Daniel Holmes, A US Navy Veteran Search and Rescue operator involved in the rescue of 4 trapped sailors who were at the very bottom of the ship operating a fuel station when they were surrounded by massive heat after several failed attempts by the ship firefighters his group of operators were asked to assist. Rescuing all 4 sailors successfully without injury.  In 2012 diagnosis of PTSD from the ship fire led him to learn about the VA healthcare system and how to navigate it. Moving forward to 2020 he was diagnosed with Membranous Nephropathy after being hospitalized for Nephrotic Syndrome. Being completely a new world to him. He quickly learned with the lack of information out there about this rare kidney disease that he needed to be as knowledgeable if not more about this disease than his own nephrologists. Night and day while fighting the symptoms of this disease he learned as much as he could attacking this disease like it was a rescue mission and failure was NOT an OPTION. Tamm Horsfall Protein : Mucoprotein made by tubular epithelial cells that precipitate out and congeal    to form casts on whatever is in the cells at the time.  (i.e. RBCs, WBCs, tubular debris) Bland Urine States- Crystalline Induced Renal Injury: obstruction and infllamatory response       - Uric Acid Neuropathy (Most common)              - Cancers, lymphomas, etc.              - Drugs: acyclovir, methotrexate, protease inhibitors, etc.              - Toxins: Ethylene glycol - Bland Urine Disease states: results from injury to tubules, instertim or pre glomerular blodd vessels, not    the filters of the kidney       - Interstital Nephritis              - Hallmark: pyuria and WBC casts                      - Biopsy: inflammatory infiltrate              - Causes:  viral, PPIs, Adenover, mizalamin, etc., Checkpoint inhibitors       - Acute Tubular Necrosis              - Hallmark: tubular epithelial cell cast                      - Granular: (course or fine) diagnostic of ATN              - Biopsy: denuded dilated tubular cells              - Causes: #1: Ischemia;  toxins, drugs, contrast dye;  pigment injury. myoglobin              - What about contrast dye?                      - Categorized under ATN                      - Per Dr. Thielen, plays a role, but injury is not solely dependent on dye alone.       - Hepatorenal Syndrome: ischemic injury to the kidney due to unopposed vasocontstriction               - Ace inhibitors cause unopposed efferent vasoconstriction + nonsteroidals cause                 unposed afferent vasoconstriction = no glomerular perfusion pressure       - Multiple Myeloma              - Hallmark: Light chain cast nephropathy or myeloma kidney                      - Light chains precipitate  out causing obstruction, inflammatory response and causes                        tubular damage              - Presentation: older possibly with anemia, bone pain and elevated creatinine with a bland urine.              - Protein to creatinine ratio: + for protein (non albumin)              - Dipstick: (which measures for albumin and not light chains) will be negative for protein aka                 bland urine       - Hypertensive Nephrosclerosis              - Small vessel vascular disease                     - Blood vessels prematurely atherosclerosis causing glomerular drop out and scarring of the                        interstim       - Scleroderma                - Limited cutaneous systemic sclerosis                - Diffuse cutaneous systemic sclerosis: 60-80% have renal injury from disease state itself                           - FANA positive                           - Concern for Scleroderma Renal Crisis = medical emergency                                   - AKI, moderate to severe HTN and bland urine                                   - Uncontrolled accumulation of collage, thickens vascular walls, narrowing and renal                                       ischemia                          - Occurs in 10-15% of those with Diffuse Cutaneous Systemic sclerosis and happens early                              in disease                                     - Left untreated: renal failure in 1-2 months and death in 1 year                          - Treatment: ACE Inhibitor Nephrotic Urine States - Urine protein: albumin excretion greater than 3.5g in 24 hours - Nephrotic Syndrome:      - Present with 3 things (nephrotic range protein, hypoalbuminemia, peripheral edema)       - Hyperlipidemia: due to increased hepatic lipogenesis                - Increased risk of renal disease and arthroscleratic       - Venous thrombotic disease:                 - Loose proteins other than albumin and develop a hypercoagulale state                 - Renal and peripheral venous thrombosis      - Lipiduria (forms fatty casts,  looks like a latese cross under microscope)  -Pathophysiology or nephrotic syndrome    - Glomerular capillary wall           - 3 layers that work as a glomerular filtration and responsible in the filtration between blood and             urine                  - Fenestrated Capillary Enothelial cells (fenestrations allow plasma through to the basement                     membrane)                 - Glomerular Basement Membrane (maintains glomerular filtration barrier; negatively charged,                     repels albumin)                 - Epithelium: Podocytes (Have highly specialized foot processes that connect and form slit                     diaphragms; Slit diaphragm important for the efficient flow of small solute and water)          - Anything that messes with any of these layers: nephrotic proteinuria - Nephrotic Disease States:     - Biopsy: anyone with nephrotic proteinuria (besides diabetics)          1) Light microscopy: high overview          2) Immunofluorescens: looks for nephritic component and identif immunce complexes          3) Electron microscopy: (EM) helps look at the ultrastructure and better identify immune deposits    - Diabetic nephropathy           - Leading cause of kidney disease in U.S. and western society           - Responsible for 30-40% of all ESRD causes           - Hyperglycemia: produces inflammatory responses, oxidative stress, and injures the podocytes and             deposits that charge and affect the ability of the kidney to filter.     - Most common cause of nephrotic syndrome in caucasion adults - 80% present with nephrotic but develops more slowly to ESRD - Primary: Major antigen identified      - antibody to trans-membrane receptor that is highly expressed on the glomerular podocyte - Secondary: Cancers (lung, breast, GI), Lupus, Thyroiditis, Hep B, Syphilis, Nonsteroidals, Monoclonal    Antibodies Nephritic Syndrome - Hematuria and proteinuria    - Hematuria: blood from kidney or outside the kidney             - In this interview, Marco discusses how he was invited to do a study on EMDR, to understand the neurological mechanisms behind the processing of the trauma. He discussed his career being a MD and a neuroscientist interested in memory. He discussed using EEG to measure what was happening in the brain during bilateral stimulation during EMDR. He explained that they were able to determine that the delta waves that were being evoked during EMDR were similar to the delta waves exhibited during sleep, and he discussed how sleep is so significantly connected to processing of memory. He discussed the processes of trauma and the mechanisms of action for EMDR. Marco Pagani, MD is a nephrologist trained in internal medicine from Jackson Memorial Hospital and attended medical school at the University of Miami School of Medicine. He has been working in neuroimaging since 1990 and has over 30 years of experiences. He works for the Italian Research Council called Sayonara. His interests are primarily in the neurobiology aspects of EMDR and in neurodegenerative disorders. He has treated Chronic Renal Disease, Nephrotic Syndrome, and Acute Renal Failure during his time as a Doctor of Internal Medicine. He has many publications regarding EMDR including Eye Movement Desensitization and Reprocessing and Slow Wave Sleep: A Putative Mechanism of Action and Neurobiological Correlates of EMDR Monitoring – An EEG Study.

Sanjeev Sethi, M.D., Ph.D., explains Mayo Clinic Laboratories' newly developed test to detect neural epidermal growth factor-like 1 protein (NELL-1), a biomarker for membranous nephropathy (MN). The immunohistochemistry assay examines tissue samples for NELL-1 antigen, which are found in 10% to 15% of MN patients. Discovered by Dr. Sethi in 2017, NELL-1 is the second most common antigen associated with MN and is correlated with underlying malignancy.

