In this episode of the PFC Podcast, the discussion revolves around patient positioning during procedural sedation, the pharmacology and application of Versed (Midazolam), and the nuances of dosing in various clinical scenarios. The speakers emphasize the importance of having a plan for airway management, the physiological implications of patient positioning, and the amnestic effects of Versed in trauma and surgical settings. They share insights from their experiences in austere environments, highlighting the need for adaptability and careful consideration of patient responses to sedation. In this conversation, the speakers delve into the complexities of IV amnestics, particularly focusing on Propofol and Midazolam, and the challenges faced during sedation in anesthesia. They discuss the importance of monitoring patient comfort, the nuances of sedation versus analgesia, and the implications of using drugs like Ketamine and Versed. The conversation highlights the critical nature of assessing patient responses and the difficulties in managing sedation effectively, emphasizing that sedation is often more challenging than general anesthesia.TakeawaysPatient positioning is crucial during procedural sedation.Recovery position may enhance airway protection during sedation.Always have a plan for airway management in different positions.Versed is a GABA agonist that promotes sedation and amnesia.Dosing of Versed varies significantly based on patient condition. In trauma cases, lower doses of sedatives may be necessary.Communication with the patient is key when administering sedatives.Understanding the pharmacodynamics of Versed aids in effective use.Amnestic effects of Versed can be dose-dependent.Clinical experience plays a significant role in medication administration. When deep sedation is required, it can be challenging to implement due to the difficulty of scheduling dedicated anesthesia coverage in the IR suite. Dr. Amy Deipolyi (interventional radiologist and Division Chief at Charleston Area Medical Center, WY) joins host Dr. Ally Baheti to explain an alternative approach to deep sedation through the use of ketamine. --- This podcast is supported by: Medtronic Abre Stent https://www.medtronic.com/en-us/healthcare-professionals/products/cardiovascular/deep-venous-stents/abre-venous-self-expanding-stent-system.html --- SYNPOSIS Dr. Deipolyi begins by sharing how she built a dedicated, academic IR program at a level 1 trauma center in West Virginia. The doctors then discuss the advantages of ketamine for deep sedation in the IR suite, and how Dr. Deipolyi gained administrative approval and implemented the change to achieve an alternative approach to deep sedation for interventional procedures. The discussion also includes how ketamine compares to traditional agents such as fentanyl and Versed. FriDudes - Getting Real. Pursuing Truth.

Normally I say Yes, you are in.  Definitely thanks for choosing to listen in.  Extra thanks when you give us input at FriDudes.com. We hear you.  More group and interviews on the way.  As for today and the subject of fires, I call Southern California home.  Yet even if you live in Alaska this will apply to you.  A reflection on fires, you all have them in life either figuratively or literally.  Here you go...Shout out to Marcus Rogers on that opener.  I bought and listening to his albums.  Some of my fave new music.  Please do the same if you dig that opening hook.  Give thanks for what is worthy.  Phil 4: 6-8, good segue as we talk fires. What fires do you have in your life?  Relationships stressed?  Finances?  Addiction?  You do have fires.  How do I know this?  Rom 3:23.Granted some worse than others.  On January 7, 2025, the LA fires have been a huge wake up call and some serious humble pie for some of the most proud and rich.  Instantly makes me think of an Insta post by my wife of a LA celebrity.  This celeb is known for her brashness.  I'm not going to name her.  For she has been humbled.  She asked that others avoid shallow posts based on beauty, etc.  Ironic part, didn't ask to pause those during the hurricanes in FL or pick your disaster of the day on this broken planet.  Truth, there are disasters and tragedy everyday for somebody on this broken planet.  For most, you only get proper perspective when it happens to you.  Think Job or any other MVP in the Bible.  That book is a tough read.  Whatever gets your mind gets you.  Rick WarrenSo many verses and so many directions we could go with this.  What gets your mind?  Which fire, good or bad?  Your wealth or desired wealth?  Your health?  Your home?  A relationship?  A drug?  Phil 4: 6-8Lastly, back to the MVP's, bad things do happen to good/great people.  A news bite I recently watched was of a man who lost his home in the Eaton Fire (Atladena), next to him was his neighbors house that looked untouched and now called 'Touched by God".  The reporter asked him why God didn't save his house.  His reply, "I guess God doesn't care about me.".  I do get that initial reaction.  I'm sure some of you do too.  Yet, it is a lie he is believing.  Versed ones.  You know what Jer 29:11 says.  You know what happened to Job and Moses and Elijah and Jacob and David and Daniel, jump ahead the disciples and Paul and most important God's own son.  Did God not care about seeing his son who was tortured and nailed to a cross?  Terrible things happen to the best.  Yet versed ones, you know Rom 8:28.  This podcast alone is full of stories of Hope and redemption out of awful.  Back to the LA fires, if you want to help.  Lots of options.  We have chosen to support Convoy of Hope.  As for further ideas hit us up and/or stay tuned.  Learn from these fires.  Palisades started with a spark.  And apparently a lack of preparation (water!) and resources.  Ironically, I can relate to that when my finances caught on fire.  I'm still working on that fire.  That's how big it was.  It's another reason I'm so passionate about Greg's GCrossSurf ministry, life planning with the Lord and infusing accountabilty in our lives.  Something I see sorely lacking in most small groups.

