The focus of this podcast is the enigmatic theme of prion disorders. I trace the history of our understanding of this neurodegenerative disorder with how Nobel laureate D. Carleton Gajdusek, described the transmission of kuru - the first pathologically defined human prion disease. I also narrate how the similarities in the clinical and pathological features of kuru and scrapie, the disease of sheep, opened the way to this breakthrough. I also chronicled how the efforts of another Nobel laureate, Stanley Prusiner, characterised the pathology, genetics and transmission of scrapie, and how he succeeded in a bitter scientific race that transformed the field of transmissible spongiform encephalopathies which he named prion diseases. I also explored the pathology of prions, their normal functions, and how they cause disease when they become misfolded. In this theme, I discussed Creutzfeldt Jakob disease extensively, with reference to the roles played by Alfons Jakob and Hans Gerherdt Creutzfeldt, and depicting its classical manifestations and investigations. I illustrated the lived experience of CJD with such memoirs as Howie V CJD, by Sandy Bosman. I also exhaustively covered the clinical and management aspects of new variant CJD, and I illustrated this with the memoir Who Killed My Son, by Christine Lord. The podcast also reviewed the other established human prion diseases such as fatal familial insomnia, and it explored the similarities between prion diseases and other neurodegenerative diseases that are also related to misfolded proteins, such as Alzheimer's, Parkinson's, and motor neurone diseases. I also covered mad cow and other prion disorders of animals. Most everyone knows of a cancer survivor. But, by and large, when a person receives a diagnosis of Alzheimer's disease, it is often assumed that this is an indication of an almost certain outcome. Alzheimer's disease is not caused by a specific flaw. While energy deficiency, inflammation, infectious agents, beta amyloid, acetylcholine deficiency, trauma, toxins, metabolic issues, and genetics are all worthy of consideration, it is clearly myopic to consider that targeting one of these popular ideas is going to solve the problem. It doesn't. Our guest today has taken a different approach. Rather than trying to find a unique drug or technique that can help the Alzheimer's population at large, this novel approach focuses on identifying how variations in multiple parameters can be targeted with the overall goal of improving brain functionality that is compromised in the developing Alzheimer's situation. This approach, a personalized medicine approach, directly challenges the notion of a one-size-fits-all mentality as it relates to treating medical problems. He established the ADDN (Alzheimer's Drug Development Network) with Dr. Varghese John in 2008, leading to the identification of new classes of therapeutics for Alzheimer's disease. He and his group developed a new approach to the treatment of Alzheimer's disease, and this approach led to the discovery of subtypes of Alzheimer's disease, followed by the first description of reversal of symptoms in patients with MCI and Alzheimer's disease, with the ReCODE (reversal of cognitive decline) protocol, published in 2014, 2016, and 2018. Dr. Bredesen is the author of the New York Times bestseller, The End of Alzheimer's, and the newly released, The End of Alzheimer's Program. Elle Russ chats with Dr. Dale Bredesen, MD - he received his undergraduate degree from Caltech and his medical degree from Duke. He served as Resident and Chief Resident in Neurology at UCSF, then was postdoctoral fellow in the laboratory of Nobel laureate Prof. Stanley Prusiner. He was a faculty member at UCLA from 1989-1994, then was recruited by the Burnham Institute to direct the Program on Aging. In 1998 he became the Founding President and CEO of the Buck Institute for Research on Aging, and Adjunct Professor at UCSF; then in 2013 he returned to UCLA as the Director of the Easton Center for Alzheimer’s Disease Research. The Bredesen Laboratory studies basic mechanisms underlying the neurodegenerative process, and the translation of this knowledge into effective therapeutics for Alzheimer’s disease and other neurodegenerative conditions, leading to the publication of over 220 research papers. He established the ADDN (Alzheimer’s Drug Development Network) with Dr. Varghese John in 2008, leading to the identification of new classes of therapeutics for Alzheimer’s disease. He and his group developed a new approach to the treatment of Alzheimer’s disease, and this approach led to the discovery of subtypes of Alzheimer’s disease, followed by the first description of reversal of symptoms in patients with MCI and Alzheimer’s disease, with the ReCODE (reversal of cognitive decline) protocol, published in 2014, 2016, and 2018. Alzheimer's disease and other neurodegenerative conditions are becoming one of the leading global health threats. As the human population ages, the prognosis is that Alzheimers will become a worldwide pandemic if effective prevention and reversal are not implemented. And here's the kicker, conventional medical opinion says that Alzheimer's can't be prevented, is untreatable and progressive. And most patients do not survive beyond three to eleven years after diagnosis. My guest today says that Alzheimer's is not just preventable, but reversible. Dr. Dale Bredesen is an internationally recognized expert in the mechanisms of neurodegenerative diseases. He has researched neurodegeneration for three decades and has published 230 peer reviewed papers and four books over his career. In those 30 years, he has developed a different opinion comparing Alzheimer's disease as a modern day equivalent to polio. He says that while it is ubiquitous now, it should be a rare disease and become a disease of the past. Dr. Bredesen also says that the buildup of amyloid plaque is not the cause for