This podcast discusses the results of four late breaking clinical trials presented at the Heart Failure congress 2023 in Prague, Czech. First, dr. Brugts shares the results of the MONITOR-HF trial, an open-label, randomized trial showing that haemodynamic monitoring using a cardioMEMS device, significantly improved quality of life and reduced heart failure hospitalizations in patients with moderate-to-severe heart failure. This trial was simultaneously published in the Lancet (Remote haemodynamic monitoring of pulmonary artery pressures in patients with chronic heart failure (MONITOR-HF): a randomised clinical trial - The Lancet). Second, dr. Cleland walks us through a population study from the greater Glasgow area that found that use of loop diuretics in patients without a diagnosis of heart failure was associated with poor outcomes. He consequently discusses possible explanations and clinical consequences. Third, dr. Campbell shares the results of the DAPA-RESIST trial, that found that in patients with diuretic resistance, dapagliflozin was not superior to metolazone at relieving congestion (Dapagliflozin versus metolazone in heart failure resistant to loop diuretics | European Heart Journal | Oxford Academic (oup.com)). Fourth, dr. Dauw discusses the results of the ENACT-HF trial where a strategy of natriuresis guided therapy as recommended by the HF guidelines, in an open label, sequential roll-out (first 10 patients standard of care, followed by 10-30 patients in the natriuresis arm), was shown to improve natriuresis and reduced length of hospital stay. No effect on change in congestion score or in-hospital mortality was observed. The podcast is concluded with a brief discussion of some HFA Young highlights at the congress.

WHAT KIDS NEED MOST IN A PARENTRATED PG: Parental Guidance Needed - Part 1Springcreek Church | Senior Pastor Keith StewartFebruary 19, 2023Parents are often exhausted, stressed out, & under constant pressure. Clear up the confusion, relieve unrealistic expectations, & get focused on the essentials that enable our kids to become all that they were meant to be.DISCUSSION QUESTIONS1. According to the Britannica Dictionary, a stereotype is an often unfair and untrue belief that many people have about all people or things with a particular characteristic. What other stereotypes exist in our society in relation to moms and dads?2. Take some time as a group to discuss the modern phenomenon of the increasingly mobile and as a result isolated nuclear family. As parenting falls more exclusively and squarely on the shoulders of just the mom and dad (as opposed to the extended family), the pressure becomes unbearable, the knowledge and limited experience take a toll and parents feel completely alone against their challenges. What can parents do to mitigate those realities? Who else do you rely on when the going gets tough? What are the benefits of having others involved in helping to raise your children?3. Take some time as a group to discuss the developmental chart that Pastor Keith shared in the message. Ask yourself about the three primary developmental needs; nurture, affection, and attention and modeling – what was your experience in those areas growing up? Which did your parent(s) do particularly well? Were any of these diminished or not what you hoped they would be? How does that affect you today? Discuss your thoughts on parental absence as it relates to these developmental needs. 4. Pastor Keith discussed the importance of play as it relates to nurturing. Did your parents play with you? Did you play with your children? Why do you think play is regarded as frivolous? If your parents didn't play with you, how did they spend time with you? Kids spell love T-I-M-E. They both want and need to be with their parents. Some parents overestimate how much time they spend with their children. Realistically, how much time did your parents spend with you? How much time did you spend with your children? 5. We desperately need both affection and attention. How affectionate were your parents? How affectionate are you? According to Ross Campbell, who wrote the book, How to Really Love Your Child, “6-year-old boys receive 1/6th as much hugging and kissing as 6 year-old-girls.” He also discussed the truth that first-grade boys get into trouble 6x's more frequently than first-grade girls. Do you think there's a correlation between those two facts? Why aren't little boys treated with the same affection as little girls? Do you think this is a good thing or not?In terms of attention, our kids really do desire ours. In particular, they want parents who listen well. In fact, they will often seek out other adults in order to get an adult perspective on their problems. They just want someone who will listen as opposed to react to everything they're saying. Were your parent's good listeners? Are you a good listener?  What are some good open-ended questions that lead to great conversations?6. There are three messages God spoke to His Son that serve as a great pattern for parenting today, I love you, I believe in you, and I'm proud of you. Talk about the importance of those messages and your personal experience with them in regard to your parents and parenting.Pastor Keith added one more message that God does not need to communicate but we most certainly do and that is “I am sorry.” Have you ever apologized to your kids? So often we show our kids love in the way WE feel loved and then wonder why they  question our love. We're doing the best we can, right?? Yes, but we can do better.Today I challenge you to stop and observe your kids for a second. Are they following you around non-stop? (Quality Time?) Are they clingy and driving you nuts? (Physical Touch?)Are they angry at you all the time and saying hurtful things? The way we give love and receive love is called our love language. The concept of love