in bodily form.” So, when Mary gave birth to her firstborn son in Bethlehem, we can say that God in all his fullness was laid to rest in the manger. And when that child grew into a man, it was God in all his fullness who was hung on the cross to die. What does that mean for you and me? There can be no doubt that the payment made on our behalf was sufficient—more than sufficient—to cancel our debt with God. The blood that was shed for us was divine and holy blood. The very God that we have offended is the One who came to redeem us to himself, and, in Jesus, his work of redemption is complete. God and man have been fully reconciled through Jesus Christ, the Lamb of God, who was both fully God and fully man. Resurrected from the dead and ascended to heaven, he continues to intercede for us before the Father. Our salvation has been brought to fullness. Prayer: Lord Jesus Christ, you are the eternal Son of God and yet you came to die in my place. Because of your holy life and innocent death, my sins have been forgiven and my debt with God has been paid in full. May this truth bring me comfort and peace, as I trust in you, my perfect Savior. Colossians 2:9-10 The Lamb of God Is God Himself When John the Baptist saw Jesus coming toward where he was baptizing in the Jordan River, he said, “Look, the Lamb of God who takes away the sin of the world!” But in what way was Jesus the “Lamb of God”? First, Jesus was a sacrifice for sin that only God himself could offer. All the lambs and other animal sacrifices brought to the temple by Jewish believers would never be enough to reconcile sinners to a holy God. Rather, they served to prepare the people for the one Lamb sent by God who alone could make full payment for our sins. This is possible because Jesus Christ, the Lamb of God, was not only sent by God, he is God. The Bible tells us that “in Christ all the fullness of the Deity lives in bodily form.” So, when Mary gave birth to her firstborn son in Bethlehem, we can say that God in all his fullness was laid to rest in the manger. And when that child grew into a man, it was God in all his fullness who was hung on the cross to die. What does that mean for you and me? There can be no doubt that the payment made on our behalf was sufficient—more than sufficient—to cancel our debt with God. The blood that was shed for us was divine and holy blood. The very God that we have offended is the One who came to redeem us to himself, and, in Jesus, his work of redemption is complete. God and man have been fully reconciled through Jesus Christ, the Lamb of God, who was both fully God and fully man. Resurrected from the dead and ascended to heaven, he continues to intercede for us before the Father. Our salvation has been brought to fullness. Prayer: Lord Jesus Christ, you are the eternal Son of God and yet you came to die in my place. Because of your holy life and innocent death, my sins have been forgiven and my debt with God has been paid in full. May this truth bring me comfort and peace, as I trust in you, my perfect Savior. The intent seems to be a return to the Monroe Doctrine by increasing the United States military presence in the Western Hemisphere, taking on the drug cartels, enhancing border security, making trade deals that are better for the United States, and enhancing American energy production. That’s not the worst high-level take on the NSS, but a look at the actual document is illustrative. The NSS states as its purpose: To ensure that America remains the world's strongest, richest, most powerful, and most successful country for decades to come, our country needs a coherent, focused strategy for how we interact with the world. And to get that right, all Americans need to know what, exactly, it is we are trying to do and why. A “strategy” is a concrete, realistic plan that explains the essential connection between ends and means: it begins from an accurate assessment of what is desired and what tools are available, or can realistically be created, to achieve the desired outcomes. A strategy must evaluate, sort, and prioritize. Not every country, region, issue, or cause—however worthy—can be the focus of American strategy. The purpose of  foreign policy is the protection of core national interests; that is the sole focus of this strategy. One of the more interesting (but not surprising) pieces of this NSS is the overt and robust return to the Monroe Doctrine, an early 19th-century policy intended to restrict further European colonization of the Western Hemisphere and to ensure American dominance in that region. The modern take on this doctrine by the Trump administration uses American power by employing both internal and external security measures. The NSS states: American policy should focus on enlisting regional champions that can help create tolerable stability in the region, even beyond those partners' borders. These nations would help us stop illegal and destabilizing migration, neutralize cartels, nearshore manufacturing, and develop local private economies, among other things. We will reward and encourage the region's governments, political parties, and movements broadly aligned with our principles and strategy. But we must not overlook governments with different outlooks with whom we nonetheless share interests and who want to work with us. Source: redstate.com The Monroe Doctrine is a foundational principle of United States foreign policy, first articulated by President James Monroe in his annual message to Congress on December 2, 1823.   It declared that the Western Hemisphere was no longer open to European colonization or interference, while affirming that the U.S. would not meddle in existing European colonies or internal affairs.   Essentially, it warned European powers—particularly those in the Holy Alliance (Russia, Austria, and Prussia)—against attempting to extend their influence or establish new colonies in the Americas, positioning the U.S. as a protector of independent nations in the region The doctrine emerged amid concerns over European monarchies potentially aiding Spain in reconquering its former Latin American colonies, which had recently gained independence. For instance: In the mid-1800s, it intertwined with Manifest Destiny to support U.S. territorial expansion, such as during the Mexican-American War.  President Theodore Roosevelt’s 1904 “Corollary” expanded it to allow U.S. intervention in Latin American countries to prevent European involvement, leading to actions like the occupation of Cuba and the Dominican Republic. It influenced Cold War policies, framing U.S. opposition to communism in the hemisphere as a defense against external threats. Critics, especially in Latin America, have viewed it as a tool for U.S. imperialism, enabling dominance over sovereign nations. Republican leadership planned Thursday to kick-off the procedural process to confirm 88 of President Donald Trump's nominees in a bloc vote, but were initially thwarted by Democratic Colorado Sen. Michael Bennet, who challenged the package for violating Senate rules. When Republicans refiled the package later on Thursday, the conference included an additional nine nominees, bringing the total to nearly 100.  The Senate has confirmed 314 civilian nominees as of Thursday evening, according to a tally by the Senate Republican Communications Center. The 97-member bloc would bring the Senate to more than 410 civilian confirmations in the first year of Trump's second term. “That far outstrips total confirmations by this point in President Biden's term, and in President Trump's first term as well,” Thune said Thursday. Thune also said that Senate Republicans have virtually cleared the nominations backlog. Before Republicans changed Senate precedent to allow for certain nominees to be confirmed in groups, more than 150 of the president's picks were awaiting floor consideration. The Senate approved a 48-member nominations package in September and an additional 108 of the president's picks in a single group vote in October.
