“We moeten gewoon samen meer leuke dingen gaan doen”, zegt hij. “Vroeger deden we veel meer samen, was er meer plezier en dat gaf het gevoel meer in een relatie te zitten. Nu leven we alleen nog voor ons werk, de kinderen en als we geluk hebben is er nog tijd voor een eigen hobby of sport. Maar echt iets samen doen, zit er al jaren niet meer in”In deze aflevering praat Job Hulshoff samen met Bart De Coninck en Laurien Engels over wat de voor- en nadelen kunnen zijn om de relatie te willen verbeteren door samen meer leuke dingen te gaan doen.  Bovenstaande anekdote leek Job een mooie aanleiding om het in deze podcast eens over te hebben. In this episode we discuss Ciara's book Memory Lane: The Perfectly Imperfect Ways We Remember, co-authored by her colleague Gillian Murphy. The book is all about how human episodic memory works and why it works the way it does. Contrary to our common assumption, a "good memory" isn't necessarily highly accurate - we don't store memories like files in a filing cabinet. Instead our memories evolved to help us function in the world. That means our memories are flexible, constantly changing, and that forgetting can be beneficial, for example. Regarding how our memories work, we discuss how memories are reconstructed each time we access them, and the role of schemas in organizing our episodic memories within the context of our previous experiences. Because our memories evolved for function and not accuracy, there's a wide range of flexibility in how we process and store memories. We're all susceptible to misinformation, all our memories are affected by our emotional states, and so on. Ciara's research explores many of the ways our memories are shaped by these various conditions, and how we should better understand our own and other's memories. Editor in Chief Cecelia E. Schmalbach, MD, MSc, is joined by Associate Editor Babak Givi, MD, and lead author Leila J. Mady MD, PhD, MPH, to discuss “Gender Differences Among Head and Neck Microvascular Reconstructive Surgeons,” which published in the November 2023 issue of Otolaryngology–Head and Neck Surgery.  The research used a survey that was sent to facial plastic and maxillofacial surgeons, in addition to microvascular surgeons, to gain a comprehensive understanding of what causes gender differences in the subspecialty. One takeaway of note was that there were no gender differences when it came to training and practice patterns. Another revealed gender differences when explaining changes in practice—for men, the reasons related to career advancement; for women, the reasons related to burnout, usually related to work-life balance and especially if they have children.

Winning Despite being Beaten with Olympic Racquetball Player Rhonda Rajsich  Rhonda Rajsich is an Olympic athlete, a professional racquetball player, speaker, coach, and author. She started playing racquetball at 2 by breaking out of the nursery at the health club her parents went to. She comes from a family of accomplished professional athletes and excellence was expected of her from a young age. Instead of being intimidated by this, she felt inspired by it and knew she would be great. Turns out she was right and today she is the most decorated professional racquetball player in United States history. However, her journey was anything but easy.  Flashback to late Saturday night, June 1st, 2008 in Hermosa Beach California. Rhonda had the World Championships coming up in just 6 months when she was blindsided and attacked by two men with brass knuckles. She had the presence of mind to stay on her feet, avoid escalation, and make bold choices that kept her alive. She ended up with several facial fractures that required an extensive 12-hour reconstructive surgery and a grueling recovery process. As hard as the attack was on Rhonda, it was devastating to her loved ones. Instead of receiving comfort, she ended up being the one comforting her girlfriend and family.  After she got home from the hospital, Rhonda was confronted for the first time by her coach with the fact that she wouldn't be able to compete in the World Championship. True to her nature, Rhonda wasn't accepting no as an answer. They never caught who did this to her, but Worlds was the only thing on her mind. At this point, she was using a walker, unable to eat solid food, and had lost 37 pounds. Despite all the odds, she did it.  Jumped on June 1st. Reconstructive surgery on June 11th. Stopped all pain medication on July 5th. Won a tournament the second week of July.  Flew to Ireland on July 29th.   Won her first world title, undefeated, on August 4th.   Leading up to the competition, she couldn't move around her house let alone train. This is where intense visualization and understanding of the mind-body connection came into play. By visualizing each step in the training process, using all five senses, and showing complete dedication to where she wanted to be, Rhonda was able to stay at the top of her game without ever stepping foot on a court. Everything takes place in the brain and rehearsing perfection results in perfection.    Today, she is still competing and still undergoing surgeries. She is still in pain and still overcoming the odds. In spite of all of this, she never entertains anger or resentment and she she says the attack gave her more than it took. Her excruciating daily pain is a reminder of everything she has to be grateful for and everything she has accomplished. She never has to prove anyone else wrong. She only has to prove herself right.  “Nothing happens to you, it happens for you.” In this episode, you'll hear: What was happening on a psychological level for Rhonda when she was being attacked?  Why a little bit of questioning medical professionals in the right circumstances can be useful sometimes. The power of using all five senses in visualization when she couldn't use her body to train. DIEP flap breast reconstruction most often requires surgical drains for post operative care. Patients often mention to me they are awkward, can become infected at the suture site where they are attached to the skin, and are cumbersome to hide under clothing after surgery when going out in public. My guest on this episode of the DiepCJourney podcast has been in communication with me over the past year in anticipation of his recently published paper, Are Surgical Drains Needed in DIEP Flap Surgery? The Drain-Free DIEP Flap Concept. Mr. Theo Nanidis, is a Consultant Plastic and Reconstructive surgeon with the NHS and private practice at The Royal Marsden. He graduated with distinctions from University College London Medical School in 2003 and completed his general surgical and higher plastic surgery training in London. Mr. Nanidis was awarded specialist fellowships at the Royal Marden and Queen Victoria Hospitals, where he spent two years specializing in microsurgery with a focus on breast reconstruction. He has a special interest in surgical innovation and was awarded The Life Sciences Entrepreneur of the Year Award from Cambridge University in 2013. His research interests include meta-analytical outcomes research, volumetric analysis of flaps and patient enhanced recovery pathways. He has published and presented his work nationally and internationally. Then, he discusses his experience of transitioning from open to robotic surgery. He discusses his experience with interviewing at programs and how he felt in the moment of being accepted into the robotic program at the University of Pennsylvania. Next, he describes the experience of completing a robotic boot camp. Dr. Cadillo faced challenges when returning to Puerto Rico, where there was a greater need for oncology and reconstructive surgeries. He learned that having a healthy obsession when learning new techniques is important, as well as having adequate resources, such as videos and mentors. In his opinion, practice and experience are the most important components of successful reconstructive surgeries. He then shares how he used mentorship and collaboration to deal with complex cases. Finally, he explains how the field of reconstructive urology in Puerto Rico has changed in the past four years with the arrival of a new generation of young urologists.