Welcome back! In today’s episode, Kent speaks with AAKP’s Ambassador, AAKP Ambassador Jennifer Jones. A retired Marine Veteran Jennifer’s life changed drastically in a matter of months due to kidney disease. Her kidneys began to fail while deployed to Afghanistan which resulted in immediate medical evacuation to Germany to receive a diagnosis of Nephrotic Syndrome. Upon returning to the states, she was immediately admitted as a patient at Walter Reed National Military Medical Center in Bethesda, Maryland. After months of testing and multiple medications, she was diagnosed with Membranoproliferative Glomerulonephritis (MPGN) shortly after her 28th birthday. The cause is unknown and there is very limited information on MPGN Type 1 Idiopathic. Jennifer was medically retired after almost 12 years of service as a Combat Camera Videographer including 3 tours to Iraq and the 1 to Afghanistan which ended her career. Facing physical and mental challenges while living alone with limited assistance from friends and family caused isolation, depression, and even suicidal ideations. She knew that she had to make a decision, is she going to become her own advocate in order to take charge of her health and well-being, or is she going to give up and succumb to this disease? Welcome back to our weekend Cabral HouseCall shows! This is where we answer our community's wellness, weight loss, and anti-aging questions to help people get back on track! Check out today's questions:  Kayla: I’ve become an avid listener the past couple of months as I have recently taken a renewed natural approach to my health. I appreciate your wealth of knowledge that you so graciously share with the world! I am 4 months post partum and am going through quite a roller coaster of thyroid issues. I was diagnosed hypothyroid during my pregnancy with a small node that was detected. The node has since shrunk in size but my thyroid problems have persisted. I have been diagnosed with postpartum thyroiditis, with a thyroid that went hyper and now is extremely hypo. My endo is sending me biweekly for blood work to constantly update my synthroid dosage. I am getting frustrated by the traditional medicine approach to thyroid management as it is seriously effecting my breast milk supply. My goal is to excusivly breastfeed my daughter, but have had to supplement with formula as a result of the thyroid issues. I want to resolve this in the most wholistic way so I can give my daughter the best nutrition. Any advice would be greatly appreciated. Gio: I started one of your protocols: Here's the backstory and my question involving it (Thank you in advance!): My friend is a big fan of your podcast and recently came across episode 760 and shared you and it with me. I am living with herpes and have done a multitude of natural remedies and probably some "snake-oil saleman" like remedies in the last few years. I've had anal warts completely disappear but one came back after a few years recently following a lot of stressful exertion and time in my life. I've been doing everything I can to rid myself of this pesky virus for good, as I believe there must be a way. I have not taken antibiotics in over 15 years (I'm currently 32 5'11" & 165 lbs), have a 95% clean diet, and have fought every sickness with whole foods. As a bird's eye view of my diet, I eat for mostly pitta, mostly vegan (lots and lots of dark leafy greens), and eat the highest quality grassfed/grassfinished ground beef & pasture raised chicken. I rarely eat processed food, exercise regularly but stress and lack of sleep wear my immune system down rather quickly. When I read up & and listened in on your Episode 760, I felt I finally found missing pieces to the puzzle in attempting to rid myself of this virus. My curiosity had wondered that maybe my immune system has never stopped fighting the virus and therefore my get exhausted more quickly as I age. Well, one week ago Friday (March 22, 2019), I was helping my roommate overcome a fever and sickness, and I believe I may have caught a bug from him, as I began to get a scratchy throat. Then that Sunday the 24th with a limited budget, I began the ACV+Cayenne+GSE water 3xdaily and the lysine+vitamin 1000mg each 3x daily, and I believe the wart shrank considerably. What is strange is that I've been extremely congested this last week and blowing a lot of green mucus (recognizing there's an infection). I was wondering if this could be die off system of the bacteria causing the infection and might be a natural culling process of the virus if in fact it's been residing within the bacteria? I'm going to continue with your regiment up to 2 weeks while saving to get the rest of your recommedations in April. Could you provide any feedback for the congestion/sickness I'm experiencing; is the congestion/infection potential/usual of he die off process for herpes?  Thank you so much for your time and consideration. MicroRNAs are present in body fluids and have the potential to serve as disease biomarkers. A study in the April 2013 issue of Clinical Chemistry explored the clinical value of microRNAs in serum and urine as biomarkers for idiopathic childhood nephrotic syndrome.  This paper was accompanied by an editorial by Johan Lorenzen and Thomas Thum, both from the Hannover Medical School in Hannover, Germany. The mechanism leading to hyperlipidemia in the nephrotic syndrome is not fully understood but may be related in part to loss of high density lipoproteins in the urine of patients with nephrosis. To prove this hypothesis, we compared serum lipoprotein profiles with the excretion of high density lipoproteins in urine in 19 nephrotic patients. Serum cholesterol ranged from 19–152 (median value 45) mg/dl in very low density lipoproteins (VLDL), from 130–443 (median 186) mg/dl in low density lipoproteins (LDL) and from 19–64 (median 33) mg/dl in high density lipoproteins (HDL). Hyperlipoproteinemia was found in 17 patients, which was classified as phenotype IIa (Fredrickson) in 2, as phenotype IIb in 9 and as phenotype IV in 6 subjects. Two patients showed normal lipoprotein patterns. VLDL- and LDL-cholesterol were not found in detectable amounts in urine, whereas HDL-cholesterol was measured in low concentrations from 0.1–8.3 mg/24 h in all samples. There was no correlation between serum HDL-cholesterol and urinary HDL-cholesterol, but a positive correlation between serum LDL-cholesterol and urinary HDL-cholesterol (r= +0.54, p < 0.05).