Yes!  You are in!  How are you doing with your New Year's resolutions?  1&Done?  More likel NoneDone  Per Greg, most people fail their resolutions by January 14th!  So you are not alone.  What works?  Lasting change?  Here you go...Yes, thank you again Marcus and the ThursDudes for that priceless nugget.  Listener, you are trying to do all these things yet do you have the Ultimate Power Source involved?  Do you know the verse Pro 19:21?Proverbs 19:21 says, "Many are the plans in a person's heart, but it is the Lord's purpose that prevails". Pro 16:9 says, We can make our plans, but the LORD determines our steps.Pro 16:3 says, Commit your actions to the LORD, and your plans will succeed.Doesn't mean you don't plan and go for it.  It does mean you best check your goals with God.  And also don't be surprised when your plans change.  Can you imagine how much better our anxiety levels would be if we could make peace with that.  Confession, I'm still working on this.Versed listeners, your goal for life is clear, to become more Christ like each day.  Can you imagine this world if we all did that?  No more wars, no more killing, no more terrorist strikes, no more mass shootings, etc.  Ok, odds are that you don't commit horrific acts.  Yet being more Christ like is loving God with all your heart, soul and mind.  This includes daily in the Word, praying, fasting and of course loving others.  Check Matt 22: 36-40.  Confession, I have room for improvement.  So do you.  How do I know that?  Check Rom 3:23.  If you think your poop doesn't stink, that is a serious problem right there. For those inspired, I am reading Practicing the Way by John Mark Comer right now.  Next you need a small/accountability group.  To learn more about Marcus, Greg, the Dudes, Alpha, Rooted, GCrossSurf, hit us up at the contact page at FriDudes.comNow go, Love God, ask him what you should be doing this year, read the Bible to get your answers, all of it, not just an isolated portion to justify something (that's where people go rogue and justify evil in their minds).  Next love others, that includes the ones not so lovable.  I love you, go...

Versed's Katherine Power and Lola Gonzalez Marra: ‘Every great brand should evolve its look and feel every five years'

“I believe that every great brand should evolve its look and feel about every five years,” said Katherine Power, serial entrepreneur and the founder of Versed skin care. “Since our launch in 2019, we've seen that our core customer, who is primarily an older millennial or young Gen X — I call them Xennials — has really grown along with us, and we wanted to continue to grow with them and to create something fresh and modern.” This included a refresh of packaging, visuals and language overseen by Lola Gonzalez Marra, Versed's longtime creative lead. Power and Marra have worked together for more than a decade on Power's many entrepreneurial projects, including editorial sites WhoWhatWear and Byrdie — which Power launched with Hillary Kerr in 2006 and 2013, respectively — as well as color cosmetics line Merit, which Power founded in 2021.  “We decided we needed to evolve enough that it's a change, but not so much that you go on Versed one day and it's a completely different brand,” said Marra. As Power and Marra explained on this episode of the Glossy Beauty Podcast, the refresh has been in the works for over a year.  Versed was launched in 2019 from data and insights learned at Byrdie and WhoWhatWear, which Power and Kerr have since sold to publisher Dotdash in 2019 and publisher Future Brands in 2022, respectively.  Thanks to Versed's omnichannel strategy where 75% of its sales are through Target, switching over merchandise has required a unique strategy. Hint: Versed is selling off older products DTC for 30% off right now. The team also rolled out tweaks to its social media aesthetic and naming formula for products. The brand also used this as an opportunity to fine-tune the assortment by making product cuts. But perhaps the biggest change is in its packaging colors, which Marra desaturated and leveraged into category IDs, all of which the duo discusses in the podcast episode.

