We all think we know the story of pregnancy. Sperm meets egg, followed by nine months of nurturing, nesting, and quiet incubation. this story isn't the nursery rhyme we think it is. In a way, it's a struggle, almost like a tiny war. And right on the front lines of that battle is another major player on the stage of pregnancy that not a single person on the planet would be here without. An entirely new organ: the placenta.In this episode, which we originally released in 2021, we take you on a journey through the 270-day life of this weird, squishy, gelatinous orb, and discover that it is so much more than an organ. It's a foreign invader. A piece of meat. A friend and parent. We're diving into the creation and purpose of ManureDB, a groundbreaking database developed to modernize and enhance the accuracy of manure book values in the U.S. With most current values dating back to pre-2003 samples, researchers at the University of Minnesota, led by Drs. Applebaum, Ph.D., Director of the Caregivers Clinic at Memorial Sloan Kettering Cancer Center. About our Guest: Dr. Allison J. Applebaum is an Associate Attending Psychologist in the Department of Psychiatry and Behavioral Sciences, Memorial Sloan Kettering Cancer Center (MSK), and an Associate Professor of Psychology in Psychiatry at Weill Cornell Medical College. She is the Founding Director of the Caregivers Clinic at MSK, housed in the Counseling Center. The Caregivers Clinic is the first of its kind and provides comprehensive psychosocial care to family members and friends of patients who experience significant distress and burden as a result of their caregiving role. Dr. Applebaum's program of research focuses broadly on the development and dissemination of psychosocial interventions for cancer caregivers, as well as understanding the impact of caregiver psychosocial wellbeing, prognostic understanding and communication skills on advanced care planning. Her interest in cancer caregiver research began during her fellowship in psycho-oncology at MSK, during which her clinical work focused primarily on providing care to patients at the end of life. Such encounters highlighted the significant distress and unmet needs experienced by those patients' caregivers, the family members and friends who are so intimately involved in the care of patients but often not recognized for - and supported in - doing so. As a result, Dr. Applebaum has devoted her career to helping to address the needs of caregivers as they become increasingly critical members of our treatment teams. A key theme that emerged in Dr. Applebaum's work in the Caregivers Clinic at MSK was "sitting with uncertainty," a challenge faced by all caregivers, regardless of their loved one's site or stage of cancer. The ability to tolerate uncertainty has benefits for caregivers' wellbeing during and after caregiving, as well as for the patients for whom caregivers provide care. Dr. Applebaum's newly funded study "A Randomized Controlled Trial of Emotion Regulation Therapy for Cancer Caregivers: A Mechanism-Targeted Approach to Addressing Caregiver Distress" seeks to test the efficacy of Emotion Regulation Therapy for Cancer Caregivers (ERT-C), a contemporary cognitive behavioral therapy (CBT) developed to deliberatively target processes underlying the distress associated with caregiving and assist caregivers with their capacity to sit with uncertainty. In this trial, caregivers are randomized to receive 8 individually delivered sessions of ERT-C, or 8 sessions of Cognitive Behavioral Therapy for Cancer Caregivers, all delivered over telepsychiatry. In addition to evaluating emotion regulation skills and symptoms of anxiety and depression, the investigative team is examining relative change in salivary markers of stress and systemic inflammation in caregivers, and quality of life in the patients for whom caregivers provide care. Melissa Wilson, PA-C, MPAS from UPMC Hillman Cancer Center, and Vernon Sondak, MD, Chair, Department of Cutaneous Oncology, Moffitt Cancer Center and Research Institute in Tampa, Florida, join host Raymond Liu, MD, for a discussion about the future of melanoma in 2023 and beyond. Treatments combining immunotherapies with targeted therapies, which disable the carcinogenic products of mutated cancer cells, have further increased treatment efficacy and durability. Toxicity and resistance, however, remain critical challenges to the field. During this podcast, our guests review past treatments and novel therapeutic interventions and discuss the term "de-escalation of care" and future directions as well as how to communicate with your healthcare team. About Our Guests Dr. Sondak is Chair of the Department of Cutaneous Oncology at the H. Lee Moffitt Cancer Center and Research Institute in Tampa, Florida. He holds the Richard M. Schulze Family Foundation Distinguished Endowed Chair in Cutaneous Oncology, and is also a Professor in the Departments of Oncologic Sciences and Surgery at the University of South Florida Morsani College of Medicine. Since 2004, when he came to Tampa, the Cutaneous Oncology Clinic at Moffitt has grown into one of the largest multidisciplinary treatment centers for pediatric melanoma and related conditions in the world. Dr. Sondak has been a leader in studies of surgical treatment of melanoma and other cutaneous malignancies, particularly in the application of sentinel lymph node biopsy and lymph node dissection to the staging and treatment of melanoma, and has been instrumental in training many surgical oncologists in these techniques over the years. Dr. Sondak also has a strong research background: he served as Principal Investigator of the Moffitt Skin SPORE, a major NCI-funded “team science” grant conducting translational research in melanoma and other cutaneous malignancies. His research interests include surgical treatment of melanoma in adults and children; adjuvant (postoperative) and neoadjuvant (preoperative) therapy of melanoma; and evaluation of new therapies for patients with localized or disseminated melanoma. Melissa Wilson, PA-C, MPAS, has a wealth of knowledge and experience helping melanoma patients and their families. Melissa is up-to-date about the latest melanoma treatments and research. A natural trailblazer, Melissa is proud of her collaborative work as an NCCPA-certified physician assistant senior since 2005 with the distinguished melanoma specialist Dr. John Kirkwood at the University of Pittsburgh in the Cancer Institute Division of Hematology/Oncology, Department of Medicine. Their treatment and research initiatives were so successful that they have been used as a template for other cancer programs throughout the country. Wilson works as a lead physician assistant for the Melanoma Program at UPMC Hillman Cancer Center. A natural teacher, Melissa has also worked with a multidisciplinary team on patient education and collaboration. Series two of Emergency on Planet Sport has arrived with new episodes dropping every Wednesday and a new co-host! After injury wrecked her Tokyo Olympic dream, international rower Melissa Wilson has new priorities; firstly, developing her organisation "Athletes of the World" and, secondly, plonking herself on the EOPS sofa next to Jonathan Overend! Last year, more than 300 British Olympians and Paralympians put their signature to a letter addressed to Prime Minister Boris Johnson, urging him to steer the country towards a ‘green recovery' amid the Covid-19 pandemic.The letter was coordinated by Champions for Earth, a group of current and former sportspeople determined to make the athlete voice a key driver in the acceleration of the low-carbon transition.Melissa Wilson, one of the guests in this episode, is part of the core Champions for Earth team, coordinating much of its effort to engage and educate athletes to understand and communicate about climate change and other environmental issues.Joining Wilson is Morten Thorsby, a professional footballer for U.C.
