He is very active in clinical research relating to sleep disordered breathing with over 80+ peer-reviewed research publications in the fields of neuroscience, head and neck surgery, and sleep-disordered breathing.Dr. Zaghi is particularly interested in studying the impact of tethered-oral tissues (such as tongue-tie) and oral myofascial dysfunction on maxillofacial development, upper airway resistance syndrome, and obstructive sleep apnea. For Kevin Jurge, what began as a sore throat in 2013, led to the removal of his tonsils, and that led to a diagnosis of Stage IV head and neck cancer.  Kevin's oncologist said Kevin didn't have to undergo chemotherapy but did require radiation treatment to attack cancerous lymph nodes.  He said the regimen made his throat feel like sunburn, pain which lingered months after the conclusion of his treatment.  He also lost his ability to taste.  However, Kevin survived, can once again taste food and beverage and seeks to help others with his book, “Go To Your Happy Place.”

This technique involves removing the tonsil but leaving the capsule intact to protect muscles of the pharyngeal wall and lateral vessels. When he performs this procedure on patients, he notices that their recovery period is less painful and their lower risk of hemorrhages when compared to extracapsular tonsillectomy patients. According to the literature, there is only 3% risk of tonsil regrowth, but he considers regrowth a safer complication than tonsillar hemorrhage. His postoperative recovery regimen includes Tylenol or Motrin for 24 hours, and his patients can start on a regular diet immediately after surgery. Then, he and Dr. Shah discuss differences between American and European guidelines on intracapsular tonsillectomy. Next, Dr. Huoh explains the type of equipment he uses and his intracapsular tonsillectomy technique. He encourages surgeons to follow their tonsillar regrowth rate, as this data can inform surgeons on whether they are removing enough tissue during the procedure. Additionally, he also emphasizes that it is never too late to learn a new procedure like intracapsular tonsillectomy. He finds it easy to add on an intracapsular tonsillectomy as a minor procedure if he is already taking a patient to the OR for another surgery because of the minimal risks of the procedure. The Life of Riley is an American radio situation comedy series of the 1940s that was adapted into a 1949 feature film, a 1950s television series, and a 1958 comic book. Radio: The radio program initially aired on the Blue Network (later known as ABC) from January 16, 1944, to July 8, 1945, it then moved to NBC, where it was broadcast from September 8, 1945, to June 29, 1951. Irving Brecher pitched the radio series for friend Groucho Marx under the title The Flotsam Family, but the sponsor balked at what would have been essentially a straight head-of-household role for Marx. (Marx would get his own series Blue Ribbon Town instead.) Brecher then saw William Bendix as taxicab company owner Tim McGuerin in Hal Roach's The McGuerins from Brooklyn (1942). Radio historian Gerald Nachman quotes Brecher as stating, "He was a Brooklyn guy and there was something about him. I thought this guy could play it. He'd made a few films, like Lifeboat, but he was not a name. So, I took The Flotsam Family script, revised it, made it a Brooklyn Family, took out the flip-flippancies and made more meat-and-potatoes, and thought of a new title, The Life of Riley. Bendix's delivery and the spin he put on his lines made it work." The reworked script cast Bendix as blundering Chester A. Riley, a wing riveter at the fictional Cunningham Aircraft plant in California. His frequent exclamation of indignation—"What a revoltin' development this is!"—became one of the most famous catchphrases of the 1940s. It was later reused by Benjamin J. Grimm of the Fantastic Four. The radio series also benefited from the immense popularity of a supporting character, Digby "Digger" O'Dell (John Brown), "the friendly undertaker." Brecher told Brown, "I want a very sepulchral voice, quavering, morbid," and he got it right away. The supporting cast featured Paula Winslowe as Riley's wife, Peg, and as Riley's mother-in law; Brown as O'Dell and as Riley's co-worker Jim Gillis; Francis "Dink" Trout as Waldo Binny; Tommy Cook, Bobby Ellis and Scotty Beckett as Junior at various times during the show's run; Barbara Eiler as Riley's daughter, Babs; Shirley Mitchell as Honeybee Gillis; Hans Conried as Uncle Baxter; and, Alan Reed as multiple characters, including Riley's boss (Mr. Stevenson) and Peg's father. Henry Morgan voiced Riley's father in one episode. Mel Blanc provided some voices as well, including that of Junior's dog Tiger as well as that of a dog catcher who claimed to have a special bond with dogs. Mitchell's Gillis often gave Riley bad information that got him into trouble, whereas Brown's Digger gave him good information that "helped him out of a hole," as he might have put it. Brown's lines as the undertaker were often repetitive, including puns based on his profession; but thanks to Brown's delivery, the audience loved him. The program was broadcast live with a studio audience, most of whom were not aware Brown played both characters. As a result, when Digger delivered his first line, it was usually greeted with howls of laughter and applause from surprised audience members. The series was co-developed by the non performing Marx Brother turned agent Gummo. The American Meat Institute (1944–45), Procter & Gamble (Teel dentifrice and Prell shampoo) (1945–49), and Pabst Blue Ribbon beer (1949–51) took turns as the radio program's sponsor. An unrelated radio show with the title Life of Riley was a summer replacement show heard on CBS from April 12, 1941, to September 6, 1941. The CBS program starred Lionel Stander as J. Riley Farnsworth and had no real connection with the more famous series that followed a few years later.