Today's guest is Dr Guido Maronati for MARA Aesthetics Dr Guido Maronati graduated in Medicine and Surgery from the University of Milan in 2000.  In 2001 he qualified to practice as a Medical Doctor after passing the Italian Medical Board qualifying exam and is registered with the Provincial Order of Physicians and Surgeons of Milan.  In his 2001-2002 Academic Year, he became the winner of the concourse to attend the School of Specialisation in Plastic and Reconstructive Surgery at the University of Udine. As part of our community health partnership with Valley Health, our conversation this month focused on plastic & reconstructive microsurgery. Dr. Neal Topham with Valley Health Plastic & Reconstructive Surgery joined me for the conversation. Dr. Topham explained the term microsurgery and talked about the various reconstructive surgery options that can aid in recovery (both mentally and physically) for cancer patients, bariatric patients, and others. We discussed his professional journey - what drew him to his specialty and why he chose to relocate his practice to Valley Health. He explained how flap surgery works and gave reasons why it can be beneficial. We talked about the types of surgery that insurance will cover and those it typically does not.

Guest Amy McLean Sprole grew up in Kansas and knew two things when she was graduating high school: She wanted to go into medicine and she wanted to spread her wings outside the midwest. She immediatly dove into the pre-med track and was so focused that she was able to do all of those requirements and still make time for international study abroad opportunities, including an art history experience in Italy. She thinks that out-of-the-ordinary adventure made her stand out in med school applications and may have been a reason for her acceptance to Cornell.She entered into med school thinking that she would pursue surgery without being able to articulate why, except that it seemed the hardest thing to do, a pattern she realizes she had had throughout life. She had to consciously set out to choose instead what she liked. This week's Urban Valor episode features Marine veteran, Cris Pacheco. Cris grew up in Chino, California. His parents immigrated to the United States from Cuba and El Salvador. Cris joined the Marines as an Amphibious Assault Vehicle Crewman AKA “Tracks.” He had an alcohol problem going into the Marines, which contributed to some mental health issues during his enlistment such as not being able to sleep for days at a time. Although he struggled mentally, Cris still received numerous awards such as Marine of the ship, Gung Ho Marine, and meritorious mast. He leaves us with his experience of how he set himself up to succeed upon transitioning out of the military. Marine Veteran Cris Pacheco is the definition of overcoming and growing. From immigration to alcohol to mental health, and overcoming reconstructive jaw surgery, he's shown that he can overcome adversity and continue to thrive and succeed. He was tentatively diagnosed with seasonal allergies around age 5 and clinically diagnosed with the skin test at 10. He had reconstructive sinus surgery at age 11 and was diagnosed with Graves disease in November of 2015.  On January 15th, 2022, he was diagnosed with Type 1 Diabetes. The day he got home from the hospital, he discovered Dr. Shawn Baker and Dr. Ken Berry on YouTube, followed by Mikaila Peterson, Dr. Ben Bikkman, Dr. Cywes, Dr. Paul Saladino, Dr. Robert Kiltz, Professor Bart Kay, and others.  Darby has had no sinus or ear infections from allergies with a total of only 8 days of symptoms this year. He has been taking no Insulin since February 1st. He has gained 26 lbs since May and has been taking no methimazole since April. His Graves' antibodies were at 301% on February 11. As of October 10, they were down to 198% with completely normal free T3/4 levels.