In this podcast episode, Dennis and Andy Fisher discuss the co-administration of ketamine and Versed. They explore the reasons behind the common practice of adding Versed to ketamine, the concept of emergence reactions, and the appropriate use of Versed. They also touch on the potential risks and side effects of both drugs and the importance of individualized dosing. The conversation emphasizes the need for critical thinking and careful consideration when administering these medications. Takeaways The practice of adding Versed to ketamine is based on historical protocols and the belief that it provides amnestic properties and prevents emergence reactions. There is no clear definition of emergence reactions, but they can include unpleasant sensations, disconnection, hallucinations, and agitation. Ketamine itself has amnestic properties and should provide adequate amnesia without the need for Versed. The use of Versed should be based on individual patient needs, such as safety and mission comfort, and not protocolized. Careful consideration of dosage, administration speed, and patient physiology is crucial to avoid adverse events and ensure optimal outcomes. The conversation discusses patient positioning during procedural sedation and the use of Versed (Midazolam) as a sedative. The speakers explore the importance of patient positioning and the need to have a plan for airway management in different positions. They also provide insights into the mechanism of action and dosing of Versed, highlighting the importance of individual patient factors and context in determining the appropriate dose. The conversation emphasizes the need for a staged approach when using sedatives and the importance of communication with the patient to assess their response. In this conversation, Dennis discusses the use of different medications for sedation and anesthesia, focusing on the challenges and considerations involved. He explains that there is no specific dose for achieving amnesia with medications like Midazolam, Propofol, and Ketamine. The dose depends on various factors, including the patient's physiological state and the presence of alcohol in their system. Dennis emphasizes the importance of closely monitoring vital signs and respiratory rate during sedation to ensure the patient remains comfortable and ventilating properly. He also discusses the use of analgesics and local anesthesia to manage pain during procedures. The conversation concludes with a discussion on the reversal agent for sedatives and the importance of considering the set and setting when administering medications like Ketamine. Takeaways Consider patient positioning during procedural sedation and have a plan for airway management in different positions. Understand the mechanism of action and dosing of Versed (Midazolam) as a sedative. Take into account individual patient factors and context when determining the appropriate dose of Versed. Use a staged approach when administering sedatives and communicate with the patient to assess their response. There is no specific dose for achieving amnesia with sedative medications like Midazolam, Propofol, and Ketamine. The dose of sedative medications depends on various factors, including the patient's physiological state and the presence of alcohol in their system. Monitoring vital signs and respiratory rate is crucial during sedation to ensure the patient remains comfortable and ventilating properly. Analgesics and local anesthesia can be used to manage pain during procedures. The reversal agent for sedatives, such as Versed, is Ramazicon, but it should be used cautiously and only after ruling out other causes for the patient not waking up. Consider the set and setting when administering sedative medications, especially in high-stress environments like combat situations.