Tonsillectomy and adenoidectomy, or T&A, is the second most common childhood surgery performed. It is important for pediatricians to have a detailed understanding of indication for referral, pre- and post-surgical management, and how this procedure has evolved over time. Today we are joined by an Otolaryngology expert to talk about tonsillectomy and adenoidectomy surgeries, when it is needed, and how this surgery is beneficial to patients. Today I am happy to be speaking with Dr. Soham Roy. Dr. Roy Soham is the chair of Pediatric Otolaryngology at Children's Hospital Colorado and is a professor and vice chair at the university of Colorado school of medicine.

This podcast highlights original research published in the April official journal of the American Academy of Otolaryngology—Head and Neck Surgery (AAO-HNS) Foundation. There is little research on the rate and risk factors for revision tonsillectomy after primary intracapsular tonsillectomy. Our study aimed to determine the revision rate following intracapsular tonsillectomy, identify patient characteristics that may increase the probability of revision surgery, and report the tonsillar hemorrhage rate after revision. In conclusion, patients below age 4 years and patients with gastroesophageal reflux disease may be at increased risk of undergoing revision tonsillectomy after primary intracapsular tonsillectomy. Surgery, whether elective or emergency, is done for many reasons. It's important for patients to be informed.  Colonoscopy? Biopsy? C-section? Tonsillectomy? All surgical procedures. Chances are, you'll need some type of surgery, whether minor or major, at some point in your life.  Surgery has changed a lot over the years, from the primitive methods of the Stone Age and Middle Ages to today's minimally invasive procedures completed with an incision the size of a keyhole. Surgeons and surgery are commonly misunderstood. And the idea of being cut open is understandably frightening, no matter how minor the procedure. But the more you know about something, the less mysterious and scary it becomes (knowledge is power!). What is a Tonsillectomy? A Tonsillectomy is a surgical procedure to remove the tonsils.  Tonsil - immune system tissue in the back of the throat.  -Ectomy = to remove something from the body. Removing the tonsils may be required if chronic tonsilitis is a problem.  Chronic tonsilitis is defined as multiple infections in a row or an infection lasting 3 months or longer.  Severe snoring and sleep apnea may be another reason to remove the tonsils, for the purpose of opening up the airway. Adenoids Adenoids are another set of immune system glands in the back of the nasal cavity.  They can also swell during infections and interfere with breathing.  Depending on the severity of the infections or the risk of sleep apnea, these may also be removed in the same or a similar procedure. Tonsillectomy Methods Since general anesthesia is used in all methods to remove the tonsils, no eating is allowed before the procedure.  They also recommend no NSAIDs (non-steroidal anti-inflammatory drugs) are to be used before and after due to their ability to thin the blood and increase the risk of bleeding.  In general, a tonsillectomy will take between 30 and 60 minutes.  Recovery time afterward is estimated to be about two weeks. Scalpel This is the old school method of using a very sharp surgical knife to cut out the tonsil.  This method has the highest risk of bleeding. Cauterization This method involves a hot knife that separates the tonsil tissue from the throat.  The heat also closes off the blood vessels so bleeding is not as severe.  A downside is that the tissue has been burned, so I imagine the pain after this method is more severe. Ultrasonic Vibration Ultrasonic waves are sound waves that have a higher frequency than the human ear hears.  The waves cause a very rapid vibration and the energy of those waves is what causes the tissue to separate as well as the blood vessels to clot.  This is the newest method developed and seems to have the least bleeding and pain. After the Tonsillectomy The side effects of a tonsillectomy include swelling of the throat, as well as the face and jaw, bleeding, and infection.  To avoid the bleeding and infection, it is important to follow all the instructions given for the recovery period.  Because of the swelling, cold foods like ice cream and popsicles are popular because cold things reduce swelling.  Popsicles can also help with hydration because swallowing bigger sips or gulps of water can be painful the first few days.  Hot foods are not recommended because the heat can increase the pain or reinjure the surgical site.  And while ice cream is the most popular post-tonsillectomy food, any soft, non-abrasive foods are fine to eat. Why do adults have more issues than kids? Kids heal faster because they are still growing and developing.  Kids also have smaller body parts than adults - the tonsils are smaller and the blood vessels are smaller.  Smaller blood vessels clot and heal faster than larger ones.  This is even true between the different sized blood vessels in your own body.  And a factor no one wants to admit: adults are terrible at following directions and truly resting after a surgery or procedure.  Rest is the best thing for a healing body of any age or size. Strep Carriers Generally, when we think of severe tonsilitis that leads to a tonsillectomy, we think of strep throat.  A carrier is someone who carries the germ around with them but does not get sick from the germ (pertussis is another example). In 1998, a study found that 5-15% of school-aged children were asymptomatic carriers of strep.  This means they tested positive on a strep swab but had no symptoms of being sick.  This causes concern because it means that you can be sick with something else that causes vague symptoms like fever and sore throat but because the strep test is positive, antibiotics are prescribed when they might not be necessary (meaning that you were sick from a virus).  This study showed that providers and caregivers were unaware of the number of unnecessary antibiotics they were prescribing.
