Brad Davis is on the Board of Directors for the San Diego Front Runners  and Walkers Association. Brad served in the Air Force for five years, active and three years on reserve. He served as a “para rescue“. He was what they called a PJ (parachute jumper) Their motto was “That Others May Live”. They were Air Force Special Forces . Their job was to save lives on land , sea , and in the air. He came out as gay in his early 20s and showed courage to come out to his family as well. This was the time when if you were gay, you are not allowed to stay in the Air Force. This  is a powerful story dedicating his life to help others, and even forgiveness of his mother and taking care of her until she died. He and I discussed what it means to “have a voice” because some of our rights are  to sliding away. All of us need a community and a voice, because if we are silent, we are invisible.

Evan Southern is a Royal Canadian Air Force (RCAF) pilot who flies the CH-149 Cormorant search and rescue helicopter, a variant of the Leonardo AW101 (EH101).This episode details the harrowing Search and Rescue (SAR) mission of 15 November 2021, where hundreds of motorists were trapped on a British Columbia highway due to landslides caused by torrential rain.You'll hear how Capt. Southern and his crew -- who are members of 442 Transport and Rescue Squadron located at Canadian Forces Base Comox -- were the first to respond to the call for help. The Squadron ultimately deployed three Cormorant helicopters in different configurations to respond to this particular SAR mission. You'll hear about the Cormorant's capabilities and some of the tactics that SAR pilots use in training and how they apply that to operational missions.The Cormorant crews worked all day to rescue over 300 people, breaking all previous records for people rescued in a singe mission by the type.If that wasn't enough, 442 Squadron also responded to two other SAR missions that day -- one for a vessel in distress, and the other for a small aircraft that was reported to be down.You'll hear about the skill and professionalism at 442 Squadron, whose motto is "That Others May Live".Thanks to Capt. Dr. Doug Kechijian (Dr. K) is a former Pararescueman, or PJ, in the US Air Force and now a Doctor of Physical Therapy and CEO / co-founder of Resilient Systems; he talks professionalism, procedure, self discipline, human performance and getting the call with Jason and Rich for Episode 036.  In his senior year at Brown -- not a typical military feeder school -- the events of 9/11 steered Doug onto a different path in medicine than the one he had been planning. He was particularly drawn to the mission of the PJs. Through his 13-year military service in the “911 of the military,” including being named as the Air National Guard's 2015 Outstanding Noncommissioned Officer of the Year, Doug learned the importance of smart training, being really really prepared and strategies to cope with challenges (medical and otherwise).  Dr. K now works with athletes and operators at every level from Major League Baseball to the NCAA to Special Operations Forces. He has been featured in Men’s Health advocating for rucking along with other items from his “menu” of training (mobility, structure, power and capacity -- all with variety). Easily one of our best vent podcasts.  Good sound! We are in the process of fixing the sound (of course by a NY PJ- not me). This talk covers the basics but also blast lung, TBI and trouble shooting from an experienced respiratory tech with deployments under his belt. He is currently at Baltimore Shock Trauma where we have a great rotation going. Your Flight Doc knows how to get you set up. Notes from Sgt Noll- Vent Bullet Points BASICS Initial calculation for TV is 6ml/kg increasing RR or TV lowers C02 decreasing RR or TV increases C02 increasing fio2 increases pao2 and sat increasing peep will increase pa02 sat o2 sat initial peep setting on vent is usually 5, can increase 1-2 at a time to 12 maximum CONTINUOUS SEDATION Primary indications for resedation of intubated Pt: Increased HR, Increased BP, Is Pt diaphoretic? Bucking? Consider using “Boom stick” Take a 10cc syringe if Flight Doc approved: Draw up 3cc Ketamine (150 mg) Draw up 5cc Fentanyl (250 mcg) Draw up 2cc Midazolam (10 mg) Recommend a 1cc push PRN every 10-15 mns Re-paralyzing the intubated patient: sTBI Patient – consider re-paralyzing w 100mg ROC if: continuous bucking and not responding to sedation and is hypertensive. Management of intubated patients (pulmonary contusion, blast lung, sTBI,) Pulmonary contusions: S/S: hypoxia, build up of fluid (blood) in lungs, high PIP ie: above 30 or 35 (if no DOPE issues) -Can be bi-lateral but more often unilateral from blunt force trauma TX: -increase Fio2 -If contusion is unilateral (only one side is affected) consider placing in recovery position with the “good” lung down (more blood will go to the lung) -consider lowering TV and increasing RR in order to maintain proper minute ventilation Blast Lung BLAST LUNG S/S: -hypoxia, dyspnea, cyanosis, increased Etco2 -high PIP over time as lung compliance decreases -reverberations for the blast can damage AC (alveolar capillary) membrane -damaged alveoli become damaged and cannot get rid of C02 properly TX: - increase fio2 -consider lowering TV and increasing RR (note: this is only temporary as Pt will clinically deteriorate as compliance will be poor) sTBI S/S per PJmed handbook Tx: per PJmed handbook Vent: maintain target 35-40 etco2 COMPARING THE IMPACT 731 AND SAVe II Impact 731 Pros: manipulation of I:E times, advanced synchronization settings, can detect patient effort and be triggered by Pt, can maintain 100% fio2 w supplemental O2, can be used down to a 5 kg Pt Cons: weight and cube size, complicated to use if not current, must manually calculated weight/size of patient SAVe II Pros: Small, light, automatically populates initial settings after pushing Pt height, simple to use Cons: Does not detect patient effort resulting in patient-ventilator “desynchrony” THAT OTHERS MAY LIVE!

Walk past the Bob Hope exhibit and through the connecting link. Turn left and go to the Combat Search and Rescue exhibit in the Southeast Asia War Gallery. Pararescueman William H. Pitsenbarger, who was born in Piqua wanted to quit high school to join the U.S. Army Special Forces’ Green Berets. His parents convinced him to stay in school, and after graduating in 1962, Pitsenbarger joined the Air Force. Arriving in Vietnam in August 1965, Pitsenbarger completed more than 250 missions, including one in which he hung from an HH-43’s cable to rescue a wounded South Vietnamese soldier from a burning minefield. This action earned him the Airman’s Medal and the Republic of Vietnam’s Medal of Military Merit and Gallantry Cross with Bronze Palm. William H. Pitsenbarger was only 21 years old when he was killed in action. But in his short life and heroic Air Force career, Pitsenbarger was an example of dedication, compassion and tenacity for all those with whom he served. In his work, and especially on his final mission, Airman 1st Class Pitsenbarger embodied the pararescueman’s motto: “That Others May Live.” For coordinating the successful rescues, caring for the wounded and sacrificing his life while aggressively defending his comrades, Pitsenbarger earned the Air Force Cross on June 30, 1966. After review, the original award was upgraded, and on December 8, 2000, the Medal of Honor was presented to his family in a ceremony here at the museum. Pitsenbarger is the 59th Medal of Honor recipient, and sixth enlisted recipient, from the Air Force and its predecessor organizations.