languages was developed by Dr. Gary Chapman with his 1992 book The Five Love Languages that has sold over 12 million copies. In 1997 he got together with Dr. Ross Campbell and authored the book The 5 Love Languages of Children. The ideas in this book are really powerful in that every child is different. If you don't speak your child's love language, they may not feel loved. In this episode, we are going to go over the 5 love languages for children so you can identify your child's primary love language so you can build a stronger relationship with them. With a 5 star rating on amazon and over 1100 reviews, this book and it's concepts have proven why it is so popular. The 5 Love Languages of Children helps us discover how to best show our children that we love them. Of course it's good to show them love in multiple ways and most children are receptive to all of them, but usually, a child (just like adults) has a primary love language. Let's go over these 5 love languages for children: #1. Words of Affirmation - A child whose primary love language is words of affirmation feels loved when we use encouraging words and phrases to them. They enjoy compliments like "you did that so well" or "thank you for helping me with cleaning." Both written and verbal expressions are powerful so you can say "I love you" or write a note for them expressing how special they are. It's the words that make them feel loved. #2 Gifts - Children with this love language like tangible tokens of love. Often these gifts can become symbols of love. Some examples of gifts may be getting them a book that you can read together or putting a favorite photo in a frame so they can put it in their room. It feels like all kids like gifts but for these children, the gifts are often seen as an extension of your love. #3. Acts of Service - Maybe your child really feels loved when you fix their favorite toy or you make their favorite meal for them. When children are younger, acts of service are often doing things for them that they may not be able to do themselves. As they get older, the acts of service are activities that cause them to feel loved and appreciated. You may sit down and do their homework with them. Figure out what acts of service you can do for them that they would really appreciate. #4 Quality Time - Quality time is doing something together and your child receiving focused attention. If you have multiple children, be sure to spend one-on-one time with your quality time child. This could be coloring or reading with them...or sitting down and playing a game. Spending time together makes them feel loved. It's not necessarily the activity but the time spent together. #5 Physical Touch - A child that is always snuggling up to you, or wants to constantly sit on your lap, this is a child who's primary love language is probably physical touch. They get fulfilled by your hugs and kisses, your backrubs or running your fingers through their hair. Physical touch is so important to them and it's how they feel close and loved. With any of these love languages, there can be some pitfalls. Think of the child that you have to always buy presents for in order for them to feel loved or the child requires a lot of your extra time to get their love cup filled. As with anything, you want to mindful that balance is important. If you are constantly doing everything for your child because their love language is acts of service, this can negative. In that case, your act of service can be teaching them to be able to do things for themselves. All of these love languages for children are about us as dads being able to create a strong bond with our child. When you understand your child's love language, your efforts are much more effective.

Derek is back at his local shop, Collected Comics and Games in Plano, TX, and this month the topic is open and general comics talk. Anything goes, and over the coarse of the conversation, Derek discovers that anything does go in talking with the shop's customers. Joining him for this free-flowing exchange are regulars Shea, Craig, Matt, and Chris, and the discussion topics are wide-ranging. They begin with a conversation on the relative disappearance of westerns and war comics as a popular genre. Sgt. Rock comes up a lot, of course, as well as Garth Ennis's recent revival of his War Stories series coming out from Avatar Press. And Craig reminds everyone about a title he brought up in the February episode, Max Brooks and Caanan White's The Harlem Hellfighters. Via a reference to one of Derek's favorites, Weird War Tales, the talk shifts to horror and mystery comics and then takes an abrupt shift into Kickstarter territory. This leads to a discussion of the recent shenanigans of Archie Comics in their Kickstarter campaign, and then a brief digression into the concept of variant covers. Webcomics is another topic that comes up, with everyone chiming in on whether or not they read them, as well as the pros and cons of digital delivery. There is a brief conversational tributary into mainstream superhero comics, specifically what's going on with Convergence and Secret Wars, and then they wrap up with a discussion of some of comics everyone has recently read for the very first time (not necessarily brand new titles). These titles include Chuck Palahniuk and Cameron Stewart's Fight Club 2, Cullen Bunn and Taylor Crook's Harrow County, Kelly Thompson and Ross Campbell's Jem and the Holograms, P. Craig Russell and Neil Gaiman's Elric: Stormbringer, Chip Zdarsky and Kagan McLeodand's Kaptara, and various books from Hans Rickheit (the latter are Derek's picks). The chat ends abruptly when the recorder is accidentally shut off (unbeknownst to the guys), but that doesn't happen until the tail end of the conversation. But while the mic is still on, there's a lot of great comics talk, as there usually is at these on-location episodes.