He was given a 24 hour window in Montana, and ends up putting down two deer, and is able to get them gutted and back home. A real testament to meat-hunters. The guys turn to two schools of thought; whole muscle vs pre-cut. Cutting whole cuts into steaks, or pre portioned packages is a great way to get to the meal quickly. James Ryan, a 50-year-old oral surgeon and former owner of Evolution Oral Surgery in Germantown, Maryland, has been sentenced to 45 years in prison for supplying his girlfriend, Sarah Harris, with addictive anesthesia solutions that ultimately led to her death. The sentencing far exceeds state recommendations for the case of depraved-heart murder. Circuit Court Judge Cheryl A. McCally, who presided over the case, showed little mercy as she delivered the sentence. Harris, who was only 25 years old and weighed a mere 83 pounds at the time of her death, succumbed to a lethal combination of ketamine, propofol, and diazepam intoxication. Prosecutors argued that Ryan played a significant role in Harris's death, as he continuously provided her with stolen drugs from his practice, even as her health deteriorated. Despite a calculated sentencing guideline of 10 to 25 years, Judge McCally emphasized the exploitation of trust and the gravity of Ryan's actions, leading her to impose the 45-year sentence. Tina Harris, Sarah's grieving mother, expressed satisfaction with the verdict. In an emotional statement in court, she clutched a lock of Sarah's braided hair and a satchel of her ashes, proclaiming, "This is all I have left of my baby. This is all I have left because you took her." The text messages showed that it was February 2021 when Ryan first introduced her to ketamine, a drug often used for surgery sedation, which he said would help her anxiety. “I can give you an injection,” he wrote. “The anxiety will be completely gone in 6 seconds.” The couple's conversations expanded to other drugs and supplies, including IV poles, saline bags, gauze, tape, and 20-gauge needles. “Can you get more Versed and ketamine,” Harris wrote on Aug. 27, 2021. “And also IV catheter.” The texts were often written while Harris was at their home and Ryan was at his office. At one point, as noted by McCally on Wednesday, Ryan wrote to Harris that he had given her some more ketamine while she slept. The judge also referenced a 10-second video, apparently taken by Ryan, of the two of them sitting next to each other. Harris had an arm up in the air as if it was floating that he would make such a video and keep it made an impression on the judge. “The fact that that seemed appropriate in your world is gut-wrenching,” she said. On the morning of January 26, 2022, Ryan discovered Harris unresponsive in their Clarksburg home and called 911. Despite efforts to revive her, paramedics pronounced Harris dead at the scene. An autopsy later confirmed the cause of death as drug intoxication. Prosecutors sought a 55-year sentence, arguing that Ryan was aware of the deadly consequences of his actions. Ryan's defense maintained that he acted out of love to alleviate Harris's pain but made the wrong decisions. They emphasized his struggles with drug abuse and mental health. Judge McCally, however, condemned Ryan's dangerous actions, pointing to text messages and a video recording where Harris appeared to be high.  Want to listen to ALL of our podcasts AD-FREE? The focus is on the importance of autonomic stability in combat situations. The best medication for amnesia in a downrange environment is Midazolam or Versed, and ketamine is a potent analgesic. The podcast also covers the MAP (mean arterial pressure) and how to maintain it in trauma patients. Finally, the podcast discusses various scenarios that require different types of sedation or analgesia, including the administration of fentanyl or calcium. Sally Mueller is the Co-founder and CEO of Womaness, modern menopause solutions from top to toe.   She is a seasoned executive with impeccable entrepreneurial acumen. Sally spent over two decades of her career at Target Corporation, first in Apparel Merchandising and then in Marketing, helping to build the bullseye from a regional discount retailer to one of the world's most recognized and influential global brands. She developed and implemented Target's designer program and Design for All platform, transforming the regional discount retailer into a global phenomenon.   Sally went on to launch her own company, Whyse Branding, where she formed a licensing relationship with Who What Wear and successfully launched the consumer products line in Target stores and ultimately achieved international distribution. She was then named Chief Brand Officer for Clique Brands, overseeing their Consumer Brands Division, where she launched Joy Lab at Target, incubated VERSED skincare and expanded Who What Wear internationally.   Sally's encore is creating a revolutionary consumer hit with the introduction of Womaness (which debuted in March 2021), a brand that is changing the conversation around menopause through innovative products that offer solutions from head to toe (and everything in-between), readily avail via Target.   Womaness provides trusted advice on symptoms from hot flashes to sleep issues to fine lines, and support from an inspired community of women who don't believe in the pause (of menopause) and exude #menopositivity, aka a dynamic, joyful outlook that embraces who we are today.   A lifelong Minnesotan, Sally, and her husband reside outside of Minneapolis, slowly acclimating to life as empty nesters.   Follow Sally on Instagram, Twitter, and LinkedIn.   Visit Womaness at Womaness.com.   Remarkable Quote:   “That night I said to my husband ‘I'm going to disrupt this industry.'”   