Alzheimer's, but a symptom of the true underlying issues. And those issues are based on a variety of complex factors making Alzheimer's not a single condition, but a host of conditions caused by a variety of insults. This is why Dr. Bredesen believes that traditional medicine, which is focused on removing the amyloid plaque, has very little benefit until the underlying issues have been addressed. And rather than proposing a single treatment, Dr. Bredesen has created a multi-pronged program that has been successful in reversing Alzheimer's. This is especially relevant for women, as statistically speaking, we're more likely to develop brain and memory-related diseases as we age. Dale provides us with some insight into why we should focus on our brain health once we turn 35. It's never too early to prepare our bodies for healthy aging. And if you're already showing signs of memory loss, Dale wholeheartedly encourages you to seek medical intervention early. This is one of the reasons it takes so long for an Alzheimer's Disease diagnosis. Because we tend to shrug off minor memory failure as just being forgetful, relying more on our long-term memory, it can be difficult to pinpoint the onset of the disease. The Keto Diet for Alzheimer's Prevention The first step to treating and preventing Alzheimer's Disease, of course, starts in the gut. Dale recommends a Ketogenic diet that relies heavily on plant-based foods - much like my KetoGreen program. There are so many different approaches to Keto you can take, especially depending on what type of Alzheimer's subset you have. But overall, Dale recommends an overnight fast, vegetable-based, and low in fats such as bacon. Dale also encourages a low Glycemic diet. This is one that's low in sugar, even those occurring in fruits, as so many of our fruits have been genetically modified over the years to have a much higher sugar count than what's natural. Finally, Dale gives some very valuable insight into the importance of our oral health. Dr. Bredesen’s research has led to new insight that explains the erosion of memory seen in Alzheimer’s disease, and has opened the door to a new therapeutic approach. He has found evidence that Alzheimer’s disease stems from an imbalance in nerve cell signaling: in the normal brain, specific signals foster nerve connections and memory making, while balancing signals support memory breaking, allowing irrelevant information to be forgotten. But in Alzheimer’s disease, the balance of these opposing signals is disturbed, nerve connections are suppressed, and memories are lost. This model is contrary to popular dogma that Alzheimer’s is a disease of toxicity, caused by the accumulation of sticky plaques in the brain. Bredesen believes the amyloid beta peptide, the source of the plaques, has a normal function in the brain — promoting signals that allow some of the nerve connections to lapse. Thus the increase in the peptide that occurs in Alzheimer’s disease shifts the memory-making vs. memory-breaking balance in favor of memory loss. This work has led to the identification of several new therapeutic candidates that are currently in pre-clinical trials. Dr. Bredesen’s novel insights into the fundamental nature of Alzheimer’s disease recently attracted an investment of $3.5 million toward a $10 million goal for initial clinical trials of these new therapeutics. This generous support came from the private venture capitalist Douglas Rosenberg, who is helping to fund the Alzheimer’s Drug Discovery Network, centered at the Buck Institute. The unit is screening drug candidates to find those that can preserve a healthy balance in the signaling pathways that support memory. Dr. Bredesen’s work on nerve cell signaling is also the focus of a collaboration between the Buck Institute and BioMarin Pharmaceuticals, Inc., which is seeking treatments for a rare form of Alzheimer’s disease, early onset Familial Alzheimer’s Disease (eFAD), which may develop in people as young as 30 years of age. Guy Our awesome guest today is Dr. Dale Bredesen. Dale, welcome to the show. Dr. Dale Thanks very much for having me. Guy Look, it’s fantastic, mate. We always kickoff and ask the same question to all our guests when they come on the show. That is, if a complete stranger stopped you on the street and asked you what you did for a living, what would you say? Dr.Dale I do everything possible to see if we can improve people who have neurodegenerative illness. [00:03:00] Guy Beautiful, perfect. I couldn’t say it any better. The other thing we always ask as well, Dale, is, would you mind just filling us a little bit in of your journey, your background, and what led you to be so passionate about this work, and making waves in the industry? Dr. Dale [00:03:30] Yeah. I came from a different place than most people who are doing this sort of medicine today. I came from a very classical science background. I was interested in mathematics and chemistry when I was a kid. I went to Caltech, and spent time at MIT as well for those reasons, working in chemistry. And then decided that if we were going to do something … I got very, very interested in the brain. When I was a freshman in college, I read a very interesting book called The Machinery of the Brain. I got very excited about that and kind of got hooked for life on neuroscience. [00:04:00] But I realized that if I was going to make any inroads into the illnesses that affect the brain, I really needed to go to medical school. So I went to medical school and got a lot of criticism for being a scientist wanting to go to medical school. But I went into neurology, and as you know, neurology has a long history of being tremendous diagnosticians. And, of course, Sherlock Holmes was actually based on a real neurologist. But in general, neurologists have been known to be the group that does not do much about the treatment side, very good with diagnosis, not so great, not so successful with treatment.