Find Us Online! In this episode, interventional radiologists Dr. Aaron Fritts, Dr. Vishal Kumar, and Dr. Chris Beck discuss types of sedation for IR procedures. --- CHECK OUT OUR SPONSOR Medtronic OBL https://www.medtronic.com/obl --- SHOW NOTES We start the discussion off by dividing the spectrum of patient sedation into three tiers: local anesthesia, moderate sedation with fentanyl and Versed, and deep sedation which encompasses anything beyond fentanyl and Versed. The conversation revolves around the fluidity of this spectrum, underscoring the ease with which patients can transition between these sedation levels, thereby adding layers of complexity to the decision-making process. Chris emphasizes the significance of embracing trauma-informed care, highlighting the potential for IR procedures to be maximally traumatic despite their minimally invasive nature. The doctors emphasize the importance of establishing comprehensive pre-procedural patient education to align expectations about pain management. Procedures that automatically trigger the requirement for an anesthesia team include procedures such as TIPS, tumor ablations, as well as declot procedures more recently. Decisions regarding anesthesia for these procedures are additionally influenced by factors such as the patient's condition, history of methadone exposure, the preference of the IR providers, and the comfort level of nursing staff. Vishal highlights the game-changing concept of having an anesthesia team present for real-time monitoring and the administration of nerve blocks during IR procedures. This collaboration is especially helpful for complex interventions. Vishal advocates for prioritizing patient safety over logistical considerations. He firmly contends IRs should be empowered to decide the level of anesthesia care since they have the most experience with the specific procedures and are best suited to recognize anesthesia needs. The conversation delves into the American Society of Anesthesiologists (ASA) scoring system, which categorizes patients based on their health status from ASA 1 (healthy) to ASA 6 (brain death). While IR procedures usually involve ASA 3 or ASA 4 patients, today's discussion uncovers the tendency among clinicians to underestimate a patient's ASA classification while overestimating their pain tolerance. The discussion raises critical questions about whether the IR community has grown complacent with moderate sedation in procedures and whether alternative approaches are warranted. --- RESOURCES American Society of Anesthesiologists Classification (ASA) Classification: https://www.ncbi.nlm.nih.gov/books/NBK441940/

Functional medicine expert Dr. Will Cole joins us to talk about his newest book, Gut-Feelings: Healing the Shame-Fueled Relationship Between What You Eat and How You Feel, and how to heal our bodies through nutrition. Versed in the Enneagram, Dr. Cole shares how it has helped him educate and empower others by learning to communicate with others in a way they can receive it.   "My Enneagram Five-ness as far as being a researcher really does suit me well, and I think that's why I've been such a voracious consumer of health information and research [...] but, there's this duality, bidirectional relationship between what I do as science and the art," explains Dr. Cole. "The art part of what we do is holding the space in between the words and really understanding how can I get this health information and help them to understand it, to empower them, to give them agency over their health, and to really help them apply these things so it's sustainable and practical, for them to reclaim their quality of life?" Sadly, it seems to be the opposite of what people often do. And yet this is what God has called us to do for others, in small ways and big ones.  Join me for a special conversation with my wonderful husband Pastor Gary Coiro as we talk about the difference between cancel culture and cover culture.  “Above all, love each other deeply because love covers a multitude of sins.” - 1 Peter 4:8

“The greatest piece of who we are is our vulnerability…our open-heartedness is our genius…”LEANN RIMES is an American treasure and one of my all-time favorite recording artists.I am so excited to welcome her to my show this week for an incredibly deep conversation about life, emotions, mental health, God and much much more. This conversation will MOVE you and when I say it's DEEP, I mean it . We are both very open and vulnerable in this conversation and it won't soon be forgotten.Trust me, we are both incredibly open in ways maybe neither one of us have been beforeLeAnn is REMARKABLE in her CREATIVITY, TALENT, and PERSEVERANCE.Think about this as you listen. That's not to say it has always been easy for LeAnn, because it hasn't.She makes no secret of her love of GOD and the role how PRESENCE has played in her life. You're going to hear what that presence means and the important role this plays in her singing. That FAITH has also been instrumental in guiding her career and her personal life.LeAnn is going to tell you how she handles ANXIETY, being PLUGGED IN all the time, and finding ways to take good care of herself through it all.  We're also going to dig into why it's dangerous to TIE YOUR IDENTITY TO EXTERNAL ACHIEVEMENTS and how LeAnn's thinking about that has evolved over time.Like any highly creative person, she's also prone to CREATIVE HIGHS and the crashes that often come after. Those ups and downs are real, not only for her but for everyone. You'll want to hear how she manages those types of emotions so you can use that wisdom in your own life. We're going to close with two things LeAnn knows a lot about…Working on getting LOVE from other people…And advice for making your DREAMS come true.I think she's well “VERSED” to talk about these things, and after you hear her, you'll think so too.
